Comparative Evaluation of Extra Capsular Extraction (ECE) with Suture and Small Incision Cataract Surgery (SICS) Manual without Suture in the university hospital of bouake (Ivory Coast)

Objective: Cataract is a partial or total opacification of the lens responsible for blindness. The treatment is surgical. The purpose of our study was to evaluate the results of the newly introduced manual small incision cataract surgery (SICS) and compare them with those of the conventional extra-capsular extraction method. Patients and Method: Analytical cross-sectional study was carried out at the ophthalmology department of the university hospital of bouaké for a period of 1 year. Results: The two surgical techniques were compared at equal proportions. The average duration of interventions was longer during the sutured Extra Capsular Extraction (ECE) compared to the SICS. Rupture of the posterior capsule with vitreous exit, which was the most observed intraoperative complication, was predominant with ECE with suture (16%) against 8% for SICS. Corneal edema observed in the early postoperative period, was predominant with sutured ECE (30%), against 20% for SICS. Corneal edema, which was more observed with EEC compared to SICS, dominated secondary and late postoperative complications. Post-operative astigmatism induced by ECE with suture was between ?2 and ?4 dioptre in 51.5% of patients and between ?0.5 and ?2 dioptre in 73.91% of SICS. Visual acuity without correction in the early days was better with SICS. Conclusion: In this comparative study of cataract surgery, we noted superimposable results for both techniques. However, the prevalence of pre- and post-operative complications was more frequent with sutured ECE. The technique without suture allows visual recovery and a quick return to normal life.

Citrin deficiency presenting as acute liver failure in an eight-month-old infant

Citrin deficiency typically presents as neonatal intrahepatic cholestasis and resolves in late *** we report a case of citrin deficiency that presented as acute liver failure in late infancy in an apparently healthy child. The full-term male infant weighed3400 g at birth, and exhibited normal development for eight months, at which time he contracted bronchial pneumonia. The infant developed jaundice and laboratory tests indicated elevated bilirubin and ammonia levels and an abnormal coagulation *** amino acid analysis showed elevated levels of tyrosine, methionine, citrulline, and arginine. Citrin deficiency was suspected, and genomic DNA analysis revealed a mutation(IVS16ins3kb) in SLC25A13, which encodes a mitochondrial aspartate-glutamate carrier protein. The infant was immediately put on a lactosefree,medium-chain-triglyceride-enriched formula with ursodeoxycholic acid and lipid-soluble ***, cholestasis and abnormal laboratory indices persisted, and the infant died at the age of 11.5 mo,two days before a scheduled liver transplantation. This case demonstrates that citrin deficiency can present in late infancy as acute liver failure triggered by infection,and may require liver transplantation.

Mitogen-activated protein kinase phosphatase 1 protects PC12 cells from amyloid beta-induced neurotoxicity

The mitogen-activated protein kinase（MAPK） signaling pathway plays an important role in the regulation of cell growth, proliferation, differentiation, transformation and death. Mitogen-activated protein kinase phosphatase 1（MKP1） has an inhibitory effect on the p38 MAPK and JNK pathways, but it is unknown whether it plays a role in Aβ-induced oxidative stress and neuronal inflammation. In this study, PC12 cells were infected with MKP1 sh RNA, MKP1 lentivirus or control lentivirus for 12 hours, and then treated with 0.1, 1, 10 or 100 μM amyloid beta 42（Aβ42）. The cell survival rate was measured using the cell counting kit-8 assay. MKP1, tumor necrosis factor-alpha（TNF-α） and interleukin-1β（IL-1β） m RNA expression levels were analyzed using quantitative real time-polymerase chain reaction. MKP1 and phospho-c-Jun N-terminal kinase（JNK） expression levels were assessed using western blot assay. Reactive oxygen species（ROS） levels were detected using 2′,7′-dichlorofluorescein diacetate. Mitochondrial membrane potential was measured using flow cytometry. Superoxide dismutase activity and malondialdehyde levels were evaluated using the colorimetric method. Lactate dehydrogenase activity was measured using a microplate reader. Caspase-3 expression levels were assessed by enzyme-linked immunosorbent assay. Apoptosis was evaluated using the terminal deoxynucleotidyl transferase d UTP nick end labeling method. MKP1 overexpression inhibited Aβ-induced JNK phosphorylation and the increase in ROS levels. It also suppressed the Aβ-induced increase in TNF-α and IL-1β levels as well as apoptosis in PC12 cells. In contrast, MKP1 knockdown by RNA interference aggravated Aβ-induced oxidative stress, inflammation and cell damage in PC12 cells. Furthermore, the JNK-specific inhibitor SP600125 abolished this effect of MKP1 knockdown on Aβ-induced neurotoxicity. COVID-19 in bouake (Côte D’Ivoire): Comparative Study of the Epidemiological, Clinical, Therapeutic, Evolutionary and Prognostic Aspects in Younger and Elderly Patients

Introduction: The clinical manifestations of many diseases vary with age, and older people often do not show typical symptoms of the disease. The present study aims to compare the epidemiological, clinical, therapeutic and evolutionary aspects of elderly patients with COVID-19 compared to young adult patients and to identify risk factors for mortality. Patients and Methods: This was a retrospective single-center analytical study conducted from January 27, 2021 to January 27, 2022 at the COVID-19 Care center of the Infectious and Tropical Diseases department of the bouaké university Teaching hospital. The study population consisted of all patients aged at least 18 years seen in consultation and/or hospitalized with a positive COVID-19 RT-PCR. The patients were divided into two groups: younger patients (Results: Of a total of 779 patients, 644 (82.7%) were young. The median age of all patients was 41 years (IQR 32 - 54, extreme 18 and 96). Of all the patients 38.5% had a comorbidity. Compared to younger patients, chronic heart disease (61.7% vs 21.5%;p Conclusion: Elderly people with COVID-19 have a different clinical presentation from younger, characterized by more atypical symptoms. Mortality risk factors are age, dyspnoea, impaired consciousness and fever. It is therefore necessary to act on its various factors to improve the prognosis of COVID-19 in this age group.

Background:Cyclosporine A and tacrolimus(TAC)are often used as a second-line treatment for children with refractory nephrotic syndrome(NS).This study was undertaken to investigate the efficacy and safety of Tacrobell®,a locally produced generic form of ***:This study was a one-year prospective,open-label,single-arm,multicenter ***-four children with steroid-dependent NS(SDNS)and 33 children with steroid-resistant NS(SRNS)were *** primary endpoints were defined as the remission rates,whereas the secondary endpoints were recognized as the duration of remission and adverse effects of ***:After one-year treatment,34(77.3%)of the 44 patients with SDNS were in complete remission,and 6(13.6%)were in partial ***(43.2%)patients did not relapse during the study;for those who did relapse,the mean duration of remission was 4.6±2.9 *** number of relapse episodes during the study period(0.90 per patient-year)was significantly lower than that in the preceding year(2.8 per patientyear).After treatment for 3 and 6 months,12(36.4%)of the 33 patients with SRNS were in remission,and after treatment for 12 months,the number of patients had increased to 13(39.4%).The mean time to achieve remission was 4.0±3.2 *** remission(duration,3.7±2.7 months),12(54.5%)of 22 patients *** fasting blood glucose and blood pressure levels during the therapy were similar to those at the time of study ***:Treatment with Tacrobell®was effective and safe for children with refractory *** efficacy of this generic form of TAC was better than that of the original TAC formula.

Syncope is a common emergency of children and adolescents. The reoccurrence of syncope seriously affects children's physical and mental health. The pathogenesis, etiology, diagnosis and treatment of pediatric syncope differ from those in adults. Most pediatric syncope is relatively transient and benign in nature. But cardiac syncope, although the incidence is relatively low as compared with the neurally-mediated syncope (NMS), poses high risks of sudden death and needs to be quickly identified and managed.

ARC syndrome with high GGT cholestasis caused by VPS33B mutations

Arthrogryposis, renal dysfunction and cholestasis (ARC) syndrome (OMIM 208085) is an autosomal recessive disorder that is caused by mutations in 2 interacting genes VPS33B and VIPAS39. Mutations in VPS33B gene account for most cases of ARC. As low or normal gamma-glutamyl transpeptidase (GGT) activity has been described in all patients with ARC syndrome identified so far, ARC syndrome is a possible diagnosis for low GGT cholestasis. Here we describe a Chinese patient with neonatal cholestasis and a high GGT level in three consecutive tests. She had other typical manifestations of ARC syndrome, including arthrogryposis multiplex congenita, renal involvement and ichthyosis. Genetic study of the VPS33B gene further confirmed the diagnosis by identification of compound heterozygosity of two known disease-causing mutations, c.403+2T > A and c.1509-1510insG. The mechanism of high GGT in this patient is unclear. Nevertheless, this case indicates that ARC syndrome cannot be excluded from the differential diagnosis of neonatal cholestasis even if high GGT activity is found.

BACKGROUND Abnormalities in the melanocortin receptor 4(MC4R)gene often lead to obesity,but are rarely associated with other conditions such as epilepsy and sleep *** SUMMARY Here,we present a case of a male obese child with a heterozygous variant in MC4R(c.494G>A,***165Gln)inherited from his father,who presented with disordered sleep and abnormal facial *** through melatonin rhythm testing and electroencephalography led to a diagnosis of sleep disorder and epilepsy,as his melatonin rhythm was markedly distorted and the electroencephalography revealed epileptic *** received treatment with an antiepileptic drug;however,the therapy was ineffective and the sleep disorder appeared to be ***,we initiated adjuvant treatment with *** re-examination,his body mass index had decreased,the sleep disturbance had resolved,and his seizures were well *** review was normal,and a typical melatonin rhythm was *** We concluded that,in addition to causing obesity,abnormalities in the MC4R gene may contribute to the development of sleep disorders and epilepsy,and that melatonin can be used as an adjuvant therapy to alleviate these symptoms.

Introduction: Neonatal mortality remains a major public health concern. According to the World Health Organisation (WHO), the number of newborns dying each year has fallen from 5 million in 1990 to 2.4 million in 2019. The aim of our study was to describe the main morbid conditions and their lethality in newborns admitted to the paediatric ward of the CSREF in Commune V. Methodology: We conducted a 12-month prospective descriptive and analytical study from August 2020 to July 2021. Results: During the 12 months of the study, out of 2654 neonates admitted to hospital, 216 met our inclusion criteria, i.e. 8%. Newborns admitted in the first few hours of life represented 99.54% of cases. Full-term newborns accounted for 65.74% of the total. Forty-eight percent of newborns had hypothermia on admission. The most common diagnosis was asphyxia in 55.56% of cases, followed by neonatal infection in 27.78% and prematurity in 10.65%. Death was recorded in 22.2% of cases. Asphyxia was the main cause of neonatal death, followed by prematurity in our context. Conclusion: Improving the prognosis of newborn babies will require a thorough understanding of neonatal pathologies and the implementation of a pre- and perinatal prevention policy.
