BACKGROUND Currently,endoscopic retrograde cholangiopancreatography(ERCP)plus laparoscopic cholecystectomy(LC)is the main treatment for cholecystolithiasis combined with ***,the treatment is unsatisfactory,and the development of better therapies is *** To determine the clinical efficacy of LC plus cholangioscopy for cholecystolithiasis combined with *** Patients(n=243)with cholecystolithiasis and choledocholithiasis admitted to The Affiliated Haixia hospital of Huaqiao University(910th hospital of Joint Logistic Support Force)between January 2019 and December 2023 were included in the study;111 patients(control group)underwent ERCP+LC and 132 patients(observation group)underwent LC+laparoscopic common bile duct exploration(LCBDE).Surgical success rates,residual stone rates,complications(pancreatitis,hyperamylasemia,biliary tract infection,and bile leakage),surgical indicators[intraoperative blood loss(IBL)and operation time(OT)],recovery indices(postoperative exhaust/defecation time and hospital stay),and serum inflammatory markers[C-reactive protein(CRP)],tumor necrosis factor-α(TNF-α),and interleukin-6(IL-6)were *** No significant differences in surgical success rates and residual stone rates were detected between the observation and control ***,the complication rate,IBL,OT,postoperative exhaust/defecation time,and hospital stays were significantly reduced in the observation group compared with the control ***,CRP,TNF-α,and IL-6 Levels after treatment were reduced in the observation group compared with the levels in the control *** These results indicate that LC+LCBDE is safer than ERCP+LC for the treatment of cholecystolithiasis combined with *** surgical risks and postoperative complications were lower in the observation group compared with the control ***,patients may recover quickly with less inflammation after LCBDE.

Synergistic effect of additional anlotinib and immunotherapy as second‐line or later‐line treatment in pancreatic cancer:A retrospective cohort study

Background:Pancreatic ductal adenocarcinoma(PDAC)is in urgent need of a second‐line or later‐line treatment *** aimed to analyze the efficacy and safety of additional anlotinib,specifically anlotinib in combination with immunotherapy,in patients with PDAC who have failed first‐line ***:Patients with pathological diagnosis of PDAC were additionally treated with anlotinib,and some patients were treated with anti‐PD‐1 agents at the same time,which could be retrospectively *** efficacy and safety of additional anlotinib were ***:A total of 23 patients were *** patients treated with additional anlotinib,the overall median progression‐free survival(PFS)was 1.8 months and the median overall survival(OS)was 6.3 months,regardless of anti‐PD‐1 *** patients receiving additional anlotinib in combination with anti‐PD‐1 agents,median PFS and OS were 1.8 and 6.5 months,*** events(AEs)were observed in 16 patients(69.6%).In patients treated with additional anlotinib,the majority of AEs were grade 1–*** analysis revealed that patients with baseline red blood cell distribution width(RDW)<14%treated with additional anlotinib plus anti‐PD‐1 agents had significantly longer OS than patients with baseline RDW≥14%(p=0.025).Patients with additional anlotinib plus anti‐PD‐1 agents as second‐line therapy had a longer OS than those treated as later‐line therapy(p=0.012).Multivariate analysis showed that baseline RDW was the only independent risk factor for OS(p=0.042).Conclusion:The combination of anlotinib and immunotherapy represents an effective add‐on therapy with tolerable AEs as second‐or later‐line therapy in patients with PDAC,particularly in patients with baseline RDW<14%.

Machine learning-based radiomics score improves prognostic prediction accuracy of stage II/III gastric cancer: A multi-cohort study

BACKGROUND Although accurately evaluating the overall survival(OS)of gastric cancer patients remains difficult,radiomics is considered an important option for studying *** To develop a robust and unbiased biomarker for predicting OS using machine learning and computed tomography(CT)image *** This study included 181 stage II/III gastric cancer patients,141 from Lichuan People's hospital,and 40 from the Cancer Imaging Archive(TCIA).Primary tumors in the preoperative unenhanced CT images were outlined as regions of interest(ROI),and approximately 1700 radiomics features were extracted from each *** skeletal muscle index(SMI)and skeletal muscle density(SMD)were measured using CT images from the lower margin of the third lumbar *** the least absolute shrinkage and selection operator regression with 5-fold cross-validation,36 radiomics features were identified as important predictors,and the OS-associated CT image radiomics score(OACRS)was cal-culated for each patient using these important *** Patients with a high OACRS had a poorer prognosis than those with a low OACRS score(P<0.05)and those in the TCIA *** and multivariate analyses revealed that OACRS was a risk factor[RR=3.023(1.896-4.365),P<0.001]independent of SMI,SMD,and pathological ***,OACRS outperformed SMI and SMD and could improve OS prediction(P<0.05).CONCLUSION A novel biomarker based on machine learning and radiomics was developed that exhibited exceptional OS discrimination potential.

Preliminary study of metabonomic changes during the progression of atherosclerosis in miniature pigs
