Glabridin Relaxes Vascular Smooth Muscles by Activating BK_(Ca) Channels and Inhibiting Phosphodiesterase in Human Saphenous Vein

The aim of the current study was to investigate the pharmacological activity of glabridinon the isolated human saphenous vein (SV) and explore the underlying mechanisms. Samples of patients' SVs were removed during bypass surgery, and 4-mm lengths of the vessels were placedin Krebs solution at ±4℃ and hung in an isolated organ bath to assess their contraction/relaxationresponses. The contraction/relaxation responses were recorded to observe if the cyclic guanosinemonophosphate (cGMP)/protein kinase G (PKG) pathway mediates the relaxant effect of glabridinafter treatment with blockers like ODQ (a guanylate cyclase inhibitor), KT5823 (a PKG inhibitor),isobutylmethylxanthine [IBMX, a phosphodiesterase (PDE) inhibitor], and cantharidin [Cant,a myosin light-chain phosphatase (MLCP) inhibitor]. Moreover, nitric oxide (nO), cGMP, andPKG levels in SV tissues were determined by ELISA after incubation with glabridin, n(o)-nitro-L-arginine methyl ester (L-name, a nO synthetase inhibitor), phenylephrine (PE), ODQ, IBMX,and KT5823. The results showed that glabridin relaxed the vascular smooth muscle of humanSV pretreated with PE in a dose-dependent manner, which was independent of the *** vasorelaxant effect of glabridin was only inhibited by iberiotoxin (IbTX), Cant, and *** increased cGMP and PKG levels in SV homogenates, whereas it did not alter the nOlevel. The enhancing efects of cGMP and PKG levels by glabridin were abolished by ODQ andKT5823. In conclusion, glabridin has a vasorelaxant effect, which is associated with the activationof BKc. channels and inhibition of PDE.

With the developments in medical technology and increased surgical experience, advanced laparoscopic surgical procedures are performed successfully. Laparoscopic abdominal surgery is one of the best examples of advanced laparoscopic surgery (LS). Today, laparoscopic abdominal surgery in general surgery clinics is the basis of all abdominal surgical interventions. Laparoscopic abdominal surgery is associated with systemic and splanchnic hemodynamic alterations. Inadequate splanchnic perfusion in critically ill patients is associated with increased morbidity and mortality. The underlying pathophysiological mechanisms are still not well understood. With experience and with an increase in the number and diversity of the resulting data, the pathophysiology of laparoscopic abdominal surgery is now better understood. The normal physiology and pathophysiology of local and systemic effects of laparoscopic abdominal surgery is extremely important for safe and effective LS. Future research projects should focus on the interplay between the physiological regulatory mechanisms in the splanchnic circulation (SC), organs, and diseases. In this review, we discuss the effects of laparoscopic abdominal surgery on the SC.

Synthesis of n-（1H-imidazoline-2-yl）-lH-benzimidazol-2-amine was carried out under microwave irradiation （MWI） conditions. Dynamic 1H nMR investigation of n-（lH-imidazoline-2-yl）-lH-benzimidazol-2-amine compound was reported at temperature range of 223--333 K in DMF-d7. Some physical parameters, such as coalescence temperature （Tc）, the free energy of activation （△G＊） and rate constant （k） values were calculated from its IH nMR spectra at various temperatures. Electrochemical feature of this compound was investigated by cyclic （CV） and square wave voltammetry （SWV）.

Violence toward emergency physicians: A prospective-descriptive study

BACKGROUnD: The highest rate of workplace violence occurs in the health sector, although most cases remain unreported. Emergency services face the majority of these incidents for many reasons, such as the patient profile, long waiting time, and overcrowding. We aimed to determine the characteristics and causes of violence toward emergency physicians. METHODS: The acts of violence toward emergency physicians over a one-year period were prospectively recorded. After a violent incident took place, a third party separately interviewed the physician exposed to the violent behavior and the perpetrator who displayed this behavior. We examined the perpetrator's reasons for violence, their demographic characteristics, and the medical complaints of patients involved in such events to determine the characteristics and causes of ***: Of the violent acts investigated, 85.1% were verbal, and most were directed toward male doctors by the young male relatives of the patients. More than half of the violent acts occurred within the 15 minutes of presentation to emergency service(60.5%) and at off-hours(69.4%). Concerning the health insurance, 20.4% of the cases were covered by the free green card system, and a small number of the perpetrators of violence lived in rural areas(38.2%). The most common reason for violent behavior was the patients' or their relatives' dissatisfaction with the examination or treatment method(38.2%).COnCLUSIOnS: Appropriate communication should be established with the patients, and they should be adequately informed about the treatments and interventions to be performed in order to prevent possible acts of violence.

BACKGROUnD ground Aspiration and evaluation of gastric residuals are commonly performed interventions before each feeding in intensive care units,especially in very low birthweight ***,there is no sufficient evidence about the necessity of routine gastric residual *** this study,we aimed to investigate the time to full enteral intake and the incidence of necrotizing enterocolitis(nEC)in preterm infants in the period with gastric residual aspiration performed before each feeding,and those in the period without gastric residual *** Preterm infants with a gestational week≤33 were included in the *** group with gastric residual control before each feeding consisted of 169 infants,and the group without routine gastric residual aspiration included 122 *** The mean gestational week was 30.37±2.58 and 29.31±3.37 in the group with gastric residual control and in the group without routine residual control,respectively(pnder,and mode of delivery were similar between both *** time to full enteral intake was shorter in the group without routine residual control(pns of parenteral nutrition,≥grade 2 nEC,weight at discharge and duration of hospitalization were similar between the *** of invasive mechanical ventilator support was shorter in the group without routine residual *** Avoidance of routine gastric residual aspiration in preterm infants shortens the time to full enteral intake without increasing the incidence of nEC.
