BACKGROUND Post-transplant lymphoproliferative disease(PTLD)is a heterogeneous group of diseases that develop after solid organ and hematopoietic stem cells transplantation related to intensive immunosuppression regimen,T-cell depletion and Epstein-Barr virus *** the improvement in the management of PTLD,the prognosis remains *** we report the management of two transplanted patients with PTLD and infections during immunochemotherapy(ICTH).CASE SUMMARY Of 65-year-old woman 11 years after kidney transplantation(first case)presented with diffuse large B-cell lymphoma(DLBCL)CS III and started ICHT according to R-CHOP *** the secondary prevention of neutropenic fever,the patient developed grade 4 neutropenia with urinary and pulmonary tract infections after the fifth *** was continued in reduced doses up to 7 cycles followed by involved-field radiation therapy of the residual *** second case presents a 49-year-old man,8 years after liver transplantation due to cirrhosis in the course of chronic hepatitis B,who started ICTH for DLBCL Burkitt-like CS *** patient received four cycles of ICTH according to RCODOX/R-IVAC protocol,with reduced *** both cases initially undertaken reduction of immunosuppression was ineffective to prevent infectious *** one incomplete ICHT treatment due to recurrent infections,both our patients remain in complete *** Reduction of immunosuppression and the doses of chemotherapeutics may be insufficient to prevent infectious complications during ICTH in PTLD patients.

The Chemopreventive Effect of Tamoxifen Combined with Celecoxib on DMBA chemically-Induced Breast Cancer

Objective： To investigate the chemopreventive effect of tamoxifen combined with a COX-2 selective inhibitor, celecoxib, on breast cancer in rats chemically induced by 7,12-dimethylben （a）anthracene （DMBA）. Methods：DMBA was irrigated into the stomaches of SD female rats to build breast cancer model. A total of 120 rats were divided into four groups： control group, tamoxifen group, celecoxib group and combined group. The incidence rate, latent period, number and volume of breast cancer were detected and analyzed. Results：The tumor incidence rate of tamoxifen group （48.15%, 13/27） and celecoxib group （50.00%, 14/28） were lower than that of control group （85.71%, 24/28）, but higher than that of combined group （21.43%, 6/28）. The tumor＇s latent period of tamoxifen group （97.54±1.85 d） and celecoxib group （96.79±2.89 d） were longer than that of control group （89.50±5.99 d）, but shorter than that of combined group （103.67±3.39 d）. The average tumor number of tamoxifen group （1.77±0.73） and celecoxib group （1.71±0.61） were less than that of control group （3.50±1.62）, but more than that of combined group （1.17_＋0.42）. The average tumor volume of tamoxifen group （1.78±0.71 cm^3） and celecoxib group （2.05±1.04 cm^3） were smaller than that of control group （6.42±3.96 cm^3）, but bigger than that of combined group （0.71±0.96 cm^3）（P 〈 0.05 respectively）. Conclusion ：Celecoxib and tamoxifen are effective drugs in preventing the occurrence of rat breast cancer chemically induced by DMBA. Furthermore, combination of them has better chemopreventive effect.

Background:The use of laparoscopic(LLR)and robotic liver resections(RLR)has been safely performed in many institutions for liver tumours.A large scale international multicenter study would provide stronger evidence and insight into application of these techniques for huge liver tumours≥10 ***:This was a retrospective review of 971 patients who underwent LLR and RLR for huge(≥10 cm)tumors at 42 international centers between ***:One hundred RLR and 699 LLR which met study criteria were *** comparison between the 2 approaches for patients with huge tumors were performed using 1:3 propensity-score matching(PSM)(73 vs.219).Before PSM,LLR was associated with significantly increased frequency of previous abdominal surgery,malignant pathology,liver cirrhosis and increased median *** PSM,RLR and LLR was associated with no significant difference in key perioperative outcomes including media operation time(242 vs.290 min,P=0.286),transfusion rate rate(19.2%vs.16.9%,P=0.652),median blood loss(200 vs.300 mL,P=0.694),open conversion rate(8.2%vs.11.0%,P=0.519),morbidity(28.8%vs.21.9%,P=0.221),major morbidity(4.1%vs.9.6%,P=0.152),mortality and postoperative length of stay(6 vs.6 days,P=0.435).Conclusions:RLR and LLR can be performed safely for selected patients with huge liver tumours with excellent *** was no significant difference in perioperative outcomes after RLR or LLR.
