Feasibility,efficacy,and safety of animal-assisted activities with visiting dogs in inpatient pediatric oncology

Background Childhood cancer entails a heavy burden for patients and their *** advances in overall survival rates have increasingly brought long-term quality of life into ***-assisted activities(AAAs)have long been hypothesized to alleviate the burden on pediatric patients and their peers in the hospital ***,their use in inpatient pediatric oncology has been a sensitive issue mainly due to the fear of infections,resulting in a lack of *** study presents data on the feasibility,safety,and efficacy of AAAs from a single German *** Between 2018 and 2022,60 patients(median age=10.3 years)diagnosed with malignancy and undergoing treatment were visited by an intervention dog(total visits=100).Patients were screened for infections as per hospital policy,with additional microbiological testing performed based on *** dog was screened for human pathogens and *** data and hospitalizations were analyzed from two months prior to the first visit until two months after the last *** of being in the hospital,both with and without planned animal-assisted interventions and pre-and post-intervention state stress,were measured using a validated visual analogue scale(0-10).Results Patients benefited from AAAs,showing increased acceptance of being in the hospital(median:7.25 vs.4.50,Ppediatric oncology are feasible and *** they hold promise for enhancing patients’well-being,further prospective studies are needed.

Chronic hepatitis B in children with or without malignancies:A 13-year follow-up

AIM:To evaluate the outcome of chronic hepatitis B(CHB)in children with or without ***:Twenty four children(15 boys and 9 girls)with malignancies,followed up by the pediatric gastroenterology outpatient clinic for CHB between January 2000 and December 2013,were enrolled in the study(Group 1).Group 2 was formed with twenty five children(11 girls and14 boys)diagnosed with CHB without *** data from the patients’records were compared between the two ***:Hepatitis B e antigen(HBe Ag)/anti HBe seroconversion was observed in 3 patients(12.5%)in group 1 and 15 patients(60%)in group 2,with annual seroconversion rates of 1.61%and 16.6%,respectively,and the difference was significant(P<0.01).One patient(6.6%)in Group 1 and 9 patients(53%)in Group 2 showed HBe Ag/anti HBe seroconversion after treatment and the difference between the two groups was significant(P<0.06)Loss of hepatitis B surface antigen was observed in one patient in each of group1 and *** clinical,laboratory and imaging findings of liver disease were observed in any of the patients at the end of the ***:HBe Ag/anti HBe seroconversion rate was lower in patients who had recovered from cancer.

Total estimated effective doses from radiologic imaging modalities of children with cancer:a single center experience

Germinal-center type B-cell classification and clinical characteristics of Chinese pediatric diffuse large B-cell lymphoma:a report of 76 cases

pediatric diffuse large B-cell lymphoma(DLBCL)is a highly aggressive disease with unique clinical *** study analyzed the germinal-center type B-cell(GCB)classification and clinical characteristics of Chinese pediatric DLBCL.A total of 76 patients with DLBCL newly diagnosed in Sun Yatsen University Cancer Center between February 2000 and May 2011,with an age younger than 18 years,were included in the *** male/female ratio was 3.47:*** median age was 12 years(range,2 to 18 years),and 47(61.8%)patients were at least 10 years *** the 76 patients,48(63.2%)had stage III/IV disease,9(11.8%)had bone marrow involvement,1(1.3%)had central nervous system(CNS)involvement,and 5(6.6%)had bone *** GCB classification was assessed in 45 patients:26(57.8%)were classified as GCB subtype,and 19(42.2%)were classified as non-GCB *** modified B-NHL-BFM-90/95 regimen was administered to 50 patients,and the 4-year event-free survival(EFS)rate was 85.8%.Among these 50 patients,31 were assessed for the GCB classification:17(54.8%)were classified as GCB subtype,with a 4-year EFS rate of 88.2%;14(45.2%)were classified as non-GCB subtype,with a 4-year EFS rate of 92.9%.Our data indicate that bone marrow involvement and stage III/IV disease are common in Chinese pediatric DLBCL patients,whereas the percentage of patients with the GCB subtype is similar to that of patients with the non-GCB *** modified B-NHL-BFM-90/95protocol is an active and effective treatment protocol for Chinese pediatric patients with DLBCL.

Safety of in vitro amplified HLA-haploidentical donor immune cell infusions for childhood malignancies

In vitro amplified human leukocyte antigen(HLA)-haploidentical donor immune cell infusion(HDICI)is not commonly used in ***,our study sought to evaluate its safety for treating childhood *** September 2011 and September 2012,12 patients with childhood malignancies underwent HDICI in Sun Yat-sen University Cancer *** median patient age was 5.1 years(range,1.7-8.4 years).Of the 12 patients,9 had high-risk neuroblastoma(NB)[7 showed complete response(CR),1 showed partial response(PR),and 1 had progressive disease(PD)after multi-modal therapies],and 3 had Epstein-Barr virus(EBV)-positive lymphoproliferative disease(EBV-LPD).The 12 patients underwent a total of 92 HDICIs at a mean dose of 1.6×108immune cells/kg body weight:71 infusions with natural killer(NK)cells,8 with cytokine-induced killer(CIK)cells,and 13 with cascade primed immune cells(CAPRIs);83 infusions with immune cells from the mothers,whereas 9 with cells from the *** cases(21.7%)of fever,including 6 cases(6.5%)accompanied with chills and 1(1.1%)with febrile convulsion,occurred during infusions and were alleviated after symptomatic *** cases(5.4%)of mild emotion changes were *** other adverse events occurred during and after the completion of *** acute nor chronic graft versus host disease(GVHD)was observed following *** a median of 5.0 months(range,1.0-11.5 months)of follow-up,the 2 NB patients with PR and PD developed PD during *** the other 7 NB patients in CR,2 relapsed in the sixth month of HDICIs,and 5 maintained CR with disease-free survival(DFS)ranging from 4.5 to 11.5 months(median,7.2months).One EBV-LPD patient achieved PR,whereas 2 had stable disease(SD).Our results show that HDICI is a safe immunotherapy for childhood malignancies,thus warranting further studies.

Homoharringtonine is a safe and effective substitute for anthracyclines in children younger than 2 years old with acute myeloid leukemia

Homoharringtonine (HHT),a plant alkaloid from Cephalotaxus harringtonia,exhibits a unique anticancer mechanism and has been widely used in China to treat patients with acute myeloid leukemia (AML) since the *** SCMC-AML-2009 presented herein was a randomized clinical study designed based on our previous findings that pediatric AML patients younger than two years old may benefit from HHT-containing chemotherapy *** randomized to arm A were treated with a standard chemotherapy regimen comprising mainly of anthracyclines and cytarabine (Ara-C),whereas patients in arm B were treated with HHTcontaining regimens in which anthracyclines in all but the initial induction therapy were replaced by *** February 2009 to November 2015,59 patients less than 2 years old with de novo AML (other than acute promyelocytic leukemia) were recruited.A total of 42 patients achieved a morphologic complete remission (CR) after the first course,with similar rates in both arms (70.6% vs.72.0%).At the end of the follow-up period,40 patients remained in CR and 5 patients underwent hematopoietic stem cell transplantation in CR,which could not be considered as events but *** 5-year event-free survival (EFS) was 60.2%± 9.6% for arm A and 88.0%± 6.5% for arm B (P=0.024).Patients in arm B experienced shorter durations of leukopenia,neutropenia,and thrombocytopenia and had a lower risk of infection during consolidation chemotherapy with high-dosage ***,the homoharringtonine-based regimen achieved excellent EFS and alleviated hematologic toxicity for children aged younger than 2 years with de novo AML compared with the anthracycline-based regimen.

Lamivudine therapy for children with chronic hepatitis B

AIM： To assess the effectiveness and side-effects of lamivudine therapy for children with chronic hepatitis B （CHB） who fail to respond to or have contraindications to interferon-α （IFN-α） therapy. METHODS： Fifty-nine children with CHB were treated with 100 mg lamivudine tablets given orally once daily for 12 too. Alanine aminotransferase （ALT） activity was evaluated monthly during the therapy and every 3 months after its discontinuation. HBe antigen, antiHBe antibodies, HBV DNA level in serum were evaluated at baseline and every six months during and after the lamivudine therapy. Sustained viral response （SVR） to lamivudine therapy was defined as permanent （not shorter than 6 mo after the end of the therapy）, namely ALl＂ activity normalization, seroconversion of HBeAg to anti-HBe antibodies, and undetectable viral HBV-DNA in serum （lower than 200 copies per mL）. The analysis of the side-effects of the lamivudine treatment was based upon interviews with the patients and their parents using a questionnaire concerning subjective and objective symptoms, clinical examinations, and laboratory tests performed during clinical visits monthly during the therapy, and every 3 mo after the therapy. RESULTS： ALT normalisation occurred in 47 （79.7%） patients between the first and 11^th mo of treatment （mean 4.4±2.95 mo, median 4.0 mo）, and in 18 （30.5%） of them after 2 mo of the therapy. There was no correlation between the time of ALT normalization and the children＇s age, the age of HBV infection, the duration of HBV infection, inflammation activity score （grading）, staging, ALT activity before treatment, serum HBV DNA level, and lamivudnie dose per kg of body weight. HBeAg/anti HBe seroconversion was achieved in 27.1% of *** higher rate of seroconversion was connected with lower serum HBV DNA level and longer duration of HBV infection. Deciphering the role of hedgehog signaling in pancreatic cancer

Pancreatic cancer, mostly pancreatic ductal adenocarcinoma （PDAC）, is a leading cause of cancer-related death in the US, with a dismal median survival of 6 months. Thus, there is an urgent unmet need to identify ways to diagnose and to treat this deadly cancer. Although a number of genetic changes have been identified in pancreatic cancer, their mechanisms of action in tumor development, progression and metastasis are not completely understood. Hedgehog signaling, which plays a major role in embryonic development and stem cell regulation, is known to be activated in pancreatic cancer; however, specific inhibitors targeting the smoothened molecule failed to improve the condition of pancreatic cancer patients in clinical trials. Furthermore, results regarding the role of Hh signaling in pancreatic cancer are controversial with some reporting tumor promoting activities whereas others tumor suppressive actions. In this review, we will summarize what we know about hedgehog signaling in pancreatic cancer, and try to explain the contradicting roles of hedgehog signaling as well as the reason（s） behind the failed clinical trials. In addition to the canonical hedgehog signaling, we will also discuss several non-canonical hedgehog signaling mechanisms.
