Improving Network Availability through Optimized Multipath Routing and Incremental Deployment Strategies

Distinct but correct: generating diversified and entity-revised medical response

Medical dialogue generation(MDG) is applied for building medical dialogue systems for intelligent consultation. Such systems can communicate with patients in real time, thereby improving the efficiency of clinical diagnosis. However, predicting correct entities and correctly generating distinct responses remain a great challenge. Inspired by actual doctors’ responses to patients, we consider MDG a two-stage task: entity prediction and dialogue generation. For entity prediction, we design an ent-mac post pre-training strategy by leveraging external medical entity knowledge to enhance the pre-trained model. For dialogue generation, we propose an entity-aware fusion MDG method in which predicted entities are integrated into the dialogue generation model through different encoding fusion mechanisms, using information from different sources. Because the diverse beam search algorithm can produce responses with entities that deviate from the predicted entities, an entity-revised diverse beam search is proposed to correct the entities entailed in the generated responses and make the generated responses more distinct. The experimental results on the China Conference on Knowledge Graph and Semantic Computing 2021(A/B tests) and the International Conference on Learning Representations 2021(online test) datasets show that the proposed method outperforms several state-of-the-art methods, which demonstrates its practicability and effectiveness.

Asperuloside Promotes Apoptosis of Cervical Cancer Cells through Endoplasmic Reticulum Stress-Mitochondrial Pathway

Objective: To investigate the effects of asperuloside on cervical cancer based on endoplasmic reticulum(ER) stress and mitochondrial pathway. Methods: Different doses(12.5–800 μg/mL) of asperuloside were used to treat cervical cancer cell lines Hela and CaSki to calculate the half maximal inhibitory concentration(IC_(50)) of asperuloside. The cell proliferation was analyzed by clone formation assay. Cell apoptosis, intracellular reactive oxygen species(ROS) and mitochondrial membrane potential were determined by flow cytometry. The protein expressions of cleaved-caspase-3, Bcl-2, Bax, Cyt-c, cleaved-caspase-4 and glucose-regulated protein 78(GRP78) were analyzed by Western blot. And the inhibitor of ER stress, 4-phenyl butyric acid(4-PBA) was used to treat cervical cancer cells to further verify the role of ER stress in the apoptosis of cervical cancer cells induced by asperuloside. Results: Asperuloside of 325, 650, and 1300 μg/mL significantly inhibited the proliferation and promoted apoptosis of Hela and CaSki cells(P<0.01). All doses of asperuloside significantly increased intracellular ROS levels, reduced mitochondrial membrane potential, significantly reduced Bcl-2 protein expression level, and increased Bax, Cyt-c, GRP78 and cleaved-caspase-4 expressions(P<0.01). In addition, 10 mmol/L 4-PBA treatment significantly promoted cell proliferation and reduced apoptosis(P<0.05), and 650 μg/mL asperuloside could reverse 4-PBA-induced increased cell proliferation, decreased apoptosis and cleaved-caspase-3,-4 and GRP78 protein expressions(P<0.05). Conclusion: Our study revealed the role of asperuloside in cervical cancer, suggesting that asperuloside promotes apoptosis of cervical cancer cells through ER stress-mitochondrial pathway.

The journey to implement cancer genomic medicine(CGM)in oncology practice began in the 1980s,which is considered the dawn of genetic and genomic cancer *** the time,a variety of activating oncogenic alterations and their functional significance were unveiled in cancer cells,which led to the development of molecular targeted therapies in the 2000s and *** CGM is still a relatively new discipline and it is difficult to predict to what extent CGM will benefit the diverse pool of cancer patients,the National Cancer Center(NCC)of Japan has already contributed considerably to CGM advancement for the conquest of *** back at these past achievements of the NCC,we predict that the future of CGM will involve the following:1)A biobank of paired cancerous and non-cancerous tissues and cells from various cancer types and stages will be *** quantity and quality of these samples will be compatible with omics *** biobank samples will be linked to longitudinal clinical information.2)New technologies,such as whole-genome sequencing and artificial intelligence,will be introduced and new bioresources for functional and pharmacologic analyses(e.g.,a patient-derived xenograft library)will be systematically deployed.3)Fast and bidirectional translational research(bench-to-bedside and bedside-to-bench)performed by basic researchers and clinical investigators,preferably working alongside each other at the same institution,will be implemented;4)Close collaborations between academia,industry,regulatory bodies,and funding agencies will be established.5)There will be an investment in the other branch of CGM,personalized preventive medicine,based on the individual's genetic predisposition to cancer.

Impacts of Roof/Ground Mitigation Strategies on Improving the Urban Thermal Environment and Human Comfort over the Yangtze River Delta, China

The combined effects of global warming and the urban heat islands exacerbate the risk of urban heat stress. It is crucial to implement effective cooling measures in urban areas to improve the comfort of the thermal environment. In this study, the Weather research and Forecasting Model(WRF), coupled with a single-layer Urban Canopy Model(UCM), was used to study the impact of heat mitigation strategies. In addition, a 5-km resolution land-cover dataset for China(ChinaLC), which is based on satellite remote sensing data, was adjusted and used, and 18 groups of numerical experiments were designed, to increase the albedo and vegetation fraction of roof/ground parameters. The experiments were conducted for four heatwave events that occurred in the summer of 2013 in the Yangtze River Delta urban agglomeration of China. The simulated results demonstrated that, for the single roof/ground schemes, the mitigation effects were directly proportional to the albedo and greening. Among all the experimental schemes, the superposed schemes presented better cooling effects. For the ground greening scheme, with similar net radiation flux and latent heat flux, its storage heat was lower than that of the roof greening scheme, resulting in more energy flux into the atmosphere, and its daytime cooling effect was not as good as that of the roof greening scheme. In terms of human thermal comfort(HTC), the improvement achieved by the ground greening scheme was better than any other single roof/ground schemes, because the increase in the relative humidity was small. The comprehensive evaluation of the mitigation effects of different schemes on the thermal environment presented in this paper provides a theoretical basis for improving the urban environment through rational urban planning and construction.

目的基于能力、机会、动机和行为(capability, opportunity, motivation, and behavior, COM-B )理论,调查我国护士抗菌药物管理(antimicrobial stewardship, AMS)参与水平及其影响因素,为制订有效策略提高AMS护理质量提供参考依据。方法采用横断面调查法。于2021年11月至2022年1月,选取湖南省的17所三级医院的4514名护士进行调查。在COM-B理论指导下,自行编制护士参与抗菌药物管理问卷(the NurseAMS Engagement Que stionnaire ,NAEQ)。该问卷包括能力(14个条目)、机会(7个条目)、动机(6个条目)和行为(12个条目)4个维度,共39个条目。结果护士NAEQ总分为( 155.08±±27.12)分,得分率为79.5% ,表明护士参与AMS处于中等水平。其中,能力维度得分为(52.33±13.48 )分,机会维度得分为(28.64±5.76)分,动机维度得分为(24.57±4.57)分,行为维度得分为(4953±8.83)分。不同职称、是否兼职感染控制护士、是否知晓AMS团队、是否知晓抗菌药物限定日剂量、科室类型、科室是否配备临床药师、参与抗菌药物培训和医师-护士联合查房的频率等特征不同的护士NAEQ得分比较,差异有统计学意义(P<0.05)。其中初级职称护士的NAEQ得分高于中级职称护士(P<0.05);担任兼职感染控制护士、知晓AMS团队、知晓抗菌药物限定日剂量的护士NAEQ得分高于其他护士(P<0.01);重症监护室和传染病科护士NAEQ得分低于五官科等其他科室的护士(P<0.01);科室配备临床药师的护士NAEQ得分高于未配备或不清楚是否配备临床药师的护士(P<0.01);参与抗菌药物培训和医师-护士联合查房次数较多的护士NAEQ得分高于其他护士(P<0.01)。结论加强教育和培训及改善多学科沟通与协作等多种策略有望提高护士参与AMS的水平。此外,应更多关注中级职称、感染控制经验较少以及特定科室工作的护士。
