Impact of continuous positive airway pressure therapy for nonalcoholic fatty liver disease in patients with obstructive sleep apnea

BACKGROUND Obstructive sleep apnea(OSA)has been suggested as an independent risk factor for nonalcoholic fatty liver disease(NAFLD),and continuous positive airway pressure(CPAP)is the first-line therapy for *** To clarify the efficacy of effective CPAP therapy on NAFLD of OSA patients by serum markers and transient elastography(TE)using FibroScan®(Echosens,Paris,France).METHODS We prospectively enrolled 123 consecutive patients with OSA who met the indications for *** fibrosis and steatosis were assessed using *** and after 6 mo of CPAP therapy,serum markers and TE were assessed for all *** mean usage rate of CPAP therapy for 6 mo was arbitrarily calculated in each patient and expressed as“mean compliance index”(m-CI).RESULTS In 50 OSA patients with NAFLD,both aspartate aminotransferase(AST)and alanine aminotransferase(ALT)levels were significantly decreased after 6 mo of CPAP *** analysis showed that decreased body weight(BW),decreased body mass index(BMI),decreased AST level,decreased hemoglobin A1c,and high m-CI were significantly related with improved ALT *** multivariate regression model adjusted for quantities of BW change during 6 mo of CPAP therapy,high m-CI tended to improve ALT level(P=0.051).All 17 OSA patients with NAFLD,high m-CI and no BMI changes showed significant improvements in AST and ALT ***,no significant changes in TE data or serum fibrosis markers were *** Some NAFLD could be associated with chronic intermittent hypoxia due to OSA independent of BW *** those cases,adequate reoxygenation from effective CPAP therapy may improve NAFLD.

We propose a new laser irradiation method for the treatment of cutaneous lesions in plastic surgery. In general, lasers with a spot size of 1 to 10 mm are used in irradiation on diseased skin. Although the target absorbs more light energy according to the theory of selective photothermolysis, the surrounding tissue, however, is still somewhat damaged. In proposed method, an f-theta lens, which is assembled by a shrink fitter, focuses the irradiation laser beam to a very fine spot with the size of 125 μm. Guided by the captured object-image, such laser beam is conducted by a pair of galvanometer-driven mirrors to irradiate only the desired tissue target without thermal damage to surrounding tissue. Moreover, an optical coherence tomography, whose probe is capable of wide field of view, can be used to provide the guidance information for the best treatment. The usefulness of the developed laser therapy apparatus was demonstrated by performing an experiment on the removal of tattoo pigment.

Dynamic nuclear self-polarization of Ⅲ–Ⅴsemiconductors

Ⅲ–Ⅴ semiconductors exhibit dynamic nuclear self-polarization(DYNASP) owing to the contact hyperfine interaction(HFI) between optically excited conduction electrons and lattice nuclei. In the self-polarization process at a low temperature, electron spin state and the nuclear polarization(magnetization) exchange a positive feedback, increasing energy splitting of the conduction electron states, thereby a large nuclear polarization. This phenomenon was theoretically predicted previously for conduction electrons excited linearly and elliptically polarized light. The polarization of the conduction electrons was represented by a parameter α in a formula for nuclear polarization(Eq.(9) in Ref. [1]); however, the effect of external magnetic fields on the nuclear polarization was not considered. Therefore, this study introduces this effect by further extending the previous studies. Herein, α′represents the combination of the effects of elliptically polarized electrons and an external magnetic field, which is used in the equations presented in previous studies. When α′ = 0, a large nuclear polarization is obtained below critical temperature Tc, but no polarization occurs above Tc. When α′ > 0, the nuclear polarization is enhanced above Tc. Below Tc, the nuclear polarization follows a hysteresis curve when α′ is partially manipulated by adjusting the degree of the polarization of the exciting laser.

Management of gastric and duodenal neuroendocrine tumors

Gastrointestinal neuroendocrine tumors (GI-NETs) are rare neoplasms, like all NETs. However, the incidence of GI-NETS has been increasing in recent years. Gastric NETs (G-NETs) and duodenal NETs (D-NETs) are the common types of upper GI-NETs based on tumor location. G-NETs are classified into three distinct subgroups: type I, II, and III. Type I G-NETs, which are the most common subtype (70%-80% of all G-NETs), are associated with chronic atrophic gastritis, including autoimmune gastritis and Helicobacter pylori associated atrophic gastritis. Type II G-NETs (5%-6%) are associated with multiple endocrine neoplasia type 1 and Zollinger-Ellison syndrome (MEN1-ZES). Both type I and II G-NETs are related to hypergastrinemia, are small in size, occur in multiple numbers, and are generally benign. In contrast, type III G-NETs (10%-15%) are not associated with hypergastrinemia, are large-sized single tumors, and are usually malignant. Therefore, surgical resection and chemotherapy are generally necessary for type III G-NETs, while endoscopic resection and follow-up, which are acceptable for the treatment of most type I and II G-NETs, are only acceptable for small and well differentiated type III G-NETs. D-NETs include gastrinomas (50%-60%), somatostatin-producing tumors (15%), nonfunctional serotonin-containing tumors (20%), poorly differentiated neuroendocrine carcinomas (< 3%), and gangliocytic paragangliomas (< 2%). Most D-NETs are located in the first or second part of the duodenum, with 20% occurring in the periampullary region. Therapy for D-NETs is based on tumor size, location, histological grade, stage, and tumor type. While endoscopic resection may be considered for small nonfunctional D-NETs (G1) located in the higher papilla region, surgical resection is necessary for most other D-NETs. However, there is no consensus regarding the ideal treatment of D-NETs.

A portable shape-shifting mobile robot system named as Amoeba Ⅱ（A-Ⅱ） is developed for the urban search and rescue application. It is designed with three degrees of freedom and two tracked drive systems. This robot consists of two modular mobile units and a joint unit. The mobile unit is a tracked mechanism to enforce the propulsion of robot. And the joint unit can transform the robot shape to get high environment adaptation. A-Ⅱ robot can not only adapt to the environment but also change its body shape according to the locus space. It behaves two work states including the linear state （named as I state） and the parallel state （named as Ⅱ state）. With the linear state the robot can climb upstairs and go through narrow space such as the pipe, cave, etc. The parallel state enables the robot with high mobility on rough ground. Also, the joint unit can propel the robot to roll in sidewise direction. Two modular A-Ⅱ robots can be connected through jointing common interfaces on the joint unit to compose a stronger shape-shifting robot, which can transform the body into four wheels-driven vehicle. The experimental results validate the adaptation and mobility of A-Ⅱ robot.

Enhanced organic solar cell performance: Multiple surface plasmon resonance and incorporation of silver nanodisks into a grating‐structure electrode

In this study, plasmonic nanostructures were examined to enhance the light harvesting of organic thin-film solar cells (OSCs) by multiple surface plasmon resonance (SPR) phenomena originating from the grating-coupled configuration with a Blu-ray Disc recordable (BD-R)-imprinted aluminum (Al) grating structure and the incorporation of a series of silver nanodisks (Ag NDs). The devices with such a configuration maximize the light utilization inside OSCs via light absorption, light scattering, and trapping via multiple surface plasmon resonances. Different types and sizes of metallic nanoparticles (NPs), i.e., gold nanoparticles (Au NPs), Ag nanospheres (Ag NSs), and Ag NDs, were used, which were blended separately in a PEDOT:PSS hole transport layer (HTL). The device structure comprised of grating- imprinted-Al/P3HT:PCBM/Ag ND:PEDOT:PSS/ITO. Results obtained from the J–V curves revealed that the power conversion efficiency (PCE) of grating-structured Al/P3HT:PCBM/PEDOT:PSS/ITO is 3.16%;this value is ~6% higher than that of a flat substrate. On the other hand, devices with flat Al and incorporated Au NPs, Ag NSs, or Ag NDs in the HTL exhibited PCEs ranging from 3.15% to 3.37%. Furthermore, OSCs with an Al grating substrate were developed by the incorporation of the Ag ND series into the PEDOT:PSS layer. Compared with that of a reference device, the PCEs of the devices increased to 3.32%–3.59%(11%–20% improvement), indicating that the light absorption enhancement at the active layer corresponds to the grating-coupled surface plasmon resonance and localized surface plasmon resonance excitations with strong near-field distributions penetrating into the active layer leading to higher efficiencies and subsequent better current generation.

Raman spectroscopy for the diagnosis of unlabeled and unstained histopathological tissue specimens

AIM To investigate the possibility of diagnosing gastric cancer from an unstained pathological tissue using Raman spectroscopy, and to compare the findings to those obtained with conventional histopathology. METHODS We produced two consecutive tissue specimens from areas with and without cancer lesions in the surgically resected stomach of a patient with gastric cancer. One of the two tissue specimens was stained with hematoxylin and eosin and used as a reference for laser irradiation positioning by the spectroscopic method. The other specimen was left unstained and used for Raman spectroscopy analysis. RESULTS A significant Raman scattering spectrum could be obtained at all measurement points. Raman scattering spectrum intensities of 725 cm^(-1) and 782 cm^(-1), are associated with the nucleotides adenine and cytosine, respectively. The Raman scattering spectrum intensity ratios of 782 cm^(-1)/620 cm^(-1), 782 cm^(-1)/756 cm^(-1), 782 cm^(-1)/1250 cm^(-1), and 782 cm^(-1)/1263 cm^(-1) in the gastric adenocarcinoma tissue were significantly higher than those in the normal stomach *** The results of this preliminary experiment suggest the feasibility of our spectroscopic method as a diagnostic tool for gastric cancer using unstained pathological specimens.

This paper proposes a novel method for analyzing a textile fabric structure to extract positional information regarding each yarn using three-dimensional X-ray computed tomography(3D CT) *** relationship among the yarns can be reconstructed using the extracted yarn positional *** this paper,a sequence of points on the center line of each yarn of the sample is defined as the yarn positional information,since the sequence can be regarded as the representative position of the *** sequence is extracted by tracing the *** yarn is traced by estimating the yarn center and direction and correlating the yarn part of the 3D CT image with a 3D yarn model,which is moved along the estimated yarn *** trajectory of the center of the yarn model corresponds to the positional information of the *** application of the proposed method is shown by experimentally applying the proposed method to a 3D CT image of a double-layered woven ***,the experimental results for a plain knitted fabric show that this method can be applied to even knitted fabrics.

BACKGROUND Sinonasal undifferentiated carcinoma(SNUC) is a rare aggressive tumor that is often unresectable. Optimal treatment for patients with unresectable,locally advanced SNUC(LA-SNUC) has not been established,and the patient outcome remains poor. We report two cases of unresectable LA-SNUC in which induction chemotherapy with docetaxel,cisplatin and fluorouracil(TPF) followed by radiotherapy with concurrent cisplatin(CCRT),a standard treatment option for locally advanced head and neck cancer,demonstrated promising *** SUMMARY A 39-year-old man presented with tearing and pain in the right eye. A biopsy of the tumor invading the sinonasal cavities,right orbit and cranial base confirmed the diagnosis of LA-SNUC. Induction TPF chemotherapy induced remarkable tumor shrinkage and rapidly improved the symptoms. He subsequently received CCRT and achieved complete remission of the disease. The other case is a 21-year-old man who presented with worsening vision. The unresectable tumor involving the nasal septum and cranial base was pathologically diagnosed as SNUC. TPF chemotherapy followed by CCRT yielded complete remission of the disease with preserved visual function. Both patients have been disease-free for44 *** Induction TPF chemotherapy followed by CCRT may remarkably improve the outcomes in LA-SNUC patients.
