The decision-making process for senior cancer patients: treatment allocation of older women with operable breast cancer in the UK

Objective: Up to 40% of women over 70 years with primary operable breast cancer in the UK are treated with primary endocrine therapy(PET) as an alternative to surgery. A variety of factors are important in determining treatment for older breast cancer patients. This study aimed to identify the patient and tumor factors associated with treatment allocation in this ***: Prospectively collected data on treatment received(surgery vs. PET) were analysed with multivariable logistic regression using the variables age, modified Charlson Comorbidity Index(CCI), activities of daily living(ADL) score, Mini-Mental State Examination(MMSE) score, HER2 status, tumour size, grade and nodal status. Results: Data were available for 1,122 cancers in 1,098 patients recruited between February 2013 and June 2015 from 51 UK hospitals. About 78% of the population were treated surgically, with the remainder being treated with PET. Increasing patient age at diagnosis, increasing CCI score, large tumor size(5 cm or more) and dependence in one or more ADL categories were all strongly associated with non-surgical treatment(P<0.05).Conclusion: Increasing comorbidity, large tumor size and reduced functional ability are associated with reduced likelihood of surgical treatment of breast cancer in older patients. However, age itself remains a significant factor for non-surgical treatment; reinforcing the need for evidence-based guidelines.

AIM:To identify genetic polymorphisms in the promoter region of the human base excision repair gene NEIL1 in gastric cancer ***:The NEIL1 promoter region in DNA from 80 Japanese patients with gastric cancer was searched for genetic polymorphisms by polymerase chain reaction-single-strand conformation polymorphism and subsequent sequencing ***:Three novel genetic polymorphisms,i.e.c.-3769C>T,c.-3170T>G,and c.-2681TA[8],were identified in the NEIL1 promoter region at an allele frequency of 0.6%,9.4%,and 4.4%,respectively,in Japanese gastric cancer ***:Three NEIL1 promoter polymorphisms detected in this study may be of importance in gastric carcinogenesis.

Impact of spiritual care on the spiritual and mental health and quality of life of patients with advanced cancer

BACKGROUND Cancer is a growing threat to human *** to the double torment of cancer and cancer treatment,patients with advanced cancer generally have a low quality of *** present,there is a lack of systematic spiritual care plans for patients with advanced cancer as well as systematic guidance plans on the specific clinical application of spiritual care for advanced cancer *** hypothesized that our care plan would be effective in improving the spiritual and mental health and quality of life of patients with advanced *** To construct a spiritual care plan suitable for Chinese patients with advanced cancer through literature *** From February to December 2018,through purpose sampling,we selected 100 advanced cancer patients from the oncology Department and Hospice ward of a tertiary hospital in Liaoning Province who met the study *** were randomly divided into experimental and control groups,with 50 cases in each *** control group received the current routine care,while the experimental group received the advanced cancer spiritual care intervention in addition to routine *** After the intervention,the overall spiritual health score for the experimental group was higher than that of the control group(4.68±1.36 vs 3.63±1.71).The difference between the groups was statistically significant(P<0.05).The proportion of anxiety-free patients in the experimental group was 95.45%,which was significantly higher than the 60.98%in the control ***,the proportion of non-depressed patients in the experimental group was 97.73%,which was significantly higher than the 85.37%in the control group(P<0.05).The overall quality of life score for the experimental group was significantly higher than that of the control group(5.36±1.16 vs 4.39±1.36,P<0.05).CONCLUSION Our spiritual care plan for patients with advanced cancer could improve their spiritual health and quality of life and reduce negative mental health sympt

Right iliac fossa abscess as first manifestation of perforated adenocarcinoma of sigmoid: a rare case report

Colorectal cancer usually present with known symptoms while there are less common manifestation including abscess formation which can be intra or extra peritoneal. A 60-year-old Caucasian male with a history of RLQ abdominal pain, nausea, vomiting and anorexia from 15 days ago referred to surgery ward. Ultrasound showed a hypoachoic lesion with diameters 50 mm x 70 mm in RLQ of abdomen and a round echogenic area in right lobe of liver with diameter 15 mm. The findings were revealed an abscess located in right lilac fossa then local drainage of abscess was performed. Four days later the patient was re-admitted because of severe abdominal distention and lack of bowel movement. Laparoscopy was performed before proceeding with further examinations, due to the poor general condition of the patient. The sigmoid was adherent into the abdominal wall and mild intestinal loop distention and apple-core view was observed during operation. Can- cer of sigmoid complicated by a right lilac fossa abscess was diagnosed and Hartman colestomy was undertaken. At the last follow-up examination 3 months after operation, the patient was in good health with no clinical evidence of recurrence.

Clinical Application of Lobaplatin Combined with Docetaxel in Adjuvant Chemotherapy for Triple-Negative Breast Cancer in Elderly Patients

To investigate the adverse reactions and efficacy of docetaxel combined with lobaplatin in adjuvant chemotherapy for triple-negative breast cancer in elderly patients.A total of 96 elderly triple-negative breast cancer patients admitted to our hospital from January 2008 to December 2011 were randomly divided into two groups.A group of 56 patients received docetaxel 75 mg·m^(-2),intravenous drip,d1;lobaplatin 30 mg·m^(-2),intravenous drip,d1;21 days repeat,a total of 6 cycles.A group of 40 patients received chemotherapy for 6 cycles with an anthracycline-containing(TEC)*** of adverse reactions and 5-year diseasefree survival in both *** incidence of thrombocytopenia was significantly higher in 56 patients with TL regimen than those with TCE-containing anthracyclines(P=0.005).But the incidence of cardiotoxicity was 32.5%in the TEC *** the difference was statistically significant(P=0.008).The 5-year disease-free survival rate was 73.2%in the TL group and 67.5%in the TEC *** was no statistical *** combined with lobaplatin in the treatment of elderly triple-negative breast cancer has no significant difference in efficacy compared with traditional anthracycline-containing drugs,but it can avoid the cardiotoxicity caused by ***’s a new option for elderly TNBC adjuvant chemotherapy,suggesting to expand the sample content for further research.

The sickle cell disease sickle cell anemia called Expired aussi is a genetic disease Caused by abnormal hemoglobin. The pediatric ward of the provincial hospital kenitra recorded a workforce of 164 children with sickle cell disease over a period of twenty four months (June 2010-June 2012). On average seven to eight (7 to 8) children are born per year with this disease. The majority of these children are admitted to the pediatric ward and have an increased susceptibility to infections. To identify socio-economic and environmental factors in the care of children and their families. The study group consists of 60 children aged 7 - 14 years. Clinical and analytical information is obtained from records and doctors during consultations of these patients. The results show that 64% of these children are from rural areas against 30% of children of urban origin, while 6% live in suburban areas, however 68% of these children use septic false. In parallel they consume well water. Clinical examination and white blood cell count revealed a prevalence of 76% for fever cases reviews some of qui-have ticemia September, 43% for leukocytosis. Moreover, we noted that diarrhea is very common in this sample. Parenting was made bacteriological examinations of well water consumed by these children. In conclusion, the present study showed a significant combination entre les frequency of sickle cell crises infectious presented by the children studied and the environmental quality of life and family in the child qui develops.

BACKGROUND The coronavirus disease 2019(COVID-19)has been the cause of a global health crisis since the end of *** countries are following the guidelines and recommendations released by the World Health Organization to decrease the spread of the *** account for only 3%-5%of COVID-19 *** data are available regarding the clinical course,disease severity,and mode of treatment in children with malignancy and *** To evaluate the treatment plan and outcome of children with malignancy who contracted *** A retrospective study of the medical files of patients with malignancy who contracted COVID-19 between July 2020 and June 2021 was *** following data were reviewed for all patients:primary disease,laboratory data,admission ward,clinical status upon admission,disease course,treatment plan,and *** patients were those with malignancy who tested positive for COVID-19 by reverse transcription polymerase chain *** A total of 40 patients who had malignancy contracted COVID-19 from July 1,2020 to June 1,*** primary diseases were as follows:34 patients(85%)had hematological malignancies(30 had acute lymphoblastic leukemia,2 had acute myeloblastic leukemia,and 2 had Hodgkin lymphoma),whereas 6 patients(15%)had solid tumors(2 had neuroblastoma,2 had rhabdomyosarcoma,and 2 had central nervous system tumors).Twelve patients(30%)did not need hospitalization and underwent home isolation only,whereas twenty-eight patients(70%)required hospitalization(26 patients were admitted in the COVID-19 ward and 2 were admitted in the pediatric intensive care unit).CONCLUSION COVID-19 with malignancy in the pediatric age group has a benign course and does not increase the risk of having severe infection compared to other children.

The decision for hospice care in patients with terminal illness in Shanghai:A mixed-method study

Objectives To investigate why patients with terminal illness and their families in Shanghai choose the hospice ward and their decision-making *** This was a mixed-method study consisting of a cross-sectional survey and a descriptive qualitative *** decision-makers for patients hospitalized in hospice wards were recruited between September 2019 and July 2021.A medical decision-maker is a family member who makes medical decisions for a *** 146 participants completed a self-developed 10-item questionnaire that included five items about their demographic characteristics and five items about the decision-making *** semi-structured interviews were conducted with nine participants to understand the family’s decision-making process when they chose a hospice *** interviews were analyzed using qualitative content *** The mean age of the 146 participants was 57.6 years *** the decision-makers,56.85%were the patients’***-dominated discussions involving other family members were the main decision-making mode(84.93%).Patient participation in the decision-making process was reported in 43.15%of *** participation of doctors(17.81%)and nurses(2.05%)were reported in a small number of *** most common reason for choosing the hospice ward was the inability to find any other hospital for the patients(82.19%).The most common ways to learn about the service were neighbors and friends(38.36%)and social media(28.77%).Two themes and six categories emerged from the *** first theme was reasons for choosing hospice *** reasons included being unable to care for the patients at home,staying in a hospice ward could reduce the psychological stress for home care,being unable to be admitted into tertiary/secondary hospitals,and thinking a hospice ward was a suitable place for the *** second theme was the decision process of choosing a hospice *** theme included the following two categ
