Comparison of visual effects of FS-LASIK for myopia centered on the coaxially sighted corneal light reflex or the line of sight

AIM：To compare visual quality after femtosecond laser in situ keratomileusis（FS-LASIK）,between the coaxially sighted corneal light reflex（CSCLR） group and conventional ablation line of sight（LOS） ***：In total,243 eyes（122 patients） were treated with centration on the CSCLR（visual axis） and 238 eyes（119 patients） treated with centration on the pupil center（LOS）.Postoperative outcomes [uncorrected visual acuity（UCVA）,best spectacle-corrected visual acuity（BSCVA）],safety index,efficacy index,refractive outcome,ablation center distance from the visual axis,corneal high-order aberrations,subjective discomfort glare and shadowing incidence rate,and contrast sensitivity at 1,3,and 6mo were measured and ***：The mean age was 27.77±7.1y in the CSCLR group and 26.03±7.70 y in the LOS ***,the manifest refraction spherical equivalent（MRSE） was-6.68±2.60 D in the CSCLR group and-6.65±2.68 D in the LOS *** postoperative UCVA,BSCVA,MRSE（-0.03±0.263 D in the CSCLR group,-0.05±0.265 D in the LOS group）,efficacy index（1.04,1.03）,and safety index（1.09,1.08） were not significantly different between the groups（all P〉0.05）.In total,3% lost one line and more of BSCVA in the CSCLR group,as 9% in the LOS group postoperatively（P〈0.05）.The ablation center deviation was 0.20±0.15 mm from the visual axis（Pentacam system default setting;range,0-0.75 mm） in the CSCLR group,and 0.43±0.22 mm（range,0-1.32 mm） in the LOS group（P〈0.0001）.Statistically significant greater augmentationof total corneal higher-order aberrations（0.15±0.10 μm and 0.20±0.12 μm respectively,P=0.03） and vertical and horizontal coma（P〈0.0001） were noted in the LOS *** discomfort glare and shadowing incidence rates were 8.59% and 17.5% in the CSCLR and LOS groups,respectively（P〈0.05）.The 1-month postoperative contrast sensitivity visual acuity in the CSCLR group was significantly higher than that in the LOS group on contrast（100%,25%,10%） with a dark background,but there

Corneal dystrophy is a common type of hereditary corneal diseases. It includes many types, which have varied pathology, histology and clinical manifestations. Recently, the examination techniques of ophthalmology and gene sequencing advance greatly, which do benefit to our understanding of these diseases. However, many aspects remain still unknown. And due to the poor knowledge of these diseases, the results of the treatments are not satisfactory. The purpose of this review was to summarize the clinical, histological and genetic characteristics of different types of corneal dystrophies.

Effects of intensive control of blood glucose and blood pressure on microvascular complications in patients with type Ⅱ diabetes mellitus

AIM: To evaluate the effects of intensive control of blood glucose and blood pressure on microvascular complications in patients with type Ⅱ diabetes by comparing the therapeutic effects of intensive and standard treatment in patients with type Ⅱ diabetes. METHODS: A total of 107 patients with type Ⅱ diabetes were randomly assigned into intensive and standard treatment groups. Patients in the intensive treatment group received preterax (perindopril/ indapamide) to control blood pressure, and gliclazide (diamicron) MR to control blood glucose. Patients in the standard treatment group received routine medications or placebo. Urinary microalbumin (UMA), urinary creatinine (UCR), the UMA/ UCR ratio, and visual acuity were monitored according to the study design of the ADVANCE trial. Direct ophthalmoscopy and seven-field stereoscopic retinal photography were used to examine the fundi at baseline,and repeated after 5 years of treatment. RESULTS: The characteristics of patients in both groups were well balanced at baseline. After 5 years of treatment, visual acuity was found to be decreased in the standard group (P=0.04), but remained stable in the intensive group. The severity of diabetic retinopathy had not progressed in patients in the intensive group, but had deteriorated in the standard group (P=0.0006). The UMA/UCR ratio was not obviously changed in patients in the intensive group, whereas it was significantly increased in the standard group (P=0.00). CONCLUSION: Intensive control of blood glucose and blood pressure can decrease the incidence or slow the progression of microvascular complications in patients with type Ⅱ diabetes, and maintain stable vision.

clinic study on silicone hydrogel contact lenses used as bandage contact lenses after LASEK surgery

AIM: To compare the clinical performance of two types of silicon hydrogel contact lenses used as bandage lenses after LASEK surgery. METHODS: A prospective, double-masked study was conducted on 42 eyes of 21 patients who received binocular LASEK surgeries. The interocular difference in spherical equivalent power was less than -1.50D. Patients were randomly assigned to wear Galyfilcon A (Lens A) bandage contact lens in one eye and Balafilcon A (Lens B) in the fellow eye after the surgery. The responses to a subjective questionnaire on comfort of wearing, corneal epithelial status, conjunctival hyperemia, limbal neovascularization, lens fitting and contact lens debris were assessed 1 and 5 days postoperatively. Corneal endothelium was assessed before and 5 days after the surgery upon bandage lens removal. RESULTS: There was no difference between the two groups in terms of conjunctival hyperemia, limbal neovascularization, contact lens fitting, corneal epithelial status, corneal endothelium cell density (CD) and endothelium cell size (CS) at any postoperative visit. Complaints of discomfort, including foreign body sensation, pain and intolerance were statistically more among Lens B wearers at any postoperative visit (P clinical performance in terms of corneal responses and lens fitting. However, Lens A showed a better performance in terms of comfort of wearing and deposit resistance.

Pediatric penetrating keratoplasty in shanghai： a retrospective multiple centre study from 2003 to 2007

Background With advances in pediatric keratoplasty over the last two decades, the success rate of surgery has increased remarkably. However, few epidemiological studies in this field have been performed in China. The present study investigated the indications and characteristics of pediatric penetrating keratoplasty in shanghai. Methods All records of pediatric keratoplasty performed on 156 eyes in 149 children （〈14 years） at four ophthalmic units in shanghai during a 5-year period （2003-2007） were used for this retrospective study. Patients were from the Eye, Ear, Nose ＆ Throat Hospital of Fudan University, shanghai No.10 People＇s Hospital, shanghai No.1 People＇s Hospital, and shanghai Peace ophthalmology Hospital; and included mostly school-age children （97 boys and 59 girls）. Results The median age at surgery was 9 years with an interval quartile range of 6-12 years. Scarring after keratitis （29.5%, 46/156） and traumatic corneal scar （19.2%, 30/156） were the most common indications. Best Corrected Snellen visual acuity （BCSVA） was reported only in 72% （112/156） of cases. Visual acuity outcomes were significantly better for keratoplasty 1 year postoperatively compared to preoperative visual acuity （P=0.001）. Of all patients, 13% （14/112） achieved a BCSVA of 6/18 or better. None of the indications was associated with a higher rate of failure compared to after 1 year follow-up. Conclusion We obtained valuable information on pediatric keratoplasty in shanghai. There was no significant difference in graft survival rate among the two indications. Vision outcomes after corneal transplantation in Chinese children were significantly better compared with pre-operation status. In conclusion, corneal graft survival can be achieved in childhood.

NITRIC OXIDE AND VEGF EXPRESSION IN ISCHEMIC-HYPOXIC RETINOPATHY

Objective To evaluate nitric oxide(NO)and vascular endothelial growth factor(VEGF)in vitreous humor and blood samples in patients with proliferative diabetic retinopathy(PDR)and in patients with branch retinal vein occlusion(BRVO).Methods NO concentrations were determined by using the Greiss reaction in plasma and vitreous humor *** levels were assayed by *** patients in the studies were divided into four groups:16 patients with PDR,5 patients with BRVO,11 patients with rhegmatogenous retinal detachment(RRD),and 10 patients with idiopathic macular hole(IMH).Results The vitreous fluid levels of NO were significantly higher in patients with PDR(15.2μmol/L,4.6-50.9μmol/L)than those in the other three groups(F=5.13,P=0.005).The concentrations of VEGF were significantly higher in patients with PDR and BRVO(1507.2 μg/mL,50.71-3722.0μg/ml;838.8μg/mL,159.6-3328.0μg/mL)than those in the other two groups(F=6.84,P=0.0008),but highest in PDR(T=3.92,P=0.001).There was no significant difference between NO and VEGF in serum in four *** was no correlation between concentrations of NO and VEGF in four groups whatever in vitreous or plasma(all P>0.05).Conclusion The results suggest that higher levels of NO and VEGF may be related to the angiogenesis in DR.

Predictors of anti-vascular endothelial growth factor treatment responses in macular edema following central vein occlusion

Retinal vein occlusion （RVO） ranged the second common cause of retinal vascular disease, consisting of central retinal vein occlusion （CRVO） and branch retinal vein occlusion （BRVO） Although the prevalence of CRVO was one-third of BRVO, it associated with worse prognosis both before and after the anti-vascular endothelial growth factor （VEGF） regimes came into clinic use. A recent pooled age- and sex-standardized analysis showed that the prevalence was 0.8 per 1 000 （CI： 0.61-0.99） for CRVO around the world. Similarly, in our country, the 10-year incidence of CRVO in Beijing （2001-2011） was 0.3±0.1 per 100 persons.
