BACKGROUND No single endoscopic feature can reliably predict the pathological nature of colorectal tumors(CRTs).AIM To establish and validate a simple online calculator to predict the pathological nature of CRTs based on white-light *** This was a single-center *** the identification stage,530 consecutive patients with CRTs were enrolled from January 2015 to December 2021 as the derivation *** regression analysis was performed.A novel online calculator to predict the pathological nature of CRTs based on white-light images was established and verified *** the validation stage,two series of 110 images obtained using white-light endoscopy were distributed to 10 endoscopists[five highly experienced endoscopists and five less experienced endoscopists(LEEs)]for external validation before and after systematic *** A total of 750 patients were included,with an average age of 63.6±10.4 *** colorectal cancer(ECRC)was detected in 351(46.8%)*** size,left semicolon site,rectal site,acanthosis,depression and an uneven surface were independent risk factors for *** C-index of the ECRC calculator prediction model was 0.906(P=0.225,Hosmer-Lemeshow test).For the LEEs,significant improvement was made in the sensitivity,specificity and accuracy(57.6%vs 75.5%;72.3%vs 82.4%;64.2%vs 80.2%;P<0.05),respectively,after training with the ECRC online calculator prediction *** A novel online calculator including tumor size,location,acanthosis,depression,and uneven surface can accurately predict the pathological nature of ECRC.

Circulating thrombospondin-2 in patients with moderate-to-severe chronic heart failure due to coronary artery disease

Chronic heart failure（CHF）remains a leading cause of morbidity and *** the current study,we aimed to evaluate the predictive value of circulating fhrombospondin-2（TSP-2）for cumulative survival in patients with ischemic CHF due to coronary artery disease（CAD）.The results showed that during a median follow-up of2.18 years,21 participants died and 106 subjects were hospitalized *** median circulating levels of TSP-2 in patients who survived and those who died were 0.63 ng/mL（95%CI=0.55-0.64 ng/mL）and 1.03 ng/mL（95%CI=0.97-1.07 ng/mL）（P〈0.001）.Circulating TSP-2 independently predicted all-cause mortality（OR=1.27;95%CI=1.08-1.59;P=0.002）,CHF-related death（OR=1.16;95%CI=1.02-1.50;P〈0.001）,and also CHF-related rehospitalization（OR=1.12;95%CI=1.07-1.25;P〈0.001）.In conclusion,among CAD patients with symptomatic CHF,increased circulating TSP-2 is correlated with increased 3-year CHF-related death,all-cause mortality,and risk for recurrent hospitalization.

clinically relevant postoperative pancreatic fistula(CR-POPF)has continued to compromise patient recovery post-pancreatectomy despite decades of research seeking to improve risk prediction and *** current diagnostic criteria for CR-POPF requires elevated drain fluid amylase to present alongside POPFrelated complications including infection,haemorrhage and organ *** worrying sequelae necessitate earlier and easily obtainable biomarkers capable of reflecting evolving *** fluid has recently emerged as a promising source of biomarkers as it is derived from the pancreas and hence,capable of reflecting its postoperative *** present review aims to summarise the current knowledge of CR-POPF drain fluid biomarkers and identify gaps in the field to invigorate future research in this critical area of clinical *** findings may provide robust diagnostic alternatives for CR-POPF and hence,to clarify their clinical utility require further reports detailing their diagnostic and/or predictive accuracy.

Rapid hemodilution is associated with increased sepsis and mortality among patients with severe acute pancreatitis

Background Hemoconcentration may be an important factor that determines the progression of severe acute pancreatitis （SAP）. In addition, it has been proposed that biomarkers may be useful in predicting subsequent necrosis in SAP. However, it is still uncertain whether hemodilution in a short term can improve outcome. We aimed to investigate the effect of rapid hemodilution on the outcome of patients with SAP. Methods One hundred and fifteen patients were admitted prospectively according to the criteria within 24 hours of SAP onset. Patients were randomly assigned to either rapid hemodilution （hematocrit （HCT）〈35%, n=56） or slow hemodilution （HCT 〉35%, n=-59） within 48 hours of onset. Balthazar CT scores were calculated on admission, day 7, and day 14, after onset of the disease. Time interval for sepsis presented, incidence of sepsis within 28 days and in-hospital survival rate were determined. Results The amount of fluid used in rapid hemodilution was significantly more than that used in slow hemodilution （P 〈0.05） on the admission day, the first day, and the second day. There were significant differences between the rapid and slow hemodilution group in terms of hematocrit, oxygenation index, pH values, APACHE II scores and organ dysfunction at different time during the first week. There were significant differences in the time interval to sepsis in rapid hemodilution （（7.4±1.9） days） compared with the slow hemodilution group （（10.2±2.3） days）, and the incidence of sepsis （78.6%） was higher in the rapid group compared to the slow （57.6%） in the first 28 days. The survival rate of the slow hemodilution group （84.7%） was better than the rapid hemodilution （66.1%. P 〈0.05）. Conclusions Rapid hemodilution can increase the incidence of sepsis within 28 days and in-hospital mortality. Hematocrit should be maintained between 30%-40% in the acute response stage.

This report describes a rare case of metastatic hepatocellular carcinoma (HCC) presenting a huge mass in the left external auditory canal (EAC). The patient was a 55-year-old man with hepatitis B virus-related HCC. He presented to our department with a three-month history of increasing left otalgia,and hearing loss with recent fresh aural bleeding. Histopathologic examination indicated that the tumor was secondary to HCC. Although external irradiation was not effective,the tumor was treated with surgical debulking and high dose rate 192 Ir remote afterloading system (RALS) for postoperative intracavitary irradiation. A review of the literature revealed only five other cases of HCC metastasis to the temporal bone,all of which mainly metastasized in the internal acoustic meatus. The present case is the first report of HCC metastasis to the EAC.

Male Breast Cancer:10-Year Experience at Mansoura University Hospital in Egypt

Objective Male breast cancer （MBC） is a rare disease representing less than 1% of all malignancies. The objective of the study is to report clinicopathological characteristics, treatment patterns, and outcomes of MBC in Mansoura University Hospital, Egypt. Methods This retrospective study focused on male breast cancer patients during 10 years （2000-2009）. The studied variables were data regarding general characteristics of patients, treatment modalities and survival. Results The series included 37 patients （0.8% of all breast cancer）. The median age was 57.7 years （range： 26-86 years）. The main clinical complaint was a mass beneath the areola ill 94.5% of the cases. Most patients had a locally advanced disease. 94.5% of tumors were invasive duct carcinomas. The treatment was essentially surgery in 91.8%, followed by adjuvant radiotherapy （in 89.2%）, hormonal therapy （in 56.7%） and chemotherapy （in 91.8%）. Follow-up period ranged from 6-115 months. Local recurrence occurred in 4 cases and metastasis in 11 cases. The 2-year and 5-year overall survival （OS） rates were 81.6% and 60.5%, respectively. The 2-year and 5-year disease-free survival （DFS） rates were 68.4%, and 52.6%, respectively. OS was not significantly affected by any of the studied parameters. Factors influencing DFS were： T stage （P=0.05）, positive lymph nodes （P=0.043）, metastasis （P=0.004）, and chemotherapy （1＇=0.046）. Conclusions MBC is a rare disease and often diagnosed at a locally advanced stage. The management of male and female breast carcinoma is identical. Future research for better understanding of this disease is needed to improve the management and prognosis of male breast cancer patients.

Relationship between interleukin-1 type I and 2 receptor gene polymorphisms and the expression level of membrane-bound receptors

The biological activity of the multifunctional cytokine interleukin-1 （IL-1） is mediated by its receptors. The aim of this study was to determine if an association exists between single nucleotide polymorphisms （SNPs） in the IL-1 type I and 2 receptor genes （ILIR1 and ILIR2） and the expression level of membrane-bound ILIRs on subpopulations of mononuclear cells or serum levels of soluble IL-1 receptors. It was observed that healthy individuals with the genotype TT in SNP rs2234650.C〉T had a lower percentage of intact CD14＋ monocytes expressing ILIR1 on their surface. The SNP rs4141134-T〉C in ILIR2 has also been associated with the percentage of intact CD3＋ T cells expressing ILIR2. Furthermore, individuals carrying the CC allele of SNP rs4141134.T〉C and the TT allele of SNP rs2071008-T〉G in ILIR2 had a lower density of ILIR2s on the surface of CD14＋ monocytes in lipopolysaccharide （LPS）-stimulated PBMC cultures. In summary, this study demonstrated that IL-1 receptor gene polymorphisms could be one of the factors influencing the expression of membrane-bound IL-1 receptors （ILIR） on immunocompetent cells.

There is currently a huge worldwide demand for donor kidneys for organ ***,numerous marginal donor kidneys,such as kidneys with microthrombi,are used to save patients'*** some studies have shown an association between the presence of microthrombi in donor kidneys and an increased risk for delayed graft function(DGF)(McCall et al.,2003;Gao et al.,2019),other studies have demonstrated that microthrombi negatively impact the rate of DGF(Batra et al.,2016;Hansen et al.,2018),but not graft survival rate(McCall et al.,2003;Batra et al.,2016;Gao et al.,2019).In contrast,Hansen et al.(2018)concluded that fibrin thrombi were not only associated with reduced graft function six months posttransplantation but also with increased graft loss within the first year of *** the other hand,Batra et al.(2016)found no significant differences in the DGF rate or one-year graft function between recipients in diffuse and focal microthrombi *** date,however,the overall influence of donor kidney microthrombi and the degree of influence on prognosis remain controversial,necessitating further research.
