ERCP in acute pancreatitis:What takes place in routine clinical practice?

AIM:To evaluate the data from a survey carried out in Italy regarding the endoscopic approach to acute pancreatitis in order to obtain a picture of what takes place after the release of an educational project on acute pancreatitis sponsored by the Italian Association for the Study of the ***:Of the 1 173 patients enrolled in our survey,the most frequent etiological category was biliary forms(69.3%) and most patients had mild pancreatitis(85.8%).RESULTS:344/1 173(29.3%) underwent endoscopic retrograde cholangiopancreatography(ERCP).The mean interval between the onset of symptoms and ERCP was 6.7 ± 5.0 d;only 89 examinations(25.9%) were performed within 72 h from the onset of *** main indications for ERCP were suspicion of common bile duct stones(90.3%),jaundice(44.5%),clini cal worsening of acute pancreatitis(14.2%) and cho langitis(6.1%).Biliary and pancreatic ducts were visua lized in 305 patients(88.7%) and in 93 patients(27.0%) *** success rate in obtaining a cholangio gram was statistically higher(P = 0.003) in patients with mild acute pancreatitis(90.6%) than in patients with severe disease(72.2%).Biliary endoscopic sphinc terotomy was performed in 295 of the 305 patients(96.7%) with no difference between mild and severe disease(P = 0.985).ERCP morbidity was 6.1% and mortality was 1.7%;the mortality was due to the complications of acute pancreatitis and not the endoscopic ***:The results of this survey,as with those carried out in other countries,indicate a lack of compliance with the guidelines for the indications for interventional endoscopy.

Background:Patient outcomes are poor for aggressive B-cell non-Hodgkin's lymphomas not responding to or progressing within 12 months after first-line *** is an anti-CD19 chimeric antigen receptor T-cell therapy approved for diffuse large B-cell lymphoma after at least two treatment ***:We conducted an international phase 3 trial involving patients with aggressive lymphoma that was refractory to or progressing within 12 months after first-line *** were randomly assigned to receive tisagenlecleucel with optional bridging therapy(tisagenlecleucel group)or salvage chemotherapy and autologous hematopoietic stem-cell transplantation(HSCT)(standard-care group).The primary end point was event-free survival,defined as the time from randomization to stable or progressive disease at or after the week 12 assessment or *** to receive tisagenlecleucel was allowed if a defined event occurred at or after the week 12 *** end points included response and safety.

In recent years,self-expandable metal stents(SEMSs)have been employed to treat benign gastrointestinal strictures secondary to several conditions:Acute diverticulitis,radiation colitis,inflammatory bowel disease(IBD),and postanastomotic leakages and *** applications include endometriosis and fistulas of the lower gastrointestinal *** it may be technically feasible to proceed to stenting in the aforementioned benign diseases of the lower gastrointestinal tract,the outcome has been reported to be *** fact,in some settings(such as complicated diverticulitis and postsurgical anastomotic strictures),stenting seems to have a limited evidence-based benefit as a bridge to surgery,while in other settings(such as endometriosis,IBD,radiation colitis,etc.),even society guidelines are not able to guide the endoscopist through decisional algorithms for SEMS *** aim of this narrative paper is to review the scientific evidence regarding the use of SEMSs in nonmalignant diseases of the lower gastrointestinal tract,both in adult and pediatric settings.

本文以1920至1940年代相關論著爲研究範圍,由'文法'與'修辭'的語義分合出發,討論背後一個根本的焦慮:漢字或漢語究竟該如何對應以英語爲主的文法。其中,一方面思考西方以詞類、句法爲主的文法,如何可能規範漢語語境下的讀書作文;另一方面思考,修辭如果並不全等於標準文法,那麽不同於拼音符號的漢語或漢字,如何在認知、指涉之外,透過本義的引申、假借、類比來引發同情共感。更重要的是,討論漢字或漢語會如何表述那些牽引在説寫/閲聽雙方的經驗、心理狀態之間紛繁如'詩'的意義線索。而這股關於漢字'多義性'的論述潮流,明顯不同於當時漢字拼音化、符號化的'現代'工程,反倒可能是20世紀上半葉,一種翻轉'現代視線'的重要現象。

在西遊叙述中,神魔鬥法前時常出現的,就是孫行者頻繁召唤土地,方能問明妖精、妖魔的出處,其頻率極高而關注者卻少,原因就是土地神僅被視爲陪襯性小神。世德堂本(以下簡稱世本)叙述其出場的方式及用意,作者其實交互使用兩種聲音:顯聲音即表現滑稽性的遊戲筆調,目的即在掩飾其真正的潛聲音,主要即借此諷喻、影射當世。這種文學旨趣在兩種聲音間交織,若隱若現,亟待解讀。世本寫定者雖曾吸收先行材料,但本身自有其創意所在,即化用當時道、佛二教的文化資源。這一點學界雖有關注者,此處即針對"召唤土地"情節,詳細解讀其使用的手法,認爲其背後皆有宗教、尤其道教知識的支持。運用從淺到深的叙述層次,其顯聲音即召唤土地的方式,軟硬兼用,從唤出到拘得,形成表面的叙述趣味;而關鍵的召唤叙述,也是簡繁俱有的捻訣、唸真言,從唵字到唵淨法界,按照當時的宗教文化的理解,即採用明代流行於文人間的準提信仰,致使作者挪用密教準提咒,並未襲用道教的召土地神咒。最值得注意的是召唤過程,叙寫行者對待土地、山神的使唤態度,而相對地,土地、山神則表現惶恐的滑稽表情。這種顯話語背後隱藏作者的潛話語,即運用"縱放之嫌"的叙述筆法,首即化用道教的鬼律、黑律知識,規範城隍——土地:境内凡有精邪而未能通告者,即有縱放之嫌而會被杖或流放;故作者叙寫土地既知妖精、妖魔存在卻任令其活動,即襲用"縱放之嫌",而叙寫行者欲用金箍棒威脅"欲打"而未打,此爲化用了道教的鬼律叙述;其次則是諷喻性的影射層次,明代中葉以前實行里甲制,小説家曾活用其兩件事:官方推動禮制性的里社壇制,但里甲居民仍然崇拜土地神,以致里社壇荒廢不用;其次里長、甲首負責里甲内的催税、徭役,但税役過重後導致居民脱逃,小説乃有逃門户、大户負擔元宵燈油等,此種叙述爲顯話語;而描述妖精、妖魔據洞稱王、差使土地,尤其後者俱從天界私下凡間,此種潛話語即諷喻明代王府與地方豪族。西遊叙述交錯活用顯、潛兩種聲音,即可知表面愈荒唐、嬉戲的語言,愈是掩飾當時習知的社會怪現狀,故時人自然領會其諷喻旨趣,今人則需從"縱放之嫌"切入,方能進行深刻的文學詮釋,確定世本能巧用多層次的語言藝術,從而爲其"奇書"作文學史的定位。

背景与目的通过意大利放射治疗和临床肿瘤学学会(Italian Association of Radiotherapy and Clinical Oncology, AIRO)的一项多中心回顾性研究,明确免疫疗法(immunotherapy, IT)和立体定向放疗(stereotactic radiotherapy,SRT)包括放射外科治疗(radiosurgery, RS)或大分割立体定向放疗(hypofractionated stereotactic radiotherapy, HFSRT)治疗非小细胞肺癌(non-small cell lung cancer, NSCLC)脑转移的疗效和毒性。方法对来自19个意大利中心接受SRT+IT的NSCLC伴脑转移患者进行分析,并与单独接受SRT的对照组患者进行比较。结果共纳入SRT+IT组100例和单独SRT组50例。接受SRT+IT的患者有更长的颅内局部无进展生存期(intracranial local progression-free survival, i LPFS)(经倾向性评分调整后,P=0.007)。在诊断为脑转移时接受IT合并有颅外进展的患者中(n=24),接受SRT后再进行IT与较好的总生存期(overall survival, OS)相关(P=0.037)。多因素分析显示组织学类型为非腺癌、卡氏体能状态(Karnofsky performance status,KPS)评分=70分以及接受HFSRT与显著减少的生存期相关(P值分别为0.019、0.017和0.007)。与SRT和IT治疗间隔>7天(n=10)相比,治疗间隔≤7天(n=90)与更长的OS相关(经倾向性评分调整后,P=0.008)。联合治疗耐受性良好。在放射性坏死方面,接受SRT+IT的患者与单独SRT的患者无显著差异。SRT和IT的治疗间隔对毒性发生率无影响。结论 SRT+IT是一种安全的联合治疗方法,与单独SRT相比,具有更长的i LPFS。
