Cerebral ischemia is a major health risk that requires preventive approaches in addition to drug therapy.Physical exercise enhances neurogenesis and synaptogenesis,and has been widely used for functional rehabilitation after stroke.In this study,we determined whether exercise training before disease onset can alleviate the severity of cerebral ischemia.We also examined the role of exercise-induced circulating factors in these effects.Adult mice were subjected to 14 days of treadmill exercise training before surgery for middle cerebral artery occlusion.We found that this exercise pre-conditioning strategy effectively attenuated brain infarct area,inhibited gliogenesis,protected synaptic proteins,and improved novel object and spatial memory function.Further analysis showed that circulating adiponectin plays a critical role in these preventive effects of exercise.Agonist activation of adiponectin receptors by Adipo Ron mimicked the effects of exercise,while inhibiting receptor activation abolished the exercise effects.In summary,our results suggest a crucial role of circulating adiponectin in the effects of exercise pre-conditioning in protecting against cerebral ischemia and supporting the health benefits of exercise.

Motor imagery is the mental representation of an action without overt movement or muscle activation. However, the effects of motor imagery on stroke-induced hand dysfunction and brain neural networks are still unknown. We conducted a randomized controlled trial in the China Rehabilitation Research center. Twenty stroke patients, including 13 males and 7 females, 32–51 years old, were recruited and randomly assigned to the traditional rehabilitation treatment group（PP group, n = 10） or the motor imagery training combined with traditional rehabilitation treatment group（MP group, n = 10）. All patients received rehabilitation training once a day, 45 minutes per session, five times per week, for 4 consecutive weeks. In the MP group, motor imagery training was performed for 45 minutes after traditional rehabilitation training, daily. Action Research Arm Test and the Fugl-Meyer Assessment of the upper extremity were used to evaluate hand functions before and after treatment. Transcranial magnetic stimulation was used to analyze motor evoked potentials in the affected extremity. Diffusion tensor imaging was used to assess changes in brain neural networks. Compared with the PP group, the MP group showed better recovery of hand function, higher amplitude of the motor evoked potential in the abductor pollicis brevis, greater fractional anisotropy of the right dorsal pathway, and an increase in the fractional anisotropy of the bilateral dorsal pathway. Our findings indicate that 4 weeks of motor imagery training combined with traditional rehabilitation treatment improves hand function in stroke patients by enhancing the dorsal pathway. This trial has been registered with the Chinese Clinical Trial Registry（registration number： Chi CTR-OCH-12002238）.

Melatonin and gamma-aminobutyric acid（GABA） have been shown to regulate sleep. The nocturnal concentrations of melatonin, GABA and total antioxidants may relate to insomnia in stroke patients. In this prospective single-center non-randomized controlled clinical trial performed in the China Rehabilitation Research center, we analyzed the relationship of nocturnal concentrations of melatonin, GABA and total antioxidants with insomnia after stroke. Patients during rehabilitation of stroke were recruited and assigned to the insomnia group or non-insomnia group. Simultaneously, persons without stroke or insomnia served as normal controls. Each group contained 25 cases. The primary outcome was nocturnal concentrations of melatonin, GABA and total antioxidants in peripheral blood. The secondary outcomes were Pittsburgh Sleep Quality Index, Insomnia Severity Index, Epworth Sleepiness Scale, Fatigue Severity Scale, Morningness-Eveningness Questionnaire（Chinese version）, and National Institute of Health Stroke Scale. The relationship of nocturnal concentrations of melatonin, GABA and total antioxidants with insomnia after stroke was analyzed and showed that they were lower in the insomnia group than in the non-insomnia group. The severity of stroke was higher in the insomnia group than in the non-insomnia group. Correlation analysis demonstrated that the nocturnal concentrations of melatonin and GABA were associated with insomnia after stroke. Stroke is the third leading cause of long-term disability.Effective stroke rehabilitation should be provided to patients with a moderate or severe stroke so as to achieve functional recovery.Also,stroke rehabilitation is moving towards more intense therapy models that incorporate technologies such as robotics and computer games.Evidence-based guidelines are useful for promoting the best possible clinical practices.This guideline summarises the modes of stroke rehabilitation in China-three-stage rehabilitation network,rehabilitation of stroke dysfunction,training for activities of daily living,stroke complications and rehabilitation care.The aim of this guideline was to provide a synopsis of best clinical practices in the stroke rehabilitation of adult patients in China.

Retinal dysfunction is the most common cause of vision loss in several retinal disorders.It has been estimated a great increase in these pathologies that are becoming more globally widespread and numerous over time,also supported by the life expectancy increment.Among different types of retinopathies,we can account some that share causes,symptoms,and treatment including diabetic retinopathy,age-related macular degeneration,glaucoma,and retinitis pigmentosa.Molecular changes,environmental factors,and genetic predisposition might be some of the main causes that drive retinal tissue to chronic inflammation and neurodegeneration in these retinopathies.The treatments available on the market contain compounds that efficiently ameliorate some of the important clinical features of these pathologies like stabilization of the intraocular pressure,reduction of eye inflammation,control of eye oxidative stress which are considered the major molecular mechanisms related to retinal dysfunction.Indeed,the most commonly used drugs are antiinflammatories,such as corticosteroids,antioxidant,hypotonic molecules and natural neuroprotective compounds.Unfortunately,these drugs,which are fundamental to treating disease symptoms,are not capable to cure the pathologies and so they are not life-changing for patients.This review provided an overview of current treatments on the market,but more interestingly,wants to be a quick window on the new treatments that are now in clinical trials.Additionally,it has been here highlighted that the recent technical enhancement of the investigation methods to identify the various retinopathies causes might be used as a sort of“precise medicine”approach to tailor the identification of molecular pathways involved and potentially study a dedicated treatment for each patient.This approach includes the use of cutting-edge technologies like gene therapy and metabolomics.

Endovascular middle cerebral arterial occlusion in a nonhuman primate model of chronic stroke

No study has reported the safety, effectiveness, and consistency of endovascular middle cerebral artery occlusion in a chronic cerebral ischemia model. Nor have studies verified the safest and most effective segment, or branch, in the embolic middle cerebral artery. In this experiment, cerebral infarction models were established at M1, and on the upper and lower trunks on the contralateral side of the handedness of rhesus monkeys by using endovascular intervention. The results confirmed a high animal survival rate in stroke models of middle cerebral artery upper trunk occlusion. There was pronounced paralysis at the acute phase, long-term upper extremity dysfunction at the chronic phase, and the models showed good repeatability and consistency. Thus, this study describes a safe and effective model of chronic stroke.

Visual Fixation Assessment in Patients with Disorders of Consciousness Based on Brain-Computer Interface

Visual fixation is an item in the visual function subscale of the Coma Recovery Scale-Revised （CRS-R）. Sometimes clinicians using the behavioral scales find it difficult to detect because of the motor impairment in patients with disorders of consciousness （DOCs）. Brain- computer interface （BCI） can be used to improve clinical assessment because it directly detects the brain response to an external stimulus in the absence of behavioral expres- sion. In this study, we designed a BCI system to assist the visual fixation assessment of DOC patients. The results from 15 patients indicated that three showed visual fixation in both CRS-R and BCI assessments and one did not show such behavior in the CRS-R assessment but achieved significant online accuracy in the BCI assessment. The results revealed that electroencephalography-based BCI can detect the brain response for visual fixation. Therefore, the proposed BCI may provide a promising method for assisting behavioral assessment using the CRS-R.

Innate social investigation behaviors are critical for animal survival and are regulated by both neural circuits and neuroendocrine factors.Our understanding of how neuropeptides regulate social interest,however,is incomplete at the current stage.In this study,we identified the expression of secretin(SCT) in a subpopulation of excitatory neurons in the basolateral amygdala.With distinct mole cular and physiological features,BLASCT+cells projected to the medial prefrontal cortex and were necessary and sufficient for promoting social investigation behaviors,whilst other basolateral amygdala neurons were anxiogenic and antagonized social behaviors.Moreover,the exogenous application of secretin effectively promoted social interest in both healthy and autism spectrum disorder model mice.These results collectively demonstrate a previously unrecognized group of amygdala neurons for mediating social behaviors and suggest promising strategies for social deficits.

Spinal cord injury(SCI)is an overwhelming and incurable disabling event accompanied by complicated inflammation-related pathological processes,such as excessive reactive oxygen species(ROS)produced by the infiltrated inflammatory immune cells and released to the extracellular microenvironment,leading to the widespread apoptosis of the neuron cells,glial and oligodendroctyes.In this study,a thioketal-containing and ROS-scavenging hydrogel was prepared for encapsulation of the bone marrow derived mesenchymal stem cells(BMSCs),which promoted the neurogenesis and axon regeneration by scavenging the overproduced ROS and re-building a regenerative microenvironment.The hydrogel could effectively encapsulate BMSCs,and played a remarkable neuroprotective role in vivo by reducing the production of endogenous ROS,attenuating ROS-mediated oxidative damage and downregulating the inflammatory cytokines such as interleukin-1 beta(IL-1β),interleukin-6(IL-6)and tumor necrosis factor-alpha(TNF-α),resulting in a reduced cell apoptosis in the spinal cord tissue.The BMSCs-encapsulated ROS-scavenging hydrogel also reduced the scar formation,and improved the neurogenesis of the spinal cord tissue,and thus distinctly enhanced the motor functional recovery of SCI rats.Our work provides a combinational strategy against ROS-mediated oxidative stress,with potential applications not only in SCI,but also in other central nervous system diseases with similar pathological conditions.
