Informing policy makers in developing countries:Practices and limitations of geriatric home medication review in Malaysia-A qualitative inquiry

Objective:To explore existing practices and challenges in the delivery of geriatric home medication review(HMR).The study was part of a larger study aimed to offer solution to expand the range of geriatric ***:This study employed qualitative exploratory design through semi-structured individual in-depth interviews with the public pharmacists involved in the delivery of geriatric HMR at public *** purpose of the interviews was to explore challenges faced by them in the delivery of geriatric ***:Based on the emerging themes from the qualitative data,the study reveals that geriatric HMR in Malaysia is integrated as part of multidisciplinary home care visits,encompassing a diverse patient population with various healthcare ***,it faces challenges such as the lack of outcome monitoring,formal training,and workforce *** these hurdles,there is a pressing need for the expansion of this service to better serve the community,and collaboration with community pharmacists holds potential to broaden its ***,the findings suggest that pharmacist-led HMR is both warranted and feasible within the Malaysian healthcare *** order to optimize medicine-use among older people living in the community,approaches for expanding geriatric HMR services in Malaysia must be ***:This study holds profound implications as it attempts to illuminate policy makers in developing countries,enabling them to formulate effective HMR *** considering the challenges highlighted within this research,policy makers can design a comprehensive HMR service that caters adeptly to the healthcare needs of the mass population.

AIM:To study the association of serum insulin-like growth factor-I(IGF-I) with diabetic retinopathy.· METHODS:Serum IGF-1 levels were measured in 25 pregnant diabetic patients and 25 pregnant non-diabetic patients who were matched for age,ethnicity,parity and period of *** examination was performed in both groups at 28,32 and 36 weeks of ***:The serum IGF-I level was significantly elevated in pregnant diabetics compared to pregnant non-diabetics(366±199μg/L vs 184±89μg/L,(P=0.0001) at 24 weeks,535±251μg/L vs 356±89μg/L,(P=0.007) at 32 weeks and 404±166μg/L vs 264±113μg/L,(P=0.003) at 36 weeks of gestation).The pregnant diabetics with established diabetes had significantly higher IGF-1 level than gestational diabetes at 28,32 and 36 weeks of *** serum IGF-I level in pregnant diabetics with retinopathy was significantly higher than in those without retinopathy at all periods of gestation.· CONCLUSION:Increased serum IGF-1 in pregnancy may increase the risks for retinopathy.

Antimicrobial activity and essential oils of Curcuma aeruginosa,Curcuma mangga,and Zingiber cassumunar from Malaysia

Objective:To analyze the chemical composition of the essential oils of Curcuma aeruginosa (***),Curcuma mangga(***),and Zingiber cassumunar(***). and study their antimicrobial ***:Essential oils obtained by steam distillation were analyzed by gas chromatography-mass speclrometry(GC-MS).The antimicrobial activity of the essential oils was evaluated against four bacteria:Bacillus cereus(***).Staphylococcus aureus(***).Escherichia coli(***).and Pseudomonas aeruginosa(***);and two fungi:Candida albicans(***) and Cyptococcus neoformans(***),using disc-diffusion and broth microdilution ***:Cycloisolongifolene,8.9-dehydro-9- formyl(35.29%) and dihydrocoslunolide(22.51%) were the major compounds in *** oil; whereas caryophyllene oxide(18.71%) and caryophyllene(12.69%) were the major compounds in *** oil:and 2,6.9,9-tetramethyl-2.6.10-cycloundecatrien-1-one(60.77%) andα-caryophyllene(23.92%) were abundant in *** *** essential oils displayed varying degrees of antimicrobial activity against all lested *** oil had the highest and most broad-spectrum activity by inhibiting all microorganisms tested,with *** being the most sensitive microorganism by having the lowest minimum inhibitory concentration(MIC) and minimum fungicidal concentration(MFC) values of 0.1μL/mL. *** oil showed mild antimicrobial activity,whereas *** had very low or weak activity against the tested ***:The preliminary results suggest promising antimicrobial properties of *** and ***,which may be useful for food preservation,pharmaceutical treatment and natural therapies.

Identification of α-glucosidase inhibitors from Clinacanthus nutans leaf extract using liquid chromatography-mass spectrometry-based metabolomics and protein-ligand interaction with molecular docking

The present study used in vitro and in silico techniques, as well as the metabolomics approach to characterise α-glucosidase inhibitors from different fractions of Clinacanthus nutans. C. nutans is a medicinal plant belonging to the Acanthaceae family, and is traditionally used to treat diabetes in Malaysia. nHexane, n-hexane: ethyl acetate(1:1, v/v), ethyl acetate, ethyl acetate: methanol(1:1, v/v), and methanol fractions were obtained via partitioning of the 80% methanolic crude extract. The in vitro α-glucosidase inhibitory activity was analyzed using all the fractions collected, followed by profiling of the metabolites using liquid chromatography combined with mass spectrometry. The partial least square(PLS) statistical model was developed using the SIMCA P^+14.0 software and the following four inhibitors were obtained:(1) 4,6,8-Megastigmatrien-3-one;(2) N-Isobutyl-2-nonen-6,8-diynamide;(3) 1′,2′-bis(acetyloxy)-3′,4′-didehydro-2′-hydro-β, ψ-carotene; and(4) 22-acetate-3-hydroxy-21-(6-methyl-2,4-octadienoate)-olean-12-en-28-oic acid. The in silico study performed via molecular docking with the crystal structure of yeast isomaltase(PDB code: 3 A4 A) involved a hydrogen bond and some hydrophobic interactions between the inhibitors and protein. The residues that interacted include ASN259, HID295, LYS156, ARG335,and GLY209 with a hydrogen bond, while TRP15, TYR158, VAL232, HIE280, ALA292, PRO312, LEU313,VAL313, PHE314, ARG315, TYR316, VAL319, and TRP343 with other forms of bonding.

Translation and Validation of Malay Version 5-Items Brief Illness Perception Questionnaire, BIP-Q5 towards COVID-19 among Malaysian Nurses

Introduction: Currently, own individual perception is recognized as one of the important factors in the prevention of disease, including coronavirus disease, COVID-19. Given the massive impact of COVID-19 on all population’s life, including nurses as one of the main health services providers in the country, this study aims to translate and validate the Malay Version 5-Items Brief Illness Perception Questionnaire, BIP-Q5 towards COVID-19 among Malaysian nurses. Materials and Methods: Forward and backward translations and pretesting of the BIP-Q5 to Malay were conducted among nurses, subject matter experts, and language professionals. The validations process was elicited through an online cross-sectional study involving 56 nurses based on a ~10:1 subject-to-items ratio sample size estimations. Results: The principal component analysis (PCA) revealed one best component with eigenvalues more than one, confirming the questionnaire’s original version. There are five items within the single component, and all are with weightage of over 43%. The scree plot supported the findings, which showed that at least one factors are suitable to be retained. The overall Cronbach’s α coefficient was 0.7 and the intraclass correlation coefficient was 0.659. The Kaiser-Meyer-Olkin was 0.655, and Bartlett’s test of sphericity p-value was Conclusion: This study showed that the translated Malay Version 5-Items Brief Illness Perception Questionnaire, BIP-Q5 has a good psychometric property, and is a valid and reliable tool to be used to measure illness perceptions towards COVID-19 among Malaysian nurses.

During the COVID-19 pandemic,medical and nursing students are faced with various challenges such as the need to attend online classes and juggling clinical postings under the new *** study aimed to assess the asso-ciation between depression,anxiety,and behavioural changes among medical and nursing students during the COVID-19 *** online self-administered questionnaire was distributed between March 2021 and July 2021 to 292 undergraduates medical and nursing students in a higher education institute on the East Coast of *** questionnaires consisted of four parts:sociodemographic data,the Generalised Anxiety Disor-der-7,the Patient Health Questionnaire-9,and questions related to behaviours during the COVID-19 *** study found that 23.6%and 33.6%of the students experienced anxiety and depression respectively,possibly caused by the COVID-19 *** was associated with a high level of behavioural changes during the pandemic with a total of 87%of the students showing behavioural *** interventions such as online counselling and consultation should be implemented by the university to reduce the prevalence of depres-sion and anxiety resulting from the COVID-19 pandemic among medical and nursing students.

Objective:To study the chemical constituents of stembark of Garcinia malaccenm（***） together with *** and antioxidant ***:Purification and structure elucidation were carried out by chromatographic and spectroscopic techniques, *** and trypan blue exclusion methods were performed to study the cytotoxic *** activity was conducted by dise diffusion and microdilulion methods, whereas antioxidant activities were done by ferric thiocyanate method and DPPH radical ***:The phylochemical study led lo the isolation ofα,β-mangostin and cycloarl-24-en-3β-ol.α-Mangostin exhibited cytotoxic activity against HSC-3 cells with an IC50 of 0.33μM.β- andα-mangostin showed activity against K562 cells with IC50 of 0.40μM and 0.48μM,respectively,α-Mangostin was active against Gram-positive bacteria, Staphylococcus aureus（***） and Bacilus anthracis（***） with inhibition zone and MIC value of（19 mm;0.02S mg/mL） and（20 mm;0.013 mg/mL）,*** antioxidant assay,α-mangostin exhibited activity as an inhibitor of lipid ***:*** presenceα- andβ-mangostin and cycloart-24-en-3β-ol.β-Mangostin was found very active against ***-3 cells and *** results suggest that mangoslins derivatives have the potential to inhibit the growth of cancer cells by inducing *** addition,α-andβ-mangostin was found inhibit the growth of Cram-positive pathogenic bacteria and also showed the activity as an inhibitor of lipid peroxidation.

Galactagogue effects of Musa x paradisiaca flower extract on lactating rats

Objective:To investigate the potential of Musa x paradisiaca(M.x paradisiaca)flower extracts in promoting milk production of lactating rats and its effects on growth of the suckling pups. Methods:Galactagogue activity was evaluated in terms of quantity of milk produced from the rats treated with petroleum ether,ethanol or water extracts of the *** rats(n=5) of Spraque Dawley with six pups each were administered with the extracts in the amount of 500 mg/kg body weight,while the control rats were given an equivalent amount of distilled *** rats were daily administered via oral feeding starting from Day 5 until Day 14 and the performance of milk production was measured along the experimental period by weight-suckle-weight method. Results were statistically analyzed using SPSS by means of ANOVA at 0.05 and was expressed as their mean±standard *** rates of pups’ growth were measured as the weight gain along the experimental ***:The rats treated with aqueous extract produced higher milk than control and ethanol *** extract was identified to increase milk production by 25%,while petroleum ether extract by 18%.The mean of yields produced by the rats during suckling period for aqueous,petroleum ether,ethanol and control were 4.62±2.45,4.37±1.93,3.65±1.89 and 3.69±1.79,*** rates of pups for the rats treated with control,aqueous, ethanol extract and petroleum ether were(1.85±0.49),(1.78±0.56),(1.65±0.46)and(1.56±0.42) g/pup, ***:The present study reveals the potential of M.x paradisiaca flower to enhance milk production of nursing mothers which could be exploited for commercialization of the isolated extract.

Role of cardiovascular magnetic resonance in assessment of acute coronary syndrome

Cardiovascular disease(CVD) is the leading cause of death in the western world and is becoming more important in the developing world. Recently,advances in monitoring,revascularisation and pharmacotherapy have resulted in a reduction in mortality. However,although mortality rates have declined,the burden of disease remains large resulting in high direct and indirect healthcare costs related to CVDs. In Australia,acute coronary syndrome(ACS) accounts for more than 300000 years of life lost due to premature death and a total cost exceeding eight billion dollars *** is also the main contributor towards the discrepancy in life expectancy between indigenous and non-indigenous Australians. The high prevalence of CVD along with its associated cost urgently requires a reliable but non-invasive and cost-effective imaging modality. The imaging modality of choice should be able to accelerate the diagnosis of ACS,aid in the risk stratification of de novo coronary artery disease and avail incrementalinformation of prognostic value such as viability which cardiovascular magnetic resonance(CMR) allows. Despite its manifold benefits,there are limitations to its wider use in routine clinical assessment and more studies are required into assessing its cost-effectiveness. It is hoped that with greater development in the technology and imaging protocols,CMR could be made less cumbersome,its imaging protocols less lengthy,the technology more inexpensive and easily applied in routine clinical practice.
