Modified Da-Cheng-Qi Decoction reduces intra-abdominal hypertension in severe acute pancreatitis： a pilot study

Background Intra-abdominal hypertension （IAH） is a recognized prognostic marker for severity of severe acute pancreatitis （SAP） and has a strong impact on the clinical course of SAP. Previous studies indicate that a Da-Cheng-Qi Decoction （DCQD） is beneficial in the treatment of SAP. The purpose of this study was to evaluate the effect of modified DCQD on IAH in patients with SAP. Methods Between January 2008 and December 2008, 42 patients from the West China Hospital were randomized into either the DCQD or control group （n=21 in each group）. Mortality, intra-abdominal pressure （lAP）, Acute Physiology and chronic Health Evaluation II （APACHE II） score, C-reactive protein （CRP）, oxygenation index, Balthazar CT score, rate of renal failure, decompression rate, intensive care unit （ICU） transfer rate, and length of hospital stay （LOS） were compared between the two groups. Results Compared to the control group, the modified DCQD treatment significantly decreased lAP （P 〈0.05） and APACHE II （P 〈0.05） scores on days 4-8, CRP on day 8 （P 〈0.01）, renal failure rate （P 〈0.05）, and LOS （P 〈0.05）. The oxygenation index was significantly improved in the DCQD group compared with the control group （P 〈0.05）. No significant differences in the Balthazar CT score, shock rate, ICU transfer rate, or mortality occurred between the two groups. Conclusions The modified DCQD can effectively relieve IAH and decrease LOS for patients with SAP. Larger clinical trials are needed to confirm these findings.

1)Background:The common factors which potentially contribute to the development of eating disorders and exercise dependence during early adulthood are still relatively *** present study aimed to examine the role of BMI,body image inflexibility,and generalized anxiety in these two behavioral problems in a sample of college students.2)Methods:In total,878 habitual exercisers(58.1%male with BMI=22.12±2.39;41.9%female with BMI=20.55±2.21)with age of 20.09±1.76 years participated in this *** main outcomes of interest are exercise dependence symptoms,eating disorders symptoms,body image inflexibility,and symptoms of generalized anxiety(as measured by Exercise Dependence Scale-Revised,Eating Disorder Examination-Questionnaire-Short Form,Body Image Acceptance and Action Questionnaire,and Generalized Anxiety Disorder-7,respectively).Pearson correlation,path analysis,and model fit information were tested.3)Results:After controlling for age,gender,and field of study,lower BMI was linked to more exercise dependence symptoms but this association was not statistically significant,while a greater BMI was significantly associated with a higher risk of developing eating disorders(β=−0.08,p<0.001).Moreover,higher body image inflexibility significantly and positively contributed to severe exercise dependence(β=0.26,p<0.001)as well as abnormal eating attitudes and behaviors(β=0.74,p<0.001).Furthermore,generalized anxiety is a significant contributor to exercise dependence symptoms(β=0.14,p<0.001)but not eating disorders symptoms.4)Conclusion:Based on our finding that body image inflexibility is a common risk factor for the development of exercise dependence and eating disorders,the prevention and treatment of these two disorders should involve the improvement of psychological *** addition,the individual with a higher BMI is more vulnerable to developing eating disorders,while those who have severer generalized anxiety symptoms should be given more attention whe

Clinicopathological Features of Nondiabetic Renal diseases from Different Age groups:An Observational Cross-sectional Study

Background:Diabetes mellitus (DM)has become the leading cause of chronic kidney disease (CKD).Nondiabetic renal diseases (NDRDs) have different clinicopathological features and prognosis from those of diabetic *** study sought to analyze the clinical and pathological features of NDRDs,in different age groups through a cross-sectional study. Methods:All patients with type 2 DM at our center who underwent renal biopsy between March 1997 and March 2017 were screened and divided into three groups by age:group 1 (youth group),18-44 years old;group 2 (middle-aged group),45-59 years old;and group 3 (elderly group),≥60 years *** analyzed the clinicopathological data and risk factors by univariate and multivariate logistic regression for NDRD of the patients to identify the features of NDRD in different age groups. Results:We included 982 patients in the final *** with NDRD accounted for 64.4%of all *** nephropathy (IgAN) was the most common pathological pattern in young patients with NDRD,accounting for 26.3%.In the middle-aged group,the two most common pathological patterns were IgAN and membranous *** nephropathy was the most common pathological pattern in elderly patients with NDRD,accounting for 29.3%.Consistent with pathological features,glomerular hematuria is a risk factor for NDRD in group 1 (odds ratio [OR],26.514;95%confidence interval [CI],2.503-280.910;P =0.006).On the other hand,rapidly increasing proteinuria or nephrotic syndrome is a risk factor for NDRD in group 2 (OR,5.921;95% CI,2.061-17.013;P =0.001)and group 3 (OR,90.409;95%CI,6.198-1318.826;P =0.001). Conclusions:This single-center study showed that the proportion and composition of NDRD differ among different age groups. Consistent with pathological features,some clinical indices such as hematuria and proteinuria showed different features among different age groups.

Monoclonal antibody technology applied to the detection of <i>Cryptosporidium parvum</i>oocysts in human and cattle fecal samples

With the discovery that the coccidian parasite Cryptosporidium sp. can cause severe symptoms in humans, many diagnostic techniques were quickly implemented such as microscopic visualization, immunofluorescence and PCR. Currently, there is no effective drug treatment and none of the current diagnostic methods is 100% accurate. In this study, a BALB/C mouse was subcutaneously immunized with Cryptosporidium parvum oocysts extract. The spleen was removed and the splenocytes were fused with SP2/0 myeloma cells in order to obtain hybridoma cells secreting antibodies specific to C. parvum antigens. Human and cattle fecal samples previously characterized by microscopy [Ziehl-Neelsen staining (ZN) and Lugol] and PCR for the presence of C. parvum and Giardia duodenalis, were analyzed by indirect immunofluorescence, using the developed hybridomas supernatants. The study shows that the selected hybridomas supernatants identify C. parvum oocysts in fecal samples in correlation with C. parvum oocysts identified using ZN/PCR. No false positive results were obtained and the two best supernatants gave 20% -30% of false negative results. No cross reaction with G. duodenalis was observed. By comparing our results with those obtained with an immunofluorescence commercial kit, it suggests the potential use of the monoclonal antibodies present in two of the hybridomas supernatants as a detection tool of C. parvum. With a reliability of 80.8% and 73.1% versus ZN and PCR methods for IFI, compared with a reliability of 76.9% and 92.3% versus ZN and PCR for commercial DIF kit, the supernatant 4.1D5 seems to be the most promising subject to further study its usefulness for C. parvum detection.

BACKGROUND Intestinal dysbiosis has been shown to be associated with the pathogenesis of alcoholic liver disease(ALD), which includes changes in the microbiota composition and bacterial overgrowth, but an effective microbe-based therapy is lacking. Pediococcus pentosaceus(P. pentosaceus) CGMCC 7049 is a newly isolated strain of probiotic that has been shown to be resistant to ethanol and bile salts. However, further studies are needed to determine whether P. pentosaceus exerts a protective effect on ALD and to elucidate the potential *** To evaluate the protective effect of the probiotic P. pentosaceus on ethanol-induced liver injury in *** A new ethanol-resistant strain of P. pentosaceus CGMCC 7049 was isolated from healthy adults in our laboratory. The chronic plus binge model of experimental ALD was established to evaluate the protective effects. Twenty-eight C57BL/6 mice were randomly divided into three groups: The control group received a pairfed control diet and oral gavage with sterile phosphate buffered saline, the EtOH group received a ten-day Lieber-DeCarli diet containing 5% ethanol and oral gavage with phosphate buffered saline, and the P. pentosaceus group received a 5% ethanol Lieber-DeCarli diet but was treated with P. pentosaceus. One dose of isocaloric maltose dextrin or ethanol was administered by oral gavage on day 11, and the mice were sacrificed nine hours later. Blood and tissue samples(liver and gut) were harvested to evaluate gut barrier function and liver injury-related parameters. Prevalence of asthma symptoms in Golestan schoolchildren aged 6-7 and 13-14 years in Northeast Iran

Asthma is the most common chronic disease among children, and its incidences are often imminent among elementary schoolchildren. This study aimed to examine the prevalence of asthma symptoms in Golestan schoolchildren aged 6-7 and 13-14 years in Northeast Iran. The prevalence rate was compared according to age group （aged 6-7 years vs. aged 13-14 years） and gender （male vs. female）. In this cross-sectional study, 1706 Iranian schoolchildren aged 6-7 and 13-14 years in Golestan Province were enrolled. Participants completed questionnaires between February and July 2014. Asthma symptoms were assessed using the questionnaire of the International Study of Asthma and Allergies in Childhood protocol in Persian. The logistic regression model was used to estimate the odds ratios （ORs） and 95% confidence intervals （CIs） of the asthma symptoms for each of the gender and age groups. The prevalence rates of ＂current asthma＂ symptoms and ＂asthma ever＂ in all the children were estimated as 9.5%and 7.5%, respectively. The prevalence of asthma （＂asthma ever＂ and ＂wheezing in the past 12 months＂） in junior high schoolchildren （aged 13-14 years） is higher than that in elementary schoolchildren （aged 6-7 years）（P 〈 0.05）. The prevalence of the severity of wheezing in girls is lower than that in boys （OR = 1.7, 95%CI = 1.06-2.96, P = 0.02）. Asthma is still a major public health problem. This study shows that the prevalence of the asthma symptoms in boys is lower than that in girls in both age groups, and the severity of asthma in girls is higher than that in boys aged 13-14 years.
