We present an improved Born-Oppenheimer method for the treatment of molecules. Our development is based on taking into account a portion of the kinetic energy which was inadvertently omitted by Born and Oppenheimer. Our theory replaces the set of standard atom-atom potentials by an evolution operator. In particular, in contrast to the traditional Born-Oppenheimer method our amended basis describes the so-called Wannier phenomenon which deforms potential surfaces, and traps one or more threshold electrons into an unstable equilibrium. Within our framework that trapping manifests itself as a Fresnel distribution. Finally we discover a π/2 phase jump in the evolution operator caused by the diffraction of a two-electron wave from a potential ridge.

Omalizumab is a humanized monoclonal antibody that binds to the high-affinity type-I Ig E Fc receptors on mast cells(MCs) and basophils, inhibiting the Ig E immune pathway. Irritable bowel syndrome(IBS) is the most common functional gastrointestinal disorder, and dysregulation of the immune system likely contributes to its etiology and/or symptomatology. Colonic biopsies from patients with IBS demonstrate considerable increase in the number of degranulating MCs releasing histamine in proximity to nerves, and this event may underlie the common IBS symptom of abdominal pain. Pharmacologic control of MC activation and mediator release is a current area of active interest in the field of IBS research. Recently, we and Pearson et al described 2 cases of patients with IBS with diarrhea(IBS-D) showing positive clinical response to omalizumab. In both cases, the female patients had severe, long-lasting IBS-D and achieved an almost complete resolution of IBS symptoms. Both patients were also able to discontinue all IBS medications after commencing the anti-Ig E therapy. For both patients, the omalizumab treatment showed a relatively rapid onset of action, resembling the efficacy observed in and previously reported for patients with chronic spontaneous urticaria. In this Editorial, we discuss the possible biological mechanisms that may underlie the clinical efficacy of omalizumab in IBS. We suggest that there is a need for a well-designed prospective study to investigate the therapeutic effects of anti-Ig E in IBS.

Einstein’s weak equivalence principle suggests that gravity and acceleration (centrifugal force) are indistinguishable from each other and, therefore, equivalent. We maintain that they are not only equivalent, but even identical, or to rephrase the main statement of this work: A gravitational force does not exist. Rather, gravity is a fictitious force, or, more pointedly: Gravity is the centrifugal force which acts upon material bodies within the rotating S3-hypersphere of the Universe. These in turn warp the adjacent space-fabric, shaping it to the well-known field geometry of general relativity.

A thorough analysis of composite inertial motion (relativistic sum) within the framework of special relativity leads to the conclusion that every translational motion must be the symmetrically composite relativistic sum of a finite number of quanta of velocity. It is shown that the resulting spacetime geometry is Gaussian and the four-vector calculus to have its roots in the complex-number algebra. Furthermore, this results in superluminality of signals travelling at or nearly at the canonical velocity of light between rest frames even if resting to each other.

High incidence of maternal vitamin B12 deficiency detected by newborn screening: first results from a study for the evaluation of 26 additional target disorders for the German newborn screening panel

Background Newborn screening (NBS) in Germany currently includes 15 target disorders. Recent diagnostic improvements suggest an extension of the screening panel. Methods Since August 2016, a prospective study evaluating 26 additional target disorders (25 metabolic disorders and vitamin B12-deficiency) in addition to the German screening panel is performed at the Newborn Screening Center Heidel-berg. First-tier results from tandem-MS screening are complemented by second-tier strategies for 15 of the additional target disorders. NBS results of seven patients diagnosed symptomatically with one of the additional target disorders by selective screening since August 2016 are retrospectively evaluated. Results Over a 13-month period, 68,418 children participated in the study. Second-tier analyses were performed in 5.4%of samples. Only 59 (0.1%) of study participants had abnormal screening results for one of the additional target disorders. Target disorders from the study panel were confirmed in 12 children: 13-hydroxy-3-methylglutaryl coenzyme A (CoA)-lyase deficiency, 1 citrullinemia type I, 1 multiple acyl-CoA dehydrogenase-deficiency, 1 methylenetetrahydrofolate reductase-deficiency, and 8 children with maternal vitamin B12-deficiency. In addition, six of seven patients diagnosed symptomatically outside the study with one of the target disorders would have been identified by the study strategy in their NBS sample. Conclusions Within 13 months, the study 'Newborn Screening 2020' identified additional 12 children with treatable con-ditions while only marginally increasing the recall rate by 0.1%. Maternal vitamin B12-deficiency was the most frequent finding. Even more children could benefit from screening for the additional target disorders by extending the NBS panel for Germany and/or other countries.
