目的评估肾皮质肿瘤化学位移MRI参数的定性定量。方法共251例连续的病人在肾切除术前行1.5TMRI检查。2名阅片者(R1,R2)独立肉眼评价,与同相位化学位移成像比较,评价所有肿瘤在反相位影像上的信号强度(SI)下降。此外,测量同、反相位(SIIP,SIOP)影像上肿瘤的SI,并计算化学位移指数作为SI变化的百分数量度。以组织病理学作为参考标准。结果肉眼所见的SI下降,透明细胞肾细胞癌(RCC)(R1,73%;R2,64%)和血管平滑肌脂肪瘤(均为80%)显著多见于嗜酸细胞瘤(29%,12%)、乳头状瘤(29%,17%)和嫌色细胞RCC(13%,9%;均P<0.05)。化学位移指数中位数在透明细胞RCC和血管平滑肌脂肪瘤明显高于其他组织学亚型(均P<0.001)。不同阅片者肉眼评价的一致性一般(κ=0.4),定量分析的一致性很好(一致性相关系数,0.80)。结论反相位化学位移成像SI下降不是透明细胞RCC或血管平滑肌脂肪瘤的识别特征,其也可见于嗜酸细胞瘤、乳头状瘤和嫌色细胞RCC。排除血管平滑肌脂肪瘤后,SI下降超过25%可诊断透明细胞RCC。要点①化学位移MRI为肾肿瘤中的脂肪提供了新信息。②所有肾皮质肿瘤均可见反相位信号减低。③信号下降超过25%对透明细胞RCC具有诊断意义。

The Identity of Human Tissue-Emigrant CD8+T Cells

Background:Proximal ureteral stones(PUS)have relatively low rates of spontaneous ***,some patients do on expectant *** aim was to compare risk factors for surgical intervention in patients with PUS who underwent primary intervention to those subjected to expectant management.Materials and methods:We retrospectively reviewed the medical charts of patients presented to the emergency room with symptoms of renal colic and underwent computerized tomography between August 2016 and August 2017.A total of 97 consecutive patients were identified with up to 10 mm *** collected patient demographics,clinical,and imaging data,and performed binary regression analysis for risk of intervention.Results:The average age was 49years(range 17-97)and average stone size was 7.1 mm(range 3-10).Forty-one patients underwent immediate intervention while the remaining 56 patients were treated *** the 56 patients treated conservatively,26 underwent delayed intervention while 30 reported spontaneous stone *** univariate analysis of all 97 patients,statistically significant risk factors for intervention were found based on stone size,age,serum lymphocyte,platelet counts,and stone *** these risk factors,stone size≥7mm(p=0.012,odds ratio=5.4)and platelet count?230K/mL(p=0.027,odds ratio=4.9)remained statistically significant on multivariate analysis.Conclusion:Stone size and platelet count were found to be risk factors for surgical intervention in patients with up to 10 mm *** findings may assist in identifying patients who are more suitable for conservative approach.

Acute kidney injury(AKI)leads to unacceptably high mortality due to difficulties in timely intervention and less efficient renal delivery of therapeutic ***,a series of polyvinylpyrrolidone(PVP)-curcumin nanoparticles(PCurNP)are designed to meet the renal excretion threshold(~45 kDa),presenting a controllable delivery nanosystem for kidney *** accumulation of the relatively small nanoparticles,^(89)Zr-PCurNP M10 with the diameter between 5 and 8 nm,is found to be 1.7 times and 1.8 times higher than the accumulation of^(89)Zr-PCurNP M29(20-50 nm)and M40(20-50 nm)as revealed by PET ***,serum creatinine analysis,kidney tissues histology,and tubular injury scores revealed that PCurNP M10 efficiently treated cisplatin-induced ***,PCurNP offers a novel and simple strategy for precise PET image-guided drug delivery of renal protective materials.

Objective Little is known about the brain systems that contribute to vulnerability to post-traumatic stress disorder （PTSD）. Comparison of the resting-state patterns of intrinsic functional synchronization, as measured by functional magnetic resonance imaging （fMRI）, between groups with and without PTSD following a traumatic event can help identify the neural mechanisms of the disorder and targets for intervention. Methods Fifty-four PTSD patients and 72 matched traumatized subjects who experienced the 2008 Sichuan earthquake were imaged with blood oxygen level-dependent （BOLD） fMRI and analyzed using the measure of regional homogeneity （ReHo） during the resting state. Results PTSD patients presented enhanced ReHo in the left inferior parietal lobule and right superior frontal gyrus, and reduced ReHo in the right middle temporal gyrus and lingual gyrus, relative to traumatized individuals without PTSD. Conclusion Our findings showed that abnormal brain activity exists under resting conditions in PTSD patients who had been exposed to a major earthquake. Alterations in the local functional connectivity of cortical regions are likely to contribute to the neural mechanisms underlying PTSD.

目的发展并验证一种用于超声的且临床可游走的双靶向(P和E选择素)对比剂(MBSelectin)在分子水平对炎症进行量化的分子成像方法,并在对鼠急性结肠炎模型不同程度炎症的可视化和量化方面,对靶向选择素超声与FDG-PET和CT进行定量相关性分析。材料与方法动物实验由斯坦福大学学术管理专家组的实验动物保护小组批准。通过共价结合一种存在于包含全氟/氮的MB脂质包壳上的天然融合于人类Fc片段的P-选择糖蛋白配体-1类似物获得MBSelectin。通过体外流式小室、体内化学诱导的鼠急性结肠炎模型评估MBSelectin的结合特异性。定量分析超声信号与PET/CT的FDG摄取和组织学等级的相关性。数据分析采用Studentt检验、方差分析和Pearson相关分析。结果在体外,与MBControl相比MBSelectin对人类和鼠P和E选择蛋白均显示高度结合(P≤0.002)。活体内,与对照组鼠[(5.0±4.5)任意单位(au)]相比,急性结肠炎组鼠[(173.8±134.8)au]超声信号显著增强(P<0.001)。超声成像信号与PET/CT影像的FDG摄取高度相关(ρ=0.89,P<0.001)。离体分析证实结肠炎小鼠的炎性过程和黏膜微血管中P蛋白和E选择蛋白的高度表达有关(P=0.014)。结论利用炎性组织的白细胞动员天然途径,MBControl超声可特异地检出并定量鼠急性结肠炎模型的炎症。MBControl超声成像与PET/CT成像的FDG摄取有良好相关性。
