Ropinirole Implants Reverse MPTP-Induced Parkinsonism in Rhesus Monkeys

Purpose: We compared efficacy and side effects of ropinirole implants with oral ropinirole in parkinsonian monkeys. Methods: Twenty monkeys received injections of 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine-hydrochloride (MPTP) to render them parkinsonian. Monkeys were then placed into 3 groups based upon clinical rating scores (CRS). Group 1 received oral ropinirole and placebo implants. Group 2 receivedropinirole implants that released 1/9th of the animals’ optimal daily oral dose and oral placebo. Group 3 received placebo implants and oral placebo. Monkeys were assessed for pharmacokinetic data, CRS, Global Dyskinesia Rating Scale, and skin irritation. Results: For the ropinirole implant group, the activity pattern was similar to that seen pre-MPTP;which extended through the weekends and was greater than control treated parkinsonian monkeys. Oral ropinirole yielded a high degree of variability for activity, with values following oral dosing being higher than the pre-MPTP periodbut levels similar to placebo treated parkinsonian animals during weekends, which were excluded from oral dosing. Implants and oral treatment achieved significant improvement in CRS between 11 - 60 days and 4 - 60 days respectively. Conclusion: Low dose ropinirole implants have the potential to provide continuous clinical improvement in bradykinesia with fewer “off periods” and lower risk for medication-induced psychosis than oral medication.

Background: Selective serotonergic reuptake inhibitors(SSRIs) are first-line pharmacologic treatments for patients with posttraumatic stress disorder(PTSD), but must be given over extended period of time before the onset of action. The use of SSRIs in PTSD patients with mild traumatic brain injury(m TBI) is problematic since SSRIs could exacerbate post-concussion syndrome(PCS) symptoms. VA/DOD guidelines identify trauma-focused psychotherapy as the best evidence-based treatment for PTSD, but overall effectiveness is limited by reduced levels of patient engagement and retention. A previous study from this research group suggested that quetiapine monotherapy, but not risperidone or valproate, could increase engagement in trauma-focused ***: We report the study protocol of a pilot study funded under the South-Central Mental Illness Research, Education, and clinical Center pilot study program from the U.S. department of Veterans Affairs. This randomized, open-label study was designed to evaluate the feasibility of completing a randomized trial of quetiapine vs. treatment as usual to promote patient engagement in PTSD patients with a history of m ***: We expect that the success of this ongoing study should provide us with the preliminary data necessary to design a full-scale randomized trial. Socio-Demographic Factors and Their Correlation with the Severity of Major Depressive Disorder: A Population Based Study

Objective: Genetic and neurobiological factors are considered to be the major causes of major depressive disorder (MDD). However, over last few years study results revealed that socio-demographic factors contribute a significant role in abnormal functional activity and recognized as major causative factors to the pathogenesis and severity of MDD. The aim of this study was to assess the socio-demographic characteristics of Bangladeshi MDD patients and to discover their role on the severity of disease. Material and Methods: A retrospective review of the case notes of psychiatry outpatients at Bangabandhu Sheikh Mujib Medical University (BSMMU) was carried out between Sep-Nov 2016. A total of 234 MDD patients (aged 18 to 60 years) were randomly recruited. Relevant information was obtained by collection of prescription details from the patients or their relatives by face to face interview. Results: Mean age and body mass index (BMI) were found 34.734 years (SEM ± 0.842) and 22.014 kg/m2 respectively. Among all respondents, 56% (133) were male and 44% (106) were female where mean income was 26.84 KBDT (Kilo Bangladeshi Taka) (SEM ± 0.749). Roughly 46% (111) patients were found to be below secondary level of education. Married people (56%) had a tendency to develop depression compare to unmarried people (36%). Urban residents (64%) are more prone to suffer from depression than rural residents (36%). There was mild depression in 28% (68), moderate in 37% (157), and severe in 14% (34) patients. Correlation analysis shows that age, sex, marital status, education, family income, and area of residence (p-values 0.004, 0.001, 0.000, 0.011, 0.000 and 0.000, respectively) are associated with the severity of disease. Background: Major Depressive Disorder (MDD) is a mental disorder characterized by a pervasive and persistent low mood which is accompanied by low self-esteem and loss of interest or pleasure in day to day activities that adversely affects a person’s family, work, and personal life. There is no sufficient laboratory test for the diagnosis of MDD and it is expected that this investigation may be helpful for better diagnosis and management of MDD. We aimed to measure serum immunoglobulin levels in MDD patients and control subjects to meet the above demand. Methods: For this purpose, we recruited 88 MDD patients from the department of psychiatry, Bangabandhu Sheikh Mujib Medical University, Dhaka and 89 healthy volunteers from Dhaka city matched with age, sex and socioeconomic status to the patient group. Turbidimetry method was applied to measure serum levels of immunoglobulin A, G, and M where immunoglobulin kit was utilized. Results: The current study revealed that mean serum concentrations of immunoglobulin A, G, and M in patients were found to be 209.07 ± 104.93, 791.50 ± 235.67 and 107.92 ± 47.53 mg/dL while those were 195.34 ± 92.16, 763.81 ± 175.89 and 99.17 ± 48.78 mg/dL in control subjects, respectively. Conclusion: Our result indicates that serum concentrations of immunoglobulin A, G and M were not significant between the groups and further studies are required to establish these findings.

This pilot study examined the psychometric properties and clinical utility of a brief neuropsychological instrument (Repeatable Battery for the Assessment of Neuropsychological Status (RBANS). The test-retest reliability, practice effects and convergent validity of RBANS were examined in participants without objective cognitive impairment. The tests were administered at two time points at approximately a two weeks’ interval, with 30 cognitively intact participants with a mean age of 63.3 ± 5.8 years. Adequate test-retest reliabilities were found for RBANS subtests, index and total scale scores with significant gain scores in immediate memory and visuospatial function. The RBANS showed good convergent validity and the RBANS supplemented with executive and language measures (Colour Trails Test and 30-item modified Boston Naming Test, respectively) demonstrated excellent convergent validity with a formal neuropsychological battery. This pilot study has provided the preliminary evidence of reliability and convergent validity of the RBANS. Additionally, it also provides insight on the practice effects so that clinicians may assess significant changes in RBANS subtests and domain indexes for clinical practice.

治疗药物监测(Therapeutic drug monitoring,TDM),如通过定量测定血清或血浆药物浓度指导用药剂量优化,已经成为对患者进行精神药物治疗的很有价值的工具。在患者用药依从性难以判断、药物耐受性不佳、治疗剂量下无效以及可能存在药代动力学药物-药物相互作用等情况下,测定药物浓度是很有用的。在精神科,有可能明显获益于TDM的主要患者群体包括儿童、孕妇、老年患者、智力障碍患者、涉及司法的患者、已知或怀疑携带药代动力学相关基因变异的患者,以及合并躯体疾病影响药代动力学的患者。然而,只有将TDM充分整合到临床治疗过程中去,才能发挥其优化药物治疗的潜在优势。为了促进TDM的合理应用,神经精神药理学与药物精神病学协会(Arbeitsgemeinschaft für Neuropsychopharmakologie und Pharmakopsychiatrie,AGNP)的TDM专家组在2004年发表了精神药物治疗药物监测指南。之后,随着知识不断更新,又有许多可能需要进行TDM的新药上市。因此,本次更新将神经精神药物的种类扩展到了128种,并将其TDM必要性划分为从"强烈推荐"到"可能有用"的四个等级。经过大量细致且全面的文献检索与分门别类的汇总整理,将基于循证医学理念的"治疗参考浓度范围"和"剂量相关参考浓度范围"呈现给大家。本共识指南引入了"实验室警戒浓度"的新概念,即实验室需要马上告知治疗医生的药物浓度上限。本共识指南还给出了诸如药物作为细胞色素P450酶的底物和抑制剂的性质,代谢物与母药浓度比值的常见范围,以及与结果解释相关的内容,还提供了何时将TDM与遗传药理学检测相结合的建议。遵循本指南,有助于改善许多患者精神药物治疗的效果,特别是那些存在药代动力学异常的患者。TDM是一门交叉学科,有时针对看起来不一致的数据,需要多学科坦诚地讨论,只有这样,患者才能从这种合作中获益。

Relationships between clozapine and norclozapine plasma concentrations, clozapine dose, and clinical response in Tunisian patients with schizophrenia-treatment resistance

The present study investigated relationships between clozapine dose, clozapine and norclozapine plasma concentrations, and clinical responses to clozapine treatment in Tunisian schizophrenics. Fourteen schizophrenia-treatment resistant patients, recruited for this study, were treated with clozapine for 45 days. Patient health improvement was assessed before and after each cycle of two weeks of clozapine therapy, using the Brief Psychiatric Rating Scale (BPRS). Plasma clozapine and norclozapine concentrations were determined by high-performance liquid chromatography (HPLC). No significant correlations between plasma clozapine and norclozapine concentrations and clinical health improvement among our schizophrenic patients were found. However, a significant correlation was observed between clinical health improvement given by BPRS scores and norclozapine plasma concentration to daily clozapine dose ratio (NCZ/D). Despite the small sample size of our study, our findings suggest that the clozapine therapy response variations observed in our patients may be, in part, explained by the interindividual differences in plasma norclozapine concentration to clozapine dose ratio (NCZ/D). So the NCZ/D parameter could be used as a good indicator for adjusting the clozapine dose-adaptation strategy and consequently for improving the clinical psychopathological state of schizophrenia-treatment resistant patients.

Ego strength and health: An empiric study in hemodialysis patients

Introduction: Ego strength represents an important variable that could be predictable about health and compliance in chronic diseases. In this study we propose a new questionnaire, E.F.E. “Ego Functioning Experience”, able to reveal the psychological functioning profile in Hemodialysis patients. The aim of this work, is to underline the existing relations between emotional profile and compliance, whit E.F.E., that may be predictive of a state of psychologycal health in hemodialysis patients. Methods: Study population included 90 hemodialysis patients. The study protocol was made by three psychological tests: the E.F.E. Questionnaire, the DMI, the Self-Liking and Self-Competence Scale. Results: Factor analysis extracts three factors: 1) “Need of Ego support”;2) “Ego activity oriented to treatment”;3) “Ego strength”. The correlation coefficients between the E.F.E. Questionnaire factors (three factors) and other measures, showed a direct correlation with the first factor and “turning against object” TAO of the DMI test (correlation is significant at the 0.001 level) and an inverse relation with set mechanisms of “Principalization”, PRN of the DMI (correlation is significant at the 0.005 level). The correlations of the second and the third factors with other measures, were not statistically significant. No relations were found about other variables. Conclusions: The E.F.E. questionnaire is a simple evaluation to detect hemo-dialysis patients who may need greater attention to the psychological health and therefore the need for treatment such as psychological support. In fact, lower presence of ego strength is indicative of poor compliance to clinical treatment in hemodialysis, but also of worsening of psychiatric symptoms such as demoralization and depressed mood. In conclusion, an increased social support is needed in hemodialysis patient in order to achieve better compliance and achieve a better state of psychological health in chronic hemodialysis patients.
