Hemodynamic Fetal Response to Maternal Isometric Exercise in Pregnant Patients with Diabetes

Background: Studies on physical activity during pregnancy and its impact on mother and fetus are still limited. International protocols consider only aerobic exercise and fail to provide information about other modalities such as isometric exercise. Isometric exercise promotes cardiorespiratory resistance and muscle strengthening, but it is rarely tested on pregnant women because it increases maternal blood pressure and can subsequently affect placental circulation. Objectives: To assess the fetal response by use of Doppler study in diabetes pregnant women submitted to isometric exercise. Methods: A cross-sectional experimental study was conducted on 25 diabetes pregnant women with gestational age between 26 and 36 weeks. The patients were submitted to isometric handgrip, and data were collected from the mother (blood pressure, heart rate and Doppler velocimetry for the uterine arteries) and from the fetus (heart rate, Doppler velocimetry for the umbilical artery, middle cerebral artery and ductus venosus). All variables were collected before, during and after the isometric handgrip. Results: There was a significant reduction in the pulsatilityindex (average values pre 0.77 ± 0.30, trans 0.65 ± 0.22, and post 0.75 ± 0.22, with p = 0.001), resistance index (average values pre 0.49 ± 0.12, trans 0.44 ± 0.10, and post 0.48 ± 0.90, with p =

Key elements required to publish a good article:consensus among executive editors of Traditional Chinese Medicine and integrative medicine journals——a Delphi study

OBJECTIVE:To explore consensus among Traditional Chinese Medicine(TCM)/ integrative medicine(IM) journal executive editors in China regarding:(a)Their perceptions about the differences between papers published in Chinese/English journals;(b)the key elements required for publishing a good quality paper;(c) how editors manage and decide on the manuscripts submitted to their ***:A three classic round Delphi was conducted with a purposive sample of TCIWIM Chinese journal executive *** key themes and items generated from six open ended questions in the first round were redistributed to the editors in two subsequent questionnaire rounds and scored on a five-point Likert ***,reliability and stability of responses were assessed between the editors in the same round and between ***:All ten editors initially agreeing to participate completed the three rounds,provided 60 comments in the first round,relating to 6 themes on:elements of publishing good quality article;differences between Chinese and English published articles;reasons for low quality of Chinese studies;maintaining the journal's good reputation;initiatives and strategies;and differences between Chinese and English versions of the same *** levels of consensus were reached for 97%***-class correlation(ICC) coefficient for agreement among editors was high in both second and third round and there was a good stability between the two ***:High inter/intra-editor agreement was identified in the differences between TCM/IM research papers published in English and *** standards and probity are keys in elevating the reputation of ***' perspectives in maintaining and improving quality journal can be viewed as fundamental for editors and researchers in publishing any articles.

An umbrella review of Lianhua Qingwen combined with Western medicine for the treatment of coronavirus disease 2019

Objective:Lianhua Qingwen combined with Western medicine(LHQW+WM)has been proposed as a viable treatment for coronavirus disease 2019(COVID-19).Interestingly,umbrella reviews of systematic reviews(SRs),which provide the most comprehensive evidence,are the best evidence in evidence-based ***,an umbrella review of SRs that summarizes and evaluates the efficacy of LHQW+WM for COVID-19 is urgently ***:Overall,6 databases were used to conduct a comprehensive literature search from inception to January 22,*** corrected covered area(CCA)was used to analyze the overlapping between ***-analysis was conducted when that of the included SRs was inappropriate.A MeaSurement Tool to Assess Systematic Reviews(AMSTAR-2)was also employed to assess the quality of the included ***:In total,12 SRs were identified,which included 12 unique primary *** included SRs ranged in quality from moderate to critically low and had an extremely high CCA(36.4%).Compared to conventional treatment,LHQW+WM showed efficacy concerning fatigue recovery[risk ratio(RR)=1.69,95%confidence interval(CI):1.04-2.73,n=2,I2=0%],cough recovery(RR=1.65,95%CI:1.09-2.51,n=3,I2=39.1%),and overall effective rates(RR=1.17,95%CI:1.07-1.28,n=3,I2=17.5%).Conclusion:LHQW+WM may improve the clinical symptoms of patients with COVID-19;however,the results should be interpreted cautiously because of the rigorous processes in the included SRs.

Hemodynamic Fetal Response to Maternal Isometric Exercise in Pregnant Patients with Hypertension

Objective: to evaluate the maternal-fetal hemodynamic response with Doppler in pregnant women with chronic arterial hypertension and preeclampsia submitted to controlled isometric activity. Methods: experimental study comparing 50 healthy and 26 hypertensive and 24 preeclamptic pregnant women, from 26 to 36 weeks of gestational age, submitted to isometric contraction with handgrip dynamometer. Maternal hemodynamic parameters (systolic and diastolic blood pressure;heart rate;uterine arteries Doppler) and fetal (heart rate;umbilical artery, middle cerebral and venous duct Doppler) were evaluated before, during and post-isometry. Results: in preeclampsia were observed higher values of blood pressure and uterine artery indexes in all times;middle cerebral artery indexes in the pre and post-isometry;and of maternal heart rate post-isometry. In hypertensive women, systolic blood pressure is increased all the times, with indexes of the right uterine and middle cerebral arteries higher in pre-isometry;middle cerebral and umbilical arteries greater during isometry;and maternal heart rate and umbilical artery indexes bigger after isometry. Conclusion: blood pressure is higher in preeclamptic and hypertension women. The right uterine artery has more resistance in preeclampsia, with a significant decrease in pre to isometry in hypertensive and preeclamptic women;and increased in contraction to post-isometry in healthy and hypertensive women. The left uterine artery increases resistance post-isometry in all groups. The fetal hemodynamic parameters did not show significant differences when comparing the before, during and post-isometry.

In recent years,the real-world studies(RWS)have attracted extensive attention,and the realworld evidence(RwE)has been acceptedto support the drug development in Chinaand ***,there is stll a lack of standards for the evaluation of the quality of *** is necessary to formulate a quality evaluation and reporting specification for RWE especially in traditional Chinese medicine(TCM).To this end,under the guidance of China Association of Chinese Medicine,the Quality Evaluation and Reporting Specification for Real-World evidence of Traditional Chinese Medicine(QUERST)Group,including 24 experts(clinical epidemiologists,clinicians,pharmacologists,ethical reviewerand statisticians),was established to develop the *** specification contains the listing of classification of RWS design and RWE,the general principles and methods of RWE quality evaluation(26 tools or scales),25 types of bias in RWS,the special considerations in evaluating the quality of RWE of TCM,and the 19 reporting standards of *** specification aims to propose the quality evaluation principles and key points of RWE,and provide guidance for the proper use of RWE in the development of TCM new drugs.
