High birth weight is a risk factor of dental caries increment during adolescence in Sweden

This study aimed to assess whether birth weight is associated with dental caries during the teenage period. In this register-based cohort study, all children of 13 yrs of age (n = 18,142) who resided in the county of Stockholm, Sweden, in 2000 were included. The cohort was followed until individuals were 19 yrs of age. Information regarding dental caries was collected from the Public Health Care Administration in Stockholm. Data concerning prenatal and perinatal factors and parental socio-demographic determinants were collected from the Swedish Medical Birth Register and National Registers at Statistics Sweden. The final logistic regression model showed that birth weight (≥4000 g), adjusted for potential confounders, was significantly associated with caries increment (DMFT ≥ 1) between 13 and 19 yrs of age (OR: 1.29, 95% CI = 1.13 - 1.48). The relatively enhanced risk OR was further increased from 1.29 to 1.52 in sub- jects with birth weight (≥4600 g). On the contrary, subjects with birth weight (<2500 g) exhibited a sig- nificantly lower risk (OR: 0.67, 95% CI = 0.50 - 0.89) for exhibiting caries experience (DMFT ≥4) at 19 yrs of age. In conclusion, birth weight can be regarded as a predictor for dental caries and birth weight (≥4000 g) is especially a risk factor for caries increment during adolescence.

Comparative outcomes of subcutaneous and transvenous cardioverter-defibrillators

Background: The comparative outcomes of subcutaneous implantable cardioverter-defibrillator (S-ICD) and transvenous (T-ICD) have not been well studied. The aim of this study was to evaluate the safety and efficacy of currently available S-ICD and T-ICD. Methods: The study included 86 patients who received an S-ICD and 1:1 matched to those who received single-chamber T-ICD by gender, age, diagnosis, left ventricular ejection fraction (LVEF), and implant year. The clinical outcomes and implant complications were compared between the two groups. Results: The mean age of the 172 patients was 45 years, and 129 (75%) were male. The most common cardiac condition was hypertrophic cardiomyopathy (HCM, 37.8%). The mean LVEF was 50%. At a mean follow-up of 23 months, the appropriate and inappropriate ICD therapy rate were 1.2% vs. 4.7%(X^2= 1.854, P=0.368) and 9.3% vs. 3.5%(X^2 = 2.428, P = 0.211) in S-ICD and T-ICD groups respectively. There were no significant differences in device-related major and minor complications between the two groups (7.0% vs. 3.5%, X^2 = 1.055, P = 0.496). The S-ICD group had higher T-wave oversensing than T-ICD group (9.3% vs. 0%, X^2 = 8.390, P=0.007). Sixty-five patients had HCM (32 in S-ICD and 33 in T-ICD). The incidence of major complications was not significantly different between the two groups. Conclusions: The efficacy of an S-ICD is comparable to that of T-ICD, especially in a dominantly HCM patient population. The S-ICD is assodated with fewer major complications demanding reoperation.

Alpha1-antitrypsin deficiency is an autosomal recessive disease characterized by reduced serum levels of alpha1-antitrypsin(AAT)due to mutations in the SERPINA1 gene causing early onset pulmonary emphysema and,occasionally,chronic liver *** report an incidental finding of a novel null AAT allele,Q0Milano,consisting of a 17 nucleotides deletion in exon 3 of SERPINA1 gene,in an Italian child with persistently increased liver enzymes,a mild decrease in circulating AAT levels and without any pulmonary disease.Q0Milano variant results in an unfunctional protein lacking of AAT active site,as the resultant protein is truncated near PiS locus involved in AAT protein stability.

Background: Splenogonadal fusion is a rare congenitalanomaly which is characterized by fusion formationbetween the spleen and ***: We report a case of a 14-month boy withspleongonadal fusion-limb deformity syndrome focusingon the importance of awareness of this ***: The patient was admitted to our clinic becauseof a left undescended testis, and preoperative diagnosiswas not made. During the operation, "spleen-like" tissueattached to the gonad induced splenogonadal fusion, whichwas confi rmed by laparoscopy. The patient also had a shortright femur, hip dysplasia and a syndromic ***: Splenogonadal fusion anomaly shouldbe considered in the evaluation of undescended testis,especially in patients with facial and limb deformities.

Feasibility of iFISH patterns in hematologic malignancies among Congolese patients at Kinshasa University clinics

Objective: To analyze the feasibility of detecting Ph1 in leukemia patients in the Kinshasa University clinics in the Democratic Republic of Congo, at KU Leuven, ***: Bone marrow and peripheral blood samples with chronic myeloid leukemia,acute myeloid leukemia or acute leukocytes leukemia were obtained from 32 patients in Kinshasa University clinics in the Democratic Republic of Congo and transferred to KU Leuven in Belgium for i FISH feasibility. Ph^1 was detected by using a remote analysis of interphase fluorescence in situ hybridization(i FISH).Results: Out of the 32 patients involved in this study, 65.6%(n = 21) of the cases were successfully tested, of which 52.4%(n = 11) were i FISH positives for the variant t(9;22)(presence of Ph^1) in chronic myeloid leukemia samples and 47.6%(n = 10) negatives in all subtypes of hematological malignancies. However, there was a female predominance in chronic myeloid leukemia samples Ph^1-positives by i FISH, whereas no sexual influence was observed on acute subtypes of ***: i FISH analysis is feasible on samples obtained from remote sites in the Democratic Republic of Congo. However, the optimization of the sample storage is necessary to further improve i FISH's performance.

Penetrating Neck Trauma in a 4-Year-Old Child

Penetrating neck injuries are uncommon in children and a post traumatic common carotid artery to internal jugular vein fistula rarely occurs. We present one case of a 4-year-old girl with a penetrating injury from flying glass fragments in the midportion of the neck, caused by an explosion in a military warehouse. She was referred to our facility upon a request of a hospital of a neighbour country, two days after sustaining a penetrating cervical trauma. The presentation, diagnostic procedures and surgical management of this very rare injury are discussed. No neck penetrating trauma should be underestimated and a selective surgical intervention based on clinical examination and investigation tests findings is associated with good long-term outcomes.
