Gestational Trophoblastic Disease encompasses a group of pregnancy-related disorders that derive from the placenta. Taking Leventhal’s Common Sense Model as a starting point, this study aims to investigate how illness perception could influence patients’ psychological adaptation to these rare diseases. Thirty-seven women completed: the Illness Perception Questionnaire-Revised, the Beck Depression Inventory Short Form, the State-Trait Anxiety Inventory, and the Fertility Problem Inventory. Results show that the perception of severe illness consequences significantly predicts the level of anxiety patients reported at the time of questionnaire completion. Furthermore, mental representations of illness present a significant association with infertility-related stress. Specifically, the belief in the efficacy of the treatment results in fewer feelings of discomfort and isolation from family and social context due to infertility-related problems. Since patients’ illness perception was found to have a specific impact on both anxiety and infertility-related stress, this variable should be considered in the planning of a clinical intervention.

Objective:Oocyte degeneration has historically been associated with the intracytoplasmic sperm injection(ICSI) technique. We sought to determine whether oocyte degeneration rates were associated with the technician performing the procedure,the baseline characteristics of the patient,and/or ovarian stimulation variables. We also evaluated whether the degeneration rate could serve as a surrogate marker for implantation potential. Design:Cohort study. Setting:Academic medical center. Patient(s):Couples undergoing ICSI. Intervention(s):Six thousand six hundred fifty-three injected oocytes were analyzed to determine whether the degeneration rate was technician dependent. Two hundred thirty first-entry down-regulated cycles were examined to identify predictors associated with oocyte degeneration. Multivariate analyses were performed using generalized linear model routines. Main Outcome Measure(s):Oocyte degeneration rates and implantation rates. Result(s):Neither the ICSI technician nor the stripping technician was associated with the oocyte degeneration rate. However,the day 3 FSH,number of mature oocytes retrieved,and E2 levels on the day of hCG were significant independent predictors of degeneration rate. Physician-adjustable ovarian stimulation variables were not associated with the degeneration rate. The degeneration rate did not appear to be associated with the implantation rate. Conclusion(s):These data suggest that oocyte degeneration is not technician or physician dependent. Degeneration is likely a function of the inherent oocyte quality in women who underwent ovarian stimulation. However,the remaining cohort of retrieved oocytes appears to be unaffected by virtue of an uncompromised implantation rate.

Objective:The objective of the study was to evaluate the relative contributions of amnion and chorion to the strength of fetal membranes and to correlate these findings with gestational age. Study design:Fetal membranes from 78 pregnancies were tested for biaxial puncture force using a blunt,instrumented probe with a low-force load cell connected through a load cell conditioner to an oscilloscope. The average of 2 to 4 tests performed on independent regions of the membrane was recorded. Means and SDs were calculated through the gestational age ranges of less than 32,32 to 36,or 37 weeks or longer. Linear regression analysis was performed across gestational age after grouping data by labor and mode of delivery. Results:There were trends toward decreasing puncture force with gestational age for both chorioamnion and amnion for both vaginal deliveries and cesarean sections. The trends were significant by linear regression for labored deliveries but not unlabored cesarean sections for both chorioamnion and amnion alone. There was no trend in chorion puncture force with either gestational age or delivery mode and the mean puncture force values were,on average,half those for the amnion. Conclusion:The amnion is significantly stronger than the chorion when subjected to biaxial strength testing. The amnion but not the chorion is significantly affected by the chemical and mechanical changes during gestation and the labor process. These data will help direct future studies on the effects of clinical and molecular modulators of inflammation on membrane rupture thresholds with special emphasis on the biochemical and structural changes in the amnion.

Trivalent lanthanide(Ln^(3+))-doped luminescent nanoparticles(NPs)have been extensively investigated as deep-tissue-penetration visual bioimaging agents owing to their exceptional upconversion and near-infrared(NIR)luminescence upon irradiation of NIR ***,in most cases,the power density of irradiation used for in vivo biological imaging is much higher than that of the reported maximum permissible exposure(MPE)value of NIR light,which inevitably does great damage to the living organisms under study and thus impedes the plausible clinical ***,by using a facile syringe pump-aided shell epitaxial growth method,we construct for the first time a new class of Ln^(3+)-doped KMgF_(3):Yb/Er@KMgF_(3)core-shell NPs that can be activated by utilizing a 980-nm xenon lamp or diode laser with an ultralow excitation power density down to 0.08 mW cm^(−2),a value that is approximately 4 orders of magnitude lower than the MPE value set by the American National Standards Institute(ANSI)for safe bioimaging in *** combining the comparative spectroscopic investigations with atomic-resolved spherical aberration corrected transmission electron microscopy(AC-TEM)characterization,we find that the reduced crystallographic defects are the primary cause underlying such an ultralow-power-excitable feature of the KMgF_(3):Yb/Er@KMgF_(3)core-shell ***,by the same token,the resultant KMgF_(3):Yb/Er@KMgF_(3)core-shell NPs also exhibit an anomalous thermo-enhanced photoluminescence(PL)behavior coupled with an excellent photothermal stability that cannot occur in other Ln^(3+)-doped core-shell *** findings described here unambiguously pave a new way to prepare high-quality Ln^(3+)-doped luminescent NPs with desirable ultralow-power-excitable capability and photothermal stability for future biomedical applications.

Objective. The endometrial origin of uterine carcinosarcoma has recently been well established. The current study investigates whether uterine carcinosarcomas can be included in protocols on high-risk endometrial cancer, given the similarities in biologic behavior of both entities. Methods. Pathological and surgical notes of patients diagnosed with grade 3 endometrioid, carcinosarcoma, serous and clear cell endometrial cancer subtypes were retrospectively analyzed with special attention to the spread pattern of the different subtypes. Information on site of relapse and time to recurrence was obtained. Results. We traced 146 patients of which 9 patients were ineligible. Histological subtypes of the remaining 137 patients were as follows: 50 (37% ) grade 3 endometrioid carcinoma, 54 (39% )-serous or clear cell carcinoma (non-endometrioid carcinoma), and 33 (24% ) carcinosarcomas. Distribution of early stage disease (Ⅰ and Ⅱ ) was 67, 46, and 78% for grade 3 endometrioid, non-endometrioid, and carcinosarcoma, respectively. Although we could not trace diffe-rences in hematogenic and transperitoneal spread among the three subtypes, non-endometrioid and carcinosarcomas were more likely to spread to pelvic and paraaortic lymph nodes (P 6, Wald statistic) and histological type appear to determine the outcome, whereas lymphovascular space infiltration (P 6% ) grade 3 endometrioid, 18/24 (75% ) non-endometrioid, and 11/25 (44% ) carcinosarcomas survive

Labor Induction with Oral Misoprostol in Term Pregnancy: A Clinical Trial

Introduction: Induction of labor has become an increasingly utilized obstetric intervention in developed countries. It contributes to reduce the maternal and perinatal morbidity and mortality. Dinoprostone derivatives are often used under cardiotocography. In poor countries, health structures have neither fetal monitoring, nor means of preserving Dinoprostone derivatives. Misoprostol therefore constitutes an alternative. This study seeks to establish the efficacy and safety of oral Misoprostol and to assess maternal and perinatal prognosis in an area with limited resource. Methods: This is a multi-center clinical trial, conducted in 3 hospitals centers in Kisangani, Tshopo Province/Democratic Republic of Congo from December 1, 2020 to May 31, 2021. Our sample was constituted with 68 pregnant women with term pregnancy. Fifty micrograms of Misoprostol was administered orally. The data were collected prospectively, their encoding was carried out on an Excel sheet 2013 and their analysis carried out using the EPI INFO software. Results: The average age of the pregnant women was 23.17 ± 5.76 years. The average parity was 1.02 ± 1.2. The average body mass index (BMI) was 24.98 ± 2.55 Kg/m2. 66 (97.1%) patients had received a maximum of 3 doses of Misoprostol. Out of the 16 parturients who received 3 doses or more, 11 (68.8%) had a BMI ≥ 25. The average duration of labor was 16.03 ± 7.99 hours. 66 (97.1%) pregnant women had delivered vaginally. 57 (83.8%) patients delivered within 24 hours. Out of the 11 patients who delivered after 24 hours, 7 (63.6%) had a BMI ≥ 25. The induction failure rate was 2.9% (2 patients). 2 (2.9%) fetuses had presented pathological modification of fetal heart rate (FHR). 2 (2.9%) patients had undergone cesarean section. Conclusion: Labor induction with oral Misoprostol is effective and associated with low maternal and perinatal morbidity. A BMI ≥ 25 kg/m2
