Introduction: Venous thromboembolism disease (VTE) is a common complication of hospitalization. Objective: To assess the practice of thromboprophylaxis in hospitalized patients in the department of medicine. Patients and Methods: It is an observational cross-sectional study conducted from 01 February to 31 March 2019 in the medical departments of the saint jean de dieu hospital of Thies. All the patients hospitalized with at least 18 years of age were included. The thromboembolic risk was assessed with the Padua score and thromboprophylaxis?assessed according to the ACCP 2012 recommendations. Results: One hundred and forty four patients were included. Seventy-one (49.3%) were in the high thromboembolic risk category. The most frequently reported risk factors were mobility impairment (n = 72;50%), age of 70 years or above (n = 35;24.3%), heart failure (n = 23;16%), acute infections (n = 14;9.7%) and cancers (n = 13;9.02%). The prevention of venous thromboembolism was practiced in 25 of the 71 patients with high thromboembolic risk or a rate of 35.2%. Thromboprophylaxis was exclusively drug-based with Enoxaparin. Conclusion: The practice of prophylaxis for VTE was low in the medical departments. Efforts still need to be made to reduce the high rate of thromboembolic complications associated with hospitalization.

Introduction: Elderly people are considered fragile and at greater risk of having malignant gastrointestinal tumors. The objective of this work was to report the reasons for performing gastrointestinal endoscopy and the lesions found during the endoscopy of this target population in Parakou. Patients and Study Methods: This was a descriptive and cross-sectional study with a retrospective collection of data from January 2016 to december 2017, then from January 2020 to december 2021. It took place in the Regional Teaching hospital of Borgou-Alibori in Parakou and in the private gastrointestinal endoscopy center of Parakou (Northern Gastrointestinal Exploration Center). All patients aged at least 60 years who had undergone an upper gastrointestinal endoscopy during the study period were included. The variables studied were: the sex, age, indications for the examination, endoscopic lesions and data from the anatomo-pathological examination. Results: In total, out of 1540 upper gastrointestinal endoscopies performed during the study period, 249 (16.17%) involved patients aged 60 years and over. The sex ratio was 1.26. The main indication for the examination was epigastric pain (123 cases, i.e. 49.40%) followed by vomiting (53 cases, i.e. 21.29%). In terms of lesions, non-tumorous gastropathy came first in the stomach (206 cases, i.e. 82.73%) while esophageal lesions were dominated by esophageal candidiasis and cardial incompetence (39 cases, i.e. 15.66% in each of the two situations). In the duodenum, ulcer was noted in 30 patients (12.05%). In 38 patients, 12 (31.58%) were tested positive for Helicobacter pylori infection. Cancers of the gastrointestinal tract were confirmed in 11 patients (4.42%). Conclusion: Upper gastrointestinal endoscopy remains an excellent examination for the exploration of the upper gastrointestinal tract. Background: Breast cancer mortality remains high in the majority of developing countries. The Ministry of Health has established two population-based cancer registries in Benin: the first one in Cotonou in 2014 and the second one in Parakou in 2017. However, there is a scarcity of data on breast cancer survival and prognosis in Benin Republic. Objective: This study sought to investigate epidemiological, diagnostic, and survival aspects of breast cancer in Parakou, based on data from its population-based cancer registry from 2017 to 2021. Method: For descriptive and analytical purposes, we used a retrospective cohort design. From January 24, 2022 to August 31, 2022, data were collected in all health facilities covered by the Parakou population-based cancer registry using an individual questionnaire. Survival and prognosis analysis were performed using KAPLAN MEIER method and David COX proportional hazard model respectively. Result: A total of 81 patients have been included in this study. The incidence rate of breast cancer in Parakou was 17.5 per 100,000 person-years with a mortality rate of 2.76 per 100,000 person-years. The median age at diagnosis was 44.50 years with extremes ranging from 19 to 76 years and a predominance of 40 - 50 years age group. The median survival time was estimated at 30 months with an overall 5-year survival of 47%. Young age at diagnosis (p-value = 0.002) and advanced stage at diagnosis (p-value = 0.000) had a negative impact on survival in women. The combination of surgery and chemotherapy improved survival (p-value = 0.018). Conclusion: Breast cancer is still a public health issue in Parakou. It comes out mandatory that resources be made available to make screening, early diagnosis and appropriate treatment of breast cancer affordable.

dear editor,Ectopic pregnancy is a medico-surgical emergency requiring prompt diagnostic and therapeutic *** is defined as the implantation of the embryo outside its normal site within the endometrial cavity.1 The fallopian tube is the most common site of ectopic implantations(96%),followed by the ovary(1%),caesarean section scar(1%–3%),cervix(1%)and viscera of the abdominal cavity(1%).2 Among tubal locations,the interstitium is the least described implantation site(2.6%),whereas the ampulla,isthmus and fimbriated infundibulum have the following respective frequencies:73.3%,12.5%and 11.6%.3 Heterotopic pregnancies are extremely rare cases.

Purpose: To describe the epidemiological, clinical and management characteristics of acute coronary syndromes (ACS) in Thies, Senegal. Methods: This was a prospective study that included consecutively from October 1st, 2018 to March 03rd, 2019, patients aged 18 years or older admitted for an ACS at the DIABCARMET department of the saint jean de dieu hospital in Thies. Results: Of the 516 patients admitted, 29 had ACS, the prevalence was 5.62%. The average age of the patients was 64.2 ± 11.6 years. Nineteen patients (65.5%) were male and 10 (34.5%) were female, the sex ratio was 1.91. The number of direct admissions was 18 (62%) versus 11 patients (38%) referred. The mode of transport was a private vehicle in 19 patients (65.5%), an ambulance in 7 patients (24.1%) and public transport in 3 patients (10.3%). STEMI (ST segment elevation myocardial infarction) was the most common presentation observed in 19 patients (65.5%). Fifteen patients (51.7%) were admitted before the 12th hour. Of the 19 patients with STEMI, 11 (57.9%) had arrived in hospital before the 12th hour and 10 patients had streptokinase thrombolysis. No patients received primary PCI. Ten patients (34.5%) had heart failure and five patients (17.2%) died during hospitalization. Conclusion: delayed consultation and high mortality characterize ACS in Thies. Prevention must be the rule.

Background: Surgical site infections (SSIs) are considered as result of the healthcare quality in hospitals. Objective: to study SSI at saint jean de dieu hospital tanguieta (SJDHT), prior to the implementation of a permanent monitoring system. Method: transversal, and descriptive study with prospective data collection was performed from 1 July to 31 janvier 2017 in the department of general surgery of SJDHT. The hospital lacks in a microbiology unit. All patients who underwent surgery during this period were included and the monitoring lasted one month. SSIs diagnostic was carried out according to WHO criteria as described in the Practical Guide for the Prevention of Nosocomial Infections published in 2002. Statistical tests (χ-square and Student’s t-test) were applied and p 0.05 were statistically significant. Results: Of 343 patients recorded, 105 (30.6%) had SSI. Their age averaged 40.3 years and the sex-ratio (men/women) was 2.8. The emergency surgery resulted in a 50.0% rate of SSI (p = 0.00). The SSI rate for clean and clean-contaminated surgery was 6.3% against 94.6% for infected surgery (p = 0.00). The SSI rates were 100% and 66.7% for NNISS = 2 and NNISS = 1 (p = 0.00), respectively. Superficial SSI rate was 13.3%, while deep SSI and organ/space SSI were 46.7% and 40%, respectively. The hospital stay of patients with SSI was three times longer than the length of patients without SSI (p = 0.00). Conclusion: SSIs are real burden at SJDHT. Appropriate measures must be adopted to reduce its prevalence.

dear Sir,I n their article, ＂A comparison between monocanalicular and pushed monocanalicular silicone intubation in the treatment of congenital nasolacrimal duct obstruction＂, Andalib et al present the results of a prospective study on congenital nasolacrimal duct obstruction treated either with a Monoka stent （using a Crawford hook for nasal retrieval） or a Masterka pushed technique for insertion of the Masterka stent. The success rates were respectively 90% and 50%.

Epidemiology of Open Limb Fractures in a Country with Low-Income

Introduction: Open limb fractures are frequent and severe. Very often they result from high velocity traffic accidents. The aim of this study was to identify the epidemiologic aspects of open limb fractures in order to improve prevention and care. Material and Methods: It was a retrospective study carried out in the department of Orthopaedic and traumatology Surgery of Afagnan hospital. It concerned the files of patients admitted from January 1st to december 31st 2016. Patients of 15 years old and older who presented open limb fractures were included in the study. The study variables were: age, sex, occupation, trauma aetiology, injury mechanism, location and type of lesions. An X-ray (antero-posterior and lateral view) of the injured limb segment was done in all patients. Results: On 87 patients, 148 cases of fractures were collected. The mean age was 38.08 years with extremes of 15 and 82. There were 23 women (26.4%) and 64 men (73.6%). The most common etiology was traffic accidents (73.5%). Motorcycle accidents (58.3%);accidents involving motorcycle and car (19.4%). Accidents at work represented (16.1%). Lower limbs were more affected (77.1%). Open fractures classified Gustillo-Anderson type III were predominant with type IIIa (51.0%), IIIb (32.6%) and IIIc (16.3%). Conclusion: Open limb fractures are severe. They are mainly due to the increase of high-velocity traffic accidents. This explains the predominance of the young male population. Road safety and all actors’ behaviour change are essential for the reduction of traffic accident morbidity.
