Trimethylamine N-oxide generation process was influenced by the proportion and source of macronutrients in the diet

Trimethylamine N-oxide(TMAO)is a risk factor of various chronic diseases,which was produced by metabolism from precursors to trimethylamine(TMA)in gut and the oxidation from TMA in liver.The TMA generation was influenced by diet,mainly due to the rich TMAO precursors in diet.However,it was still unclear that the effects of different proportion and source of macronutrients in different dietary pattern on the production process of TMAO.Here,the generation of TMA from precursors and TMAO from TMA was determined after single oral choline chloride and intraperitoneal injection TMA,respectively,in mice fed with carbohydrates,proteins and fats in different proportion and sources.The results suggested that the generation of TMAO was increased by low non-meat protein and high fat via enhancing the production of TMAO from TMA,and decreased by plant protein and refined sugar via reducing TMA production from precursors in gut and TMAO transformation from TMA in liver.The high fat and high sugar diets accelerating the development of atherosclerosis did not increase the production of TMAO,the risk factor for atherosclerosis,which indicated that the dietary compositions rather than the elevated TMAO level might be a more key risk factor for atherosclerosis.

Relationship between nutritional therapy and beneficial bacteria ratio in severe disease

Objective:To evaluate relationship between changes in the beneficial bacteria in intensive care unit(ICU)patients and nutritional therapy type.Methods:Ten patients aged≥18 years admitted to the ICU between January and December 2020,were included.Good enteral nutrition was defined as early achievement of target calorie intake through enteral feeding.The ratio of beneficial bacteria at the first and second bowel movements after each patient’s admission was calculated and the patients were classified into the increase or decrease group.Among all patients,five each were in the increase and decrease groups.We investigated patient background,changes in sequential organ failure assessment(SOFA)and acute physiology and chronic health evaluation(APACHE)Ⅱscores,nutritional doses or methods,and clinical outcomes.Results:No relationship was found between changes in the ratio of beneficial bacteria and changes in SOFA/APACHEⅡscores at the time of admission.The rate of good enteral nutrition was significantly higher in the increase group than in the decrease group(4/5 vs.0/5,P=0.01).Conclusions:An increase in beneficial bacteria may be significantly related to the early establishment of enteral nutrition.In the future,accumulating cases may make it possible to establish a new nutritional strategy for critically ill patients from an intestinal microbiota perspective.

Fucoxanthin promotes the chain lengthening reaction of polyunsaturated fatty acids in vivo by enhancing FADS2 enzyme activity

Alpha-linolenic acid (ALA) is capable of synthesizing EPA and DHA in vivo， but the conversion rate is hard to meet the body's need for rapid DHA supplementation. Studies have shown that fucoxanthin can increase DHA content in the liver of mice， but the mechanism is not clear. Moreover， the effect of fucoxanthin and its metabolite fucoxanthinol on the chain lengthening reaction of n-6 polyunsaturated fatty acids (PUFAs) is little studied. In this study， the effects of fucoxanthin and fucoxanthinol on the chain elongation of n-3 and n-6 series PUFAs were investigated in mice. Results showed that fucoxanthin and fucoxanthinol significantly increased the conversion of n-3 series ALA to DHA and n-6 series LA to AA in the serum， liver， small intestine， and heart of mice. In addition， fucoxanthin accelerated the rate of DHA supplementation by ALA in the brain. Further mechanistic studies revealed that the role of fucoxanthin in promoting PUFAs conversion was not regulated at transcriptional level， but by increasing the activity of fatty acid desaturase 2 (FADS2)， a key enzyme of chain elongation reaction of n-3 and n-6 PUFAs. This study fundamentally provided theoretical bases for a new strategy of dietary supplementation of n-3 and n-6 PUFAs.

According to the general opinion,grains should occupy the largest amount of human food each day,but this grain-based eating habit does not allow human life to be extended.Grains are high in carbohydrates and five commonly recognized plant hormones,namely auxin,gibberellin,cytokinin,abscisic acid,and ethylene.Through in-depth analysis of ancient books,it was found that five grains for support:support is a restriction,and it means the essence,which means that grains should be eaten less,and it is the essence of the least daily intake.To correctly understand the Chinese dietary concept of"five grains for support"is the basis for improving Asian dietary health and promoting Asian human health.

Prediction of return of spontaneous circulation in outof-hospital cardiac arrest with non-shockable initial rhythm using point-of-care testing: a retrospective observational study

BACKGROUND:Out-of-hospital cardiac arrest(OHCA) is a public health concern, and many studies have been conducted on return of spontaneous circulation(ROSC) and its prognostic factors.Rotational thromboelastometry(ROTEM?), a point-of-care testing(POCT) method, has been useful for predicting ROSC in patients with OHCA, but very few studies have focused on patients with non-shockable rhythm. We examined whether the parameters of POCT could predict ROSC in patients with OHCA and accompanying non-shockable rhythm.METHODS:This is a single-center, retrospective observational study. Complete blood count,blood gas, and ROTEM POCT measurements were used. This study included patients with nontraumatic OHCA aged 18 years or older who were transported to the emergency department and evaluated using POCT between January 2013 and December 2021. The patients were divided into the ROSC and non-ROSC groups. Prehospital information and POCT parameters were compared using receiver operating characteristic(ROC) curve analysis, and further logistic regression analysis was performed.RESULTS:Sixty-seven and 135 patients were in the ROSC and non-ROSC groups,respectively. The ROC curves showed a high area under the curve(AUC) for K^(+) of 0.77(95%confidence interval [CI]:0.71–0.83) and EXTEM amplitude 5 min after clotting time(A5) of 0.70(95%CI:0.62–0.77). The odds ratios for ROSC were as follows:female sex 3.67(95%CI:1.67–8.04);K^(+)0.64(95%CI:0.48–0.84);and EXTEM A5 1.03(95%CI:1.01–1.06).CONCLUSION:In OHCA patients with non-shockable rhythm, K^(+) level and the ROTEM parameter EXTEM A5 may be useful in predicting ROSC.

Disturbed passage of jejunal limb near esophageal hiatus after overlapped esophagojejunostomy following laparoscopic total gastrectomy

BACKGROUND Overlapped esophagojejunostomy(OEJ) is a secure purely laparoscopic reconstruction after laparoscopic total gastrectomy(LTG). However, long-term surgical results have not been documented well.AIM In this paper, we report unusual patients who manifested jejunal limb stricture near the esophageal hiatus without anastomotic stenosis during long-term observation after surgery.METHODS From April 2009 until May 2020, we retrospectively reviewed 211 patients underwent LTG following by OEJ for gastric carcinoma and took a standard surveillance program. We aimed to characterize a novel complicated disorder observed in these patients to assist treatment and prevention.RESULTS Five patients(2.4%) had unusual jejunal limb stricture after LTG and OEJ,occurring at a mean of 10 mo after initial radical LTG. All five patients had disturbed oral intake and marked weight loss, and two had aspiration pneumonia.Various diagnostic modalities and intraoperative findings in each patient revealed an intact anastomosis, bent or tortuous jejunal limb resulting from loose fibrous adhesions on the left crus at the esophageal hiatus and no cancer recurrence. All five patients were successfully treated by reoperation for adhesiolysis, division of the left crus and rearrangement of the jejunal limb.CONCLUSION Disturbed passage through the jejunal limb near the hiatus can occur after some types of OEJ following LTG. We speculate that it may result from a short remnant esophagus, excessive mobilization of the jejunal limb that permits bending or tortuosity and adhesions on the left crus at the hiatus. Prevention for this complication is possible during the original LTG procedure.

This study focuses on the use of heavy fuel oil in construction material in Burkina Faso. Its mixture with silty or clayey soil is used as a coating to reinforce the walls of raw earth constructions which are very sensitive to water. The interest of this study is to determine erodibility, water content, while highlighting the influence of the porosity accessible by water on thermal diffusion in construction material containing heavy fuel oil. The heavy fuel oil was mixed with a silty-clayey soil, in different proportions, and water to make bricks samples on which tests were carried out. At the end of the experimental tests, it appears that the water content increases gradually, but not significantly with the addition of heavy fuel oil, which causes a slight increase in the speed of heat propagation through the material with reduced porosity, particularly those containing higher quantities of heavy fuel oil. Conversely, we note a good performance of heavy fuel oil in terms of water resistance properties such as porosity accessible by water and erodibility. This allows us to conclude that the mixture of heavy fuel oil and silty-clayey soil used as a coating material could greatly reduce water infiltration into the walls of housing constructions with raw earthen materials.

Growth promotion of interspecific hybrid embryos between Cucumis anguria and Melon(C. melo) by fruit heating

Wild species of the genus Cucumis, such as C. anguria, are resistant to various melon(C. melo) diseases such as Fusarium wilt race 1.2 and are anticipated as genetic resources of melon. However, in the interspecific crosses between C. anguria and melon, the abortion of the embryos was observed in the early growth stage and could not be regenerated even when cultured. Therefore, for embryo rescue, it is necessary to promote embryo growth during abortion occurrence. This study investigated the effects of fruit heating on embryo growth in interspecific hybrids of C. anguria and melon. In the heating device, the minimum night temperature around the fruit was maintained above 21.5 °C. The ovule grain area of C. anguria × MR-1 with fruit heating was significantly enlarged compared to that without fruit-heating. It was also significantly enlarged compared with the ovule grain area of C. anguria self-pollination. Although, in C. anguria × MR-1 without fruit-heating, no embryo was observed until 10 days after pollination(DAP), and pre-embryos were observed at 13 DAP. Conversely, in C. anguria × MR-1 with fruit-heating, pre-embryos were even observed at 10 DAP. These results suggest that the possibility of embryo or ovule culture regeneration has increased due to embryo growth promotion.

Two cases of childhood absence epilepsy who showed seizure disappearance after ethosuximide drug eruption

Background Recent studies suggest potential roles of immune response in the pathophysiology of epilepsy.Anti-seizure medications(ASMs)are known to have side effects of drug eruption caused by immune responses.A few reports in adults have demonstrated disappearance of seizures after an ASM drug eruption episode.In this paper,we described 2 cases of childhood absence epilepsy(CAE)who showed seizure disappearance after ethosuximide(ESM)drug eruption,suggesting the possibility that the epilepsy disappears due to immune responses to ASM.Case presentation Case 1 was an 8-year-old girl diagnosed with CAE.She was treated with valproate acid(VPA)initially,and then ESM was administered as an additional treatment.Her epileptic seizure disappeared 4 days after initiation of ESM.However,drug eruption appeared 1 week after the administration of ESM.Even after discontinuation of ESM administration,she maintains no seizure after the drug eruption.Case 2 was a 5-year-old boy diagnosed as CAE.He was treated with VPA initially,and ESM was administered additionally.Drug eruption appeared 1 month after the administration of ESM.Even after ESM was terminated,he maintained seizure freedom after the appearance of eruption.Conclusions Epileptic seizures may have been suppressed due to the immune responses caused by ASM eruption.Further studies are needed to elucidate the pathophysiologic effects of drug eruption on epilepsy through immune responses.
