Introduction Tuberculosis(TB)is one of the most prevalent infectious diseases in the world,causing major public health problems in developing *** rate of TB incidence in Iran was estimated to be 13 per 100,000 in *** study aimed to estimate the reproduction number and serial interval for pulmonary tuberculosis in *** and methods The present national historical cohort study was conducted from March 2018 to March 2022 based on data from the National Tuberculosis and Leprosy Registration center of Iran's Ministry of health and Medical Education(MOHME).The study included 30,762 tuberculosis cases and 16,165 new smear-positive pulmonary tuberculosis patients in *** estimated the reproduction number of pulmonary tuberculosis in a Bayesian framework,which can incorporate uncertainty in estimating *** analyses were accomplished in R *** The mean age at diagnosis of patients was 52.3±21.2 years,and most patients were in the 35–63 age group(37.1%).Among the data,9121(56.4%)cases were males,and 7044(43.6%)were *** patients,7459(46.1%)had a delayed diagnosis between 1 and 3 ***,3039(18.8%)cases were non-Iranians,and 2978(98%)were *** time-varying reproduction number for pulmonary tuberculosis disease was calculated at an average of 1.06±0.05(95%Crl 0.96–1.15).Conclusions In this study,the incidence and the time-varying reproduction number of pulmonary tuberculosis showed the same *** mean of the time-varying reproduction number indicated that each infected person is causing at least one new infection over time,and the chain of transmission is not being disrupted.

The interplay between DNA replication stress and immune microenvironment alterations is known to play a crucial role in colorectal tumorigenesis,but a comprehensive understanding of their association with and relevant biomarkers involved in colorectal tumorigenesis is *** address this gap,we conducted a study aiming to investigate this association and identify relevant *** analyzed transcriptomic and proteomic profiles of 904 colorectal tumor tissues and 342 normal tissues to examine pathway enrichment,biological activity,and the immune ***,we evaluated genetic effects of single variants and genes on colorectal cancer susceptibility using data from genome-wide association studies(GWASs)involving both East Asian(7062 cases and 195745 controls)and European(24476 cases and 23073 controls)*** employed mediation analysis to infer the causal pathway,and applied multiplex immunofluorescence to visualize colocalized biomarkers in colorectal tumors and immune *** findings revealed that both DNA replication activity and the flap structure-specific endonuclease 1(FEN1)gene were significantly enriched in colorectal tumor tissues,compared with normal ***,a genetic variant rs4246215 G>T in FEN1 was associated with a decreased risk of colorectal cancer(odds ratio=0.94,95%confidence interval:0.90–0.97,P_(meta)=4.70×10^(-9)).Importantly,we identified basophils and eosinophils that both exhibited a significantly decreased infiltration in colorectal tumors,and were regulated by rs4246215 through causal pathways involving both FEN1 and DNA *** conclusion,this trans-omics incorporating GWAS data provides insights into a plausible pathway connecting DNA replication and immunity,expanding biological knowledge of colorectal tumorigenesis and therapeutic targets.

Limited evidence exists on the effect of submicronic particulate matter(PM_(1)) on hypertension hospitalization. Evidence based on causal inference and large cohorts is even more scarce. In 2015, 36,271 participants were enrolled in South China and followed up through 2020. Each participant was assigned single-year, lag0–1, and lag0–2 moving average concentration of PM_(1)and fine inhalable particulate matter(PM2.5) simulated based on satellite data at a 1-km resolution. We used an inverse probability weighting approach to balance confounders and utilized a marginal structural Cox model to evaluate the underlying causal links between PM_(1)exposure and hypertension hospitalization, with PM2.5-hypertension association for comparison. Several sensitivity studies and the analyses of effect modification were also conducted. We found that a higher hospitalization risk from both overall(HR: 1.13, 95% CI:1.05–1.22) and essential hypertension(HR: 1.15, 95% CI: 1.06–1.25) was linked to each 1 μg/m3increase in the yearly average PM_(1)concentration. At lag0–1 and lag0–2, we observed a 17%–21% higher risk of hypertension associated with PM_(1). The effect of PM_(1)was 6%–11% higher compared with PM2.5. Linear concentration-exposure associations between PM_(1)exposure and hypertension were identified, without safety thresholds. Women and participants that engaged in physical exercise exhibited higher susceptibility, with 4%–22% greater risk than their counterparts. This large cohort study identified a detrimental relationship between chronic PM_(1)exposure and hypertension hospitalization, which was more pronounced compared with PM2.5and among certain groups.

Background and purpose Previous studies,mostly focusing on the European population,have reported polygenic risk scores(PRSs)might achieve risk stratification of *** aimed to examine the association strengths of PRSs with risks of stroke and its subtypes in the Chinese *** Participants with genome-wide genotypic data in China Kadoorie Biobank were split into a potential training set(n=22191)and a population-based testing set(n=72150).Four previously developed PRSs were included,and new PRSs for stroke and its subtypes were *** PRSs showing the strongest association with risks of stroke or its subtypes in the training set were further evaluated in the testing *** proportional hazards regression models were used to estimate the association strengths of different PRSs with risks of stroke and its subtypes(ischaemic stroke(IS),intracerebral haemorrhage(ICH)and subarachnoid haemorrhage(SAH)).Results In the testing set,during 872919 person-years of follow-up,8514 incident stroke events were *** PRSs of any stroke(AS)and IS were both positively associated with risks of AS,IS and ICH(p<0.05).The HR for per SD increment(HR_(SD))of PRSAS was 1.10(95%CI 1.07 to 1.12),1.10(95%CI 1.07 to 1.12)and 1.13(95%CI 1.07 to 1.20)for AS,IS and ICH,*** corresponding HR_(SD) of PRS_(IS) was 1.08(95%CI 1.06 to 1.11),1.08(95%CI 1.06 to 1.11)and 1.09(95%CI 1.03 to 1.15).PRS_(ICH) was positively associated with the risk of ICH(HR_(SD)=1.07,95%CI 1.01 to 1.14).PRSS_(AH) was not associated with risks of stroke and its *** addition of current PRSs offered little to no improvement in stroke risk prediction and risk *** In this Chinese population,the association strengths of current PRSs with risks of stroke and its subtypes were moderate,suggesting a limited value for improving risk prediction over traditional risk factors in the context of current genome-wide association study under-representing the East Asia
