Hyperinsulinemic hypoglycemia due to adult nesidioblastosis in insulin-dependent diabetes

In neonates, persistent hyperinsulinemic hypoglycemia (PHH) is associated with nesidioblastosis. In adults, PHH is usually caused by solitary benign insulinomas. We report on an adult patient who suffered from insulin-de- pendent diabetes mellitus, and subsequently developed PHH caused by diffuse nesidioblastosis. Mutations of the MEN1 and Mody 2/3 genes were ruled out. Preoperative diagnostic procedures, the histopathological criteria and the surgical treatment options of adult nesidioblastosis are discussed. So far only one similar case of adult nesidioblastosis subsequent to diabetes mellitus Ⅱ has been reported in the literature. In case of conversion of diabetes into hyperinsulinemic hypoglycemia syndrome, nesidioblastosis in addition to insulinoma should be con- sidered.

Currently, a growing number of programs become available in statistical software for multiple imputation of missing values. Among others, two algorithms are mainly implemented: Expectation Maximization (EM) and Multiple Imputation by Chained Equations (MICE). They have been shown to work well in large samples or when only small proportions of missing data are to be imputed. However, some researchers have begun to impute large proportions of missing data or to apply the method to small samples. A simulation was performed using MICE on datasets with 50, 100 or 200 cases and four or eleven variables. A varying proportion of data (3% - 63%) was set as missing completely at random and subsequently substituted using multiple imputation by chained equations. In a logistic regression model, four coefficients, i.e. non-zero and zero main effects as well as non-zero and zero interaction effects were examined. Estimations of all main and interaction effects were unbiased. There was a considerable variance in the estimates, increasing with the proportion of missing data and decreasing with sample size. The imputation of missing data by chained equations is a useful tool for imputing small to moderate proportions of missing data. The method has its limits, however. In small samples, there are considerable random errors for all effects.

The tissue culture and autoradiography techniques were used to study the effect of infiltration by inflammatory cells and of HP infection on the synthesis of glycoproteins by the gastric antrum. The results showed that in active phase of gastritis, the synthesis of glycoprotein was increased but no correlation between HP infection and synthesis of glycoprotein was noted. The machanism of increased synthesis in chronic gastritis was discussed. It is suggested that the inflammatory cells and media released by them may play important role.

AIM: To examine the effect of tachykinins on the ascending reflex pathway in rat small intestine, we used different selective neurokinin (NK) receptor antagonists (RA); a) NK1-RA; GR-82334 and CP-96.345, b) NK2-RA: MEN-10.376 and L-659.877. The aim was further to investigate the effect of substance P (SP) on the ascending excitatory and descending inhibitory reflex pathway. METHODS: The whole segments of rat ileum (10 cm in length) were studied in an organ bath. Ascending contraction of circle muscle was elicited by anal electrical stimulation (3 Hz, 1ms, 20 V) and measured as change of intraluminal pressure by a perfused manometric system 2 cm and 4 cm orad of the stimulation site. RESULTS: GR-82334 and CP-96.345 (NK1-RA) caused a significant dose-related inhibition of the oral contraction at a distance of 4 cm: GR-82334 [area: -10 % ± 8 % (10 nmol/L); - 29 %±10 % (1000 nmol/L). Zusammenfassung： Evaluierung der Effektivitat yon Bletilla striata bei transerteriellen Chemoembolisation （TACE） im Kleintiermodell. Nach subkapsularer Implantation （Tag 0） eines soliden Morris Hepatoms （2 mm^3） bei ACI-Ratten （n=30） wurden am 13. Tag die Tumorvolumina （V1） magnettomographisch bestimmt. Im Anschlu erfolgten nach Laparatomie die retrograde Katheterisierung der Arteria gastroduodenalis und die Anwendung folgender Therapieprotokolle.. （A） TACE （0,1 mg Mitomycin C ＋ 0,1 mL Lipiodol）＋ Bletilla striata （1,0 mg）（n=10）; （B） TACE （ Bletilla striata （1,0mg）＋ Ligatur （n=10）;（C） TACE allein （Kontrollgruppe, n=10）. Zur Effektivitatsbeurteilung der unterschiedlichen Therapieprotokolle erfolgte am 28. Tag eine erneute magnettomographische Bestimmung der Tumorvolumina （V2）. Das durchschnittliche Tumorvolumen der Gruppen A, B und C vor und nach interventioneller Therapie betrug 0,0355 cm^3 und 0, 2248 cm^3 , 0,0374 cm^3 und 0,0573 cm^3 sowie 0,0380 cm^3 und 0,3674 cm^3. Das errechnete durchschnittliche Verhaltnis （V2/V1） betrug 6,28, 1,53 sowie 9,14. Im Vergleich zur Kontrollgruppe C konnte somit fur Therapiegruppe B eine signifikant （P〈0.01） geringere Tumorvolumenzunahme im Beobachtungszeitraum ermitteh werden, wkhrend zwischen Gruppe A und Gruppe C keine statistisch signifikanten （P〉0.05） Unterschiede im Tumorwachstum festgestelh werden konnten. Das Wachstum der induzierten Lebertumoren konnte unter Behandlung mittels TACE ＋ Bletilla striata ＋Ligatur im Tiermodell statistisch signifikant im Vergleich zur Kontrollgruppe gehemmt werden. Glucose is an important regulator of cell growth and metabolism. Uridine diphosphate sugars (UDP-sugars), as the intermediate products of metabolism, play pivotal roles as precursors in the synthesis of complex carbohydrates and glycolipids as well as lectose. It is very important to study their metabolism in cells in clinical biochemistry. A capillary electrophoretic method has been developed for the analysis of UDP-sugars and nucleotides. By using an uncoated capillary (70cm×50μm) and 20 mmol/L borax buffer (pH 9), 4 important UDP-sugars can be analyzed in 15 min at 22 kV with satisfactory precision and sensitivity. The developed method has been applied to analyze UDP-sugars concentrations in lymphocytes, fibroblasts and mesangial cells, and the results show it not only is much better than HPLC method, but also can be used to measure the energy charge of cells.
