＜正＞Purpose:To evaluate the activity and the toxicity of ACNU （1 - （4 - amino - 2methyl - 5pyrimidinyl） - methyl - 3 - （2 -cholroethyl） - 3 - nitrosourea hydrochloride） administered with cisplatin by intravenous infusion for 72 hours in select patients with recurrent *** and methods:From April 1996 to 2002,thirty-seven patients with histologically proven glioblastoma,who showedre-currence on image study after operation and radiation, met the eligibility criteria of our cohort. The mean time to recurrence was 9.7± 7.0（1 - 26 months） .Treatment response was evaluated every 6 weeks using magnetic resonance imaging （mRI）. Complete blood counts were collected every week to monitor and treat possible bone marrow suppression from the *** rates were analyzed using the Kaplan - meier and log rank test. Results: Post - chemotherapy mRI was available in 36 of 37 patients. Response to treatment was observed in 19 patients （53%） including 2 cases of complete *** patients （17%） showed progression （Pd） and 11 patients （31%） had stable disease （Sd）. Two or more cycles of chemotherapy was the only factor that predicted response to treatment. The overall median survival for all patients was 17.0±3.3 *** （＜ 40 years） and time to recurrence （＜ 1 year） were the clinical factors that predicted improved overall *** gain after chemotherapy was 9 ***’s median survival in those who responded and thoee with Sd after treatment （11 months） had a longer median survival compared to Pd （5 months）（p = 0.01） .myelosuppression was severe （grade III/IV leukopenia in 15 patients （40%） and grade III/IV thrombocytopenia in 19 patients （52%）） but most recovered more than WHO grade II at the end of the chemotherapy cycles. There was only one fatality due to sepsis from pneumonia during the initial leukopenic state. Conclusion: ACNU and cisplatin chemotherapy can be an effective salvage therapy for recurrent glioblastoma patients.

Alpha-fetoprotein-targeted reporter gene expression imaging in hepatocellular carcinoma

Hepatocellular carcinoma (HCC) is one of the most common cancers in Eastern Asia, and its incidence is increasing globally. Numerous experimental models have been developed to better our understanding of the pathogenic mechanism of HCC and to evaluate novel therapeutic approaches. molecular imaging is a convenient and up-to-date biomedical tool that enables the visualization, characterization and quantification of biologic processes in a living subject. molecular imaging based on reporter gene expression, in particular, can elucidate tumor-specific events or processes by acquiring images of a reporter gene’s expression driven by tumor-specific enhancers/promoters. In this review, we discuss the advantages and disadvantages of various experimental HCC mouse models and we present in vivo images of tumor-specific reporter gene expression driven by an alpha-fetoprotein (AFP) enhancer/promoter system in a mouse model of HCC. The current mouse models of HCC development are established by xenograft, carcinogen induction and genetic engineering, representing the spectrum of tumor-inducing factors and tumor locations. The imaging analysis approach of reporter genes driven by AFP enhancer/promoter is presented for these different HCC mouse models. Such molecular imaging can provide longitudinal information about carcinogenesis and tumor progression. We expect that clinical application of AFP-targeted reporter gene expression imaging systems will be useful for the detection of AFP-expressing HCC tumors and screening of increased/decreased AFP levels due to disease or drug treatment.

Liver stiffness and perfusion changes for hepatic sinusoidal obstruction syndrome in rabbit model

BACKGROUNd Hepatic sinusoidal obstruction syndrome(SOS)is caused by damage to hepatic sinusoidal endothelial cells that results in fibrous obliteration of intrahepatic venules and necrosis of *** the diagnosis is primarily based on nonspecific clinical features and invasive liver ***,noninvasive imaging methods are required for the early diagnosis and severity assessment of hepatic *** To determine the effectiveness of supersonic shear wave imaging(SSI)and dual energy computed tomography(dECT)for diagnosing hepatic SOS using a rabbit *** Among nine New Zealand white rabbits(3-4 kg,male),three in control group ingested normal saline for 20 d and six in the SOS group ingested 6-thioguanine(5 mg/kg/d)for 20 *** stiffness was measured using SSI on days 0,3,10,and *** the same days,liver perfusion was evaluated from virtual monochromatic images of 55 keV and iodine map using *** changes in the liver were assessed using *** pathology scores were compared between the two *** stiffness and perfusion parameters were compared according to the groups,days,and pathology *** Final pathology scores were significantly higher in the SOS than the control group(median 22 vs 2,P=0.024).No gross morphologic changes were seen in *** stiffness,Hounsfield Unit values,and iodine concentrations were higher in the SOS compared to the control group on days 10 and 20(all,P≤0.007).Compared to day 0,liver stiffness and perfusion parameters were higher on day 20 in the SOS group(all,P≤0.001).Correlation coefficients for liver stiffness(r=0.635),Hounsfield Unit values(r=0.587),and iodine concentration(r=0.611)with final pathology scores were positive without significance(all,P>0.05).CONCLUSION Liver stiffness and perfusion parameters were significantly increased in the livers of a rabbit SOS *** and dECT might aid in early diagnosis of hepatic SOS.

Sex-specific Cutoff Values of Visceral Fat Area for Lean vs. Overweight/Obese Nonalcoholic Fatty Liver disease in Asians

Background and Aims:Visceral obesity is a risk factor for nonalcoholic fatty liver disease(NAFLd).We investigated sex-specific optimal cutoff values for visceral fat area(VFA)associated with lean and overweight/obese NAFLd in an Asian ***:This retrospective study includ-ed 678 potential living liver donors(mean age,30.8±9.4 years;434 men and 244 women)who had undergone ab-dominal computed tomography(CT)imaging and liver bi-opsy between November 2016 and October *** was measured using single-slice abdominal *** was eval-uated by liver biopsy(≥5%hepatic steatosis).Receiver op-erating characteristic curve analysis was used to determine cutoff values for VFA associated with lean(body mass index[BmI]m2)and overweight/obese(BmI≥23 kg/m2)***:Area under the curve(AUC)values with 95%confidence intervals(CI)for VFA were 0.82(95%CI,0.75-0.88)for lean and 0.74(95%CI,0.69-0.79)for over-weight/obese men with *** AUC values were 0.67(95%CI,0.58-0.75)for lean and 0.71(95%CI,0.62-0.80)for overweight/obese women with *** cutoff values for VFA associated with lean NAFLd were 50.2 cm2 in men and 40.5 cm2 in *** optimal cutoff values for VFA associated with overweight/obese NAFLd were 100.6 cm2 in men and 68.0 cm2 in ***:Sex-specific cutoff values for VFA may be useful for identifying subjects at risk of lean and overweight/obese NAFLd.

Stereotactic body radiotherapy for isolated paraaortic lymph node recurrence from colorectal cancer

AIm： To evaluate the efficacy and complications of stereotactic body radiotherapy in localized paraaortic lymph node recurrence from colorectal cancer. mETHOdS： From 2003 to 2009, 7 patients with paraaortic lymph node recurrence （1-3 lesions） from colorectal cancer were treated with stereotactic body radiotherapy. Total gross tumor volumes ranged from 4 to 40 mL. The doses were escalated from 36 Gy/patient to 51 Gy/patient and were delivered in 3 fractions. RESULTS： One and 3 year overall survival rates were 100% and 71.4%, respectively, and median survival was 37 mo. Grade IV intestinal obstruction was reported in 1 of 7 patients. This patient received 48 Gy in 3 fractions with a maximum point dose to the intestine of 53 Gy and V45Gy = 3.6 mL. However, 6 patients received an intestinal maximum point dose of 〈 51 Gy and V45Gy of 〈 1 mL, and did not develop any severe complications. CONCLUSION： This pilot study suggests selected paraaortic lymph node recurrence （1-3 closed lesions） that failed to respond to chemotherapy can be potentially salvaged by stereotactic body radiotherapy.
