目的:探讨儿童NTRK重排间叶源性肿瘤临床和病理学特征,以提高对该类疾病的认识。方法:收集上海交通大学医学院附属上海儿童医学中心及新加坡KK Women′s and Children′s hospital从2017年1月至2019年9月5例手术切除标本,采用EnVision法检测免疫组织化学表型,荧光原位杂交(FISH)检测相关基因并对NTRK基因重排进行克隆性分析。结果:该组5例患儿,3例男性,2例女性。年龄从3个月到13岁,部位包括软组织(膝、胸腔、腹壁)和肾,肿瘤大小为4.5~12.5 cm。组织学检查主要呈梭形细胞肿瘤,浸润性生长,可伴有炎性细胞。免疫表型上,肿瘤细胞阳性表达Pan-TRK,分子检测均存在NTRK基因重排,包括DCTN1-NTRK1的发现。除2例目前接受靶向药物治疗,其余均为无疾病进展病例,随访时间9~29个月。结论:NTRK重排间叶源性肿瘤,其肿瘤发生部位广泛,组织学图像多变。免疫组织化学Pan-TRK可以帮助诊断,NTRK基因检测确认其存在重排是诊断的金标准,对不能完整切除或有复发、转移病例建议靶向药物治疗。

Purpose of the study: Research of the clinical and diagnostic significance of determination of Lipocalin-2 associated with neutrophilic gelatinases (uNGAL) in the urine of children with urinary tract infection (UTI) and pyelonephritis. Materials and methods: We examined 30 children with acute pyelonephritis and UTI aged 1 to 16 years (average age 7.32 ± 4.52) including 26 girls and 4 boys. Verification of the diagnosis was conducted on the basis of clinical and laboratory data, medical history and instrumental examination of patients. All children were divided into 2 groups: 1st group—15 children with acute pyelonephritis, 2nd group—15 children with urinary tract infection. uNGAL was measured in the urine by enzyme-linked immunosorbent assay (EISA) (BioVendor Laboratoty Medicine). Results: It is found, that the urine level of NGAL depends on the damage degree of renal parenchyma. The correlation of medium strength was found between the excretion level of uNGAL during the acute period of pyelonephritis and the detection of renal scars according to the DMSA-nephroscintigraphy data. In the group of children with the acute pyelonephritis the direct correlation of medium strength was found between the excretion level of uNGAL/creatinine and leukocytosis value and also with the CRP blood level. Conclusion: The results allow us to recommend the determination of the excretion level of uNGAL/creatinine as an additional non-invasive marker for the early detection of renal parenchyma injury.

Background:New-onset diabetes after transplantation(NODAT)is a serious complication following liver transplantation(LT).The present study aimed to investigate the incidence of and risk factors for NODAT using the Korean Organ Transplantation Registry(KOTRY)***:Patients with history of pediatric transplantation(age≤18 years),re-transplantation,multi-organ transplantation,or pre-existing diabetes mellitus were excluded.A total of 1,919 non-diabetic adult patients who underwent a primary LT between May 2014 and December 2017 were *** factors were identified using Cox regression ***:NODAT occurred in 19.7%(n=377)of adult liver transplant *** analysis showed steroid use,increased age,and high body mass index(BMI)in recipients,and implantation of a left-side liver graft was closely associated with NODAT in adult *** living donor liver transplant(LDLT)patients(n=1,473),open donor hepatectomy in the living donors,steroid use,small for size liver graft(graft to recipient weight ratio≤0.8),increased age,and high BMI in the recipient were predictive factors for *** use of antimetabolite and basiliximab induction reduced the incidence of NODAT in adult LT and in adult ***:Basiliximab induction,early steroid withdrawal,and antimetabolite therapy may prevent NODAT after adult *** BMI or advanced age in liver recipients,open donor hepatectomy in living donors,and small size liver graft can predict the occurrence of NODAT after adult LT or LDLT.

Effects of the combination of mask preconditioning with midazolam pretreatment on anxiety and mask acceptance during pediatric inhalational induction and postoperative mask fear in children

Background Anxiety and fear frequently causes an aversion to applying a face mask and increases difficulty during pediatric induction. There is at present little study of this problem. Therefore, the aim of this study was to investigate the effect of the combination of mask preconditioning and midazolam pretreatment on mask acceptance during pediatric induction and on postoperative mask fear. Methods One hundred and sixty children were randomly assigned into four groups： the mask preconditioning group （MaG）, the midazolam pretreatment group （MiG）, the mask/midazolam combination group （Ma/MiG）, and the saline group （SAG）. The Modified Yale Preoperative Anxiety Scale （m-YPAS） was employed to assess the anxiety in the operation room （OR）. A Mask Acceptance Score （MAS） was measured during inhalational induction and the incidence of mask fear （MAS 〈2） was evaluated postoperatively. Results The MaG and Ma/MiG groups had the highest mask acceptance scores but there were no differences between these two groups （P 〈0.05）. The average anxiety level of children entering the OR was much lower in the MaG and Ma/MiG groups than in the SaG group （P 〈0.05）. During induction, the anxiety level increased in the SaG and MaG groups but decreased in the MiG and Ma/MiG groups （P 〈0.05）. At the postoperative third day, the incidence of mask fears was as high as 23% in the SaG group, 15% in the MiG group, but only 2.5% in the MaG and Ma/MiG groups. Conclusions The single use of mask preconditioning has a better influence than midazolam for increasing mask acceptance during inhalational induction and reducing postoperative mask fear, reducing the anxiety level during induction, improving induction compliance and shortening the total mask time. A mask preconditioning and midazolam combination did not increase mask acceptance during inhalational induction, reduce mask fears postoperatively, improve induction compliance, nor shorten the total mask time. Respiratory infections in children increase the risk of fatal lung disease,making effective identification and analysis of breath sounds ***,most studies have focused on adults ignoring pediatric patients whose lungs are more vulnerable due to an imperfect immune system,and the scarcity of medical data has limited the development of deep learning methods toward reliability and high classification *** this work,we collected three types of breath sounds from children with normal(120 recordings),bronchitis(120 recordings),and pneumonia(120 recordings)at the posterior chest position using an off-the-shelf 3M electronic *** features were extracted from the wavelet denoised signal:spectrogram,mel-frequency cepstral coefficients(MFCCs),and Delta *** recog-nition model is based on transfer learning techniques and combines fine-tuned MobileNetV2 and modified ResNet50 to classify breath sounds,along with software for displaying analysis *** experiments on a real dataset demonstrate the effectiveness and superior performance of the proposed model,with average accuracy,precision,recall,specificity and F1 scores of 97.96%,97.83%,97.89%,98.89%and 0.98,respectively,achieving superior performance with a small *** proposed detection system,with a high-performance model and software,can help parents perform lung screening at home and also has the potential for a vast screening of children for lung disease.

Background and Objective: Nowadays, Computer Tomography is one of the best radiological imaging technics which can give right diagnostic information, among the detection of multiphasic adenomas, the detection of cardiac, cerebral and vascular abnormalities. Although these good qualities, this technic is too radiant for the patient. In this paper, we based on the irradiation doses delivered from the current protocols to find a practical method of their optimization during the pediatric cranial scan. Materials and Methods: This work relies on a collection of data from patients in the hospitals, so that analyze them, give the conclusions and, propose an optimal practical method to decrease the irradiation doses. To collect data, we performed a prospective study of seventeen months (from December 2017 to May 2019) carried out simultaneously in three hospitals of the city: The Centre Medical la Cathédrale (H1), the Yaoundé Central hospital (H2) and the Yaoundé Gyneaco-Obstetric and pediatric hospital (H3). This study included a total of 192 cases of cerebral trauma, of which 11 cases excluded for incomplete information. The dosimetry quality control (CTDIvol) using the PMMA phantoms of 16 cm and 32 cm fulfilled. The scanographic parameters of the patient acquisition protocol were recorded and analyzed. Some of those parameters were modified and entered the CT with the help of a biomedical engineer to reduce the delivered dose. The relationship between CTDIvol and kV is statistically significant (p Results: Among patients, 172 are boys, and the remaining 9 are girls all were in the 0 to 15 age group. CTDIvol values varied from 34.2 mGy to 107.8 mGy and PDLs from 107.8 *** to 2214.5 *** in H1. In H2, CTDIvol varied from 5.8 mGy to 44 mGy and PDLs from 91.4 *** to 665.5 ***. CTDIvol varied between 9.34 mGy to 92.81 mGy and PDLs from 162.38 *** to 2713.67 *** in H3. All values are taken at 75th percentile, with or without contrast injection. Conclusion: The

Purpose: To report a case of a pediatric corneal limbal dermoid with eyelashes and to describe post-operative changes after excision with reconstruction using amniotic membrane grafting, sutures and fibrinogen-thrombin glue. Case Report: One pediatric patient was identified with a grade II infratemporal corneal-limbal dermoid with conjunctival eyelashes. The dermoid was surgically excised and the cornea reconstructed with amniotic membrane using sutures and fibrinogen/thrombin glue. Preoperative and postoperative measurement of astigmatism, anisometropia and presence of exposure keratopathy were performed.

pediatric cholesteatoma causes extensive destruction within the middle ear and adjacent structures. Despite advancements in surgical techniques, the management of pediatric cholesteatoma remains controversial due to the varied presentations and its rapid progression. This study aims to evaluate the presentation of symptoms, otoscopic findings, surgical outcomes, and functional results in a series of pediatric cholesteatoma patients at our institution. A total of 18 patients, aged 5 - 14 years, underwent canal wall down (CWD) or intact canal wall (ICW) surgery based on the extent of disease as assessed intraoperatively. The study focused on the sites involved by cholesteatoma, the surgical challenges encountered, and the techniques employed. Outcomes measured included the incidence of residual and recurrent cholesteatoma, as well as hearing function at follow-up. Our results indicated a 69% improvement in the air-bone gap (ABG) among patients, with residual perforation observed in 4% of cases and no recurrences during the follow-up period. The increased selection of the CWD technique correlated with the extensive nature of the disease presentation. This study underscores the necessity for individualized treatment plans in pediatric cholesteatoma management, considering the aggressive nature of the disease and the need for a balance between eradication and hearing preservation.
