How Can Practitioners Support Citizen Volunteers in Disaster Risk Reduction? Insight from “Good and Ready” in Aotearoa New Zealand

Global and national policy frameworks emphasize the importance of people's participation and volunteers' role in disaster risk reduction. While research has extensively focused on volunteers in disaster response and recovery, less attention has been paid on how organizations involved in disaster risk management can support volunteers in leading and coordinating communitybased disaster risk reduction. In 2019, the New Zealand Red Cross piloted the Good and Ready initiative in Auckland,Aotearoa New Zealand, with the objective to empower local people in resilience building with a focus on volunteers and community participation. This research examined the positive and negative outcomes of Good and Ready and investigated volunteers' experiences in the disaster resilience initiative. It involved the codesign of a questionnaire-based survey using participatory methods with Good and Ready volunteers, the dissemination of the survey to gather volunteers' viewpoints, and a focus group discussion with participatory activities with Red Cross volunteers. The findings highlight that a key challenge lies in finding a balance between a program that provides flexibility to address contextual issues and fosters communities' ownership, versus a prescriptive and standardized approach that leaves little room for creativity and self-initiative. It pinpoints that supporting volunteers with technical training is critical but that soft skills training such as coordinating, communicating,or facilitating activities at the local level are needed. It concludes that the sustainability of Good and Ready requires understanding and meeting volunteers' motivations and expectations and that enhancing partnerships with local emergency management agencies would strengthen the program.

Influence of preterm birth on the association between gestational diabetes mellitus and childhood developmental vulnerability:a causal mediation analysis

Background Epidemiological studies examining the direct and indirect effects of gestational diabetes mellitus(GDM)on offspring early childhood developmental vulnerability are ***,the aims of this study were to estimate the direct and indirect effects of GDM(through preterm birth)on early childhood developmental *** We conducted a retrospective population-based cohort study on the association between gestational diabetes mellitus and early childhood developmental vulnerability in children born in Western Australia(WA)using maternal,infant and birth records from the Midwives Notification,Hospitalizations,Developmental Anomalies,and the Australian Early Development Census(AEDC)*** used two aggregated outcome measures:developmentally vulnerable on at least one AEDC domain(DV1)and developmentally vulnerable on at least two AEDC domains(DV2).Causal mediation analysis was applied to estimate the natural direct(NDE),indirect(NIE),and total(TE)effects as relative risks(RR).Results In the whole cohort(n=64,356),approximately 22%were classified as DV1 and 11%as DV2 on AEDC *** of the natural direct effect suggested that children exposed to GDM were more likely to be classified as DV1(RR=1.20,95%CI:1.10-1.31)and DV2(RR=1.34,95%CI:1.19-1.50)after adjusting for potential *** 6%and 4%of the effect of GDM on early childhood developmental vulnerability was mediated by preterm birth for DV1 and DV2,*** Children exposed to gestational diabetes mellitus were more likely to be developmentally vulnerable in one or more AEDC *** biological mechanism for these associations is not well explained by mediation through preterm birth.

Transcranial photobiomodulation for the brain: a wide range of clinical applications

Photobiomodulation therapy(PBMT)is a rapidly growing approach to the healing,stimulation,protection,and regeneration of many human organs and tissue *** started in the 1960s as low-level laser therapy for wound healing,but since then the introduction of light-emitting diodes(LEDs)has dramatically increased the number of applications and reports of positive *** generally uses red(620-700 nm)and/or near-infrared(780-1270 nm)wavelengths of light at an intensity that causes no tissue heating,and its activity is based on well-established biological and cellular mechanisms(de Freitas and Hamblin,2016).While laser therapists continue to use various types of laser in their office practice,LEDs are ideally suited for home use devices because they are completely safe and without any known significant adverse *** the various body parts on which PBMT has been shown to exert beneficial effects,the brain stands out as perhaps the most promising *** has been shown to reduce neuroinflammation,while increasing mitochondrial function,oxygen consumption,and blood flow within the brain(Hamblin,2016).Moreover,PBMT can stimulate the processes of synaptogenesis,neurogenesis,and neuroplasticity thus helping the brain to heal itself.

Beta-lactam antibiotic resistance among Escherichia coli isolates from patients with urinary tract infections in Vietnam:A systematic review and meta-analysis

Objective:To investigate the evolving patterns of antimicrobial resistance against beta-lactam antibiotics in Escherichia coli isolates from urinary tract infection patients in Vietnam,covering the period from 2005 to 2022.Methods:29 Descriptive studies published between 2010 and 2022 in English and Vietnamese were included in the *** on resistance rates to beta-lactam antibiotics,including cephalosporins and carbapenems,were extracted and *** pooled resistance rates were calculated using random-effects *** trends in resistance were assessed using linear regression analysis.Results:Among the beta-lactam antibiotics studied,Escherichia coli exhibited varying levels of resistance,with cephalosporins showing higher resistance rates compared to *** pooled resistance rates were 66％for cefotaxime,65％for ceftriaxone,54％for ceftazidime,and 56％for *** contrast,carbapenems demonstrated lower resistance rates,with weighted pooled resistance rates ranging from 3％to 4％for meropenem,ertapenem,and *** rates were also observed for amoxicillin/clavulanate(35％),ticarcillin/clavulanate(26％),and piperacillin/tazobactam(12％).Significant annual increases in resistance were noted for imipenem(0.56％,P＜0.001).Conclusions:This study highlights the critical issue of antimicrobial resistance in urinary tract infections in Vietnam and emphasizes the importance of prudent antibiotic use and the regular monitoring of resistance *** insights are useful for guiding healthcare professionals in optimizing treatment strategies and for policymakers in formulating evidence-based clinical guidelines to combat antibiotic resistance effectively.
