Clinical outcomes with a new design in multifocal intraocular lens:a pilot study

Purpose:To evaluate the clinical and visual outcomes,quality of near vision and the influence of photic phenomena in patients bilaterally implanted with a new Precizon Presbyopic multifocal intraocular lens(IOL).Methods:In this prospective consecutive case series,20 eyes of 10 patients were included(mean age 63.80±12.55 years).Uncorrected and corrected visual acuity(far,intermediate and near),subjective refraction,binocular defocus curve,contrast sensitivity(CsV-1000)and quality of vision and satisfaction questionnaires were *** follow-up was 12 months after ***:At 12 months after surgery,uncorrected distance visual acuity(UDVA)improved with surgery(p=0.001)with a value of 0.08±0.08 *** near visual acuity(UnVA)was 0.22±0.12 logMAR and distance corrected near visual acuity(DCnVA)was 0.16±0.13 *** distance visual acuity(UIVA)was 0.22±0.10 *** sensitivity outcomes were similar to normal population in photopic conditions and slightly reduced in mesopic conditions of *** curve showed that this multifocal IOL was able to provide a visual acuity(VA)equal or better to 0.16 logMAR between defocus levels of+1.00 to−2.50 *** patient satisfaction was obtained in quality of vision and satisfaction questionnaires ***:The Precizon Presbyopic nVA IOL(OPHTEC BV)provides good visual *** multifocal IOL provides a high percentage of spectacle independence due to good VA at far,intermediate and near distances and satisfactory contrast *** patient satisfaction was observed in quality of vision and satisfaction questionnaires with a low percentage of patients manifesting photic phenomena.

Background:To evaluate the refractive outcomes and the optical performance as well as the quality of life in patients implanted with a new diffractive multifocal intraocular lens(IOL).Methods:Prospective,clinical study including 41 cases of patients who underwent cataract surgery and were divided in two groups:group 1,including 20 eyes implanted with the multifocal IOL seeLens MF(Hanita Lenses,Israel);group 2,21 eyes implanted with the Acrysof sA60AT *** acuity,defocus curve,intraocular aberrations,contrast sensitivity function and quality of life were assessed during a follow up period of 6 ***:significant improvement was observed in the uncorrected distance visual acuity(UDVA)and corrected distance visual acuity(CDVA)in both groups(pshowed better results in terms of uncorrected near visual acuity(UnVA)and distance-corrected near visual acuity(DCnVA)(pson of both groups showed better visual acuities for the multifocal IOL group in defocus levels from-3.0 D to-1.50 D(p≤0.01).At 6 months,there was a significant reduction of the internal higher order aberrations(p≤0.04).A significant increase in scotopic contrast sensitivity was detected for 6 cycles/°spatial frequency during follow up(p=0.04),but no significant changes were observed for the rest of spatial frequencies(p≥0.06).Visual Functioning Index(VF-14)questionnaire showed that patients reported high levels of satisfaction when performing daily ***:The seeLens MF IOL is able to successfully restore distance,near and intermediate visions after cataract *** also provides functional intermediate vision with optimal intraocular optical quality.

Presbyopia still remains the last frontier of refractive *** surgical management is under constant evolution due to the limitations that exist today with respect to its management,which is probably in relation with the multifactorial basis in which presbyopia is clinically developed in the *** currently,virtually all surgical techniques that have been proposed for its correction are based on the induction of pseudoaccommodation in the presbyopic eye,including ***,the real restoration of accommodation is more complex,and it has been tried by the use of different,so called,“accommodative”pseudophakic intraocular lenses(AIOL).Overall,the reported results with these lenses by independent authors have been modest in relation with the restoration of the accommodative power of the eye and these modest benefits are usually lost with time due to the long term changes in the capsular *** fact made these lenses to be almost abandoned in the last few years,but there are currently other AIOL models being used with innovative mechanisms of action and different anatomical support outside the capsular bag that offer encouraging preliminary results that could bring a new potential of application to these types of *** this article,we will update the modern refractive surgeon about the fundamentals and provide updated information about the outcomes of AIOLs by reviewing the concept of accommodation,the different attempts that have been accomplished in the past,their demonstrated published results in human clinical trials,and the future alternatives that may arrive in the near future.

surgical options for correction of refractive error following cataract surgery

Refractive errors are frequently found following cataract surgery and refractive lens *** biometric analysis,selection and calculation of the adequate intraocular lens(IOL)and modern techniques for cataract surgery all contribute to achieving the goal of cataract surgery as a refractive procedure with no refractive ***,in spite of all these advances,residual refractive error still occasionally occurs after cataract surgery and laser in situ keratomileusis(LAsIK)can be considered the most accurate method for its ***-based procedures,such as IOL exchange or piggyback lens implantation are also possible alternatives especially in cases with extreme ametropia,corneal abnormalities,or in situations where excimer laser is *** our review,we have found that piggyback IOL is safer and more accurate than IOL *** aim is to provide a review of the recent literature regarding target refraction and residual refractive error in cataract surgery.

Cataract surgery due to advances in small incision surgery evolved from a procedure concerned with the primary focus on the safe removal of cataractous lens to a procedure focused on the best possible postoperative refractive *** the outcomes of cataract surgery became better,the use of lens surgery as a refractive modality in patients without cataracts has increased in interest and in *** of the crystalline lens for refractive purposes or refractive lens exchange(RLE)presents several advantages over corneal refractive *** with high degrees of myopia,hyperopia and astigmatism are still not good candidates for laser ***,presbyopia can currently only be corrected with monovision or reading *** supplemented with multifocal or accommodating intraocular lenses(IOLs)in combination with corneal astigmatic procedures might address all refractive errors including presbyopia,and eliminate the future need for cataract surgery.

Purpose: To evaluate the optical and densitometric changes that take place in the crystalline lens with aging. design: Crosssectional study. Participants: seventy-two eyes of 72 patients of different ages (8-80 years) with a clear lens, a visual acuity of 20/25 or better, and no ocular disease. Methods: In each case, the lens thickness, optical density, modulation transfer function (MTF), and intraocular aberrations were measured. Main Outcome Measures: Embryonic and fetal nucleus density, lens thickness, intraocular high-order aberration (HOA), and 0.1 MTF. Results: Embryonic, anterior, and posterior fetal nucleus densities show a positive correlation with age (P nd P sp ectively). Lens thickness also shows a positive correlation with age (Pnd corneal HOAs for a 6-mm pupil show a positive correlation with age (P=0.036 and Pspectively). Ocular and corneal Zernike polynomials Z4i+Zi6 and intraocular spherical aberration (Z04) also show a positive correlation with age (P=0.001, P=0.039, and P=0.001, respectively). Intraocular coma aberration (Z-13) shows a negative linear correlation with age (Pn addition, 0.1 MTF decreased with age from 18.557 to 10.100 cycles per degree. Conclusion: There is a degradation of the optical quality of the crystalline lens with aging that is associated with morphological changes (thickness and density). These results are important for the consideration of lens replacement in the absence of evident cataract.

Purpose: To investigate in a pilot study the potential of an anterior chamber phakic refractive multifocal intraocular lens (IOL) prototype for the correction of near and far vision in those with myopic and hyperopic presbyopia. design: A multicenter, open-label, prospective, noncomparative pilot evaluation. Participants: There were 17 patients (34 eyes), 16 women and 1 man, with a mean age of 52± 3.94 years (range, 46- 62 years). six eyes were myopic (mean spherical equivalent [sE],-9.3± 3.83 diopters (D)) and 28 were hyperopic (mean sE, + 2.3± 0.77 D), with astigmatism less than 1.5 D. Methods: The prototype multifocal phakic IOL was implanted through a 6.5 mm temporal incision. The dominant eye was targeted for emmetropia and the nondominant eye for-1.0 D. Main Outcome Measures: The efficacy of the implant was assessed after surgery by measuring monocular and binocular uncorrected and distance-corrected visual acuity (VA) at distance, intermediate, and near, and distance-corrected near VA with near add. Distance-corrected distance and near VA also were determined in low contrast (25% ). The safety index and efficacy index were calculated, and patient satisfaction questionnaires also were administered. Follow-up was at 1- 3 days, 5- 9 days, 21 days, 3 months, 6 months, and 1 year after surgery. Results: spherical equivalent refraction ranged from -11.75 to+ 4.25 D before surgery, and from -2.0 to + 1.85 D 1 year after surgery. The binocular efficacy index was 0.68 (near) and 1.0 (distance), and the binocular safety index was 1.0 for both near and distance. Mean binocular uncorrected distance VA improved from 20/59 (± 4 lines) before surgery to 20/18 (± 1 line) 1 year after surgery. Mean binocular uncorrected intermediate VA improved from 20/125 (± 10 lines) to 20/21 (± 1 line), and mean binocular uncorrected near VA improved from 20/78 (± 5 lines) to 20/32 (± 1 line). Purpose: To investigate the safety of LAsIK in patients with a known history of controlled rheumatic diseases. design: Retrospective consecutive observational clinical study. Participants: Forty-two eyes (22 patients) known to have had a history of rheumatic diseases underwent *** patients had a history of controlled rheumatic diseases, including rheumatoid arthritis, systemic lupus erythematosus, dermatomyositis, scleroderma, ankylosing spondylitis, psoriatic arthritis, inflammatory bowel disease, arthritis, or Behet’s disease. Methods: Assessments were made preoperatively and at 1, 3, and 6 months postoperatively and included visual acuity (VA), refraction, keratometric readings, slit-lamp biomicroscopy, and corneal topography. All the patients underwent LAsIK. Postoperative treatment included a topical steroid-antibiotic combination. Main Outcome Measures: Visual acuity, refraction, keratometric readings, and slit-lamp biomicroscopy findings at 1, 3, and 6 months postoperatively. Results: LAsIK was performed uneventfully on all patients. The mean pre-LAsIK spherical equivalent (sE)-was-5.39±2.25, and the sixth month postoperative sE was-0.15±0.38 diopters (D) (Pncorrected VA 6 months after the LAsIK procedure was 20/25. Enhancement procedures were performed in 6 of 42 eyes (14.3%) at variable times after the primary procedure. Postoperative follow-up showed development of a moderate degree of dry eye syndrome in 4 eyes (9.5%). Corneal haze, melting, flap, or interface complications were not observed in any of the study eyes. Conclusions : In this small series, we found good outcomes when correcting refractive errors using LAsIK in selected patients with controlled rheumatic diseases. In this series, a favorable postoperative visual outcome was obtained with no operative or postoperative vision-threatening complications.

Cellular therapy of the corneal stroma,with either ocular or extraocular stem cells,has been gaining a lot of interest over the last *** publications from different research groups are showing its potential benefits in relation to its capacity to improve or alleviate corneal scars,improve corneal transparency in metabolic diseases by enhancing the catabolism of the accumulated molecules,generate new organized collagen within the host stroma,and its immunosuppressive and immunomodulatory *** extraocular stem cells do not require a healthy contralateral eye and they do not involve any ophthalmic procedures for their *** stem cells have been the most widely assayed and have the best potential to differentiate into functional adult keratocytes in vivo and in *** embryonic stem cells have been partially abandoned due to ethical implications,the discovery of the induced pluripotent stem cells(iPsC)has opened a new and very promising field for future research as they are pluripotent cells with the capacity to theoretically differentiate into any cell type,with the special advantage that they are obtained from adult differentiated *** delivery into the corneal stroma has been experimentally assayed in vivo in multiple ways:systemic versus local injections with or without a *** preliminary human clinical data is already available although still very limited,and further research is necessary in order to consolidate the clinical applications of this novel therapeutic line.
