Role of fast-setting cements in arthroplasty: A comparative analysis of characteristics

AIM To evaluate the behaviour of two fast-setting polymethylmethacrylate(PMMA) cements CMW~? 2 G and Palacos~? fast R + G, as reference: Standard-setting Palacos~? R + *** The fast-setting cements CMW~? 2 G and Palacos~? fast R + G were studied, using standard-setting high viscosity Palacos~? R + G as a reference. Eleven units(of two batch numbers) of each cement were tested. All cements were mixed as specified by the manufacturer and analysed on the following parameters: Handling properties(mixing, waiting, working and hardening phase) according to Kuehn, Mechanical properties according to ISO 5833 and DIN 53435, Fatigue strength according to ISO 16402, Benzoyl Peroxide(BPO)-Content by titration, powder/liquid-ratio by weighing, antibiotic elution profile by High Performance Liquid Chromatography. All tests were done in an acclimatised laboratory with temperatures set at 23.5 ℃± 0.5 ℃ and a humidity of > 40%. RESULTS Palacos~? fast R + G showed slightly shorter handling properties(doughing, hardening phase, n = 12) than CMW~? 2 G, allowing to reduce operative time and to optimise cemented cup implantation. Data of the quasistatic properties of ISO 5833 and DIN 53435 of both cements tested was comparable. The ISO compressive strength(MPa) of Palacos~? fast R + G was significantly higher than CMW~? 2 G, resulting in ANOVA(P < 0.01) and two sample t-test(P < 0.01) at 0.05 level of significance(n = 20). Palacos~? fast R + G showed a higher fatigue strength of about 18% mean(ISO 16402) of 15.3 MPa instead of 13.0 MPa for CMW~? 2 G(n = 5 × 10~6 cycles). Palacos~? fast R + G and CMW~? 2 G differed only by 0.11%(n = 6) with the former having the higher content. The BPO-content of both cements were therefore comparable. CMW~? 2 G had a powder/liquid ratio of 2:1, Palacos~? fast R + G of 2.550:1 due to a higher powder content. Despite its higher gentamicin content, CMW~? 2 G showed a significantly lower antibiotic elution over time than Palacos~? fast R + G(n = 3). CON

Postoperative tunnel enlargement has been frequently reported after anterior cruciate ligament（ACL）*** screw,as a surgical implant in ACL reconstruction,may influence natural loading transmission and contribute to tunnel *** aims of this study are（1）to quantify the alteration of strain energy density（SED）distribution after the anatomic single-bundle ACL reconstruction;and（2）to characterize the influence of screw length and diameter on the degree of the SED alteration.A validated finite element model of human knee joint was *** screw length ranging from 20 to 30 mm with screw diameter ranging from 7 to 9 mm were *** the post-operative knee,the SED increased steeply at the extra-articular tunnel aperture under compressive and complex loadings,whereas the SED decreased beneath the screw shaft and nearby the intra-articular tunnel *** the screw length could lower the SED deprivation in the proximal part of the bone tunnel;whereas increasing either screw length or diameter could aggravate the SED deprivation in the distal part of the bone *** the elastic modulus of the screw could lower the bone SED deprivation around the *** consideration of both graft stability and SED alteration,a biodegradable interference screw with a long length is recommended,which could provide a beneficial mechanical environment at the distal part of the tunnel,and meanwhile decrease the bone-graft motion and synovial fluid propagation at the proximal part of the *** findings together with the clinical and histological factors could help to improve surgical outcome,and serve as a preliminary knowledge for the following study of biodegradable interference screw.

Chronic carpal tunnel syndrome caused by covert tophaceous gout: A case report

Carpal tunnel syndrome(CTS) is well recognized as the most common type of peripheral neuropathy. A rare cause of CTS is tophaceous gout. Tophi deposits can accumulate in various structures including the flexor tendons, tendon sheaths, the carpal tunnel floor, transverse carpal ligament, and even the median nerve, causing various symptoms such as pain, numbness, and weakness. Tophi forming in the carpal canal can compress the median nerve, leading to CTS. Here, we describe a 25-year-old male with a family history of tophaceous gout who presented with typical CTS symptoms. Although he had chronic numbness in his right hand, he failed to present with any obvious pal-pable masses on his forearm or hand. However, his family history, laboratory, clinical, and magnetic re-sonance imaging findings were consistent with tophi deposits. CTS symptoms were eased through surgical removal of tophi and decompression of the median nerve. No recurrences of gout and CTS symptoms were reported at a one-year follow-up. This case shows that CTS symptoms could be the initial manifestation of tophaceous gout. In patients with a family history of gout and with CTS symptoms, imaging examinations are critical for early diagnosis and selecting appropriate treatment. Surgical removal of "covert" tophi and decompression of the median nerve is an effective option for eliminating symptoms.

Reliability of dermatomal somatosensory-evoked potential in the evaluation of lumbosacral nerve root injury A concurrent case-control study

BACKGROUND： It has been reported that dermatomal somatosensory evoked potential （DSEP） can be used for diagnosing nerve root injury in patients with lumbar disc herniation （LDH）, and that 83% 95% of patients suffer from the disease. Body height correction is not performed prior to determinations of latency and latency difference between the healthy and affected sides. However, latency noticeably correlates to body height. OBJECTIVE： This study aims to determine the lumbosacral nerve root injury in patients with LDH by DSEP, and to evaluate the sensitivity of the DSEP difference between the healthy and affected sides using a diagnostic index following body height correction. DESIGN： A case-control observation. SETTING： Department of Orthopedic Surgery, Hainan Provincial People＇s Hospital. PARTICIPANTS： Ninety-six patients, comprised of 67 males and 29 females, with an average age of 43 years and a mean body height of 1.65 m （range 1.48-1.81 m）, were recruited for this study. These patients suffered from unilateral lower limb radiation pain and received treatment at the Department of Orthopedic Surgery, Hainan Provincial People＇s Hospital between January 2004 and December 2006. All patients were confirmed to suffer from LDH at the L3-4, L-5, and/or Ls-SI by CT and/or MRI examinations. Central nervous system diseases were excluded. In order to obtain a normal reference value, DSEP was determined for a group of 50 subjects, who concurrently received health examinations in the same department. The subjects had no previous history of back leg pain or nervous system disease. The group of healthy controls included 26 males and 24 females, with an average age of 37 years and a mean body height of 1.63 m （range 1.50-1.80 m）.
