Background： Appropriate expression and regulation of the transcriptome, which mainly comprise ofmRNAs and lncRNAs, are important for all biological and cellular processes including the physiological activities of bone microvascular endothelial cells （BMECs）. Through an intricate intraeellular signaling systems, the transcriptome regulates the pharmacological response of the cells. Although studies have elucidated the impact of glucocorticoids （GCs） cell-specific gene expression signatures, it remains necessary to comprehensively characterize the impact of lncRNAs to transcriptional changes. Methods： BMECs were divided into two groups. One was treated with GCs and the other left untreated as a paired control. Differential expression was analyzed with GeneSpring software V12.0 （Agilent, santa clara, CA, USA） and hierarchical clustering was conducted using Cluster 3,0 software. The Gene Ontology （GO） analysis was performed with Molecular Annotation System provided by CapitalBio Corporation. Results： Our results highlight the involvement of genes implicated in development, differentiation and apoptosis following GC stimulation. Elucidation of differential gene expression emphasizes the importance of regulatory gene networks induced by GCs. We identified 73 up-regulated and 166 down-regulated long noncoding RNAs, the expression of 107 of which significantly correlated with 172 mRNAs induced by hydrocortisone. Conclusions： Transcriptome analysis of BMECs from human samples was performed to identify specific gene networks induced by GCs. Our results identified complex RNA crosstalk underlying the pathogenesis of steroid-induced necrosis of femoral head.

Introduction: The Ho:YAG-Laser is categorized as a potentially dangerous lithotripsy device (DIN: Class 4) for perforation which is mainly caused by the photonic energy the laser emits. Long time complications like ureteral strictures seem to be directed by thermal and mechanical injury. In this study different energy settings a) are being investigated, a DJ (double J stent) is placed beside the laser to simulate a therapy of a forgotten stent with reduction of the lumen b) due to the volume exploitation of the DJ, and direct contact between the laser fiber and the DJ in the ureter c) is simulated during laser exposure. Materials and Methods: We used the Ho:YAG-laser (Vera PulseTM, Coherent, santa clara USA) with a 365 μm diameter laser fiber. The settings of the laser were 0.6 J and 1 J pulse energy with a frequency of 5 Hz. The experimental setup was closely aligned with the clinical situation. The tip of the thermometer was attached inside the catheter through a puncture. The laser fiber was guided by means of a rigid URS video device (11.5 Ch). We had four different settings for a), b) and c) during the measurement: 1) Distance of 0.5 cm between the laser and the thermometer;without irrigation, 2) Distance of 0.5 cm between the laser and the thermometer;with irrigation, 3) Distance of 1 cm between the laser and the thermometer;without irrigation, 4) Distance of 1 cm between the laser and the thermometer;with irrigation. Results: The temperature in an empty ureter rises approximately by 5°C, when the laser energy is increased from 0.6 J to 1 J. When a DJ is inserted in the artificial ureter there is surprisingly almost no difference in the maximum temperature between the lower energy level (0.6 J) and the high energy level (1 J). However the time needed to reach the maximum temperature is noticibly less when using high energy levels. Background:Studies have suggested that use of prolonged dual antiplatelet therapy (DAPT) following new generation drug-eluting stent implantation may increase costs and potential bleeding *** study aimed to investigate the association of DAPT status with clinical safety in patients undergoing everolimus-eluting stent (EES) implantation in the SEEDS study (A Registry to Evaluate Safety and Effectiveness of Everolimus Drug-eluting Stent for Coronary Revascularization) at 2-year ***:The SEEDS study is a prospective,multicenter study,where patients (n =1900) with small vessel,long lesion,or multi-vessel diseases underwent EES *** DAPT status was collected at baseline,6-month,1-and *** interruption was defined as any interruption of aspirin and/or clopidogrel more than 14 *** net adverse clinical events (NACE,a composite endpoint of all-cause death,all myocardial infarction (MI),stroke,definite/probable stent thrombosis (ST),and major bleeding (Bleeding Academic Research Consortium Ⅱ-Ⅴ)) were investigated according to the DAPT status at 2-year ***:DAPT was used in 97.8％ of patients at 6 months,69.5％ at 12 months and 35.4％ at 2 *** was observed that the incidence of NACE was low (8.1％) at 2 years follow-up,especially its components of all-cause death (0.9％),stroke (1.1％),and definite/probable ST (0.7％).DAPT was not an independent predictor of composite endpoint of all-cause death/MI/stroke (hazard ratio [HR]:0.693,95％confidence interval [CI]:0.096-4.980,P =0.715) and NACE (HR:1.041,95％ CI:0.145-7.454,P =0.968).Of 73 patients who had DAPT interruption,no patient had ST at 12-month,and only 1 patient experienced ST between 1-and 2-year (1.4％).There was a high frequency of major bleeding events (53/65,82.5％) occurred in patients receiving DAPT ***:Prolonged DAPT use was not associated with improved clinical *** study emphasized that duration of DAPT needs to be shortened in Chine

AIM: To investigate physicians' knowledge including chronic hepatitis B(CHB) diagnosis, screening, and management in various stages of their ***: A voluntary 20-question survey was administered in santa clara County, CA where Asian and Pacific Islanders(API) account for a third of the population. Among the 219 physician participants,there were 63 interns, 60 second-year residents, 26 chief residents and 70 attending physicians. The survey asked questions regarding respondents' demographics,general hepatitis B virus knowledge questions(i.e.,transmission, prevalence, diagnostic testing, prevention,and treatment options), as well as, self-reported practice behavior and confidence in ***: Knowledge about screening and managing patients with CHB was poor: only 24% identified the correct tests to screen for CHB, 13% knew the next steps for patients testing positive for CHB, 18% knew the high prevalence rate among API, and 31% knew how to screen for liver cancer. Wald chi-square analysis determined the effect of training level on knowledge; in all cases except for knowledge of liver cancer screening(p = 0.0032), knowledge did not significantly increase with length in residency training or completion of ***: Even in a high-risk region, both medical school and residency training have not adequatelyprepared physicians in the screening and management of CHB.
