Is There Any Relationship between Maternal Vitamin D Levels and Asymptomatic Bacteriuria? A Preliminary Study

Hypothesis: Vitamin D has been considered to play important roles in maintaining pregnancy. Also, this vitamin is considered to regulate immune response and thus may be important for pregnant women also to maintain no-infection of which preterm delivery is especially important. Since vitamin D regulates maternal anti-infection function and since Asymptomatic bacteriuria (ASB) causes various adverse outcomes in pregnancy, we hypothesized that maternal serum vitamin D level may be related with the presence/absence of ASB. The present study was an effort to determine this. The study included 215 pregnant women, attended the Ondokuz Mayis University Hospital antenatal care polyclinic, Samsun, Turkey, in the summer for initial visits between 6- and 14-weeks’ gestation. Women with symptoms suggestive of urinary tract infection, a history of antibiotic use within the previous two weeks, ongoing antibiotic therapy or urinary system instrumentation, a diagnosis of vitamin D insufficiency or deficiency, and those taking vitamin D supplements were excluded. Demographic data features of the sampled women were analyzed retrospectively. 10 ml maternal peripheral venous blood samples were collected and the concentrations of 25(OH)D3 in the serum were analyzed. Mid-stream morning urine samples were obtained and analyzed at the microbiology laboratory according to established procedures. Endometrial Polyps:Which Patients Should Be Selected for Hysteroscopic Surgery?--A Study Using Data from the Swedish National Quality Registry of Gynecological Surgery

Objective: To illuminate the findings after hysteroscopic surgery of endometrial polyps. Method: Data were extracted from The Swedish National Quality Registry of Gynecological Surgery. Endometrial polyps were identified in 1934 cases in a total of 4512 hysteroscopic operations. Data on all hysteroscopic procedures registered as surgery of endometrial polyps between 1997 and January 2013 were analyzed with logistic regression analysis and effect size was calculated. Main outcome measure was malignancy in endometrial polyps. Results: The most frequently reported symptoms of endometrial polyps were: postmenopausal bleeding, metrorrhagia, pain, and infertility. Among the registered biopsies, there were: 30 malignancies (1.8%), and 41 dysplasias (2.5%). The remaining polyps were benign. Only 1/30 cancer patients were <52 years old (p < 0.001). Among those women with dysplasia, 12/41 (29%) were p = 0.07). All cancer patients at age ≥52 had postmenopausal bleeding. The <52-year-old cancer patient had treatment-resistant bleeding. Increasing weight was a risk factor associated with tissue alterations in endometrial polyps (p = 0.014), controlling for age as a confounding factor. Conclusion: Hysteroscopic surgery should be recommended for women with postmenopausal bleeding and presence of endometrial polyps according to the findings of this study. The results further indicate a low risk of malignancy in premenopausal women <52 years with endometrial polyps. In those cases, it seems to be safe to refrain from surgery.

Modifications of sexual activity during uncomplicated pregnancy: A prospective investigation of Spanish women

Introduction: Pregnancy is a time of profound changes in a woman’s life. These changes can influence her sexual behaviour and the relationship with her partner. The purpose of this article is to study sexual behaviour modification before and during pregnancy in a cohort of healthy Spanish pregnant women. Methods: This is a prospective cohort study in which 111 pregnant women took part. They completed a questionnaire about their sexual habits before pregnancy and at 20, 30 and 38 weeks (frequency of intercourse and orgasm, satisfaction with their relationships and pain or discomfort). Results: Intercourse frequency decreased progressively during pregnancy at all times studied relative to pre-regnancy periods, to a significant degree (p < 0.0001), accompanied by a reduction in sexual satisfaction. The incidence of orgasm was lightly modified, except in the weeks before parturition in which it underwent a notable decrease (p < 0.0001) with an increase in the rate of hypoorgasmia/ anorgasmia from 19% pre-pregnancy to 43% at 38th week. Conclusions: The longitudinal monitoring of this cohort permits the conclusion that during pregnancy, sexual behaviour undergoes a general decline with respect to prepregnancy levels. The differences vary depending on the sexual parameter considered, with intercourse frequency being the most affected variable in contrast to dyspareunia, which is the least affected.

Improved Prediction of In Vitro Fertilization Clinical Outcomes by Combining Ultrasound, Serum Hormones and Body Mass Index

Objective To identify efficient predictors of clinical outcomes of in vitro fertilization （IVF）. Methods Two hundred and one women undergoing their first IVF cycle were measured for antral follicle count （AFC） and endometrial thickness via ultrasound. On day 2 or 3 of the menstrual cycle and the day of human chorionic gonadotropin administration, serum levels of follicle-stimulating hormone （FSH）, luteinizing hormone （LH）, estradiol, and progesterone were determined. The difference of ovarian response and clinical pregnancy rates were measured. Results Basal FSH was the single best predictor of ovarian response in women 〈35 years old before the initiation of treatment, with an area under the receiver operating characteristic curve （A UC） of 0. 752. For women ≥ 35 years old, none of parameters was a single good predictor of lVF pregnancy. For women older than 35 years, adding body mass index （BMI） with AFC and basal FSH improved prediction of ovarian response （A UC=0. 859）. Prediction of clinical pregnancy for women ≥ 35 years was improved either by combining only AFC and basal FSH （AUC=0. 757） or adding BMI to the combined AFC and FSH （AUC=0. 722）. Conclusion Combining AFC, basal FSH, and BMI was the best predictor of ovarian response and clinical pregnancy after IVF in women ≥ 35 years.
