Global distribution of Thermosteric Contribution to Sea Level Rising Trend

The sea level derived from TOPEX/Poseidon （T/P） altimetry data shows prominent long term trend and inter-annual variability. The global mean sea level rising rate during 1993-2003 was 2.9mm a^-1. The T/P sea level trend maps the geographical variability. In the Northern Hemisphere （15°-64°N）, the sea level rise is very fast at the mid-latitude （20°-40°N） but much slower at the high-latitude, for example, only 0.5 mm a^-1 in the latitude band 40°-50°N. In the Southern Hemisphere, the sea level shows high rising rate both in mid-latitude and high-latitude areas, for example, 5.1 mm a^-1 in the band 40°- 50°S. The global thermosteric sea level （TSL） derived from Ishii temperature data was rising during 1993-2003 at a rate of 1.2 mm a^-1 and accounted for more than 40% of the global T/P sea level rise. The contributions of the TSL distribution are not spatially uniform; for instance, the percentage is 67% for the Northern Hemisphere and only 29% for the Southern Hemisphere （15°-64°S） and the maximum thermosteric contribution appears in the Pacific Ocean, which contributes more than 60% of the global TSL. The sea level change trend in tropical ocean is mainly caused by the thermosteric effect, which is different from the case of seasonal variability in this area. The TSL variability dominates the T/P sea level rise in the North Atlantic, but it is small in other areas, and shows negative trend at the high-latitude area （40°-60°N, and 50°-60°S）. The global TSL during 1945-2003 showed obvious rising trend with the rate of about 0.3 mm a-l and striking inter-annual and decadal variability with period of 20 years. In the past 60 years, the Atlantic TSL was rising continuously and remarkably, contributing 38% to the global TSL rising. Is There Comparable morbidity in Pylorus-preserving and Pylorus-resecting Pancreaticoduodenectomy? A meta-analysis

Summary： Pancreaticoduodenectomy （Pd） is the most effective treatment for patients with pancreatic head or periampullary lesions. Two major strategies exist： pylorus-preserving pancreaticoduodenectomy （PPPd） and pylorus-resecting pancreaticoduodenectomy （PRPd）. However, it is yet unclear regarding the morbidity after PPPd and PRPd. This study analyzed the morbidity after PPPd and PRPd to de- termine the optimal surgical treatment of masses in the pancreatic head or periampullary region. A sys- tematic search of databases identifying randomized controlled trials （RCTs） from the Cochrane Library, Pubmed, EmBASE and Web of Science was performed. Outcome was compared by postoperative mor- bidity including overall morbidity, pancreatic fistulas, wound infections, postoperative bleeding, biliary leakage, ascites and delayed gastric emptying （dGE） rate between PPPd and PRPd. The dGE rate in the PRPd subgroups （conventional Pd [CPd] and subtotal stomach-preserving Pd [SSPPd], respec- tively） was also analyzed. The results showed that 9 RCTs including 722 participants were included for meta-analysis. Among these RCTs, 7 manuscripts described PRPd as CPd, and 2 manuscripts described PRPd as SSPPd. There were no significant differences in the overall morbidity, pancreatic fistulas, wound infections, postoperative bleeding, or biliary leakage between PPPd and PRPd. There was a lower rate of dGE with PRPd than that with PPPd （RR=2.15, P=0.03, 95% CI, 1.09-4.23）. Further subgroup analysis indicated a comparable dGE rate for the CPd but a lower dGE rate for the SSPPd group than the PPPd group. However, the result did not indicate any difference between CPd and SSPPd regarding the dGE rate （P=-0.92）. It is suggested that PPPd is comparable to PRPd in overall morbidity, pancreatic fistulas, wound infections, postoperative bleeding and biliary leakage. Cardiovascular risk factors for acute stroke: Risk profiles in the different subtypes of ischemic stroke

Timely diagnosis and control of cardiovascular risk factors is a priority objective for adequate primary and secondary prevention of acute stroke. Hypertension, atrial fibrillation and diabetes mellitus are the most common risk factors for acute cerebrovascular events, although novel risk factors, such as sleep-disordered breathing, inflammatory markers or carotid intima-media thickness have been identified. However, the cardiovascular risk factors profile differs according to the different subtypes of ischemic stroke. Atrial fibrillation and ischemic heart disease are more frequent in patients with cardioembolic infarction, hypertension and diabetes in patients with lacunar stroke, and vascular peripheral disease, hypertension, diabetes, previous transient ischemic attack and chronic obstructive pulmonary disease in patients with atherothrombotic infarction. This review aims to present updated data on risk factors for acute ischemic stroke as well as to describe the usefulness of new and emerging vascular risk factors in stroke patients.

目的探讨微格教学模式在创伤骨科行外固定术患者出院后健康教育中应用的效果评价。方法2013年1月至2015年1月在本科室行外固定手术患者64例，随机分为观察组和对照组各32例，对照组在出院前按常规方法进行护理及健康教育，观察组按微格教学模式实施相关护理及健康教育。观察两组患者外固定健康教育内容的掌握程度、手术后并发症情况、患肢功能活动情况及对护理工作的满意度。结果观察组患者对相关外固定知识的掌握程度，患肢功能活动情况，并发症发生情况，对护理工作的满意度，均优于对照组，两组比较差异有统计学意义。结论将微格教学模式应用于外固定术患者的健康教育中，能使患者较全面地掌握相关外固定健康教育知识，预防外固定术后并发症的发生及促进健康恢复有着积极的推动作用，从而提高了护理满意度，体现了以人为本的服务理念。

Background:COVId-19 vaccines that trigger a strong secretory antibody response in breast milk may achieve effective passive protection of vulnerable newborns and breastfed infants of immunized *** aim of this work was to investigate the presence of SARS-CoV-2 spike RBd-specific IgA and IgG antibodies in breast milk,5 and 9 weeks after vaccination with 3 doses of the protein subunit vaccine Abdala,compared to those found in breast milk from COVId-19-recovered women,collected at least 40 days after the ***:SARS-CoV-2 spike RBd-specific IgA and IgG antibodies were semi-quantified by indirect ELISA,using a homemade standard generated by pooling twenty breast milk samples with high absorbance values according to preliminary *** validity of the standard curves was proved following the European medicines Agency *** breast milk samples from 2 unvaccinated women who had not been infected with COVId-19 were included as negative *** neutralizing antibodies was assessed by a SARS-CoV-2 surrogate virus neutralization ***:High levels of anti-RBd IgA antibodies were detected in breast milk samples 9 weeks after vaccination and anti-RBd IgG antibodies rise from the fifth to the ninth *** the post-COVId-19 time that was evaluated,the IgG-type response was notably higher compared to both post-vaccination *** antibody titers were similar in breast milk from vaccinated and COVId-19 recovered ***:This is the first report about the immune response in breast milk after the administration of a COVId-19 protein subunit vaccine,which could provide analogous protection to that conferred by SARS-CoV-2 *** implies a potential passive immunity that breastfed infants receive from their mothers vaccinated with Abdala.

Inguinal Hernia Repair with Local Anesthesia in the Outpatient—10 Year Experience

Objective: To demonstrate the feasibility of inguinal hernia repair with local anesthesia in an out-patient regime, with safety, efficacy and short learning curve. methods: We prospectively evaluated 1186 patients undergoing inguinal hernia repair under local anesthesia on an outpatient basis between November 2004 and march 2014. Of the total number of hernias surgically treated in this period, 755 were operated on the right, 394 on the left and 37 bilateral. We used clinical, surgical and psychosocial criteria for inclusion in the procedure. The parameters for exclusion were complex, irreducible or recurrent hernia, obesity (BmI greater than 30 kg/m2), patient’s refusal and psychiatric disorder. All patients underwent elective surgery and were analyzed regarding surgical outcome, complications and hospital stay. Results: All operations were completed successfully. In no case there was a need to change the anesthetic method. Surgical time was similar to that conducted with other methods of anesthesia and there were no cases of adverse effects of local anesthetics. Intra-operative complications amounted to approximately 2.64%. There was no need for hospital admissions greater than 24 hours. Conclusion: The procedure is feasible and causes no perioperative significant pain, is safe, can be performed by residents under supervision, has satisfactory patient acceptance and complications similar to those observed in a conventional herniorrhaphy, allowing lower time and cost of hospitalization and faster access to treatment.
