Objective: The aim of the study was to evaluate the feasibility of functional MR in predicting the clinical response to chemotherapy in patients with colorectal liver metastases(CLM).Methods: A total of 196 eligible patients were enrolled in the study between August 2016 and January 2023. Functional MR was performed at baseline and after one cycle of chemotherapy. The diffusion kurtosis radiomic texture features were extracted and a signature model was built using the R package. The initial 100 cases were designated as the training set, the following 48 cases were designated as the validation set, and the final 48 cases were designated as the intervention validation ***: Good performance for the response prediction(AUC = 0.818 in the training set and 0.755 in the validation set) was demonstrated. The objective response rates(ORRs) in the high-risk subgroup were significantly lower than the low-risk subgroup in the training and validation sets. Worse progression-free survival and overall survival rates were noted in the high-risk population. In the intervention set 22.9%(11/48) of the chemotherapy regimens for patients were changed in response to the model-predicted results and the ORR reached 77.1%(37/48), which was significantly higher than the training and validation sets [47.97%(71/148); P = 0.000].Conclusions: A functional MR signature effectively predicted the chemotherapy response and long-term survival. The adjustment of the regimen guided by the model significantly improved the ORR.

The general objective of our study was to describe the clinical and epidemiological aspects of anxiety disorders. This was a retrospective descriptive cross-sectional study, lasting 5 years, from January 01, 2019 to December 31, 2023, on 225 patients seen in consultation for anxiety disorders in the department. The prevalence of anxiety disorders was 2.5% in consultation. The 20 - 29 age group was the most represented at 33.3%. Females were slightly represented at 55.1%, with a sex ratio of 1.2. The secondary level was the most affected, accounting for 36.4% of cases. Insomnia was the most frequent reason for consultation, accounting for 71.1% of cases. A larger general population study would be necessary to better assess the extent of anxiety disorders in Guinea.

Objective: European lung cancer screening studies using computed tomography(CT) have shown that a management protocol based on measuring lung nodule volume and volume doubling time(VDT) is more specific for early lung cancer detection than a diameter-based protocol. However, whether this also applies to a Chinese population is unclear. The aim of this study is to compare the diagnostic performance of a volume-based protocol with a diameter-based protocol for lung cancer detection and optimize the nodule management criteria for a Chinese ***: This study has a population-based, prospective cohort design and includes 4000 participants from the Hexi district of Tianjin, China. Participants will undergo low-dose chest CT at baseline and after 1 year. Initially, detected lung nodules will be evaluated for diameter and managed according to a routine diameter-based protocol(Clinical Practice Guideline in Oncology for Lung cancer Screening, Version 2.2018). Subsequently, lung nodules will be evaluated for volume and management will be simulated according to a volume-based protocol and VDT(a European lung nodule management protocol). Participants will be followed up for 4 years to evaluate lung cancer incidence and mortality. The primary outcome is the diagnostic performance of the European volume-based protocol compared to diameter-based management regarding lung nodules detected using low-dose ***: The diagnostic performance of volume-and diameter-based management for lung nodules in a Chinese population will be estimated and ***: Through the study, we expect to improve the management of lung nodules and early detection of lung cancer in Chinese populations.

Objective:To analyze the efficacy and safety o f apatinib in the treatment of stage IV osteogenic sarcoma after chemotherapy failure through a single-arm,prospective,and open clinical phase II ***:Information on 34 patients with stage IV osteogenic sarcoma treated with apatinib after failure o f chemotherapy in Tianjin Medical University cancer Institute and Hospital between September 2015 and December 2019 was collected and *** participants included 23 males and 11 females,with an average age of 35.24 years(11-73 years).The objective response rate(ORR),disease control rate(DCR),progression-free survival(PFS),PFS rate(PFR),and overall survival(OS)were *** treatmentrelated adverse events(AEs)and safety of apatinib were also ***:O f the 34 patients,33 were able to be evaluated for *** patient received apatinib treatment for less than one cycle;therefore,only safety analysis was *** 12-week clinical evaluation showed that 2 patients had a partial response(PR),24 patients had stable disease(SD),and 7 patients had progressive disease(PD).The ORR,DCR,and PFR at 12 weeks were 6.06%(2/33),78.79%(26/33),and 82%,*** the end of the follow-up,6 patients had SD(18.18%,6/33),27 patients had PD(81.82%,27/33),and 15 patients died because of disease progression(45.45%,15/33).The ORR was 0(0/33),the DCR was 18.18%(6/33),and the median PFS(mPFS)was 7.89 months(95%Cl:4.56-11.21).The median OS(mOS)was 17.61 months(95%Cl:10.85-24.37).The most common treatment-related AEs were hand-foot syndrome(35.29%,12/34),proteinuria(32.35%,11/34),and hypertension(32.35%,11/34).

Bariatric surgery induces alterations in effective connectivity between the orbitofrontal cortex and limbic regions in obese patients

Obese subjects show enhanced brain responses in motivation and reward neurocircuitry encompassing sensory and somatic integration-interception, motivation-reward(striatal), emotion, and memory processes, which attenuate frontal region activation during food cues. Bariatric surgery(BS) is the only reliable treatment for morbid obesity. Unfortunately, it is unknown how BS affects neurocircuitry after weight loss. We aimed to examine effects of BS on the basal activity of brain areas involved in reward and motivation processing, emotion, memory, and gut-brain interaction. We combined resting-state fMRI with amplitude of low-frequency fluctuation(ALFF) and Granger causality analysis(GCA) to assess interactions between regions within the frontal-mesolimbic circuitry in 16 obese subjects(OB) and 22 normal-weight(NW) *** OB group was studied at baseline and 1 month post BS. Comparisons between OB and NW, and preand post BS showed significant differences in ALFF in areas involved in drive(caudate, orbitofrontal cortex(OFC)), arousal(thalamus), and conditioning/memory(amygdala, hippocampus)(P < 0.05, FDR correction). GCA revealed that in the OB group, the OFC had greater connectivity to limbic regions(amygdala,hippocampus, and medial thalamus) and the caudate. Post BS, the connectivity of the OFC to limbic regions decreased, whereas the connectivity from the amygdala and hippocampus to the caudate and thalamus was enhanced, particularly in subjects with lower body mass index(BMI). OFC activation in the OB group was associated with BMI prior to surgery, and changes in OFC post surgery were associated with alterations in BMI. Overall, the functional connectivity of the OFC was significantly decreased.
