“Time flies over us,but leaves its shadow behind”(Nathaniel Hawthorne,1860,The Marble Faun).The shadow that separates the latest guidance on vascular liver disorders of the American Association for the Study of Liver Diseases(AASLD)from its former counterpart is 12 years long(1,2).Along the course,several guidelines from different societies have been published on this topic(3-7).As acknowledged in the preamble,the lack of high-quality evidence in the field led the AASLD to commission a guidance from an expert panel based on formal review and analysis of the *** thus differs from other guidelines that perform systematic reviews with explicit methods of searching,selection,and rating the quality of evidence and,if appropriate,meta-analysis on certain clinical questions(2).

Erectile dysfunction （ED） is considered a condition with a broad range of etiologies. Obstructive sleep apnea （OSA） syndrome is one of the lesser studied risk factors for ED. We intend to summarize the current evidence on the relationship between OSA and sexual impairment, focusing on the results in terms of erectile function of the different therapies offered to OSA patients. A systematic review was conducted, selecting articles related to the physiology of OSA and ED, and to the treatments of OSA syndrome and their reported outcomes in erectile and sexual function. Higher prevalences of ED in the OSA groups have been published. However, whether this effect on the erectile function occurs in the entire range of OSA severities remains unclear. Several hypotheses were proposed to explain the physiology of this association. Continuous Positive Airway Pressure as a treatment for OSA patients with ED has achieved a significative improvement in the sexual parameters in most of the studies. Phosphodiesterase type 5 inhibitors （iPde5） on demand are useful as a treatment for ED in this subgroup of patients, with high satisfaction rates. The surgical treatment for the OSA evidenced benefits over the erectile function, and the effect on the sexual satisfaction of the therapy using Mandibular Advancement devices is still undefined.

While the association between obesity,type 2 diabetes(T2D)and the presence of steatosis has been established since the beginning of the last century(1),it was not until 1980 that the first references were made about a form of histologic liver lesion designated as non-alcoholic steatohepatitis(NASH).NASH is characterised by the presence of a variable degree of steatosis accompanied by lobular inflammatory infiltrate and hepatocellular damage with or without ***,the broader term of non-alcoholic fatty liver disease(NAFLD)was accepted,which more accurately encompass the entire spectrum of lesions that appear in these patients(2).Ever since then,NAFLD has become progressively the primary cause of chronic liver disease in developed countries(3).

This paper presents an experimental and analytical investigation of the influence of variables related to soil weathering on the geomechanical performance of sand-silt mixtures containing lateritic soils,*** weathered tropical soils with the influence of interparticle *** sand-silt mixtures containing different relative proportions between uniform sand and lateritic soil were produced,and geomechanical soil characterization tests were *** on the results,a transition from a primarily coarse-to a fine-grained prevailing soil structure was found to cause considerable impact on the geomechanical performance of these soils,as evidenced by design variables related to soil mineralogy and size distribution ***,fines contents of both individual soil particles and soil aggregations were found to correlate with experimental results,while the relative proportion between sesquioxides(aluminum,and iron oxides),and silica,***-silica ratios(SSR^(-1)),facilitated estimates concerning changes in geomechanical ***,the application of the sandsilt mixtures containing lateritic soil on soil walls reinforced with polymeric strips was also evaluated,further emphasizing the potential advantages of adopting variables related to soil weathering on design guidelines concerning tropical soils.

Background:This study is aimed at describing the prevalence of and risk factors associated with early post-operative complications after Crohn’s disease-related intestinal ***:This was a retrospective analysis of data from the PRACTICROHN *** Crohn’s disease patients who underwent ileocolonic resection with ileocolonic anastomosis between January 2007 and december 2010 were *** complications evaluated included death,ileus,anastomotic leak,abscess,wound infection,catheter-related infection,digestive bleeding and other extra-abdominal infections that occurred in the 30 days after ***:A total of 364 patients(median age at surgery 38 years and 50%men)were *** for surgery was:stricturing disease(46.4%),penetrating disease(31.3%),penetrating and stricturing disease(14.0%)or resistance to medical treatment(5.8%).Early complications were recorded in 100(27.5%)patients,with wound infection,intra-abdominal abscess and anastomotic leakage being the most frequent *** hospitalization duration was 16 days for patients with complications vs.9 days without complications(Pde at the time of surgery(15/31,48.4%)compared with the rest(85/331,25.7%)(P=0.013).Medication received at the time of surgery did not affect the rate of ***:Almost a quarter of patients developed early complications after intestinal *** disease and urgent surgery were associated with an increased risk of complications.

The anatomy of the penile urethra presents additional challenges when compared to other urethral segments during open stricture surgery particularly because of its unsuitability for excision and primary anastomosis and its relatively deficient corpus spongiosum. Stricture aetiology, location, length and previous surgical intervention remain the primary factors influencing the choice of penile urethroplasty technique. We have identified what we feel are the most important challenges and controversies in penile urethral stricture reconstruction, namely the use of flaps vs grafts, use of skin or oral mucosal tissue for augmentation/substitution and when a single or a staged approach is indicated to give the best possible outcome. The management of more complex cases such as panurethral lichen-sclerosus strictures and hypospadias "cripples" is outlined and potential developments for the future are presented.

Autoimmune hepatitis (AIH) is a rare chronic inflammatory disease of the liver, which affects a group of patients who lost their immunological tolerance to antigens of the liver. It is clinically characterized by hypergammaglobulinemia, elevated liver enzymes, presence of autoantibodies and histological changes. Although being rare in children, it represents a serious cause of chronic hepatic disease that can lead to cirrhosis and hepatic failure. Clinical findings, exclusion of more common liver disorders and the detection of antibodies antinuclear antibodies, smooth muscle antibodies and anti-LKM1 are usually enough for diagnosis on clinical practice. The pathogenic mechanisms that lead to AIH remain obscure, but some research findings suggest the participation of immunologic and genetic factors. It is not yet knew the triggering factor or factors that stimulate inflammatory response. Several mechanisms proposed partially explain the immunologic findings of AIH. The knowledge of immune factors evolved might result in better markers of prognosis and response to treatment. In this review, we aim to evaluate the findings of research about genetic and immune markers and their perspectives of application in clinical practice especially in pediatric population.

Mg-Zn-Zr(ZK)alloys exhibit notably high mechanical strength amongst all magnesium alloy ***,due to the formation of low melting point Mg_3Zn_7-precipitates,these alloys are susceptible to hot cracking,thus compromising their metallurgical *** addition of rare earths to ZK alloys is an alternative to form higher melting point intermetallic compounds,speed up dynamic recrystallization,refine grain size,enhance corrosion resistance and extend the service temperature due to improved creep *** work deals with the effect of Ce-base mischmetal addition on the hot rolling behaviour of as-cast ZK60 *** microstructure investigation conducted using electron microscopy and X-Ray diffraction shows that precipitation of Mg_(7)Zn_(3) intermetallics occur during hot rolling,whereas no further precipitation is observed for the ZK60-Mm *** fragmentation of the intermetallic compounds occur during hot rolling and finer particles of Mg_(7)Zn_(3) are observed for the ZK60,whereas Mg_(7)Zn_(3) and Mg Zn_(2) Ce intermetallics are formed in the alloy modified with mischmetal addition.A higher fraction of dynamically recrystallized grains is observed for the ZK60-Mm in comparison to the *** recrystallization takes place in ZK60 with the formation of sub-grains near to the intermetallics and the addition of mischmetal promotes the occurrence of discontinuous recrystallization with the nucleation of new grains close to the *** mechanical strength and,in particular,the ductility of the hot-rolled alloys are notably improved when compared to the same alloys in the as-cast *** mechanical strength is,however,higher for the ZK60 *** solid solution strengthening,softer Mg Zn_(2) Ce intermetallics and more extensive recrystallization contribute to reduce the mechanical strength of *** in both alloys are initiated at coarse intermetallics and propagate through intermetallic-rich regions.
