随着新生儿微创辅助通气时代的到来及"开肺策略"的提出,新生儿的无创通气技术理念在世界范围内的NICU得到普及。该文复习了近年来新生儿无创通气技术的有关文献并作简要综述。持续气道正压通气(CPAP)主要原理为在呼气末予以压力支持的一种通气模式,又称之为持续扩张压或呼气末正压通气,经过世界范围内多年的临床实践,CPAP已被证实是一种有效的对患有呼吸系统疾病的新生儿实施呼吸支持的辅助通气模式。由于CPAP价格相对便宜,且易于在临床应用,因此更适合于在发展中国家推广。CPAP辅助通气,除可显著地改善患儿机体的氧合外,对有不同程度气道阻塞的患儿,它可以减轻气道塌陷,因此有助于减少患儿呼吸暂停的发生。文献研究表明,对于32周以下的早产儿,早期应用CPAP可减少患儿气管插管机械通气的机率。该文将试图介绍几种不同的CPAP应用装置和几种不同的CPAP应用方法。虽然对于早期应用CPAP是否能够降低新生儿死亡率及患病率目前尚不十分清楚,但临床实践已经表明,与有创通气比较,CPAP对患儿的损伤小,是一种较为舒适的通气模式,早期应用CPAP还可减少肺泡表面活性物质的使用频率。另外,该文还介绍CPAP的应用指征、优点及其局限性,CPAP的衍生模式如经鼻间歇正压辅助通气以及infant flow CPAP装置等。

ZINC DEPLETION IN SEVERE PRE-ECLAMPSIA

＜正＞The renin-angiotensin system plays an important role in cardiovascular control. Intracerebroventricular （i.c.v.） angiotensin （ANG） Ⅱcauses a reliable pressor response in the fetus at 90% gestation in utero. To determine roles of brain AT1 and AT2 receptors in this response, the effects of central AT1 and AT2 receptor antagonists, losartan and PD123319, were investigated in chronically prepared near-term ovine fetuses. Losartan at 0.5mg/kg （i.c.v.） abolished central ANG Ⅱ-induced pressor responses. High-dose i.c.v. losartan （5mg/kg） showed a potentiation of the pressor response to i.c.v. ANG Ⅱ, accompanied by bradycardia. Associated with the pressor responses, c-fos expression in the cardiovascular controlling areas was significantly different between the low and high doses of losartan. These areas include the subfornical organ, median preoptic nucleus, organum vasculosum of the lamina terminalis, and paraventricular nuclei in the forebrain, and the tractus solitarius nuclei, lateral parabrachial nuclei in the hindbrain. The low-dose losartan markedly reduced c-fos in these areas after i.c,v. ANG Ⅱ, while the high-dose losartan together with ANG Ⅱ, elicited a much stronger FOS-immunoreactivity in these areas than that induced by i.c.v. ANG Ⅱalone. This is a novel finding that c-fos expression in the brain can be inhibited. Central ANG Ⅱinduced fetal pressor responses were not altered by PD123319. These results indicate that i.c.v. losartan at a high and a low dose has strikingly different effects on central ANG Ⅱ-induced pressor responses in fetuses at the late gestation, and that the AT1 mechanism plays an important role in fetal cardiovascular regulation. In summary, the present study is the first to report that i.c.v. losartan at a low-dose can inhibit central ANG Il-induced pressor responses and neural activity in the fetus at the last third of gestation, while a high-doses of losartan enhances i.c.v. 52 cases with severe pre-eclampsia were studied prospectivelyfor the probable relationship between zinc metabolism and pregnancy-induced hypertension. Zinc level of maternal blood in the study group was significantly lower than that in the non-hypertensive control group (P<0.01). A positive correlation was found btween zinc level in cord blood and newborn’s weight. The lower the maternal plasma zinc concentration is, the earlier the development of the disease (r=0.76, P<0.001); the larger the amount of protein in urine and the lower the zinc level in the cord blood of their fetuses will be. The disease would affect maternal zinc level and improvement of maternal zinc status might benefit the augmentation of zinc levels in the fetuses. It is only when the exhaustion status of zinc to some degree that the growth of the fetus would be involved.

The Spontaneously Diabetic Torii (SDT) rat is a novel model for nonobese type 2 diabetes. In this study we investigated the glycolipid metabolic changes with phlorizin-treatment, which inhibits intestinal glucose uptake and renal glucose reabsorption, in male SDT rats. Phlorizin (100 mg/kg, b.i.d., s.c.) was administered for 4 weeks to SDT rats from 20 to 24 weeks of age. As a result, phlorizin reduced the development of hyperglycemia and decreased the hemoglo-bin A1c (HbA1c) levels. In the liver, phlorizin increased mRNA levels of glucokinase, the enzymes related with the glycogen cascade and the proteins associated with lipid metabolism. In conclusion, chronic administration of phlorizin in SDT rats produced a good glycemic control and an improvement in liver function.

Objectives: The objectives of this study were to evaluate and compare perinatal outcomes and frequency of congenital malformations in pregnancy that are complicated with type 1 and type 2 diabetes. Study Design: This prospective study included 557 pregnant women with type 1 diabetes and 149 pregnant women with type 2 diabetes that gave birth in the Clinic for Diabetes in Pregnancy in Zagreb, from January 2000 to December 2012. Results: Women with type 2 diabetes were significantly older than women with type 1 diabetes (p perinatal mortality was higher in pregnant women with type 1 diabetes than in women with type 2 diabetes (4 versus 2) but not statistically significant. Conclusion: Comparing perinatal mortality and frequency of congenital malformations in women with type 1 and type 2 diabetes, we didn’t find any important statistical differences. There is no

Objective To compare the efficacy and safety of medical abortion of different regimens for termination of pregnancy at 8-16 weeks of gestation. Methods Healthy pregnant women requesting medical abortion at 8-16 weeks＇ gestation within 12 hospitals in Shanghai were randomly allocated to four treatment groups. Three intervention groups were given mifepristone 200 mg as a single dose then 24 h later misoprostol 600 #g at 3 h intervals vaginally, orally or vaginally followed by orally, respectively. Control group was given mifepristone 100 mg for 2 d, followed at 48 h by initiation of misoprostol 600 μg vaginally every 12 h. The primary outcome measures were the successful abortion rate, the induction-to-abortion interval, vaginal bleeding and side effects. Results Efficacy outcomes were analyzed for 1 112 women （92.67%）, excluding 88 protocol violations. Termination successful rates were similar among the four groups from 97.1% to 97.8%. The average dose of misoprostol and the incidence of side effects in control group were lower than those in three intervention groups. Stratified analysis showed that the interval of induction-to-abortion at gestation of 1116 weeks was decreased in control group. Conclusion The four regimens have the similar termination successful rates in spite of different administration intervals or routes. Control group was recommended for the advantages of reduced dose of misoprostol and fewer side effects.
