Incidence and predictors of futile recanalisation after endovascular therapy in acute vertebrobasilar artery occlusion patients:insight from the ANGEL-ACT registry

Objectives To identify the occurrence rate and predictors of futile recanalisation after endovascular therapy(EVT)for acute vertebrobasilar artery occlusion(VBAO).Methods Participants of the Endovascular Treatment Key Technique and Emergency Workflow Improvement of Acute Ischaemic Stroke(ANGEL-ACT)registry were selected for the *** recanalisation was defined as patients did not achieve a 90-day good outcome(modified Rankin Scale≤3)despite successful recanalisation(modified Treatment in Cerebral Ischaemia Scale≥2b)after the *** logistic regression analysis was conducted to find independent predictors of futile recanalisation in VBAO patients undergoing *** Three hundred and fifteen patients with VBAO who achieved successful recanalisation after EVT were included in current analysis,of whom,155(49.2%)suffered futile recanalisation,and 160 achieved effective *** the multivariable analysis,we found admission National Institutes of Health Stroke Scale(NIHSS)≥19(OR 4.81,95%CI 2.76 to 8.39,p<0.001),platelet-lymphocyte ratio(PLR)≥162.2(OR 1.93,95%CI 1.14 to 3.27,p=0.001),onset-to-puncture time(OTP)≥334min(OR 2.15,95%CI 1.25 to 3.68,p=0.005)and use of general anesthesia(GA)(OR 1.87,95%CI 1.09 to 3.22,p=0.024)were associated with futile *** Futile recanalisation after EVT occurred 49.2%of VBAO patients in the ANGEL-ACT ***≥19,PLR≥162.2,OTP≥334min and use of GA were independent predictors of futile recanalisation.

Background and purpose Recent observational studies have reported that serum total homocysteine(tHcy)is associated with intracranial aneurysms(IAs).However,the causal effect of tHcy on IAs is *** leveraged large-scale genetic association and real-world data to investigate the causal effect of tHcy on IA *** We performed a two-sample Mendelian randomisation(MR)using publicly available genome-wide association studies summary statistics to investigate the causal relationship between tHcy and IAs,following the recommendations of the Strengthening the Reporting of Observational Studies in Epidemiology-MR ***,a propensity score matching(PSM)analysis was conducted to evaluate the detailed effects of tHcy on risk of IA formation by utilizing real-world multicentre data,including 9902 patients with and without IAs(1:1 matched).Further interaction and subgroup analyses were performed to elucidate how tHcy affects risk of IA *** MR analyses indicated that genetically determined tHcy was causally associated with IA risk(OR,1.38,95%CI 1.07 to 1.79;p=0.018).This is consistent with the more conservative weighted median analysis(OR,1.41,95%CI 1.03 to 1.93;p=0.039).Further sensitivity analyses showed no evidence of horizontal pleiotropy or heterogeneity of single nucleotide polymorphisms in causal *** to the PSM study,we found that,compared with low tHcy(≤15μmol/L),moderate tHcy(>15-30μmol/L)(OR 2.13,95%CI 1.93 to 2.36)and high tHcy(>30μmol/L)(OR 3.66,95%CI 2.71 to 4.95)were associated with a higher IA risk(p trend<0.001).Subgroup analyses demonstrated significant ORs of tHcy in each subgroup when stratified by traditional cardiovascular risk ***,there was also a synergistic effect of tHcy and hypertension on IA risk(p interaction<0.001;the relative excess risk due to interaction=1.65,95%CI 1.29 to 2.01).Conclusion Both large-scale genetic evidence and multicentre real-world data support a causal as

Objective To explore the effectiveness of a nutritional intervention in rescuers who screened positive for *** A randomized controlled trial design was *** June to August,2022,4,460 rescuers were screened using the Self-Rating Depression Scale(SDS),and 1,615 positive cases were ***-one volunteers were recruited and randomly divided into a nutritional intervention group and a control *** intervention group received health education and nutritional intervention(a compound paste therapy primarily composed of red roses and Seville orange flowers),while the control group received psychological *** scores were assessed before and after the *** There was a statistically significant decline in SDS scores in the nutritional intervention group after the intervention(P<0.05).Furthermore,the SDS scores of the intervention group were significantly lower than those of the control group,both before and after the intervention(P<0.05).No severe adverse reactions were observed during safety *** The nutritional intervention effectively reduced the depression scores in *** nutritional intervention is recommended for rescuers who initially screen positive for depression.

Cuprotosis,a new type of cell death,provides great opportunities for the treatment of oral squamous cell carcinoma,as nanocarriers of copper ions can induce cuprotosis and immunogenic ***,we studied an editor that enables production of a nanoparticle“storm”in oral squamous cell carcinoma,maximizing the toxic effect of these particles and reprogramming the tumor microenvironment;as a result,T cells and natural killer(NK)cells can infiltrate the tumor microenvironment to activate an antitumor immune *** this basis,the editor can be combined with optical therapy to improve patient *** this study,the metal ratio was regulated in response to the nanocarrier of acid response ***,in the presence of a specific copper ion content,the nanocarrier could change the permeability of the tumor cell *** on these results,the nanoparticles were cracked in an acidic environment and then released copper ***,the nanoparticles contributed to cuprotosis and immunogenic *** addition,the editor could inhibit murine oral cancer 1(MOC1)tumors in C57BL/6 without *** rate of tumor growth inhibition was as high as approximately 80%.This strategy provides a new idea for ***,it can improve the interaction between immunotherapy and the copper-induced death of oral squamous cell *** all,this study will provide a new opportunity for the effective treatment of oral squamous cell carcinoma.

Mesenchymal stromal cells modulate unfolded protein response and preserve β-cell mass in type 1 diabetes

Introduction:Transplantation of mesenchymal stromal cells(MSCs)is a promising therapy for type 1 diabetes(T1D).However,whether the infused MSCs affect the endoplasmic reticulum stress or subsequent unfolded protein response inβcells remains ***:To investigate this,we induced early-onset T1D in non-obese diabetic mice using ***,T1D mice were randomly assigned to receive either MSCs or phosphate-buffered *** observed the in vivo homing of MSCs and assessed their effectiveness by analyzing blood glucose levels,body weight,histopathology,pancreatic protein expression,and serum levels of cytokines,proinsulin,and ***:Infused MSCs were found in the lungs,liver,spleen,and pancreas of T1D *** exhibited various effects,including reducing blood glucose levels,regulating immunity,inhibiting inﬂammation,increasingβ-cell areas,and reducing the expression of key proteins in the unfolded protein response *** serum proinsulin and C-peptide levels were signiﬁcantly higher in the MSCs treatment group than in the T1D model ***,there was no signiﬁcant difference in the biomarker ofβ-cell endoplasmic reticulum stress,the ratio of fasting serum proinsulin to C-peptide,between the two ***:Ourﬁndings reveal that MSCs infusion does not alleviate endoplasmic reticulum stress inβcells directly but modulates the unfolded protein response pathway to preserveβ-cell mass and function in T1D mice.

Damage or microstructural alterations of the white matter can cause dysfunction of the intrinsic neural networks in a condition termed as white matter disease(WMD).Frequently detected on brain computed tomography and magnetic resonance imaging scans,WMD is commonly presented in inflammatory demyelinating diseases like multiple sclerosis(MS)and vascular diseases such as cerebral small vessel disease(CSVD).Prevention of MS and CSVD progression requires early treatments with drastically different medications and approaches,as such,early and accurate diagnosis of WMD,derived from vascular or demyelinating etiologies,is of paramount ***,the clinical and imaging similarities between MS,especially during the early stage,and CSVD,pose a significant dilemma in differentiating these two *** this review,we attempt to summarize and contrast the distinguishing features of MS and CSVD for aiding accurate diagnosis to ensure timely corresponding management in the early stages of MS and CSVD.

Intellectual disability(ID)is a condition characterized by cognitive impairment and difficulties in adaptive *** our research,we identified two de novo mutations(c.955C>T and c.732C>A)at the KDM2A locus in individuals with varying degrees of *** addition,by using the Gene4Denovo database,we discovered five additional cases of de novo mutations in *** mutations we identified significantly decreased the expression of the KDM2A *** investigate the role of KDM2A in neural development,we used both 2D neural stem cell models and 3D cerebral *** findings demonstrated that the reduced expression of KDM2A impairs the proliferation of neural progenitor cells(NPCs),increases apoptosis,induces premature neuronal differentiation,and affects synapse *** ChIP-Seq analysis,we found that KDM2A exhibited binding to the transcription start site regions of genes involved in *** addition,the knockdown of KDM2A hindered H3K36me2 binding to the downstream regulatory elements of *** integrating ChIP-Seq and RNA-Seq data,we made a significant discovery of the core genes'remarkable enrichment in the MAPK signaling ***,this enrichment was specifically linked to the p38 MAPK ***,disease enrichment analysis linked the differentially-expressed genes identified from RNA-Seq of NPCs and cerebral organoids to neurodevelopmental disorders such as ID,autism spectrum disorder,and ***,our findings suggest that KDM2A plays a crucial role in regulating the H3K36me2 modification of downstream genes,thereby modulating the MAPK signaling pathway and potentially impacting early brain development.
