In tropical countries like India,malaria has been one of the most common parasitic illnesses leading to frequent hospitalization and causing major economic burden among the masses. Although Plasmodium vivax infection is considered to be benign,in contrast to Plasmodium, falciparum infection which is notorious for its severe splenic complications can occur frequently. Splenomegaly tends not to receive special attention,as it is not usually accompanied by any symptoms and can be gradually resolved via standard antimalarial *** infarction, although rarely attributable to malaria in an endemic region with high parasitemia,can be a rare presentation of this disease entity.

AIM To study the levels of neutrophil gelatinase associated lipocalin(NGAL) in head and neck squamous cell carcinoma(HNSCC).METHODS This was a non randomized case control study conducted at Department of Biochemistry,in collaboration with Regional Cancer Center over a period of one *** study population included 50 adult newly diagnosed HNSCC patients reporting in outpatient department at Regional Cancer Center and compared with 50 healthy *** was estimated by ELISA *** t test and χ~2 test were applied for comparison of means of study *** between groups were analyzed using Pearson correlation coefficient(r) *** Patients with HNSCC exhibited significantly increased levels of NGAL(P < 0.05) as compared to healthy controls(978.88 ± 261.39 ng/mL vs 34.83 ± 7.59 ng/mL).Out of 50,26 patients(52%) were in stage Ⅳ,21(42%) in stage Ⅲ,1(2%) patient in stage Ⅱ and 2(4%) patients were in stage Ⅰ.Metastasis was absent in 98% patients and mean NGAL levels were highest in these patients but P value was not *** NGAL levels were highest in stage Ⅳ [1041.54 ± 222.15 ng/mL(stage Ⅳ) vs 1040 ± 0.00 ng/mL(stage Ⅰ);900 ± 0.00 ng/mL(stage Ⅱ) and 1031.90 ± 202.55 ng/mL(stage Ⅲ)] and χ~2 test was highly significant(P < 0.001).Thirty-six patients(72%) were having moderately differentiated HNSCC and mean NGAL levels were maximum in patients with well differentiated HNSCC(1164 ± 315.64 ng/mL vs 1013.33 ± 161.19 ng/mL in moderately differentiated and 890 ± 11.55 ng/mL in poorly differentiated) and the results were also highly significant(P < 0.001,χ~2 test).CONCLUSION The present work demonstrates a potential role of NGAL as cancer biomarker and its use in monitoring the HNSCC progression.

Hepatitis C virus(HCV)causes a clinically important disease affecting 3%of the world *** is a single-stranded,positive-sense RNA virus belonging to the genus Hepacivirus within the Flaviviridae *** virus establishes a chronic infection in the face of an active host oxidative defence,thus adaptation to oxidative stress is key to virus *** a small RNA virus with a limited genomic capacity,we speculate that HCV deploys a different strategy to evade host oxidative *** of counteracting oxidative stress,it utilizes oxidative stress to facilitate its own *** is the first step in the replication of a plus strand RNA virus so it would make sense if the virus can exploit the host oxidative defence in facilitating this very first *** is particularly true when HCV utilizes an internal ribosome entry site element in translation,which is distinctive from that of cap-dependent translation of the vast majority of cellular genes,thus allowing selective translation of genes under conditions when global protein synthesis is ***,we were the first to show that HCV translation was stimulated by an important prooxidant-hydrogen peroxide in hepatocytes,suggesting that HCV is able to adapt to and utilize the host antiviral response to facilitate its own translation thus allowing the virus to thrive under oxidative stress condition to establish *** how HCV translation is regulated under oxidative stress condition will advance our knowledge on how HCV establishes *** chronicity is the initiator step in disease progression this will eventually lead to a better understanding of pathogenicity,which is particularly relevant to the development of anti-virals and improved treatments of HCV patients using anti-oxidants.

Genetic load in marine animals： a review

Marine invertebrates and fish are well known for their remarkable genetic diversity, which is commonly explained by large population size and the characteristic dispersive nature of their early, planktonic life history. Other potential sources of diversity in marine animals, such as a higher mutation rate, have been much less considered, though evidence for a high genetic load in marine bivalves has been accumulating for nearly half a century. In this review, I examine evidence for a higher genetic load in marine animals from studies of molecular marker segregation and linkage over the last 40 years, and survey recent work examining mutational load with molecular evolution approaches. Overall, marine animals appear to have higher genetic load than terrestrial animals （higher dn/ds ratios, inbreeding load, and segregation dis＇tortion）, though results are mixed for marine fish and data are lacking for many marine animal groups. Bivalves （oysters） have the highest loads observed among marine animals, comparable only to long-lived plants; however, more data is needed from other bivalves and more marine invertebrate taxa generally. For oysters, a higher load may be related to a chronically lower effective population size that, in concert with a higher mutational rate, elevate the number of deleterious mutations observed. I suggest that future studies use high-throughput sequencing approaches to examine （1） polymorphism in genomescale datasets across a wider range of marine animals at the population level and （2） intergenerational mutational changes between parents and offspring in crosses of aquaculture species to quantify mutation rates.

Genetic factors and gene polymorphisms leading to the onset of autoimmune response in autoimmune hepatitis(AIH)are still not full *** the CTLA-4 molecule is a key modulator of the lymphocytes responses we hypothezied that deficiencies or mutations in the gene encoding CTLA4 protein may be involved in AIH susceptibility and trigger the autoimmune *** investigated 3 distinct polymorphic sites(t49A>G,CT60 G>A and e318C>T)of the CTLA4 gene in 50 AIH patients and 100 healthy controls using the KASP genotyping technology.A significant positive association with AIH susceptibility was found for the GG genotype in t49 position of the CTLA4 gene which was significantly higher in AIH patients compared to controls(28%vs 9%,pZ0.003,ORZ3.93[1.56e9.88]).The CTLA4 A/A genotype in position CT60 was more significantly frequent in controls comparing to AIH patients and could be considered as a protective genotype for the tunisian ***4 genotyping in position318 did not show any statistically significant difference in genotype or allele *** CTLA4 gene polymorphism in position t49 is associated to AIH susceptibility in the Tunisian *** in the CTLA4 gene may lead to a modification of the CTLA4 protein structure that could have functional relevance in AIH pathogenesis and onset.

Public stigma and self-stigma impact negatively on the lives of people with mental health *** people in society stereotype and discriminate against people with mental ill-health,and often this negative process of marginalisation is internalised by people with lived ***,this negative internalisation leads to the development of *** this article,I reflect on my own experiences of shame and self-stigma as a person with mental ill-health socially bullied by peers from my community and social groups.I present a personal narrative of both public and self-stigmatisation which I hope will enable me to exorcise memories of internalised stigma,which are encountered as my demons of lived *** reflexivity,a process used widely in health and social care fields,I consider how social bullying shattered my fragile confidence,selfesteem,and self-efficacy in the early days of my recovery;the impact of associative stigma on family members is also *** this,the potential to empower people who experience shame and stigma is explored alongside effective anti-stigma processes which challenge discrimination.I connect the concept of recovery with the notion of empowerment,both of which emphasise the importance of agency and self-efficacy for people with mental ***,I consider how the concepts of empowerment and recovery can challenge both the public stigma held by peers in the community and the selfstigma of those with lived experiences.

Ileal intussusception is the invagination of the small intestine within itself and accounts for 1% of cases of acute obstruction. However, physicians do not initially consider intussusception as a possible diagnosis of obstruction due to its rarity in adults. Herein, we report the case of a 22-year-old male who was admitted to the Emergency Department with continuous abdominal pain. Ultrasonography and computed tomography revealed an ileal intussusception. The patient underwent surgical removal of the segment of the small bowel. Unexpectedly, pathology revealed that the invagination occurred due to a parasite egg, with features suggestive of Schistosoma species. Schistosomiasis, although considered a parasitic disease in tropical countries, is not absent from Europe and though it is highly improbable, it may be responsible for cases of intussusception in adults.
