Potential benefit of lymph node dissection during radical nephrectomy for kidney cancer:A review and critical analysis of current literature

Objective:The role of lymph node dissection(LND)is still controversial in patients with renal cell carcinoma undergoing *** aimed to provide a comprehensive review of the literature about the effect of LND on survival,prognosis,surgical outcomes,as well as patient selection and available LND ***:Recent literature(from January 2011 to December 2021)was assessed through PubMed and MEDLINE databases.A narrative review of most relevant articles was ***:The frequencies in which LNDs are being carried out are decreasing due to an increase in minimally invasive and nephron sparing ***,randomized clinical trials and meta-analyses failed to show any survival advantage of LND versus no ***,retrospective studies suggest a survival benefit of LND in high-risk patients(bulky tumors,T3-4 stage,and cN1 patients).Moreover,extended LND might provide important staging information,which could be of interest for adjuvant treatment ***:No level 1 evidence of any survival advantage deriving from LND is currently available in ***,the role of LND is limited to staging ***,low grade evidence suggests a possible role of LND in high-risk *** clinical trials are warranted to corroborate these findings.

Structure and Activity of Strigolactones： New Plant Hormones with a Rich Future

Strigolactones （SLs） constitute a new class of plant hormones which are active as germination stimulants for seeds of parasitic weeds of Striga, Orobanche, and Pelipanchi spp, in hyphal branching of arbuscular mycorrhizal （AM） fungi and as inhibitors of shoot branching. In this review, the focus is on molecular features of these SLs. The occurrence of SLs in root exudates of host plants is described. The naming protocol for SL according to the International Union of Pure and Applied Chemistry （IUPAC） rules and the ＇at a glance＇ method is explained. The total synthesis of some natural SLs is described with details for all eight stereoisomers of strigol. The problems encountered with assign- ing the correct structure of natural SLs are analyzed for orobanchol, alectrol, and solanacol. The structure-activity relationship of SLs as germination stimulants leads to the identification of the bioactiphore of SLs. Together with a tentative mechanism for the mode of action, a model has been derived that can be used to design and prepare active SL analogs. This working model has been used for the preparation of a series of new SL analogs such as nijmegen-1, and analogs derived from simple ketones, keto enols, and saccharine. The serendipitous finding of SL mimics which are derived from the D-ring in SLs （appropriately substituted butenolides） is reported. For SL mimics, a mode of action is proposed as well. Recent new results support this proposal. The stability of SLs and SL analogs towards hydrolysis is described and some details of the mechanism of hydrolysis are discussed as well. The attempted isolation of the protein receptor for germination and the current status concerning the biosynthesis of natural SLs are briefly discussed. Some non-SLs as germinating agents are mentioned. The structure-activity relationship for SLs in hyphal branching of AM fungi and in repression of shoot branching is also analyzed. Systematic review of cystic duct closure techniques in relation to prevention of bile duct leakage after laparoscopic cholecystectomy

AIM To study the effect of different techniques of cystic duct closure on bile leakage after laparoscopic cholecystectomy(LC) for biliary *** A systematic search of MEDLINE, Cochrane and EMBASE was performed. Rate of cystic duct leakage(CDL) was the primary outcome. Risk of bias was evaluated. Odds ratios were analyzed for comparison of techniques and pooled event rates for non-comparative analyses. Pooled event rates were compared for each of included *** Out of 1491 articles, 38 studies were included. A total of 47491 patients were included, of which 38683(81.5%)underwent cystic duct closure with non-locking(metal)clips. All studies were of low-moderate methodological quality. Only two studies reported separate data on uncomplicated and complicated gallbladder disease. For overall CDL, an odds ratio of 0.4(95%CI: 0.06-2.48)was found for harmonic energy vs clip closure and an odds ratio of 0.17(95%CI: 0.03-0.93) for locking vs non-locking clips. Pooled CDL rate was around 1% for harmonic energy and metal clips, and 0% for locking clips and ligatures. CONCLUSION Based on available evidence it is not possible to either recommend or discourage any of the techniques for cystic duct closure during LC with respects to CDL,although data point out a slight preference for locking clips and ligatures vs other techniques. No separate recommendation can be made for complicated gallbladder disease.

Validation of a Device for Clinical Measurement of Frontal Tibiofemoral Alignment

Study Design: A prospective instrumental study design. Objectives: In this study, the validity of a device was investigated. Background: Frontal tibiofemoral malalignment is a predisposing factor for developing gonarthrosis. In a previous study a repositioning device was developed to clinically measure frontal tibiofemoral alignment. This device appeared to be more accurate than the conventional method. Case description: This study has examined three types of measurement: conventional method, repositioning device and a bilateral full-leg standing radiograph (Rx). We compared the results of the repositioning device and the conventional method to Rx. To determine the validity of the new repositioning device, n = 41 patients under 18 years of age were included in this study. They all underwent a standing Rx in their history. Outcome: In our study we found a negative correlation between the repositioning device and the already known Rx. Conclusion: The repositioning device was validated, but is not accurate enough to replace Rx. However, physiotherapists can use this device to obtain a more accurate clinical measurement of frontal tibiofemoral alignment.

Expression Compilation of Several Putative Cancer Stem Cell Markers by Primary Ovarian Carcinoma

Cancer stem cells (CSCs) or tumor initiating cells are rare cells that are able to establish a tumor or metastasis. Identification of those CSCs is, however, cumbersome even in established cell lines. Several cancer stem cell markers were reported to be expressed by ovarian cancer. Those cancer stem cells are gifted with lower vulnerability to irradiation and cytostatic drugs explaining the high incidence of recurrence after treatment. A variety of different cancer stem cell markers were described for epithelial tumors. Also, cancer cell lines were assessed for stem cell markers with no common denominator. The expression of CD24, CD44, CD117, CD133, ABCG2, ALDH was determined for cells from 22 patients. Ovarian cancer cells were collected from ascites. Part of the tumor cells were analyzed immediately and stained for the above mentioned cancer stem cell markers. The remainder of the cells was cultured for several weeks using standard stem cell culture conditions. We observed a large variety in expression of putative stem cell markers for primary tumors. After two weeks of culture spheres were seen in several cultures, indicative for cancer stem cells, though not all patients’ cells were able to form spheres. Our data show for the first time the heterogeneity in marker display in primary tumors. Also for the cultured cells stem cell markers were determined. None of the stem cell markers was expressed by all patients’ cells. No correlation with tumor type was demonstrated. The complexity of expression challenges the isolation of cancer stem

The objective of this appraisal is to shed light on the various approaches to screen sensory information in the human gut. Understanding and characterization of sensory symptoms in gastrointestinal disorders is poor. Experimental methods allowing the investigator to control stimulus intensity and modality, as well as using validated methods for assessing sensory response have contributed to the understanding of pain mechanisms. Mechanical stimulation based on impedance planimetry allows direct recordings of luminal cross-sectional areas, and combined with ultrasound and magnetic resonance imaging, the contribution of different gut layers can be estimated. Electrical stimulation depolarizes free nerve endings non-selectively. Consequently, the stimulation paradigm (single, train, tetanic) influences the involved sensory nerves. Visual controlled electrical stimulation combines the probes with an endoscopic approach, which allows the investigator to inspect and obtain small biopsies from the stimulation site. Thermal stimulation (cold or warm) activates selectively mucosal receptors, and chemical substances such as acid and capsaicin (either alone or in combination) are used to evoke pain and sensitization. The possibility of multimodal (e.g. mechanical, electrical, thermal and chemical) stimulation in different gut segments has developed visceral pain research. The major advantage is involvement of distinctive receptors, various sensory nerves and different pain pathways mimicking clinical pain that favors investigation of central pain mechanisms involved in allodynia, hyperalgesia and referred pain. As impairment of descending control mechanisms partly underlies the pathogenesis in chronic pain, a cold pressor test that indirectly stimulates such control mechanisms can be added. 新型冠状病毒肺炎(COVID-19)的重症和死亡与年龄有关,儿童感染者很少重症和死亡,其机制尚不完全清楚.我们回顾性地研究了湖北省十堰市太和医院就诊的186例新冠肺炎患者,发现10岁以下患儿主要临床表现均为轻症,而非成人的严重病症.进一步年龄分层分析发现, 2岁以下患儿更不易合并肺炎支原体共感染,康复时间平均缩短6 d,治疗后淋巴细胞计数回升更明显.患儿康复时间缩短与中西药治疗没有显著关联性,而与其频繁接种疫苗有关.因此, 2岁以下频繁接种疫苗可能激活免疫系统,对新型冠状病毒感染及肺炎支原体共感染具有更强的免疫力.

Health insurance and switching behavior: Evidence from the Netherlands

Introduction: Since the introduction of the Health Insurance Act in the Netherlands in 2006, insurers are incentivized to compete on prices for basic health insurance, and on price and quality for supplementary insurance. The new health insurance system aimed to create a more competitive market in which consumers would switch health plans, thereby stimulating insurers to price competition and quality improvement. This article evaluates the switching behavior of Dutch consumers and evaluates whether this behavior is advantageous to the goals of the reform. Methods: Three surveys were conducted: from 2005-2006 (n = 478), 2008-2009 (n = 389), and 2010-2011 (n = 191). Results: In 2005-2006, almost 20 percent of the Dutch consumers switched their insurance company. In between 2006 and 2012, however, the percentage of switchers decreased to less than four percent. The main cause of this decrease is that consumers no longer perceive sufficient differences between insurance companies in terms of premium and service. In addition, consumers have difficulties finding the proper information making the right decision and believe they may not be accepted for the supplementary insurance. Consequently, insurance companies only perceive limited incentives to create a more competitive market. Conclusion: Clear and unambiguous information, combined with an obligatory acceptance for the supplementary insurance might help to improve the potential mobility of Dutch consumers.

Interface and Surface Properties of Nano-hydroxyapatite /Poly (1,4-PhenyleneSulfide)-Poly(2,4-Phenylene Sulfide Acid) Copolymer Composite

The interface and surface properties of nano-hydroxyapatite(n-HA) and poly(1,4-phenylene sulfide)-poly (2,4-phenylene sulfide acid)(PPS-PPSA) copolymer composite were investigated. The results show that there are some strong interface combinations of calcium ion (Ca2+), carboxyl (-COO-) and phosphate radicle ion (PO3-4) between copolymer and n-HA in the composite. The presence of the 2,4-phenylene sulfide acid in copolymer can increase the affinity to n-HA, which causes the formation of chemical bindings between the PPS-PPSA copolymer and n-HA. XRD analysis and IR surface analysis indicate that n-HA is not encapsulated by copolymer but exposed on the surface of the composite, and has same structure and properties with the original n-HA. The presence of the interface chemical bindings between the PPS-PPSA copolymer and n-HA can increase the content of n-HA in composite but does not cause the decrease of the composite mechanical strength.
