Congenital Vaginal Atresia:A Report of 39 Cases in a Regional obstetrics and gynecology hospital

To investigate the clinical course and management of congenital vaginal atresia. This retrospective analysis included patients with congenital vaginal atresia treated from March 2004 to August 2014 at the obstetrics and gynecology hospital of Fudan University. Thirty-nine patients were included in this study. Their average age was 16.87±2.2 years when they came to our hospital. Totally, 51% of the patients had isolated congenital vaginal atresia with a normal cervix, whereas the others had either cervical atresia or imperforate hymen. The primary presenting signs and symptoms included primary amenorrhea（71.8%）, periodic abdominalgia（41.0%）, abdominal pain（36.0%）, dyspareunia（10.3%）, menstrual disorders（5.1%）, and pelvic mass（5.1%）. Ultrasound and magnetic resonance imaging（MRI） were effective inspection methods for the screening of urogenital tract-associated anomalies. Vaginoplasty mainly included simple vagina reconstruction with insertion of a mold（n=22） and split-thickness skin grafting（n=4）. In 64% of surgical patients, normal menstrual bleeding was achieved. Four of the patients subsequently became pregnant and delivered at term. Primary amenorrhea, periodic abdominalgia and abdominal pain are the main reasons for the post pubertal patients to visit doctors. Surgical methods can successfully provide these patients an opportunity for subsequent conservative management, can result in normal menstrual bleeding, resolve cyclic pelvic pain, and provide some potential for fertility.

Spectrum of Diseases in Female Children and Adolescents Admitted to the obstetrics and gynecology hospital, Shanghai, between 2003 and 2017

Objective:To investigate the spectrum of diseases in female children and adolescents hospitalized for obstetric and gynecological conditions.Methods:This retrospective study included patients(age≤19 years)admitted to the obstetrics and gynecology hospital of Fudan University,Shanghai,between January 2003 and December 2017.Their medical records-age,principal diagnosis,and treatment-were reviewed and analyzed.Diagnoses were divided into eight disease classifications.Comparison between groups was analyzed with Student’s t-test.Results:This study enrolled 4,628 inpatients.The number of inpatients increased each year,especially in the groups of gynecologic tumors,pregnancy-related conditions,and disorders of the breast.The mean age was 16.33±2.42 years(range:2-19 years).Of all the inpatients,41.9%(n=1,939)were hospitalized for gynecologic tumors,and 85.5%(n=1,657)of the tumors were benign.Altogether,ovarian cysts(28.66%,n=1,325)were the most common disease observed in this group.In addition,32.9%(n=1,524)of the inpatients were hospitalized for pregnancy,childbirth,and puerperium;within this group,21.9%(n=1,014)were hospitalized for artificial abortions.Conclusions:The efforts of all stakeholders toward the implementation of government policies to promote sexual and reproductive health among children and adolescents in China have been effective.

Introduction: Obesity and pregnancy is a major public health problem worldwide, both maternal and fetal. Objective: This is to describe the epidemiological and prognostic aspects of obesity and pregnancy in the gynecology-obstetrics department at the Sylvanus Olympio University hospital Center (CHU SO) in Lomé. Methodology: This was a descriptive cross-sectional study concerning obesed pregnant women. The survey was conducted from the 1st to the 30th of June 2022 at the CHU SO. Results: We enrolled 55 obese pregnant women. The frequency of obesity and pregnancy was 5.14%. Resellers were represented at 41.8%. The average age was 31 years old. As risk factors, 85.5% claimed to have a fatty diet and 76% did not practice sports. The gestational pathologies found during pregnancy were hypertension in 47.4% of cases, preeclampsia in 24.6% and gestational diabetes in 7%. Caesarean section was the way of delivery in 63.6% of cases and those who gave birth vaginally presented a tear of the soft tissues in 85% of cases. Birth weight was abnormal (low weight and excess weight) in 61.8% of cases. Conclusion: The association between obesity and pregnancy constitutes an important risk factor for the mother and the fetus.

Ovarian vein thrombosis (OVT) is a rare thrombotic condition commonly attributed to the puerperium. The complications of OVT can be significant, and diagnosis relies on a high clinical index of suspicion. It can occur with lower quadrant pain that may mimic a surgical abdomen, especially in the setting of recent pregnancy, abdominal surgery, inflammatory disease, or malignancy. Diagnosis can be made with confidence using ultrasound, computed tomography, or magnetic resonance imaging. Management of OVT is particularly important in postpartum patients, with anticoagulation therapy being the treatment of choice.

To the Editor:Gestational diabetes mellitus(GDM)is a pathologic condition in which glucose intolerance and insulin resistance develop to different degrees during pregnancy.Women with GDM have higher odds of adverse maternal and fetal outcomes.Patients with a history of GDM were at a significantly increased risk for recurrence of GDM in subsequent pregnancies.Our previous and some other studies found an increased incidence of adverse perinatal outcomes in pregnant with recurrent GDM.^([1])So far,women with a prior history of GDM are not recommended to refrain from subsequent pregnancy in light of the limited available data.What about those women with a history of recurrent GDM?Should women with recurrent GDM be recommended to avoid further pregnancy?Currently,studies on perinatal outcomes of women with re-recurrent GDM and their prognostic factors are very rare.Therefore,this retrospective cohort study was performed to explore perinatal outcomes of pregnant woman with re-recurrent GDM and their prognostic factors.

Glycogen synthase kinase-3β positively regulates the proliferation of human ovarian cancer cells

Although glycogen synthase kinase-3(GSK-3)might act as a tumor suppressor since its inhibition is expected tomimic the activation of Wnt-signaling pathway,GSK-3β may contribute to NF-κ3 activation in cancer cells leading toincreased cancer cell proliferation and survival.Here we report that GSK-3β activity was involved in the proliferationof human ovarian cancer cell both in vitro and in vivo.Inhibition of GSK-3 activity by pharmacological inhibitors sup-pressed proliferation of the ovarian cancer cells.Overexpressing constitutively active form of GSK-3β induced entryinto the S phase,increased cyclin D1 expression and facilitated the proliferation of ovarian cancer cells.Furthermore,GSK-3 inhibition prevented the formation of the tumor in nude mice generated by the inoculation of human ovariancancer cells.Our findings thus suggest that GSK-3β activity is important for the proliferation of ovarian cancer cells,implicating this kinase as a potential therapeutic target in ovarian cancer.

Gynecological and Obstetrical Emergencies at the University Clinic of gynecology-obstetrics of the National hospital Donka Guinea

Gynecological and obstetrical emergencies are found all over the world, especially in developing countries where women pay a heavy price for giving birth. They can occur at any time during pregnancy and outside of pregnancy often in a socio-economic context. The objectives of this study were to describe the sociodemographic characteristics and the maternal and fetal prognosis of gynecological and obstetric emergencies. Patients and Methods: This was a descriptive cross-sectional study with prospective data collection, conducted at the University Clinic of Gynecology and Obstetrics of the Donka National Hospital between June 1 and September 30, 2015. It involved all patients admitted to our department in emergency for a gynecological or obstetrical complaint. Results: We collected 361 cases of gynecological and obstetrical emergencies out of a total of 1779 consultations, i.e. a frequency of 20.29%. Obstetrical emergencies were predominant with 91.41% and gynecological emergencies represented 8.59%. The average age of patients was 29.5 with extremes of 14 and 47. Nulliparous women were the most numerous (34.35%). More than half of the patients did not attend school (52.08%) and 56.70% were evacuees. Abdominopelvic pain and hemorrhage were the main reasons for consultation (54.29% and 49.58%). Admission diagnoses were dominated by acute fetal distress and hemorrhage in the last quarter of pregnancy (52.3% and 36.01%). The caesarean section rate was high (82.12%). Clinical observation on 90 cases of primary dysmenorrhea treated by buccal acupuncture therapy:a randomized controlled study

OBJECTIVE:To explore the short-term and long-term efficacy of buccal acupuncture therapy in the treatment of patients with primary dysmenorrhea(PD).METHODS:A total of 90 patients with PD who were admitted to the Second Affiliated hospital of Zhejiang University of Traditional Chinese Medicine from January 2020 to December 2021 were included in this study and randomly divided into three groups,including the observation group,the control group,and the placebo group,with 30 patients in each group.Patients in the observation group were treated according to the syndrome types of Traditional Chinese Medicine,those in the control group were treated with Saridon,and those in the placebo group received a placebo.The visual analog scale(VAS)scores of these patients were evaluated at different time points(before treatment,5 min after needle retention,30 min after needle retention,1 h after treatment,2 h after treatment,1 d after treatment,and 2 d after treatment).The Cox Menstrual Symptom Scale(CMSS)scores were assessed before the treatment and after three courses of treatment.Besides,these two scales were also evaluated during follow-up.Moreover,the pain score and adverse reactions of these patients were assessed during treatment.RESULTS:After treatment,the VAS and CMSS scores in the observation group decreased significantly(P<0.05)5 and 30 min after needle retention and after treatment.CONCLUSION:Buccal acupuncture therapy is verified to have short-term efficacy in the treatment of PD.In addition,this therapy has an immediate analgesic effect and long-term efficacy in reducing the severity and frequency of PD.

Histological Features of Uterine Myometrial Dysfunction:Possible Involvement of Localized Inflammation

Objective The latest perspective suggests that elevated levels of inflammation and cytokines are implicated in atonic postpartum hemorrhage.Lipopolysaccharide(LPS)has been widely used to induce inflammation in animal models.Therefore,this study aimed to induce uterine inflammation using LPS to investigate whether local inflammation triggers dysfunction and atrophy in the myometrium,as well as the potential underlying molecular mechanisms involved.Methods In vivo,an animal model was established by intraperitoneal injection of 300μg/kg LPS in rats on gestational day 21.Hematoxylin-eosin(H&E)staining and Masson staining were employed to determine morphological changes in the rat uterine smooth muscle.Enzyme-linked immunosorbent assay(ELISA)was used to detect inflammatory cytokines.Immunohistochemistry,tissue fluorescence,and Western blotting were conducted to assess the expression levels of the uterine contraction-related proteins Toll-like receptor 4(TLR4)and the nuclear factor kappa-B(NF-κB)signaling pathway.In vitro,human uterine smooth muscle cells(HUtSMCs)were exposed to 2μg/mL LPS to further elucidate the involvement of the TLR4/NF-κB signaling pathway in LPS-mediated inflammation.Results In this study,LPS induced uterine myometrial dysfunction in rats,leading to a disorganized arrangement,a significant increase in collagen fiber deposition,and widespread infiltration of inflammatory cells.In both in vivo animal models and in vitro HUtSMCs,LPS elevated IL-6,IL-1β,and TNF-αlevels while concurrently suppressing the expression of connexin 43(Cx43)and oxytocin receptor(OXTR).Mechanistically,the LPS-treated group exhibited TLR4 activation,and the phosphorylation levels of p65 and IκBαwere notably increased.Conclusion LPS triggered the TLR4/NF-κB signaling pathway,inducing an inflammatory response in the myometrium and leading to uterine myometrial dysfunction and uterine atony.
