China faces the greatest challenge from stroke in the world.The death rate for cerebrovascular diseases in China was 149.49 per 100000,accounting for 1.57 million deaths in 2018.It ranked third among the leading causes of death behind malignant tumours and heart disease.The age-standardised prevalence and incidence of stroke in 2013 were 1114.8 per 100000 population and 246.8 per 100000 person-years,respectively.According to the Global Burden of disease Study 2017,the years of life lost(YLLs)per 100000 population for stroke increased by 14.6%;YLLs due to stroke rose from third highest among all causes in 1990 to the highest in 2017.The absolute numbers and rates per 100000 population for all-age disability-adjusted life years(DALYs)for stroke increased substantially between 1990 and 2017,and stroke was the leading cause of all-age DALYs in 2017.The main contributors to cerebrovascular diseases include behavioural risk factors(smoking and alcohol use)and pre-existing conditions(hypertension,diabetes mellitus,dyslipidaemia and atrial fibrillation(AF)).The most prevalent risk factors among stroke survivors were hypertension(63.0%-84.2%)and smoking(31.7%-47.6%).The least prevalent was AF(2.7%-7.4%).The prevalences for major risk factors for stroke are high and most have increased over time.Based on the latest national epidemiological data,26.6%of adults aged≥15 years(307.6 million adults)smoked tobacco products.For those aged≥18 years,age-adjusted prevalence of hypertension was 25.2%;adjusted prevalence of hypercholesterolaemia was 5.8%;and the standardised prevalence of diabetes was 10.9%.For those aged≥40 years,the standardised prevalence of AF was 2.31%.Data from the Hospital Quality Monitoring System showed that 3010204 inpatients with stroke were admitted to 1853 tertiary care hospitals during 2018.Of those,2466785(81.9%)were ischaemic strokes(ISs);447609(14.9%)were intracerebral haemorrhages(ICHs);and 95810(3.2%)were subarachnoid haemorrhages(SAHs).The average age of patients admitted was 66 years old,and nearl

Cancer Mortality and Cause Eliminated Life Expectancy in Key Areas of Four Provinces——China,2008-2018

Introduction:Cancer is a major health problem in China.Integrated interventions have been implemented in key areas of Anhui,Henan,Jiangsu,and Shandong provinces with historically higher than average cancer mortality.Assessing the cancer mortality trend and its impact on life expectancy(LE)could help evaluate the effectiveness of interventions in these regions.Methods:Based on the national Cause-of-Death Surveillance,we analyzed the standardized mortality rate(SMR)of cancer,cause eliminated life expectancy(CELE),potential gains in life expectancy(PGLEs),and rate of life lost in key areas of 4 provinces from 2008 to 2018.Joinpoint program was used to compute the average annual percentage change(AAPC)of cancer mortality.Arriaga’s decomposition method was used to estimate the contribution of cancer to LE in each age group.Results:From 2008 to 2018,cancer SMR decreased in the study region(AAPC=−3.09%,P<0.001),which increased LE.The positive effect was the greatest in the 75-79 age group(0.120 years,2.90%),and the negative effect was the greatest in the 50-54 age group(-0.094 years,−2.20%).Compared to 2008,cancer CELE increased by 3.95 years,PGLEs increased by 0.32 years,and rate of life lost increased by 0.21%in 2018.Conclusions:Cancer SMR decreased in key areas of 4 provinces from 2008 to 2018.This change had a positive effect on the increase of LE.However,the rate of life lost due to cancer increased.Integrated interventions should continue to further reduce the cancer burden.

Association between physical activity energy expenditure and cardiometabolic risk factor clustering among Chinese adults in 2015

To understand the association between cardiometabolic risk factor(CMRF)clustering and physical activity(PA)levels,we included 86520 Chinese adults aged 18–64 years having at least one CMRF(hypertension,diabetes,dyslipidemia,or obesity)from the China chronic disease and Nutrition Surveillance survey in 2015,a nationally and provincially representative investigation with a multistage clustering sampling design.Self-reported PA information was collected with the Global Physical Activity Questionnaire through face-to-face interviews.In view of the obesity epidemic in CMRF patients,PA energy expenditure(PAEE)per kilogram body weight was used,and was defined into four categories:(i)inactivity:0 kJ/kg/day;(ii)low activity:0–5 kJ/kg/day;(iii)moderate activity:6–11 kJ/kg/day;and(iv)vigorous activity:≥12 kJ/kg/day.The estimated weighted prevalence(95%confidence interval[CI])of having 1,2,3,and 4 CMRFs was 60.57%(59.48%–61.67%),28.10%(27.40%–28.79%),9.82%(9.22%–15.42%)and 1.50%(1.37%–1.63%),respectively.The rate(95%CI)of inactivity,low activity,moderate activity,and vigorous activity was 34.52%(32.69%–36.35%),22.22%(21.37%–23.37%),15.98%(15.38%–16.58%)and 27.28%(26.02%–28.53%),respectively.For those having 2,3 and 4 CMRFs(compared to those having 1 CMRF),the adjusted odds ratio(95%CI)for moderate activity and vigorous activity were 0.91(0.85–0.98)and 0.92(0.85–0.99),0.87(0.80–0.95)and 0.84(0.77–0.92),and 0.77(0.67–0.89)and 0.85(0.72–1.00),respectively.In conclusion,CMRF clustering was a pandemic among Chinese adults in 2015 and was inversely associated with PA level.PAEE(in kJ/kg/day)may be introduced into PA management practice,especially for populations with high body weight.

What is already known about this topic?Esophageal cancer(EC)is one of the most common malignant tumors in China.The new cases and deaths in China account for more than half of the world,and the disease burden of esophageal cancer is serious.What is added by this report?From 1990 to 2019,the disease burden of EC in China showed a decrease overall;it first increased between 1990 and 2004,but then decreased between 2004 and 2019.The burden of EC in men was much higher than that in women.Age was an important factor affecting the burden of EC,with disease burden rising rapidly after 40 years old.What are the implications for public health practices?The screening,early diagnosis,and treatment for EC should continue to be strengthened in China.Middleaged and elderly men are high-risk groups of EC and should be a key population for EC prevention and control.

In the past 30 years,the prevalence of diabetes in China has been increasing,but the awareness rate,treatment rate and control rate of diabetes are relatively low,and the death rate,disability rate,disease burden of diabetes and its complications are constantly increasing,which brings heavy burden to individuals,families and social economy.The Healthy China Initiative(2019-2030)is a roadmap to a healthy China and an innovative public policy system project.The Diabetes prevention and control Action is one of the four chronic and noncommunicable diseases prevention and control actions in the Healthy China Initiative(2019-2030).Based on the background,basic principles and key contents of diabetes prevention and control,this paper interpreted the diabetes prevention and control action from five aspects,that is,healthy lifestyle is the foundation,which emphasized that individuals are the first responsibility for their health,and advocates reducing the risk of diabetes by practicing healthy diet,increasing physical activity and maintaining a healthy weight;Early detection and early intervention are key links,which emphasized to pay attention to individual blood glucose level,high risk individuals regularly test blood glucose;Standardize health management is the priority,which emphasized to strengthen the implementation of graded diagnosis and treatment standards,and medical institutions provide guidance on self-management of health for patients with diabetes;The improvement of capacity in primary level is the guarantee,where emphasis is placed on improving the ability of medical personnel to screen,intervention and standardize diagnosis and treatment;And innovative health service models are the key measures which emphasized the use of information technology to improve the efficiency of health management.

Mortality of Common Gastrointestinal Tract Cancers--Huai River Basin, 2008–2018

What is already known on this topic?Gastrointestinal(GI)tract cancer is a leading cause of death and produces a heavy disease burden.GI tract cancer in the Huai River Basin was reportedly higher than the national level during 2004–2006,while current mortality rates and variations have not been reported recently.What is added by this report?During 2008 to 2018,significant decreases were observed in the rates of esophageal cancer(from 28.5 to 13.2 per 100,000)and stomach cancer(from 32.1 to 16.5 per 100,000).There was no statistical difference for the mortality rates of colorectal cancer,which actually showed a significant increase among men aged 45 to 54 years and women aged below 55 years.Substantial disparities exist among different sexes,age groups,and geographical regions.What are the implications for public health practice?These results highlight that the mortality of GI tract cancers in the Huai River Basin in 2018 are similar to national levels and still produce a heavy disease burden.More attention is needed to provide important evidence for evaluating the improvement and remaining gaps in cancer prevention and control strategies in the Huai River Basin.

What is already known about this topic?Parkinson’s disease(PD)affects 1%of the population over the age of 60 years old and 4%or more over the age of 80 years old.An estimated nearly 3 million elderly people are currently affected by Parkinson’s disease in China,and the number of patients of all ages will be close to 5 million by 2030.What is added by this report?Rates of awareness,drug treatment,and rehabilitation among PD patients were 32.4%,37.8%,and 16.0%,respectively.The rates of awareness and treatment were lower in rural areas than in urban areas and in older age groups.What are the implications for public health practice?Efforts should be made to improve health education among the elderly,to improve professional training of primary care and other health care institutions,and to strengthen the construction of community-based rehabilitative intervention for the elderly with PD.
