Background: To investigate the effects of the association of semi-ablative fractional radiofrequency (RFF) with growth factors in stretch marks. Methods: This is a randomized controlled trial. The sample consisted of 28 female patients with complaints of gluteus or abdomen striae and divided into 2 equitable groups, named G1 (semi-ablative RFF without active principle association) and G2 (semi-ablative RFF with active principle association), being subdivided into G-1A and G-2A (abdominal applications) and G-1B and G-2B (gluteus applications). A photographic and histological analysis was performed using questionnaires about inflammation, presence of adverse effects and satisfaction of volunteers. Photographic analysis showed a reduction in the amount of striae alba in all groups. Results: The histological analysis showed an increase in fibroblast activity, collagen type I, and blood vessels, especially in the groups that associated the RFF with the active principle. In the inflammation analysis, the groups that performed intervention with active ingredients showed lower inflammatory signal. All groups had a reasonable level of satisfaction after the interventions;however, the group that used active ingredients reported greater results. Conclusion: The association of semi ablative RFF with growth factors showed a reduction in clinical inflammatory signs, mainly in the first 15 days, as well as higher collagen production, verified in 30 days, with satisfactory clinical results in all groups.

Wounds are a serious public health problem in our country. Access to effective, low-cost treatment is still far from the reality for many patients leading to a chronic and serious condition. They interfere in the quality of life of these individuals who isolate themselves socially constrained by the appearance of their wounds. The report shows the case of a patient with type II diabetes mellitus, with chronic wounds in lymphedema in the lower limbs who was treated with the Scar Acceleration Methodology associated with Complex Decongestive Therapy, leading to an improvement in her social participation and quality of life.

The effects of Park sham needles: a pilot study

BACKGROUND： When performing clinical trials involving acupuncture, it can be difficult to choose a placebo control. OBJECTIVE： To validate the Park sham needle for use as a double-blind control intervention. DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS： Two different methods were employed. First, a researcher blinded to needle type administered the Park sham or a real needle, chosen at random, on the arms of 16 healthy volunteers. The researcher and the volunteers independently recorded which needle type they thought had been applied at each acupuncture point. Second, 19 patients with shoulder impingement syndrome were randomly assigned to receive acupuncture applied with the real needle or the Park sham needle, once a week for 6 weeks alongside a course of physiotherapy. At the end of the 6 sessions the patients recorded which needle type they thought was being used. The results were analyzed with a Fisher＇s exact test. This study was carried out in the Outpatient Department of physiotherapy in Chafing Cross Hospital, London, UK, on healthy volunteers and patients with shoulder impingement pain. The age range was 23 to 54 and 22 to 74 years respectively. RESULTS： Of the healthy volunteers, there was no difference （P=0.23） between the number of needles that were correctly （n=43） or incorrectly identified （n=53）. All patients thought that they had received the real needles. The researcher correctly identified all needles that were applied. CONCLUSION： The researcher delivering the acupuncture recognized the needle type. However, both healthy and patient volunteers were blind to the needle type. This demonstrates that the Park sham needle is an effective single-blind control. It should be noted that the number of patients recruited was small and the study was underpowered to detect an effect of treatment.

Vulvovaginal candidiasis is an infection of the genital mucosa, which involves the vulva and vagina caused by yeast. It is considered a recurrent pathology and a public health problem that causes discomfort by the triggers and that, when left untreated, can lead to health problems. The present study is a report of two cases treated with the MAC® - Scar Acceleration Method - (MAC®) methodology at the physiotherapy School Clinic of Teaching Center of Campos Gerais (CESCAGE) as a pilot for the proposal to implement the protocol at the Municipal Center for Women’s Health—CMM in the municipality of Ponta Grossa—PR, Brazil. The patients were referred by Basic Health Units in the city with complaints of pruritus, vaginal discharge and local burning with the diagnosis of candidiasis evidenced by culture and antibiogram. Patients were treated with photodynamic therapy using the MAC® method for 14 consecutive days with 660 nm red laser phototherapy, 100 mW of power, 9 joules of energy, photosensitizing the drug Turmeric Long in one of the participants and propolis in the other, using 1% methylene blue dye. Intensive care units’ acquired muscle weakness is present in approximately 50% of the patients. Although active muscle training can attenuate weakness, a large proportion of critical patients cannot participate in any active mobilization. Neuromuscular electrical stimulation may be an alternative strategy to reverse muscle weakness. The objective of the study was to review the scientific publications on the use of neuromuscular electrical stimulation and its parameters and the main results in patients hospitalized in intensive care units. This is an integrative review surveying studies in online databases. The studies were selected from the following descriptors: neuromuscular electrical stimulation AND parameters AND intensive care units AND muscle weakness. The inclusion criteria included articles that addressed the topic of neuromuscular electrical stimulation and the parameters used in patients admitted to intensive care units, aged 18 years or older. Exclusion criteria were studies involving animals, case reports, letters to the editor and book chapters. The search comprised articles in the Portuguese, English and Spanish languages from January 2013 to March 2019. Of the 185 articles identified, nine met the eligibility criteria. The studies were evaluated assessing the level of evidence, and the relevant information was presented in the table and discussed. The parameters of the neuromuscular electrical stimulation employed in the studies showed positive results for the maintenance of strength and muscle mass. There was evidence of benefits in the local and systemic microcirculation, potentially mobilizing endothelial stem cells, to prevent atrophy, to reduce mechanical ventilation time and stay in intensive care unit;and when incorporated into the usual physiotherapy care, proved to be more effective than usual care. Its use is safe and viable in critically ill patients.

Effect of bloodletting therapy at local myofascial trigger points and acupuncture at Jiaji(EX-B 2) points on upper back myofascial pain syndrome: a randomized controlled trial

OBJECTIVE: To observe the clinical efficacy of bloodletting therapy and acupuncture at Jiaji points for treating upper back myofascial pain syndrome(MPS), and compare this with lidocaine block ***: A total of 66 upper back MPS patients were randomly assigned to either the treatment group or the control group in a 1∶1 ratio. The treatment group(n = 33) were treated with bloodletting therapy at local myofascial trigger points and acupuncture at Jiaji(EX-B 2) points; one treatment course consisted of five, single 20-min-treatments with a 2-day break between each treatment. The control group(n = 33) were treated with a lidocaine block at trigger points; one treatment course consisted of five sessions of lidocaine block therapy with a 2-day break between each session. The simplified Mc Gill Scale(SF-MPQ) and tenderness threshold determination were used to assess pain before and after a course of ***: After the third and fifth treatment, the SF-MPQ values were significantly decreased(P 0.05). There were five cases with minor adverse reactions reported in the control patients, while no adverse reactions were reported in the treatment ***: Bloodletting therapy at local myofascial trigger points and acupuncture at Jiaji points was effective in treating upper back ***, bloodletting and acupuncture therapy had the same efficacy as the lidocaine block therapy, with fewer adverse reactions.

Patellar Tendinopathy: A Physiotherapist’s Perspective

The pathology of patellar tendinopathy and the mechanisms of pain are poorly understood. The most popular treatment modality is the Eccentric overload Exercise. However, there is insufficient evidence to recommend a specific protocol and yet it is the most commonly used treatment modality. The aim of this case study is to present the possible mechanisms of pain and a treatment approach on a patient who was diagnosed as having patellar tendinopathy. It is suggested that hyper extension of the knee might be the source of patellar tendon pain in this patient. Following a course of pulsed short wave diathermy the patient was symptom free and remained so at 6 month review. Further studies to evaluate effectiveness of pulsed short wave diathermy is suggested.

TREATMENT OF 150 CASES OF FACIAL PALSY WITH ACUPUNCTURE PLUS TDP IRRIDIATION

In the present paper, the author summarizes the results of treatment of 150 cases of facial palsy with acupuncture plus TDP irradiation. Main acupoints used are Baihui (GV 20), Yintang (EX HN 3), Sibai (ST 2), Dicang (ST 4) and Hegu (LI 4). After insertion, the needles are retained but not manipulated, and the acupoints are irradiated with TDP for 20-60 min every time. The treatment is conducted once daily, with 10 sessions being a therapeutic course. Following 6-28 sessions of treatment, of the 150 cases, 145 are cured, 2 experience remarkable improvement and 3 have improvement, with the total effective rate being 100%. The fact indicates that acupuncture plus TDP irradiation is rather effective in the treatment of facial palsy.

COMBINED TREATMENT OF 198 CASES OF INSOMNIA WITH ACUPUNCTURE OF BODY ACUPOINTS PLUS PELLET PRESSING OF OTOPOINTS

Objective: To observe the therapeutic effect of acupuncture of body acupoints plus otopoint pellet-pressing for treatment of insomnia. Methods: A total of 198 cases of insomnia patients were observed. Straight inserted the filiform needles into Shenmen (HT 7), Neiguan (PC 6), Zhaohai (KI 6) and Zusanli (ST 36) and manipulated the needles with uniform reinforcing-reducing method, then otopoints as Ershenmen (MA-TF 1), Shen (MA-SC), Gan (MA-SC 5), etc. were stimulated by pressing the stuck vaccaria seeds. Ten treatments were considered as a therapeutic course. Results: Of the 198 cases, 65 (32%) were cured, 76 (38%) had remarkable improvement, 53 (26%) had improvement and 4 (2%) had no effect, with the total effective rate being 97%. Conclusion: Acupuncture of body acupoints plus otopoint pellet pressing works well in treatment of insomnia.
