Reducing the production costs of clean energy carriers such as hydrogen through scalable water electrolysis is a potential solution for advancing the hydrogen *** the various material candidates,our group demonstrated transition-metal-based materials with tunable electronic characteristics,various phases,and ***,electrochemical water oxidation using Cu_(2)Se-V_(2)O_(5) as a non-precious metallic electrocatalyst via a hydrothermal approach is *** water-splitting performance of all the fabricated electrocatalysts was evaluated after direct growth on a stainless-steel *** electrochemically tuned Cu_(2)Se-V_(2)O_(5) catalyst exhibited a reduced overpotential of 128 mV and provided a reduced Tafel slope of 57 mV·dec^(−1) to meet the maximum current density of 250 mA·cm^(−2).The optimized strategy for interfacial coupling of the fabricated Cu_(2)Se-V_(2)O_(5) catalyst resulted in a porous structure with accessible active sites,which enabled adsorption of the intermediates and afforded an effective charge transfer rate for promoting the oxygen evolution ***,the combined effect of the catalyst components provided long-term stability for over 110 h in an alkaline solution,which makes the catalyst promising for large-scale practical *** aforementioned advantages of the composite catalyst overcome the limitations of low conductivity,agglomeration,and poor stability of the pure catalysts(Cu2Se and V2O5).

Correlation between Radiological Staging of Chronic Rhinosinusitis and Revision of Endoscopic Sinus Surgery

Introduction: Chronic rhinosinusitis (CRS) is an inflammatory condition affecting the sinonasal mucosa with duration of more than 12 weeks. Multifactorial aetiology contributes to its refractoriness. Functional endoscopic sinus surgery (FESS) is the most common surgical technique preferred in CRS not responding to medical management, showing high success rates. However, some failure has been noted, requiring revision FESS. CT (Computed Tomography) is a diagnostic tool for CRS, and aids in improving surgical outcomes. Combined with the Lund-Mackay scoring system, CT can help stage the CRS, predicting the need for revision surgery. Methods: In our paper, we aimed to study the relationship between severity of CRS using radiological staging and revision of functional endoscopic sinus surgery, assessing outcome after at least 1 year of follow-up. There was a single centre retrospective study involving 150 patients selected by random sampling, to study the correlation between the severity of chronic rhinosinusitis and revision of functional endoscopic sinus surgery (FESS). The study was conducted in the ENT department of king hamad university hospital, Bahrain for a time period of 6 months. Records were scanned and scored using Lund-Mackay system. Lund Mackay score of 10 and above was considered severe. The relation between increased severity with Lund Mackay score on the CT scan and the need for revision sinus surgeries was then determined. Results and Conclusion: This study concludes that LMS score in CT scans can be used to predict the outcomes of primary FESS procedure and the need for revision surgery in the management of CRS.

Clinical epidemiology of ulcerative colitis in Arabs based on the Montréal classification

AIM:To determine the clinical,epidemiological and phenotypic characteristics of ulcerative colitis(UC)in Saudi Arabia by studying the largest cohort of Arab UC ***:Data from UC patients attending gastroenterology clinics in four tertiary care centers in three cities between September 2009 and September 2013were entered into a validated web-based registry,inflammatory bowel disease information system(IBDIS).The IBDIS database covers numerous aspects of inflammatory bowel *** characteristics,disease phenotype and behavior,age at diagnosis,course of the disease,and extraintestinal manifestations were ***:Among 394 UC patients,males comprised51.0%and females 49.0%.According to the Montréal classification of age,the major chunk of our patients belonged to the A2 category for age of diagnosis at17-40 years(68.4%),while 24.2%belonged to the A3category for age of diagnosis at>40 *** to the same classification,a majority of patients had extensive UC(42.7%),35.3%had left-sided colitis and29.2%had only ***,51.3%were in remission,16.6%had mild UC,23.4%had moderate UC and 8.6%had severe *** relapse occurred in17.4%patients,infrequent relapse in 77%and 4.8%had chronic disease.A majority(85.2%)of patients was steroid *** regard to extraintestinal manifestations,arthritis was present in 16.4%,osteopenia in 31.4%,osteoporosis in 17.1%and cutaneous involvement in 7.0%.CONCLUSION:The majority of UC cases were young people(17-40 years),with a male *** the disease course was found to be similar to that reported in Western countries,more similarities were found with Asian countries with regards to the extent of the disease and response to steroid therapy.

46 XX karyotype during male fertility evaluation; case series and literature review

Forty-six XX disorder of sex development is an uncommon medical condition observed at times during the evaluation of a man＇s fertility. The following is a case series and literature review of phenotypically normal men diagnosed with this karyotype. Our goal is to comprehend the patients＇ clinical presentation as well as their laboratory results aiming to explore options available for their management. A formal literature review through PubMed and MEDLINE databases was performed using ＂46 XX man＂ as a word search. A total of 55 patients, including those conveyed in this article were diagnosed with a 46 XX karyotype during their fertility evaluation. The patients＇ mean age _＋ s.d. was 34 ＋ 10 years and their mean height ＋ s.d. was 166 ＋ 6.5 cm. Overall, they presented with hypergonadotropic hypogonadism. Sexual dysfunction, reduced hair distribution, and gynecomastia were reported in 20% （4120）, 25.8% （8/31）, and 42% （13131） of the patients, respectively. The SRYgene was detected in 36 （83.7%） and was absent in the remaining seven （16.3%） patients. We found that a multidisciplinary approach to management is preferred in 46 XX patients. Screening for remnants of the mullerian ducts and for malignant transformation in dysgenetic gonads is imperative. Hypogonadism should be addressed, while fertility options are in vitro fertilization with donor sperm or adoption.

Safety and efficacy of amplatzer duct occluder for percutaneous closure of ventricular septal defects with tunnel shape aneurysm: Medium term follow up

Objectives: Different devices including Amplatzer duct occluder has been used for percutaneous closure of ventricular septal defects. This study reports our medium term follow up of perimembranous and muscular ventricular septal defects with tunnel shape aneurysm closure using the Amplatzer duct occluder. Materials and Methods: From May 2006-December 2012, we used Amplatzer duct occluder in seven ventricular septal defect patients here athamad General hospital,Doha,Qatar. There were 4 male and 3 female patients with an age range of 4 - 32 years with a median of 8 years and weight range of 16 - 63 kgwith a median of33 kg. In this group, 6 were perimembranous and 1 muscular and all these ventricular septal defects had a tunnel shape aneurysm. Transesophageal echocardiographic diameter ranged from 4 - 8 mmand Qp/Qs was 1 - 1.6. Angiographically, the diameter on the left ventricular side measured 3.5 - 10 mmand on right ventricular side 2.4?- 5 mm. 8/6 mmAmplatzer duct occluder was used to close these ventricular septal defects. Results: There were no major complications and immediately after the procedure there was no residual shunt in any of these patients and all the patients remained in normal sinus rhythm. One patient was expatriate and no further follow up was available. The rest of the 6 patients had 1 - 80 months with a median of 54 months follow up and none of these patients had any residual shunt and all remained in normal sinus rhythm. Two patients developed trivial aortic valve regurgitation immediate post procedure, one remained unchanged and the 2nd has progressed to mild at this latest follow up. Conclusion: Amplatzer duct occluder is feasible and a safe device for percutaneous closure of selective tunnel shape aneurysmal perimembranous and muscular ventricular septal defects.

The effects of temperature on corrosion and the electrochemical behavior of Ni82.3Cr7Fe3Si4.5B3.2 glassy alloy in HC1,H2SO4,and H3PO4 acids were studied using AC and DC *** data reveal that the susceptibility to localized corrosion increases with increasing *** polarization curves reveal that the bulk glassy alloy is spontaneously passivated at all the investigated temperature in H2SO4 and H3PO4 solutions.A localized corrosion effect in HCl solution is clearly *** apparent activation energies in the regions of Tafel,active,and passive,as well as the enthalpies and entropies of the dissolution process were determined and *** high apparent activation energy（Ea） value for H3PO4 solution in Tafel region is explained by the low aggressivity of PO4^3- ions.

Reversible postural orthostatic tachycardia syndrome

Postural orthostatic tachycardia syndrome(POTS) is a relatively rare syndrome recognised since 1940. It is a heterogenous condition with orthostatic intolerance due to dysautonomia and is characterised by rise in heart rate above 30 bpm from base line or to more than 120 bpm within 5-10 min of standing with or without change in blood pressure which returns to base line on resuming supine position. This condition present with various disabling symptoms such as light headedness, near syncope, fatigue, nausea, vomiting, tremor, palpitations and mental clouding, etc. However there are no identifiable signs on clinical examination and patients are often diagnosed to have anxiety disorder. The condition predominantly affects young female between the ages of 15-50 but is rarely described in older people. We describe an older patient who developed POTS which recovered over 12 mo. Recognising this condition is important as there are treatment options available to alleviate the disabling symptoms.
