Behavioral Effects of Deep Brain Stimulation of the Anterior Nucleus of Thalamus, Entorhinal Cortex and Fornix in a Rat Model of Alzheimer＇s Disease

Background：Recent clinical and preclinical studies have suggested that deep brain stimulation （DBS） can be used as a tool to enhance cognitive functions.The aim of the present study was to investigate the impact of DBS at three separate targets in the Papez circuit,including the anterior nucleus of thalamus （ANT）,the entorhinal cortex （EC）,and the fornix （FX）,on cognitive behaviors in an Alzheimer＇s disease （AD） rat model.Methods：Forty-eight rats were subjected to an intrahippocampal injection ofamyloid peptides 1-42 to induce an AD model.Rats were divided into six groups：DBS and sham DBS groups of ANT,EC,and FX.Spatial learning and memory were assessed by the Morris water maze （MWM）.Recognition memory was investigated by the novel object recognition memory test （NORM）.Locomotor and anxiety-related behaviors were detected by the open field test （OF）.By using two-way analysis of variance （ANOVA）,behavior differences between the six groups were analyzed.Results：In the MWM,the ANT,EC,and FX DBS groups performed differently in terms of the time spent in the platform zone （F（2.23） =6.04,P ＜ 0.01）,the frequency of platform crossing （F（2,23） =11.53,P ＜ 0.001）,and the percent time spent within the platform quadrant （F（2,23） =6.29,P ＜ 0.01）.In the NORM,the EC and FX DBS groups spent more time with the novel object,although the ANT DBS group did not （F（2,23） =10.03,P ＜ 0.001）.In the OF,all of the groups showed a similar total distance moved （F（1.42） =1.14,P =0.29）and relative time spent in the center （F（2,42） =0.56,P =0.58）.Conclusions：Our results demonstrated that DBS of the EC and FX facilitated hippocampus-dependent spatial memory more prominently thanANT DBS.In addition,hippocampus-independent recognition memory was enhanced by EC and FX DBS.None of the targets showed side-effects of anxiety or locomotor behaviors.

Acute Fornix Deep Brain Stimulation Improves Hippocampal Glucose Metabolism in Aged Mice

Background： A beneficial memory effect of acute formix deep brain stimulation （DBS） has been reported in clinical studies. The aim of this study was to investigate the acute changes in glucose metabolism induced by fornix DBS. Methods： First, the Morris water maze test and novel object recognition memory test were used to confirm declined memory in aged mice （C57BL/6, 20-22 months old）. Then, four groups of mice were used as follows： aged mice with stimulation （n = 12）, aged mice with sham-stimulation （n = 8）, adult mice （3-4 months old） with stimulation （n = 12）, and adult mice with sham-stimulation （n = 8）. lpsilateral hippocampal glucose metabolism and glutamate levels were measured in vivo by microdialysis before, during, and after fornix DBS treatment. Histological staining was used to verify the localization of electrodes and mice with inaccurate placement were excluded from subsequent analyses. The effects of fornix DBS on extracellular glucose, lactate, pyruvate, and glutamate levels over time were analyzed by repeated-measures analysis of variance followed by Fisher＇s least significant difference post hoc test. Results： The aged mice had a higher basal lactate/pyruvate ratio （LPR） and lactate/glucose ratio （LGR） than the adult mice （LPR： 0.34 ± 0.04 vs. 0.13 ± 0.02, t = 4.626, P 〈 0.0001 ; LGR： 6.06± 0.59 vs. 4.14 ± 0.36, t = 2.823, P 〈 0.01 ）. Fomix DBS decreased the ipsilateral hippocampal pyruvate and lactate levels （P 〈 0.05）, but the glucose levels were not obviously changed in aged mice. Similarly, the LGR and LPR also decreased in aged mice after fornix DBS treatment （P 〈 0.05）. Glucose metabolism in adult mice was not significantly influenced by tbrnix DBS. In addition, fornix DBS significantly decreased the ipsilateral hippocampal extracellular levels of glutamate in aged mice （P 〈 0.05）, while significant alterations were not found in the adult mice. Background To explore the clinical,radiological,and surgical characteristics of anterior perforated substance(APS)gliomas.Methods Twenty patients with APS glioma who were treated with surgery between March 2019 and January 2022 from Tiantan hospital were retrospectively reviewed.The clinical,histological and radiological data were collected.Results Twenty patients,including 7 males(55%)and 13 females(45%),with a mean age at diagnosis of 37.9 years(range,28-53 years)underwent operative intervention for APS.Headaches and dizziness were the most common preoperative symptoms in the majority patients(14,70%).Based on radiological features of MRI,the APS was classified into two subtypes,type A and type B.Seven patients(40%)in type A indicated a clear tumor margin,while 13 patients(60%)in type B showed an ill-defined margin.The surgical approach including frontal,temporal,and coronal frontal incisions for type A and type B tumors,respectively.Three patients in type A received total resection,while one patient in type B were total resected.Pathologically,12 cases(60%,12/20)were diagnosed as astrocytoma and 8 cases(20%,8/20)were oligodendroglioma.Meanwhile,17 cases(85%,17/20)had MGMT promotor methylation.Conclusion In this study,we performed the first systematic research of patients with APS glioma.Most of patients with APS presented headaches and dizziness symptoms.The APS glioma was further divided into two major radiological subtypes with relevant different surgical approaches.The APS glioma in type A were more likely to receive total resection.

The brain is a complex organ that requires precise mapping to understand its structure and function.Brain atlases provide a powerful tool for studying brain circuits,discovering biological markers for early diagnosis,and developing personalized treatments for neuropsychiatric disorders.Neuromodulation techniques,such as transcranial magnetic stimulation and deep brain stimulation,have revolutionized clinical therapies for neuropsychiatric disorders.However,the lack of fine-scale brain atlases limits the precision and effectiveness of these techniques.Advances in neuroimaging and machine learning techniques have led to the emergence of stereotactic-assisted neurosurgery and navigation systems.Still,the individual variability among patients and the diversity of brain diseases make it necessary to develop personalized solutions.The article provides an overview of recent advances in individualized brain mapping and navigated neuromodulation and discusses the methodological profiles,advantages,disadvantages,and future trends of these techniques.The article concludes by posing open questions about the future development of individualized brain mapping and navigated neuromodulation.

Dorsal root entry zone coagulation for treatment of deafferentation pain syndromes

Background Deafferentation pain is a kind of chronic pain syndrome and hard to manipulate. To evaluate the effectiveness and safety of junctional dorsal root entry zone （DREZ） coagulation, 23 consecutive patients with intractable deafferentation pain syndrome were studied.Methods Twenty-three patients underwent junctional DREZ coagulation （C5-T1 for upper extremities and L2-S1 for lower extremities） under general anesthesia. The pain severity was evaluated by the short McGill pain questionnaire （MPQ） and the visual analog scale （VAS）, and the depression and anxiety of patients were assessed by Hamilton rating scale for depression （HRSD）, Hamilton anxiety scale （HAMA）, self-rating anxiety scale （SAS） and self-rating depression scale （SDS）.Results All the patients experienced significant pain reduction immediately after surgery. The scales of short MPQ and VAS at pre-operation, 6-month and 12-month follow-up were 31.5±3.4 and 8.8±1.5, 6.5±1.9 and 2.5±2.2, 7.1±2.1 and 2.9±1.9, respectively. The postoperative scores comparing to pre-operative scores showed a statistically significant difference （P 〈0.01）. The depression and anxiety state was also significantly relieved. At 12-month follow-up 6 patients had complete pain relief, 11 had excellent results with more than 75% pain relief, 17 had good results with more than 50% pain relief （73.9%）. The main postoperative complications were transient slight hemiplegia （8）, hypesthesia and paresthesia （15）, a bearing down feeling of affected extremity （6）, and deep sensory disability in the lower limbs （4） on the operated side. Because of the long time and prone position of the operation, 13 cases had a transient hyperalgesia in the upper chest. Conclusion DREZ coagulation is a safe and effective procedure in the treatment of deafferentation pain syndromes.

Evaluation of lordosis recovery after lumbar arthrodesis and its clinical impact

Background Posterior lumbar arthrodesis has become a widely used therapeutic option to correct sagittal imbalances in patients suffering from degenerative lumbar conditions.However,in western Africa,there is no study have reported long-term outcome of posterior lumbar arthrodesis.The aim of this study was to investigate the relationship between the restoration of adequate lordosis and the patient’s postoperative quality of life.Method The study was retrospective.From January 2012 to December 2019,80 patients who underwent posterior lumbar arthrodesis for lumbar degenerative diseases were included with a mean follow-up of 43.2 months.Mean age was 50.8 years(SD=12.2).Preoperative and postoperative patients'symptoms were assessed by the visual analog scale(VAS),Oswestry Disability Index(ODI),and 12-item Short Form(SF-12).Pre-and post-operative radiographic evaluation included lumbar lordosis measured(LLm),pelvic incidence(PI),sacral slope(SS),and pelvic stilt(PS).Theoretical lumbar lordosis(LLt)was defined by the following:LL=0.54×PI+27.6.Data analysis was done using the statistical software"R."The risk of error was 5%(poperative and postoperative lumbar lordosis(p=0.2567).There was no statistical difference between preoperative and post-operative PI-LL(p=0.179).There was a statistically significant difference between the pre and postoperative clinical scores(p<0.001).Statistical analysis showed a correlation between recovery of lumbar lordosis and improvement in physical component of SF-12(PCS)(p<0.05)and lumbar and radicular VAS(p<0.05)for the subgroup of narrow lumbar spine.There was a statistical relationship between the restoration of lumbar lordosis and improvement in PCS(p=0.004)and VAS(p=0.003)for the subgroup of isthmic lysis spondylolisthesis.Discussion The root decompression performed in most patients could explain the clinical improvement regardless of recovery of lordosis.The failure to consid

Antagonism of Protease-Activated Receptor 4 Protects Against Traumatic Brain Injury by Suppressing Neuroinflammation via Inhibition of Tab2/NF-κB Signaling

Traumatic brain injury(TBI)triggers the activation of the endogenous coagulation mechanism,and a large amount of thrombin is released to curb uncontrollable bleeding through thrombin receptors,also known as protease-activated receptors(PARs).However,thrombin is one of the most critical factors in secondary brain injury.Thus,the PARs may be effective targets against hemorrhagic brain injury.Since the PAR1 antagonist has an increased bleeding risk in clinical practice,PAR4 blockade has been suggested as a more promising treatment.Here,we explored the expression pattern of PAR4 in the brain of mice after TBI,and explored the effect and possible mechanism of BMS-986120(BMS),a novel selective and reversible PAR4 antagonist on secondary brain injury.Treatment with BMS protected against TBI in mice.mRNA-seq analysis,Western blot,and qRT-PCR verification in vitro showed that BMS significantly inhibited thrombin-induced inflammation in astrocytes,and suggested that the Tab2/ERK/NF-κB signaling pathway plays a key role in this process.Our findings provide reliable evidence that blocking PAR4 is a safe and effective intervention for TBI,and suggest that BMS has a potential clinical application in the management of TBI.

Surgical site infections after elective craniotomy for brain tumor:a study on potential risk factors and related treatments

Background Surgical site infection(SSI)is a common complication following craniotomy that increases morbidity,mortality,and medical expenses.The objectives of this study were to determine the relevant risk factors associated with SSI after elective craniotomy for brain tumor and analyse the treatments for SSI.Methods A retrospective nested case-control study was conducted using data from patients who underwent craniotomy for brain tumor resection at the Neurosurgical Oncology Department No.6 of Beijing Tiantan Hospital,Capital Medical University,between January 2019 and December 2021.Risk factors for SSI were determined using multivariate logistic regression analysis.We analyzed microbiological and related treatment data for different SSI types.Results Among 2061 patients who underwent craniotomy for brain tumor,31 had SSI(1.50%).In the multivariate logistic regression analysis,body mass index(BMI)and operative duration were identified as independent risk factors for SSI.The most common microorganism isolated from SSIs was Staphylococcus epidermidis(22.9%),and drug sensitivity results showed that gram-positive bacteria were sensitive to linezolid,vancomycin and tigecycline,whereas gram-negative bacteria were sensitive to meropenem,cefepime and ceftazidime.Six of the seven patients who underwent bone flap removal due to osteomyelitis were infected with gram-negative bacteria.Conclusions BMI and operative duration were identified as independent risk factors for SSI.Diabetes mellitus,previous ratio therapy,type of incision,recurrence tumor and other risk factors were not found to be associated with the occurrence of SSI in this study.

Anti-epileptic effects of neuropeptide Y gene transfection into the rat brain

Neuropeptide Y gene transfection into normal rat brain tissue can provide gene overexpression, which can attenuate the severity of kainic acid-induced seizures. In this study, a recombinant adeno-associated virus carrying the neuropeptide Y gene was transfected into brain tissue of rats with kainic acid-induced epilepsy through stereotactic methods. Following these transfections, we verified overexpression of the neuropeptide Y gene in the epileptic brain. Electroencephalograms showed that seizure severity was significantly inhibited and seizure latency was significantly prolonged up to 4 weeks after gene transfection. Moreover, quantitative fluorescent PCR and western blot assays revealed that the mRNA and protein expression of the N-methyI-D-aspartate receptor subunits NR1, NR2A, and NR2B was inhibited in the hippocampus of epileptic rats. These findings indicate that neuropeptide Y may inhibit seizures via down-regulation of the functional expression of N-methyI-D-aspartate receptors.
