BACKGROUND:Prompt pathogen identification can have a substantial impact on the optimization of antimicrobial *** objective of the study was to assess the diagnostic value of next-generation sequencing(NGS)for identifying pathogen and its clinical impact on antimicrobial intervention in immunocompromised patients with suspected ***:This was a retrospective *** January and August 2020,47 adult immunocompromised patients underwent NGS testing under the following clinical conditions:1)prolonged fever and negative conventional cultures;2)new-onset fever despite empiric antimicrobial treatment;and 3)afebrile with suspected infections on *** data,including conventional microbial test results and antimicrobial treatment before and after NGS,were *** were analyzed according to documented changes in antimicrobial treatment(escalated,no change,or deescalated)after the NGS ***:The median time from hospitalization to NGS sampling was 19 *** relevant pathogens were detected via NGS in 61.7% of patients(29/47),more than half of whom suffered from fungemia(n=17),resulting in an antimicrobial escalation in 53.2% of patients(25/47)and antimicrobial de-escalation in 0.2% of patients(1/47).Antimicrobial changes were mostly due to the identification of fastidious organisms such as Legionella,Pneumocystis jirovecii,and *** the remaining three cases,NGS detected clinically relevant pathogens also detected by conventional cultures a few days *** antimicrobial treatment was subsequently adjusted according to the susceptibility test ***,NGS changed antimicrobial management in 55.3%(26/47)of patients,and conventional culture detected clinically relevant pathogens in 14.9% of the patients(7/47).CONCLUSION:With its rapid identification and high sensitivity,NGS could be a promising tool for identifying relevant pathogens and enabling rapid appropriate treatment in immunocompromised patients w

Level pinning of anti-PT-symmetric circuits for efficient wireless power transfer

Wireless power transfer(WPT) technology based on magnetic resonance(a basic physical phenomenon)can directly transfer energy from the source to the load without wires and other physical contacts, and has been successfully applied to implantable medical devices, electric vehicles, robotic arms and other ***, due to the frequency splitting of near-field coupling, the resonant WPT system has some unique limitations, such as poor transmission stability and low efficiency. Here, we propose anti-resonance with level pinning for high-performance WPT. By introducing the anti-resonance mode into the basic WPT platform, we uncover the competition between dissipative coupling and coherent coupling to achieve novel level pinning, and construct an effective anti-parity-time(anti-PT)-symmetric non-Hermitian system that is superior to previous PT-symmetric WPT schemes. On the one hand, the eigenvalue of the anti-PT-symmetric system at resonance frequency is always pure real in both strong and weak coupling regions, and can be used to overcome the transmission efficiency decrease caused by weak coupling, as brought about by, for example, a large size ratio of the transmitter to receiver, or a long transmission *** the other hand, due to the level pinning effect of the two kinds of coupling mechanisms, the working frequency of the system is guaranteed to be locked, so frequency tracking is not required when the position and size of the receiver change. Even if the system deviates from the matching condition, an efficient WPT can be realized, thereby demonstrating the robustness of the level pinning. The experimental results show that when the size ratio of the transmitter coil to the receiver coil is 4.29(which is in the weak coupling region), the transfer efficiency of the anti-PT-symmetric system is nearly 4.3(3.2) times higher than that of the PT-symmetric system when the matching conditions are satisfied(deviated). The first exothermic peak of cement-based material occurs a few minutes after mixing,and the properties of three dimensional(3D)printed concrete,such as setting time,are very sensitive to *** this background,based on the classical Park cement exothermic model of hydration,we propose and construct a numerical model of the first exothermic peak,taking into account the proportions of C_(3)S,C_(3)A and quicklime in *** calculated parameters are calibrated by means of relevant published exothermic test *** is found that this developed model offers a good simulation of the first exothermic peak of hydration for C_(3)S and C_(3)A proportions from 0 to 100% of cement clinker and reflects the effect of quicklime content at 8%-10%.The unique value of this research is provision of an important computational tool for applications that are sensitive to the first exothermic peak of hydration,such as 3D printing.

Schistosoma infection is one of the major causes of liver *** roles of hepatic progenitor cells(HPCs)in the pathogenesis of liver fibrosis have been ***,the precise mechanism underlying the role of HPCs in liver fibrosis in schistosomiasis remains *** study examined how autophagy in HPCs affects schistosomiasis-induced liver fibrosis by modulating exosomal *** activation of HPCs was verified by immunohistochemistry(IHC)and immunofluorescence(IF)staining in fibrotic liver from patients and mice with Schistosoma japonicum *** coculturing HPCs with hepatic stellate cells(HSCs)and assessing the autophagy level in HPCs by proteomic analysis and in vitro phenotypic assays,we found that impaired autophagy degradation in these activated HPCs was mediated by lysosomal *** autophagy by the autophagy inhibitor chloroquine(CQ)significantly diminished liver fibrosis and granuloma formation in ***-infected ***-secreted extracellular vehicles(EVs)were further isolated and studied by miRNA ***-1306-3p,miR-493-3p,and miR-34a-5p were identified,and their distribution into EVs was inhibited due to impaired autophagy in HPCs,which contributed to suppressing HSC *** conclusion,we showed that the altered autophagy process upon HPC activation may prevent liver fibrosis by modulating exosomal miRNA release and inhibiting HSC activation in *** the autophagy degradation process may be a therapeutic strategy for liver fibrosis during Schistosoma infection.

Survival Analysis of Patients Undergoing Intraoperative Contrast-enhanced Ultrasound in the Surgical Treatment of Malignant Glioma

Holography can provide the desired wavefront phase and/or amplitude for imaging,particle manipulation,bacteria trapping,and cell patterning in optics and ***,previous work on acoustic holography is mostly based on local design optimization,either using active control of the sound source or relying on the structural design to provide the desired *** precise control over the acoustic field remains a significant ***,we realize refined single-plane symmetric binary amplitude,asymmetric intensity gradient amplitude,and bi-objective hologram through the non-local holographic imaging theory that considers the acoustic coupling of structural units in *** taking into account the self-radiation and mutual radiation between many small units on a plate of well-designed thickness,as well as the transmission through the plate's apertures,we can effectively regulate the sound field behind the *** demonstrate the effectiveness of our approach through numerical simulations and experiments,showcasing a circle,a black hole,and a bi-objective with a circle and a square ***,the acoustic black hole hologram precisely reconstructs the intensity gradient distribution at two bright *** non-local holographic imaging theory is valuable for the fine-intensity regulation of the sound field and is expected to be applied in ultrasound diagnosis and treatment,medical imaging,and other fields.
