Efficacy and safety of Jiawei simiao powder combined with celecoxib for acute gouty arthritis:A meta-analysis

Objective To evaluate the efficacy and safety of Jiawei simiao powder(JWsMP)combined with celecoxib for the treatment of acute gouty arthritis by conducting a meta-analysis of randomized controlled trials(RCTs).Methods The Chinese National Knowledge Infrastructure Databases,Chinese scientific Journal Database,Wanfang,Cochrane Library,EMBAsE,PubMed,and Web of science databases were searched from inception until December *** variables were analyzed using the mean difference(MD)for analysis,and dichotomous variables were used as risk *** with similar characteristics were pooled for meta-analysis,and heterogeneity was assessed using *** Cochrane Handbook was used to assess the risk of bias and *** 5.3 software was used to perform the *** Thirteen RCTs involving 1007 patients were included in the *** quality of the included studies was low(unclear randomization processes and insufficient blinding reporting).The group receiving JWsMP combined with celecoxib showed significantly lower levels of serum uric acid(suA,MD=−66.32,95%confidence interval(CI):−80.97 to−51.67,Psedimentation rate(EsR,MD=−6.05,95%CI:−8.29 to−3.82,Pscore(VAs score,MD=−2.14,95%CI:−2.4 to−1.88,PsMP combined group had a higher total effective rate(risk ratio=1.22,95%CI:1.14 to 1.29,Pse compared to celecoxib *** JWsMP combined with celecoxib is more effective than celecoxib alone in improving the total efficacy rate,alleviating joint pain,and improving suA,EsR,and CRP *** also reduced the occurrence of adverse events caused by ***,the quality of the included studies was low,highlighting the need for further high-quality research with larger sample sizes and robust methodologies,such as double-blind randomization,to confirm these findings.

Alcoholic liver disease (ALD) is a major cause of morbidity and mortality worldwide. The incidence of hepatocellular carcinoma (HCC) is increasing in the united states, and chronic, excessive alcohol consumption is responsible for 32%-45% of all the liver cancer cases in the united states. Avoidance of chronic or excessive alcohol intake is the best protection against alcohol-related liver injury; however, the social presence and addictive power of alcohol are strong. Induction of the cytochrome P450 2E1 (CYP2E1) enzyme by chronic and excessive alcohol intake is known to play a role in the pathogenesis of ALD. High intake of tomatoes, rich in the carotenoid lycopene, is associated with a decreased risk of chronic disease. The review will overview the prevention of ALD and HCC through dietary tomato rich in lycopene as an effective intervention strategy and the crucial role of CYP2E1 induction as a molecular target. The review also indicates a need for caution among individuals consuming both alcohol and high dose lycopene as a dietary supplement.

New Nutrient Recommended IntakesThe united states and Canada have together revamped the nutrient intake recommendations for the North American population. The new Dietary Reference Intakes (DRIs) is a collective term which includes several reference values: the Estimated Average Requirement (EAR) value meets the nutrient needs of 50% of the population, the Recommended Dietary Allowance (RDA) value meets the requirements for 97.5% of the population. The EAR is a value which is useful for determining nutrient intake adequacy of population groups, whereas the RDA is intended to serve as a goal for daily intakes by individuals, In addition to setting EAR and RDA values, for the first time, tolerable upper intake level is the highest level of daily nutrient intake that is likely to pose no risk of an adverse health effect to almost all individuals in the general population. BACKGROuND The results obtained to date concerning food groups,diet quality and colorectal cancer(CRC)risk vary according to criteria used and the study *** To study the relationships between food groups,diet quality and CRC risk,in an adult population of the Basque Country(North of spain).METHODs This observational study included 308 patients diagnosed with CRC and 308 ageand sex-matched subjects as *** recruitment,dietary,anthropometric,lifestyle,socioeconomic,demographic and health status information was *** to the dietary recommendations was evaluated utilizing the Healthy Eating Index for the spanish Diet and the *** logistic regressions were used to evaluate the associations of food group intakes,diet quality scores,categorized in tertiles,with CRC *** The adjusted models for potential confounding factors showed a direct association between milk and dairy products consumption,in particular high-fat cheeses[odds ratio(OR)third tertile vs first tertile=1.87,95%confidence intervals(CI):1.11-3.16],and CRC *** the consumption of fiber-containing foods,especially whole grains(OR third tertile vs first tertile=0.62,95%CI:0.39-0.98),and fatty fish(OR third tertile vs first tertile=0.53,95%CI:0.27-0.99)was associated with a lower risk for ***,higher MD adherence was associated with a reduced CRC risk in adjusted models(OR third tertile vs first tertile=0.40,95%CI:0.20-0.80).CONCLusION Direct associations were found for high-fat cheese,whereas an inverse relation was reported for fiber-containing foods and fatty fish,as well as adherence to a Mediterranean dietary pattern.

目的:了解中国成人体内β-胡萝卜素( β-carotene, β-C ) 转化维生素A (VA)的效率。方法:使用稳定同位素稀释法对11名50～60岁健康志愿者(男7,女4)进行 β-C人体代谢实验。2 w适应期和8 w实验期内,志愿者接受常规膳食,避免大量VA和 β-C摄入及烟、酒和营养制剂。实验D1 ,给受试者含6 mg氘标记 β-C(2H8 β-C )玉米油胶丸,随半流质早餐(脂肪热能比25%)一起摄入。实验D4 ,同样方法摄入含3 mg氘标记醋酸视黄醇(2H8 -RAc ) 油剂胶丸。实验D1 和D4 摄入标记物后0、3、5、7、9、11、13 h时,实验D2、3、 5、6、7、8、9、10、14、21、28、35、42、49、56 晨空腹时,采静脉血。用高效液相色谱仪 (HPLC) 分离血清 β-C和VA 组分,再分别使用气相质谱仪 ( GC/Ms )和液相质谱仪 ( LC/Ms ) 测定VA和 β-C组分的同位素丰度。根据VA和 β-C的浓度和同位素丰度,描述标记VA和 β-C在体内应答的血液动力学曲线。结果: 11名受试者对口服2H8–RAc和2H8-C的应答均非常明显;52 d时血清中2H4视黄醇(来自2H8 β-C)的曲线下面积(AuC )平均为1 102 ± 658 nmol/d,52 d的2H8视黄醇(来自2H8 -RAc)的AuC平均为3659 ±787 nmol/d, 以摄入的2H8 –RAc作为VA参照,计算来自摄入2H8-C的2H4 视黄醇平均为2599 ±1180 nmol/d 。由此计算

背景：对于载脂蛋白与传统脂质测量指标预测冠心病（coronaryheartdisease，CHD）危险的性能，目前有关证据尚有矛盾。目的：采用鉴别和标准化特征（discrimination and calibration characteristics）以及危险再分类对不同脂质测量指标预测CHD的性能进行比较；评估载脂蛋白预测CHD的效果是否优于传统的脂质指标。设计、地点及参试者：以人群为基础的前瞻性队列来自FraminghaiR，Massachusetts。我们评估了血清总胆固醇、高密度脂蛋白胆固醇（HDL-C）、低密度脂蛋白胆固醇（LDL-C）、non-HDL-C、载脂蛋白（apo）A-I和apoB以及三种脂质比值（总胆固醇：HDL-C，LDL-c：HDL-C以及apoB：apoA-I）。332例中年白人参试者均参加了第4次后代检查（1987～1991年），没有心血管病。53％的参试者为女性。主要观测指标：首次CHD事件（已确诊或未确诊之心肌梗死、心绞痛、冠状动脉不全或冠心病死亡）。结果：在中位随访15年后，291例参试者（198例男性）发生CHD。在校正非脂质危险因素的多变量模型中，apoB：apoA-I比值可预测CHD（在男性每增加1个sD的风险比[hazardratio，HR]，1．39；95％可信区间[CI]，1．23～1．58。女性HR，1．40；95％,CI，1．16～1．67），但是其危险比与总胆固醇：HDL—C（男性HR，1．39；95％CI，1．22～1．58。女性HR，1．39；95％CI，1．17～1．66）、LDL—C：HDL—C（男性HR，1．35；95％CI，1．18～1．54。女性HR，1．36；95％CI，1．14～1．63）相似。在男性及女性，使用apoB：apoA—I比值的模型证实其操作特征与其他脂质相似但不优于其他脂质比值。apoB：：apoA—I比值在包含所有Framinghan危险评分因子的模型，包括总胆固醇：HDL—C（男性、P=0．12；女性P=0．58），不能预测CHR危险。结论：在这个大型人群队列，apoB：apoA-I比值预测CHD的性能与传统的脂质比值相近，但并不优于总胆固醇：HDL-C比值。在已有总胆固醇及HDL-C测量结果的条：件下，这些数据并不支持apoB或apoA-I的临床应用。
