Importance:The incidence of obstructive sleep apnea syndrome(OSAS)in children has increased year by year *** pressure research of OSAS children can help understand the occurrence of OSAS related *** detection and intervention of blood pressure changes in children with OSAS can reduce the incidence of cardiovascular disease in later ***:To investigate the differences in blood pressure among different groups of snoring children and different sleep ***:Habitually snoring children(snoring frequency of≥3 nights per week)aged3 to 11 years were recruited from Beijing Children's Hospital from 1 January 2017 to 30 June *** children underwent polysomnography,and their blood pressure was monitored and calculated by the pulse transit *** children were divided into those with primary snoring(PS),mild OSAS,and moderate to severe OSAS according to their obstructive apnea-hypopnea index(OAHI).Results:In total,140 children were ***-seven had PS,24 had mild OSAS,and 19 had moderate to severe *** were no differences in age,sex,or body mass index z-score among the *** significant differences were found in the OAHI,oxygen desaturation index 3%,respiratory arousal index,and lowest oxygen saturation among the three *** with moderate to severe OSAS had higher systolic and diastolic blood pressure than those with mild OSAS andPS(P<0.001).In all children,systolic and diastolic blood pressure was higher in the rapid eye movement(REM)sleep stage than in the non-REM sleep stage(P<0.05).Interpretation:Children with moderate to severe OSAS had higher blood pressure than those with PS and mild OSAS in all sleep *** pressure in the REM sleep stage was higher than that in other sleep stages in all groups of children.

Gastroesophageal reflux disease:A review of surgical decision making

Gastroesophageal reflux disease(GERD) is a very common disorder with increasing prevalence. It is estimated that up to 20%-25% of Americans experience symptoms of GERD weekly. Excessive reflux of acidic often with alkaline bile salt gastric and duodenal contents results in a multitude of symptoms for the patient including heartburn, regurgitation, cough, and dysphagia. There are also associated complications of GERD including erosive esophagitis, Barrett's esophagus, stricture and adenocarcinoma of the esophagus. While first line treatments for GERD involve mainly lifestyle and non-surgical therapies, surgical interventions have proven to be effective in appropriate circumstances. Anti-reflux operations are aimed at creating an effective barrier to reflux at the gastroesophageal junction and thus attempt to improve physiologic and mechanical issues that may be involved in the pathogenesis of GERD. The decision for surgical intervention in the treatment of GERD, moreover, requires an objective confirmation of the diagnosis. Confirmation is achieved using various preoperative evaluations including: ambulatory p H monitoring, esophageal manometry, upper endoscopy(esophagogastroduodenoscopy) and barium swallow. Upon confirmation of the diagnosis and with appropriate patient criteria met, an antireflux operation is a good alternative to prolonged medical therapy. Currently, minimally invasive gastroesophageal fundoplication is the gold standard for surgical intervention of GERD. Our review outlines the many factors that are involved in surgical decisionmaking. We will review the prominent features that reflect appropriate anti-reflux surgery and present suggestions that are pertinent to surgical practices, based on evidence-based studies.

Self-Perception and General Perception of the Safety Impact of Autonomous Vehicles on Pedestrians, Bicyclists, and People with ambulatory Disability

For autonomous vehicles (AVs) to receive general acceptance, society must have a positive perception about their safety impact on vulnerable road users. Using data from a statewide random-digit-dialing telephone survey of 1001 adults, this paper examines how New Jersey residents perceive the safety impact of AVs on pedestrians, bicyclists, and people with ambulatory disability. It uses a combination of confirmatory factor analysis and ordered probit models. Confirmatory factor analysis is used to create latent variables on socioeconomic status and built environment. Three ordered probit models are used to examine people’s perception of AV safety impact on each of the three population groups. The models also examine how frequent walkers, bicyclists, and people with ambulatory disability perceive their own safety as well as the safety of the other two groups. All three models examine the effect of familiarity with AV, gender, age, income, education, race, ethnicity, number of vehicles in household, political party affiliation, as well as built environment and socioeconomic status of the municipalities where the survey respondents live. The analysis showed that men, people with familiarity with the AV concept, Democrats, bicyclists, and people with high household income generally have a positive perception about the safety impact of AVs. While frequent walkers are ambivalent about their own safety as pedestrians, bicyclists have a positive perception about their own safety and the safety of pedestrians, whereas people with ambulatory disability have a strong negative perception about their own safety. The models did not show statistically significant effects of socioeconomic status or built environment of municipalities on AV safety perception.

背景手术后恶心、呕吐（PONV）是门诊手术常见的并发症。本试验目的是观察透皮东莨菪碱贴剂（TDS）联合静脉注射昂丹司琼（OND）对门诊患者PONV的疗效是否优于单独应用昂丹司琼。方法此项随机双盲多中心的临床试验纳入了620例接受门诊腹腔镜或隆胸手术的PONV高危女性患者，进入手术室前2小时分别给患者贴上TDS贴剂或外观相同的空白贴剂。在全麻诱导前2—5分钟给所有患者静脉注射OND4mg。手术后24~48小时评估止吐是否完善（彻底止吐标准为无呕吐或干呕，且没有追加止吐药）。同时记录发生呕吐、干呕、恶心以及追加止吐药的患者比例，记录手术后首次发生PONV的时间及离院时间，记录呕吐、干呕、恶心的发作次数与严重程度及患者对于止吐治疗的满意度。结果TDS＋OND组与OND组相比，手术后24小时恶心、呕吐的发生率显著降低，但手术后48小时两者差异无显著性。TDS＋OND组没有呕吐、干呕且未追加过止吐药的患者占48％，而OND组占39％（P〈0．02）。TDS＋OND组的彻底止吐（无恶心、呕吐或干呕，且未追加止吐药）率显著高于OND组（分别为35％和25％，P〈0．01）。TDS＋OND组手术后首次发生恶心、呕吐、干呕及追加止吐药的时间明显晚于OND组（P〈0．05）。前者不良反应的总发生率也低于后者（分别为36．7％和49％，P〈0．01）。结论与单独应用OND相比。TDS＋OND能更大程度地减少PONV，主要是通过减少不良反应而获得。

ambulatory Radio Iodine Therapy in the Management of Hyperthyroidism in Africa: African Systematic Review and Perspectives in Burkina Faso

Introduction: In hyperthyroidism, selective irradiation of the thyroid gland with radioactive iodine is a radical treatment and an alternative to surgery. The aim of this review is to assess the medium-term efficacy of outpatient treatment of hyperthyroidism with iodine-131 in Africa. Methods: We identified the studies carried out in Africa on outpatient radiation therapy between 2016 and 2020. For each article included, we noted the country concerned and the year of publication, the numbers studied, the socio-demographic characteristics of the patients, the indications for radio iodine therapy, the dose administered, the results of the hormonal dosage 6 months after radiation. Results: 13 retrospective studies were included to constitute a total population of 925 patients. The average age was 40.77 years, the sex ratio of 1/5.4 with a clear female predominance. The 3 main etiologies of hyperthyroidism justifying outpatient radio iodine therapy were Graves’ disease (55.89%), toxic multinodular goiter (22.70%) and toxic adenoma (21.40%). The average dose of iodine 131 administered per course is 13.7 mCi. No short-and medium-term complications were reported. The radio iodine therapy was effective in 86.08% (n = 796) of the patients with extremes of 72% and 100%. Conclusion: Radio iodine therapy is effective in Africa. It is simple, inexpensive on an outpatient basis and well tolerated. The introduction of outpatient radio iodine therapy could improve the management of patients with hyperthyroidism in Burkina Faso.

Evaluating Health Planning Issues at the Community Level

This study was a brief review of issues related to health planning in the metropolitan area of Syracuse, New York. It suggested that population demographics and health care utilization can have an impact on the use of services at the community level. The study suggested that the utilization of services can be associated with patient demographics. In this study, younger populations were associated with lower use of hospital inpatient care. The data have also suggested that larger numbers of younger populations are not necessarily related to opportunities for expanding local health care providers. Younger populations can be associated with lower per capita use of services. The data demonstrated that there were substantial reductions in hospital discharges for adult medicine. Total discharges declined by 3999 patients between the seven months in 2019 and 2023. The data also demonstrated that there were notable reductions in hospital discharges for adult surgery.

Consensus and inconsistency between different consensus documents on renal denervation worldwide:the way forward

Given the unsatisfactory hypertension control rates and high rates of non-adherence to antihypertensive medications worldwide,device therapy which can safely provide durable blood pressure-lowering effects can fulfill the unmet need.A series of second-generation randomized sham-controlled renal denervation(RDN)trials have demonstrated the efficacy and safety of RDN in a wide range of hypertensive *** four representative consensus documents on RDN(from the Chinese Taiwan Hypertension Society and Taiwan Society of Cardiology[THS/TSOC 2019],Asia Renal Denervation Consortium 2019,European Society of Hypertension[ESH 2021],and Society for Cardiovascular Angiography&Intervention and National Kidney Foundation[SCAI/NKF 2021])consistently recommend RDN as an alternative or complementary treatment strategy for patients with uncontrolled *** addition,both documents from Asia further recommend that RDN can be considered as an initial treatment strategy for drug-naïve hypertensive *** is still inconsistency regarding whether ambulatory blood pressure monitoring should be used routinely both before and after RDN,and whether patients with a secondary cause of hypertension could be treated with RDN if their blood pressure remains uncontrolled after definitive treatment(treatment-resistant secondary hypertension).The THS/TSOC consensus provides acronyms to summarize key aspects of patient selection(RDNi2)and pre-RDN assessments(RAS).The ESH and SCAI/NKF documents recommend establishing structured pathways for clinical practice and issues regarding *** documents identify knowledge gaps in RDN,from identifying predictors of super-responders to demonstrating effects on cardiovascular *** gaps should be urgently filled to facilitate the wider application of this device therapy for patients with hypertension.

Objective: To construct gene co-occurrence network of hypertension and liver-fire hyperactivity syndrome, to investigate the biological basis of hypertension and liver-fire hyperactivity syndrome and the characteristics of the molecular network from gene *** and Methods: Applying Gen CLip 2.0 online platform to retrieve the up-to-date literature referred to essential hypertension from PubMed database, cluster the abnormal expression of essential hypertension-related genes and analyze their function, combining Kyoto encyclopedia of genes and genomes-pathway analysis to investigate the closely related genes and the signaling *** on the genes closely related to hypertension, standard diagnostic symptoms of liver-fire hyperactivity were used as keywords to conduct hypertension liver-fire hyperactivity-related gene cluster ***: The top 1000 genes of essential hypertension were retrieved from GenCLip 2.0 online platform, which mainly clustered in the regulation of ambulatory blood pressure, regulation of renin-angiotensin-aldosterone system(RAAS), and sympathetic nervous system activity, as well as endothelial dysfunction; the closely related genes of hypertension with liver-fire hyperactivity are related to RAAS, gene REN, angiotensin converting enzyme, angiotensinogen, and cytochrome P450 family ***: A combination of literature mining and data mining can construct the gene network of hypertension and the syndrome-related genes, which provides a new method for the study of the biological basis of hypertension from the genetic level.
