Objective:To determine post-discharge mortality and associated factors of the first-wave multicenter Turkish Thoracic Society(TTD)-TURCOVID ***:In this retrospective cohort study,we analyzed the data of 18 of 26 centers included in the first TTD-TURCOVID study,and 1112 cases diagnosed with COVID-19 between 11 March and 31 July 2020 participated in the *** causes of death after COVID-19 discharge were ***:The mean age of the patients was(51.07±16.93)years,with 57.6%male *** the cohort group,89.1%of COVID-19 treatment locations were hospital wards,3.6%were intensive care units(ICUs),and 7.2%were community *** the longterm follow-up,the in-hospital mortality rate was 3.6%(95%CI 2.6-4.8),the post-discharge mortality rate was 2.8%(95%CI 1.9-3.9),and the total mortality was 6.3%(95%CI 5.0-7.8).After discharge,63.3%of mortality overall occurred during the first six *** rates in post-discharge follow-ups were 12.7%(95%CI 8.0-30.6)in cancer patients,10.8%(95%CI 6.3-22.9)in chronic obstructive pulmonary disease patients,11.1%(95%CI 4.4-22.7)in heart failure patients,7.8(95%CI 3.8-14.3)in atherosclerotic heart disease patients,and 2.3%(95%CI 0.8-5.6)in diabetes mellitus *** smokers/ex-smokers,the all-mortality rates were higher than in ***:This multicenter study showed that patients over 65 years of age,males,former/active smoker,ICU stay,lung,heart disease,and malignancy should be followed up for at least the first six months after discharge due to COVID-19.

In this study, exergy dynamic and advanced exergy analyses are applied to theturbojet engine to assess its mexogenous, endogenous, exogenous, avoidable and unavoidableexergies under the environment conditions of 15 C temperature and 1 bar pressure. Themaximum exergy point in the turbojet engine is found for the combustor in which C11H23(Jet-A1) fuel is combusted with air, while the minimum one is determined for the aircompressor head where the free air enters. The combustion chamber has the maximum fuel,product and irreversibility rates and the air compressor has the minimum fuel and product ex-ergy values, while the minimum irreversibility is found for the turbine. Maximum improvementpotential rate is found for the combustion chamber (5141.27 kW), while minimum rate is deter-mined for the turbine of system (6.95 kW). Also, the turbine component has the highest exergyefficiency (97.20%) due to its expansion process, while combustion chamber component hasthe lowest exergy efficiency (55.39%) due to low efficient combustion process of the ***, the mexogenous exergy destructions from maximum to minimum are found for the combustion chamber, air compressor and gas turbine units, respectively. Considering exergydynamic analysis, the mexogenous exergy destruction rates of the combustion chamber, aircompressor and gas turbine are found as 184.4 kW, 103.97 kW and 9.99 kW, *** all results, the combustion chamber is the primer component to be handled for bet-ter efficiency and improvement.

In the present study, safflower biodiesel (safflower oil methyl ester) was produced through transesterification. Biodiesel fuel obtained from safflower was blended with diesel fuel in ratios of 2% (B2) and 20% (B20). Kinematic viscosity, density, water content, heating value, flash point, clouding point, pour point, ash content, copper strip corrosion and CFPP (cold filter plug point) tests were performed on B2, B20, B100 fuel blends and diesel fuel. A four stroke, single cylinder, water cooled diesel engine with a direct injection fuel system was operated with B2, B20, B100 and diesel fuels at partial load for 100 hours and samples were taken from the engine lubricating oil at certain hours. The wear elements iron, aluminum, lead, copper and chromium contents of the samples taken from the engine lubricating oil were examined.

Signalized intersections are widely used in today"s cities where the traffic flows from various directions and the pedestrians have right to pass one by one. In this study, two of the intersections in Konya were investigated, Kule and Nal?ac?–Sille signalized intersections. The intersections are important in terms of urban traffic. New cycle times were proposed with the aim to minimize delays and to increase the capacities and level of services in these intersections. The intersections were examined using Sidra Intersection 5.1 software based on the Australian methods. Also, signalization calculations can be made according to the American HCM (Highway Capacity Manual) methods by using the same software. The intersection analyses were performed using both methods based on the current cycle times and optimum cycle times proposed by the methods. The obtained analytical results were compared and some solutions were suggested. After analysis, the obtained values using the Australian and American methods were observed to be close. Additionally, the decrease in the delays and the increase in the capacities at the intersections were generally observed as a result of the proposed cycle times.

Digital single-operator video cholangioscopy improves endoscopic management in patients with primary sclerosing cholangitis-a retrospective observational study

BACKGROUND Patients with primary sclerosing cholangitis(PSC)are at a high risk of developing cholestatic liver disease and biliary cancer,and endoscopy is crucial for the complex management of these *** To clarify the utility of recently introduced digital single-operator video cholangioscopy(SOVC)for the endoscopic management of PSC *** In this observational study,all patients with a history of PSC and in whom digital SOVC(using the SpyGlass DS System)was performed between 2015 and 2019 were included and retrospectively *** were performed at a tertiary referral centre in *** total,46 SOVCs performed in 38 patients with a history of PSC were *** primary endpoint was the evaluation of dominant biliary strictures using digital SOVC,and the secondary endpoints were the performance of selective guidewire passage across biliary strictures and the diagnosis and treatment of biliary stone disease in PSC *** The 22 of 38 patients had a dominant biliary stricture(57.9%).In 4 of these 22 patients,a cholangiocellular carcinoma was diagnosed within the stricture(18.2%).Diagnostic evaluation of dominant biliary strictures using optical signs showed a sensitivity of 75%and a specificity of 94.4%to detect malignant strictures,whereas SOVC-guided biopsies to gain tissue for histopathological analysis showed a sensitivity of 50%and a specificity of 100%.In 13%of examinations,SOVC was helpful for guidewire passage across biliary strictures that could not be passed by conventional methods(technical success rate 100%).Biliary stone disease was observed in 17.4%of examinations;of these,in 37.5%of examinations,biliary stones could only be visualized by SOVC and not by standard *** stone treatment was successful in all cases(100%);25%required SOVC-assisted electrohydraulic ***,such as postinterventional cholangitis and pancreatitis,occurred in 13%of examinations;however,no procedur

This study has been investigated some pipeline lines after collapsing process of Union of Soviet Socialist Republics from Azerbaijan to Europe and another alternative ways from Russia. As we know Russia is a big power as from economic, defensive, and political aspects, as that"s in ruler position in the region. So, USA from another side of the world as a super power of the world could make dimension of those pipelines from transit of Turkey. That"s why new Nabucco project has been also cancelled by Russia"s power. This article has been studied newly established project of Trans Anatolian Natural Gas Line Project (TANAP) from Azerbaijan"s oil and gas department, Ministry"s State Petroleum Company"s (SOCAR) project has not been cancelled by Turkish Petroleum Company (TPAO). So TANAP"s advantages and disadvantages have been researched in this article.

Objective:The role of lymph node dissection(LND)is still controversial in patients with renal cell carcinoma undergoing *** aimed to provide a comprehensive review of the literature about the effect of LND on survival,prognosis,surgical outcomes,as well as patient selection and available LND ***:Recent literature(from January 2011 to December 2021)was assessed through PubMed and MEDLINE databases.A narrative review of most relevant articles was ***:The frequencies in which LNDs are being carried out are decreasing due to an increase in minimally invasive and nephron sparing ***,randomized clinical trials and meta-analyses failed to show any survival advantage of LND versus no ***,retrospective studies suggest a survival benefit of LND in high-risk patients(bulky tumors,T3-4 stage,and cN1 patients).Moreover,extended LND might provide important staging information,which could be of interest for adjuvant treatment ***:No level 1 evidence of any survival advantage deriving from LND is currently available in ***,the role of LND is limited to staging ***,low grade evidence suggests a possible role of LND in high-risk *** clinical trials are warranted to corroborate these findings.

Comparison of Biopsy Results and Imaging Methods,in the Diagnosis of Breast Cancer

Purpose: In this study, we aimed at comparing the sensitivity and selectivity rates of Ultrasonography (USG), Mamography (MG) and Magnetic Resonance (MR), based on the biopsy results of breast mass lesions. Materials and methods: Between January 2009 and December 2010 in Konya Training and Research Hospital the biopsy results and reports of imaging tecniques such as USG, MG and MR, were obtained from the hospital automation. The sensitivity and selectivity of the USG, MG and MR were calculated. Results: The avarage age of the 112 patients included in this study is 49 ± 10 (23 - 71). 27 (24%) of the patients were found to have breast cancer after the histopathologic examination. USG was used with the 95 (94%) of the patients and 17 (18%) of the patients were diagnosed to have cancer. MG was used with the 75 (67%) of the patients and 15 (20%) of the patients were diagnosed to have cancer, and MR was used with the 112 (100%) of the patients and 25 (22%) of the patients were diagnosed to have cancer. In the pathologic diagnosis of cancer, the sensitivity of USG was found 85%, the sensitivity of MG was found 89%, and the sensitivity of MR was found 92%. The selectivity of these tecniques were 58%, 87% and 57% respectively. Conclusion: MR and MG have similar sensitivity rates in the diagnosis of breast cancer. Breast MR is an appropriate imaging tecnique that can be used in suitable indications in addition to USG and MG.
