Feasibility,efficacy,and safety of animal-assisted activities with visiting dogs in inpatient pediatric oncology

Background Childhood cancer entails a heavy burden for patients and their *** advances in overall survival rates have increasingly brought long-term quality of life into ***-assisted activities(AAAs)have long been hypothesized to alleviate the burden on pediatric patients and their peers in the hospital ***,their use in inpatient pediatric oncology has been a sensitive issue mainly due to the fear of infections,resulting in a lack of *** study presents data on the feasibility,safety,and efficacy of AAAs from a single German *** Between 2018 and 2022,60 patients(median age=10.3 years)diagnosed with malignancy and undergoing treatment were visited by an intervention dog(total visits=100).Patients were screened for infections as per hospital policy,with additional microbiological testing performed based on *** dog was screened for human pathogens and *** data and hospitalizations were analyzed from two months prior to the first visit until two months after the last *** of being in the hospital,both with and without planned animal-assisted interventions and pre-and post-intervention state stress,were measured using a validated visual analogue scale(0-10).Results Patients benefited from AAAs,showing increased acceptance of being in the hospital(median:7.25 vs.4.50,Ppediatric oncology are feasible and *** they hold promise for enhancing patients’well-being,further prospective studies are needed.

Update on Healthcare-Associated Blood Stream Infections in Febrile Neutropenic pediatric oncology Patients

Background: Continuous surveillance of pattern of blood stream infection is necessary in febrile neutropenia especially with the recent escalating trend in the management of pediatric cancer patients towards intensified regimens and with the increase in infections caused by resistant organisms limiting the choice of antibiotics. Aim: Monitoring if a change has occurred in pattern of blood stream infections (BSI) in febrile neutropenic (FN) pediatric cancer patients. Methods: Surveillance of FN episodes with positive BSI was prospectively monitored and compared to a previous surveillance in the same pediatric oncology unit. Results: A total of 232 BSI positive episodes were documented in 192 patients during a 6 months period. The results of recent surveillance analysis showed an increase in intensified regimens of chemotherapy, antimicrobial resistance, and prolonged duration of episodes when compared to previous surveillance, with a p value of <0.001, 0.005, and <0.001, respectively. There was an apparent decrease in the crude mortality but this was not statistically significant, 6% in 2011 and 10% in 2006. Conclusion: The pattern of BSI at our institution is still inclining towards gram positive organisms but is showing a shift towards more antibiotic resistance and prolonged episodes.

Clinico-Pathological Profile of Malignant Abdominal Tumors in Children: Experience of the pediatric oncology Unit in Bamako

Introduction: the objective was to describe the epidemiological and histological aspects of malignant abdominal tumors in children. Methodology: This was a descriptive, cross-sectional study conducted from January 1, 2017 to December 31, 2018. We included all patients aged 0 to 15 years with histologically confirmed abdominal malignancies. Results: We collected 46 files of malignant tumors of the abdomen (35% of pediatric cancers). The majority of patients were under 5 years old (65%). The sex ratio was 1.7. They were generally from disadvantaged socio-economic backgrounds (67%). The average consultation time was 50 days. The main reasons for consultation were abdominal mass (48%), pain (17%) or distension (9%). The mass was large, hard (89%), immobile (71%) and painful (46%). Malnutrition was found in 80% of patients. The mass was retro-peritoneal in 63% of cases and renal in 54%. It was solid in 94% of cases and heterogeneous in 76% of cases. The assay for tumor markers was not performed. Anemia was present in all patients. Fine needle aspiration was performed in 43%, biopsy in 37% and myelogram in 15% of cases. spinal cord involvement was observed in 28% of Burkitt’s cases. The main cancers diagnosed were nephroblastoma (52%), Burkitt’s lymphoma (26%) and neuroblastoma (13%). Stage 1 and stage 2 were frequent in 20% and 22% of cases, respectively;36% of cases were not staged. The average duration of chemotherapy was 11 weeks. Total resection was performed in 30% of cases. The outcome of the patients was marked by total remission in 65%. Conclusion: Abdominal malignancies are very common in children. The diagnosis of certainty is late in our context.

Prevalence and Prognosis of Relapse of Nephroblastoma at the pediatric oncology Unit of Bamako

The nephroblastoma is the third pediatric cancer in Mali, this study aimed to describe the prevalence and prognosis of nephroblastoma relapses. Methods: It was a descriptive retrospective study over a 10-year period from January 2005 to March 2015. We collected children aged 0 to 15 years followed for relapse of nephroblastoma in the pediatric oncology unit of university hospital center (UHC) Gabriel Toure. Results: The frequency of relapse of nephroblastoma was 7.4% (19 cases) whose mean age was 42 months with a sex ratio of 1.3. The relapse occurred before the end of the postoperative course in 16% of cases (3 patients). It was local recurrence in 52% of cases (10 patients), pulmonary 16% (3 patients), and hepatic 11% (2 patients). According to the SIOP classification, 47% of patients were diagnosed in stage III (9 patients) and 21% (4 patients) in stage IV. The tumor was high risk in 37%. Palliative chemotherapy was performed in 63% of the patients (12 patients) and the remaining seven were put on a high risk diet. The overall survival at 5 years was 21% or 4 patients. Conclusion: Our results showed all the difficulties in the management of nephroblastoma relapses in our context.

Although many inflammatory cytokines are prognostic in sepsis,the utility of cytokines in evaluating disease severity in pediatric hematology/oncology patients with septic shock was rarely *** the other hand,a single particular cytokine is far from ideal in guiding therapeutic intervention,but combination of multiple biomarkers improves the *** this prospective observational study,111 episodes of septic shock in pediatric hematology/oncology patients were enrolled from 2006 through *** samples were taken for inflammatory cytokine measurement by cytometric bead array(CBA) technology at the initial onset of septic ***(IL)-6 and IL-10 were significantly elevated in majority of patients,while tumor necrosis factor(TNF)-αand interferon(IFN)-γwere markedly increased in patients with high pediatric index of mortality 2(PIM2) score and *** the four cytokines paralleled the PIM2 score and differentially correlated with hemodynamic disorder and fatal *** pediatric multiplex cytokine score(PMCS),which integrated the four cytokines into one score system,was related to hemodynamic disorder and mortality as well,but showed more powerful prediction ability than each of the four *** was an independent predictive factor for fatal outcome,presenting similar discriminative power with PIM2,with accuracy of 0.83(95%CI,0.71-0.94).In conclusion,this study develops a cytokine scoring system based on CBA technique,which performs well in disease severity and fatality prediction in pediatric hematology/oncology patients with septic shock.

Homoharringtonine (HHT),a plant alkaloid from Cephalotaxus harringtonia,exhibits a unique anticancer mechanism and has been widely used in China to treat patients with acute myeloid leukemia (AML) since the *** SCMC-AML-2009 presented herein was a randomized clinical study designed based on our previous findings that pediatric AML patients younger than two years old may benefit from HHT-containing chemotherapy *** randomized to arm A were treated with a standard chemotherapy regimen comprising mainly of anthracyclines and cytarabine (Ara-C),whereas patients in arm B were treated with HHTcontaining regimens in which anthracyclines in all but the initial induction therapy were replaced by *** February 2009 to November 2015,59 patients less than 2 years old with de novo AML (other than acute promyelocytic leukemia) were recruited.A total of 42 patients achieved a morphologic complete remission (CR) after the first course,with similar rates in both arms (70.6% vs.72.0%).At the end of the follow-up period,40 patients remained in CR and 5 patients underwent hematopoietic stem cell transplantation in CR,which could not be considered as events but *** 5-year event-free survival (EFS) was 60.2%± 9.6% for arm A and 88.0%± 6.5% for arm B (P=0.024).Patients in arm B experienced shorter durations of leukopenia,neutropenia,and thrombocytopenia and had a lower risk of infection during consolidation chemotherapy with high-dosage ***,the homoharringtonine-based regimen achieved excellent EFS and alleviated hematologic toxicity for children aged younger than 2 years with de novo AML compared with the anthracycline-based regimen.
