Can patients with mild non-neoplastic lesions diagnosed at baseline screening be safely exempt from surveillance: evidence from multicenter community-based cohorts

Surveillance recommendations for gastric cancer（GC） in current guidelines focused on advanced precancerous lesions and were based on precise diagnosis of severity/extent of baseline lesions. We aimed to develop a less endoscopy-related equipment-dependent risk-stratification tool, and assessed whether mild-precursor-lesion patients can be safely exempt from surveillance. In the multicenter communitybased cohort, 75,051 participants receiving baseline endoscopy were enrolled during 2015–2017 and followed-up until 2021. Cumulative incidence rates（CIRs） of GC for precancerous-conditions were calculated by Kaplan-Meier method and compared by Log-rank tests. Mixedeffects Cox regression models were used to detect potential factors for progression towards GC. A risk score was calculated as counts of selected factors. An independent cohort, including 26,586 participants was used for external validation. During a median follow-up of 6.25 years, CIRs of GC were 0.302%, 0.436%, and 4.756% for normal group, non-neoplastic（atrophic gastritis/intestinal metaplasia） and neoplastic lesions（low-grade/high-grade dysplasia）, respectively(Ptrend<0.001). Four predictors, including male, ≥60 years, smoking, and limited vegetable consumption, were selected for risk-stratification. High-risk patients（≥3 risk factors） with non-neoplastic lesions showed higher GC risks（adjusted HR=7.73, 95%CI: 4.29–13.92）, and their four-year CIR reached the one-year CIR of neoplastic lesions. Further categorizing non-neoplastic lesions by histological grade, both patients with moderate-to-severe lesions（aHR=3.07, 95%CI: 1.67–5.64） and high-risk patients with mild lesions（aHR=7.29, 95%CI: 3.58–14.86） showed higher risks. Consistent trends were observed in validation cohort. High-risk mild-precursor-lesion patients should receive surveillance within 3–5 years after baseline screening. Our study provides evidence on supplementing current guideline recommendations.

Several studies have shown that activation of unfolded protein response and endoplasmic reticulum(ER)stress plays a crucial role in severe cerebral ischemia/reperfusion *** occurs within hours after cerebral ischemia,but the relationship between ER stress and autophagy remains *** this study,we established experimental models using oxygen-glucose deprivation/reoxygenation in PC12 cells and primary neurons to simulate cerebral ischemia/reperfusion *** found that prolongation of oxygen-glucose deprivation activated the ER stress pathway protein kinase-like endoplasmic reticulum kinase(PERK)/eukaryotic translation initiation factor 2 subunit alpha(e IF2α)-activating transcription factor 4(ATF4)-C/EBP homologous protein(CHOP),increased neuronal apoptosis,and induced ***,inhibition of ER stress using inhibitors or by si RNA knockdown of the PERK gene significantly attenuated excessive autophagy and neuronal apoptosis,indicating an interaction between autophagy and ER stress and suggesting PERK as an essential target for regulating *** autophagy with chloroquine exacerbated ER stress-induced apoptosis,indicating that normal levels of autophagy play a protective role in neuronal injury following cerebral ischemia/reperfusion *** from this study indicate that cerebral ischemia/reperfusion injury can trigger neuronal ER stress and promote autophagy,and suggest that PERK is a possible target for inhibiting excessive autophagy in cerebral ischemia/reperfusion injury.

An investigation on patient dose in screen-film diagnostic radiology in Lhasa City,Xizang Autonomous Region,China

This study aimed to investigate patient dose in diagnostic screen-film radiographic examinations in the city of Lhasa,*** out of the twenty-six hospitals registered with the Lhasa Health Bureau were included in the *** entrance surface air Kerma(ESAK)of seven conventional screen-film radiology X-ray equipment in these hospitals was measured with a QA dosimeter in September *** X-ray examinations were divided into three categories:PA(posterior-anterior)chest,upper/lower limb,and AP(anterior-posterior)lumbar *** each category,ESAKs were calculated and *** mean ESAK was 0.6 mGy for PA chest,0.3 mGy for upper/lower limb,and 1.8 mGy for AP lumbar *** addition,the mean ESAK value recorded for PA chest X-ray examinations exceeded the corresponding value recommended by the International Atomic Energy Agency(0.4 mGy).

Epidemiological Features of Foodborne disease Outbreaks in Catering Service Facilities—China,2010–2020

Introduction:In terms of food preparation settings,catering service facilities have been identified as locations with the highest incidence of foodborne disease outbreaks in *** 2010,the China National center for Food Safety Risk Assessment has established the Foodborne disease Outbreak Surveillance System(FDOSS)to monitor foodborne disease ***,data from the FDOSS has provided a more accurate depiction of the epidemic characteristics of outbreaks within these ***:From 2010 to 2020,the FDOSS gathered data related to the prevalence of outbreaks,cases,hospitalizations,and deaths linked to foodborne disease outbreaks in catering service *** study examined the temporal and geographical distribution,pathogenic factors,and contributing variables of these outbreaks over the course of the ***:From 2010 to 2020,China’s catering service facilities reported 18,331 outbreaks,which resulted in 206,718 illnesses,68,561 hospitalizations,and 201 *** second and third quarters of the year accounted for 76.12%of the outbreaks and 72.93%of the *** primary pathogenic factors were pathogenic organisms,which caused 4,883(26.64%)outbreaks,94,047(45.50%)cases,32,170(46.92%)hospitalizations,and 21(10.45%)*** were 5,607(30.59%)outbreaks in restaurants,2,876(15.69%)outbreaks from street vendors,and 2,560(13.97%)outbreaks in employee canteens in ***:The implementation of relevant control methods,including health education and promotion,is critical for addressing foodborne diseases in catering service *** food safety training sessions for restaurant personnel and managers are essential to ensuring the effective management of these health risks.

Pneumoconiosis is a systemic occupational disease that commonly manifests as diffuse fibrosis of the lungs caused by long-term inhalation of silica dust in the production process. The development of pneumoconiosis is a chronic and progressive process;once it occurs, it is irreversible, and gradually develops into lung injury and respiratory failure, and can cause death regardless of exposure to dust. The incidence of pneumoconiosis is the highest in China and the direct and indirect economic loss as a result of pneumoconiosis is approximately 9 billion yuan per year.
