The study of various medical conditions is limited by the current state of knowledge. New tech-nologies enable researchers to advance their understanding of various medical conditions and to advance their understanding of particular pathologies. Strannik Virtual Scanning (SVS) is such a technology. SVS is able to determine the genetic and non-genetic components of every pathology in each of the 30 main organs, and it is able to determine the onset of pathologies at a much earlier stage than any other technologies or techniques. This makes it an ideal tool to screen for the onset and progression of pathologies which are implicated in multi-systemic conditions and to determine the spectrum of pathologies i.e. comorbidities which are involved in diabetes, cardiovascular disease, migraine, chronic fatigue syndrome, fibromyalgia, depression, chronic kidney disease, etc. This case study illustrates how the technology can be usefully and cost-effectively deployed to determine the spectrum of pathologies which characterise Reynaud’s phenomenon. This includes pathologies in the liver and kidneys e.g. portal hypertension, renal insufficiency, nephritis, liver insufficiency, pre-symptomatic onset of heart attack, presymptomatic indications of dementia, pancreatic cancer, and ovarian cancer, which are difficult to diagnose and/or for which there is currently an unmet clinical need.

Background: The organs in the body function in coherent organ networks. These organ networks are commonly known as physiological systems. Blood Glucose, Blood Pressure and pH exhibit the characteristics of neurally regulated Physiological Systems. Any medical condition, such as diabetes, has origins which are due to systemic dysfunction. This influences the genetic expression of proteins and the rate at which such expressed proteins subsequently react. Increased levels of acidity influence the levels of available minerals, protein conformation, and hence the rate at which expressed proteins such as insulin and leptin react or function. This is particularly significant in diabetes etiology where a deficiency of insulin and insulin-“resistance” are significant features of type 1 and type 2 diabetes. Proteins such as Insulin absorb and emit light. Moreover, the spectrum and intensity of the bioluminescence emitted from glycated proteins (which are more significantly bioluminescent) influence colour perception. Accordingly, changes to the diabetic’s colour perception can be used as the basis of a cognitive screening technique which is able to quantify the influence of genotype and phenotype. This may have significant advantages over current biomarker techniques which are not able to satisfactorily determine the earliest onset of?diabetes or distinguish between the symptomatic and presymptomatic onset of diabetes. Such methodology, based upon the properties of proteins, i.e. effectively, the rate at which proteins are expressed and the rate at which such expressed proteins subsequently react, allows the clinician to quantify genotype and phenotype and may contribute to a greater understanding of the processes responsible for what are commonly known as type 1 and type 2 diabetes. New technologies bring new insights. The existence of the first mathematical model of how the brain regulates the autonomic nervous system and physiological systems, and knowledge of how this can be applied to screen or treat the diabetic, enables the author to take a comprehensive view of the etiology of diabetes and obesity. This paper illustrates the dynamic relationship between the brain, physiological systems and visceral organs. It highlights that 1) blood glucose is a neurally regulated physiological system, and/or network of organs, which function coherently in order to maintain blood glucose within normal physiological limits;and 2) that dysregulation of this physiological system (regulation of blood glucose levels), by genetic or phenotypic pathologies in the organs in this system, influences the prevailing levels of insulin, directly influences brain function and hence the coherent function of this and other physiological systems and organs e.g. blood pressure, sleep, pH, digestion, sexual function, etc. In addition 3) the observation that proteins may be coiled and reactive, or uncoiled and resist reacting with their reactive substrates, leads to a greater level of understanding of the mechanisms responsible for type 2 diabetes and obesity and how such conditions could be screened and treated.

Is a split-dose regimen of 2 L polyethylene glycol plus ascorbic acid tolerable for colonoscopy in an early morning visit to a comprehensive medical check-up?

To evaluate the effectiveness and tolerability of a split-dose 2 L polyethylene glycol (PEG)/ascorbic acid (AA) regimen for healthy examinees who visited for comprehensive medical check-up in the early *** February 2015 to March 2015, examinees of average risk who were scheduled for a colonoscopy in the morning were retrospectively *** 189 examinees were divided into split-dose and non-split-dose groups. The adequacy of bowel preparation for the split-dose group vs the non-split-dose group was 96.8% vs 85.2%, respectively, P < 0.001, and the compliance of the last meal restriction was 74.6% vs 58.2%, respectively, P < 0.001. The sleep disturbance (P < 0.001) was more prevalent in the split-dose group, however the willingness to repeat the same preparation method (P = 0.243) was not different in both groups. The split-dose regimen was the most important factor influencing adequate bowel preparation in multivariate analysis (HR = 10.89, 95%CI: 6.53-18.17, P < 0.001).CONCLUSIONA split-dose regimen of 2 L PEG/AA for an early morning colonoscopy was more effective and showed better compliance for diet restriction without any difference in satisfaction and discomfort. Introducing a split-dose regimen of 2 L PEG/AA to morning colonoscopy examinees is effective and tolerable in a comprehensive medical check-up setting.

Surrogate potency assays:Comparison of binding profiles complements dose response curves for unambiguous assessment of relative potencies

Surface plasmon resonance(SPR) systems are widely used for detailed characterization of antibody activities including antigen and Fc-receptor binding. During the later stages of development, where the focus is to ensure that established critical quality attributes(CQAs) are maintained during cell culture,purification and formulation processes, analysis is simplified, and relative potencies are often determined. Here, simulation of binding data revealed that relative potency values, determined via parallel line analysis(PLA) and half maximal effective concentration(EC50) analysis accurately reflect changes in active concentration only if binding kinetics remain unchanged. Changes in the association rate constant shifted dose response curves, and therefore relative potencies, in the same way as changes in analyte concentration do. However, for interactions characterized by stable binding, changes in the dissociation rate constant did not result in any shift, suggesting that this type of change may go unnoticed in the dose response curve. Thus, EC50 and PLA analyses of dose response curves obtained with an anti-TNF-α antibody were complemented with the Biacore functionality for sensorgram comparison analysis, whereby changes in antigen and Fc-receptor binding profiles could be detected. Next, analysis of temperature stressed TNF-α antibody revealed that calibration free concentration analysis(CFCA) data correlated perfectly with relative potency values. Together, these results demonstrate that combinations of SPR based dose response curves, sensorgram comparison and CFCA can be used to strengthen the confidence in relative potency assessments, and suggest that SPR can potentially be used as a surrogate potency assay in the quality control of biotherapeutic medicines.
